A Case Report on Osgood-Schlatter Disease Treatment Using Complex Korean Medicine Therