Human Sparganosis in Korea

  • Background: Sparganosis is a larval cestodiasis caused by the plerocercoid of Spirometra spp. Since the first description of human sparganosis in 1924, several hundred cases have been reported in Korea. However, systematic approaches for literature surveys of Korean sparganosis have seldom appeared. Methods: We searched publicly available databases such as PubMed, Research Information Sharing Service, and Korea Medical Citation Index with relevant Medical Subject Headings. Results: At least 438 Korean sparganosis cases have been described from 1924 to 2015. Preoperative diagnosis has been significantly increased since the 1980s due to popularization of serological and imaging diagnostics. Cases were largely detected from fifth decades in general, but cerebral sparganosis was detected in relatively young age groups (third and fourth decades). Sparganosis was prevalent in men (75.9%). Consumption of frog/snake and drinking unfiltered water were found in 63.4% and 16.9% of patients, respectively. Most frequently affected sites were subcutaneous tissues (49.9%), followed by the central nervous system (36.2%). Involvements of visceral organs (7.6%), ocular regions (3.6%), and muscles (2.7%) were noticed. In women, breast sparganosis constituted a large proportion (34.2%). Sparganosis associated with immunocompromised patients has recently been reported. Conclusion: Sparganosis has been continuously reported in Korea during the past 90 years, although its incidence has decreased during the last 20 years. The disease is mostly characterized by subcutaneous nodule, but infection of the worm in vital organs often results in serious illness. Continuous awareness is warranted to monitor sparganosis occurrence and associated clinical consequences.

Associations between Hashimoto Thyroiditis and Clinical Outcomes of Papillary Thyroid Cancer: A Meta-Analysis of Observational Studies

  • Background: Epidemiological studies have suggested an association between Hashimoto thyroiditis (HT) and papillary thyroid cancer (PTC) development. Other studies, however, have reported a protective role of HT against PTC progression. Through this updated meta-analysis, we aimed to clarify the effects of HT on the progression of PTC. Methods: We searched citation databases, including PubMed and Embase, for relevant studies from inception to September 2017. From these studies, we calculated the pooled odds ratios (ORs) of clinicopathologic features and the relative risk (RR) of PTC recurrence with 95% confidence intervals (CIs) using the Mantel-Haenszel method. Additionally, the Higgins $I^2$ statistic was used to test for heterogeneity. Results: The meta-analysis included 71 published studies with 44,034 participants, among whom 11,132 had HT. We observed negative associations between PTC with comorbid HT and extrathyroidal extension (OR, 0.74; 95% CI, 0.68 to 0.81), lymph node metastasis (OR, 0.82; 95% CI, 0.72 to 0.94), distant metastasis (OR, 0.49; 95% CI, 0.32 to 0.76), and recurrence (RR, 0.50; 95% CI, 0.41 to 0.61). Conclusion: In this meta-analysis, PTC patients with HT appeared to exhibit more favorable clinicopathologic characteristics and a better prognosis than those without HT.

  • Objectives Osteoarthritis is hard to manage with both conventional and Korean medicine treatment. The core outcome set (COS) to demonstrate the effectiveness of Korean medicine treatment has not been established yet. We aimed to present preliminary data of COS by performing a literature review on the evaluation indices used in existing clinical research. Methods We examined the literature from 2000 to 2017 in two Korean electronic databases (Korea citation index and oriental medicine advanced searching integrated system) by searching for the following 3 terms 'total knee replacement (Korean)', 'total knee replacement,' and 'knee surgery.' We found 333 articles; among them, 50 duplicates were removed. Finally, we selected 160 articles after complete screening. We then extracted measured indices and clinical outcomes from the selected articles and categorized the relevant criteria. Results According to this study, the hospital for special surgery and knee society, range of movement angle, cross leg, Berg balance scale and balance ability, muscle strength, 6 minutes walking test, visual analogue scale, self-efficacy, the 12-item and 36-item short form survey and self-rated health status are the most commonly used outcomes of knee. Conclusions This study found that the several categories after total knee replacement (TKR) are being evaluated in the literature, and we were able to verify the most frequently used evaluation indices in these categories. The results of this study will be used to establish evaluation indices for the treatment of TKR in the future using Korean medicine.

한방병원에 근무하는 간호사의 정신건강에 대한 연구동향: 국내연구를 중심으로 (Scoping Review of Research on Mental Health of Nurses Working in Korean Medicine Hospitals)

  • Objectives: The aim of this scoping review was to analyze research trends about mental health of nurses working in Korean medicine (KM) hospitals in Republic of Korea. Methods: Searches were conducted using four electronic databases including Oriental Medicine Advanced Searching Integrated System, Korean Studies Information Service System, Research Information Sharing Service, Korea Citation Index to collect relevant studies. The search date was March 4, 2021. All studies published up to the search date were considered. Observational studies reporting mental health outcomes of nurses working at KM Hospital were included. Results: A total of 11 cross-sectional observation studies were included. Four of them compared mental health of nurses working on KM and Western medicine (WM). Commonly reported outcomes related to mental health were job satisfaction, role conflict, and work stress. As a result of meta-analysis based on a 5-point Likert scale, the following factors were related to mental health of KM nurses: job satisfaction (2.844±0.067 points), role conflict (3.678±0.058 points), work stress (3.142±0.021 points), turnover intention (3.483±0.028 points), and burnout (3.180±0.033). Compared to WM nurses, KM nurses had significantly less work stress (p=0.000), role conflict (p=0.039), and job satisfaction (p=0.000). Conclusions: Mental health problems of nurses are known to be very common. Although improving them is an important social task, studies on mental health of KM nurses remain insufficient. Based on findings of this study, more cooperation between nurses and KM doctors should be made to improve the mental health of KM nurses, especially their job satisfaction in the future. The Korean Society of Oriental Neuropsychiatry needs to pay more attention to this topic.

슬개대퇴통증증후군의 침 치료에 대한 체계적 문헌고찰의 프로토콜 (Acupuncture Treatment for Patellofemoral Pain Syndrome: A Protocol for a Systematic Review)

  • Objectives The purpose of this systematic review is to analyse randomized controlled trials about acupuncture for patellofemoral pain syndrome (PFPS) and evaluate it's pain improvement effect. Methods Studies will be searched from ten databases (Medline/PubMed, Cochrane Library, EMBASE, China National Knowledge Infrastructure [CNKI], WanFang Data, Citation Information by NII [CiNII], ScienceON, Koreanstudies Information Service System [KISS], KMBASE, and KoreaMed). Participants will be patients diagnosed with PFPS, regardless of cause, gender, age or race, and interventions will be acupuncture, electroacupuncture, dry needling, fire needle, and acupotomy. Other treatments than interventions will be the control group and the main outcome will be reviewed by visual analogue scale (VAS). Results Data will be extracted and assessed according to the Preferred Reporting Items for Systematic Review and Meta-Analysis flow chart and Cochrane risk of bias tools. Meta-analysis will be conducted using random effect model within 95% confidence interval. Conclusions We expect this systematic review to provide useful data about the pain improvements of patellofemoral pain using acupuncture and expect to stimulate and systematize acupuncture treatment on PFPS in the future.

심장판막 치환술 후 한의학적 치료에 대한 체계적 문헌고찰 및 메타분석 (East Asian Traditional Medicine Treatment for Patients after Heart Valve Replacements: A Systematic Review with Meta-Analysis)

  • Objectives: To compare the effectiveness and safety of East Asian traditional medicine treatments (EATMT) versus conventional management in patients following heart valve replacement surgery. Methods: We searched several databases, including the Korean Studies Information Service System, PubMed, China National Knowledge Infrastructure, and Citation Information by NII. The search range included randomized controlled trials from each first issue until June 27, 2021. Two review authors independently extracted the data. We assessed the risk of systematic errors by evaluating risk domains using the "Risk of bias" tool. Results: We included 5 trials in the review. In the EATMT, the investigators reported significant improvements in reshaping of the heart structure: left ventricular end diastolic diameter (MD -4.43, 95% CI -6.06 to -2.79; 130 participants; 2 studies; high evidence). Comparisons with usual care revealed a significant decrease in gastrointestinal complications rate (OR 0.30, 95% CI 0.20 to 0.47; 503 participants; 2 studies; high evidence). We assessed 4 studies as having a low risk of bias and 1 study as having a high risk of bias. Conclusion: This systematic review suggests that East Asian traditional medicine interventions may be effective in preventing and alleviating complications, but we found evidence of important trade-offs between known benefits and known adverse effects in cardiac dysfunction and inflammation following heart valve replacement. Consequently, additional high-quality studies should be conducted.

파킨슨병에 동반된 수면장애의 한약 치료에 대한 임상 연구 동향 : 무작위 대조연구를 중심으로 (Efficacy of Herbal Medicine on Sleep Disorders in Parkinson's Disease: A Review of Randomized Controlled Trials)

  • Objectives: This study reviewed randomized controlled trials (RCTs) investigating the efficacy of herbal medicine on sleep disorders associated with Parkinson's disease and suggests a better research process. Methods: We searched for RCTs for herbal medicine treatments for sleep disorders related to Parkinson's disease on July 31, 2023 using eight databases (PubMed, Embase, the Cochrane library, China National Knowledge Infrastructure [CNKI], the Research Information Service System [RISS], Science ON, the Oriental Medicine Advanced Searching Integrated System [OASIS], and the Korea Citation Index [KCI]). Cochrane's risk of bias tool was used to assess the quality of the RCTs. Results: A total of 16 RCTs met all the inclusion criteria, and in most reports, the treatment group showed a significant improvement in sleep disorders compared to the control group. Total effective rate (TER), Pittsburgh Sleep Quality Index (PSQI), Unified Parkinson's Disease Rating Scale (UPDRS), TCM Symptom Score (TSS), Parkinson's Disease Sleep Scale (PDSS), etc., were used as evaluation indicators. Conclusion: Herbal medicine is a potential treatment for sleep disorders associated with Parkinson's disease. However, the selected RCTs were of poor quality, and it is necessary to perform more systematic studies.

국내 학술지에 게재된 합곡혈 임상연구 동향 (Trends in Clinical Studies Including LI4 Acupoint Stimulation in Korean Journals)

  • Objectives : Hapgok (LI4) is one of the most widely used acupoints in clinical practice. This study aims to investigate the trend of clinical studies including LI4 acupoint stimulation in Korea. Methods : Papers of clinical researches including LI4 acupoint stimulation in Korean journals were searched in Korean databases of Korean Traditional Knowledge Portal, Korean Studies Information Service System, Korea Citation Index, DBpia, Research Information Sharing Service, and ScienceON. The papers were then analyzed focusing on the number of publications by year, the types of the clinical research, interventions, research topics, institutes, and journals. Results : A total of 154 publications were included in this study. The first paper was published in 1981, and the number of papers has gradually increased since the 1990s. Although LI4 acupoint stimulation was included, most of the papers had additional acupoints and interventions. Diseases of the nervous system were the most common, followed by diseases of the digestive system and diseases of the circulatory system. Kyung Hee University is the institution that published the most papers, and the journal that published the most papers is The Journal of Korean Oriental Internal Medicine. Conclusions : Since the 1990s, the number of clinical studies on LI4 acupoint has increased. However, the results of the studies are insufficient to prove the efficacy of LI4 acupoint because most of the studies were case reports using additional acupoints and interventions. Therefore, more rigorous studies are needed to prove the efficacy of LI4 acupoint.

Cardiovascular Magnetic Resonance Versus Histopathologic Study for Diagnosis of Benign and Malignant Cardiac Tumours: A Systematic Review and Meta-Analysis

  • BACKGROUND: The gold standard for diagnosis of cardiac tumours is histopathological examination. Cardiovascular magnetic resonance (CMR) is a valuable non-invasive, radiation-free tool for identifying and characterizing cardiac tumours. Our aim is to understand CMR diagnosis of cardiac tumours by distinguishing benign vs. malignant tumours compared to the gold standard. METHODS: A systematic search was performed in the PubMed, Web of Science, and Scopus databases up to December 2022, and the results were reviewed by 2 independent investigators. Studies reporting CMR diagnosis were included in a meta-analysis, and pooled measures were obtained. The risk of bias was assessed using the Quality Assessment Tools from the National Institutes of Health. RESULTS: A total of 2,321 results was obtained; 10 studies were eligible, including one identified by citation search. Eight studies were included in the meta-analysis, which presented a pooled sensitivity of 93% and specificity of 94%, a diagnostic odds ratio of 185, and an area under the curve of 0.98 for CMR diagnosis of benign vs. malignant tumours. Additionally, 4 studies evaluated whether CMR diagnosis of cardiac tumours matched specific histopathological subtypes, with 73.6% achieving the correct diagnosis. CONCLUSIONS: To the best of our knowledge, this is the first published systematic review on CMR diagnosis of cardiac tumours. Compared to histopathological results, the ability to discriminate benign from malignant tumours was good but not outstanding. However, significant heterogeneity may have had an impact on our findings.

뇌전증의 병인, 변증, 한약 치료에 대한 고찰 -한국, 일본, 중국 데이터베이스를 중심으로- (A Review of Etiology, Syndrome Differentiation, and Herbal Medicine of Epilepsy)

  • Objectives This study aimed to establish a foundation for research on Korean medicine for epilepsy by analyzing its etiology, syndrome differentiation, and the use of herbal medicines. Methods Articles were extracted from five databases in Korea, Japan, and China: the Oriental Medicine Advanced Searching Integrated System (OASIS), Citation Information by the National Institute of Informatics (CiNii), Chinese National Knowledge Infrastructure (CNKI), Wangfang Database, and Chinese Scientific Journal Database (VIP). The etiology, syndrome differentiation, and herbal medicines used for epilepsy were investigated. Results A total of 64 articles were selected, including seven clinical studies, 16 case reports, and 41 reviews. Epilepsy is a complex disease with various etiologies, and among the five viscera, syndrome differentiation mainly involved the liver, spleen, and kidney, with pathological factors that included phlegm, wind, and blood stasis. Frequently used herbal medicines included Modified Jeongganhwan, Sihogyejitang, Yukgunjatang, and Sihogayonggolmoryotang. Conclusions This study analyzed epilepsy's etiology, syndrome differentiation, and herbal treatments. Further evidence is needed to better understand the efficacy and safety of Korean herbal medicine for treating epilepsy.
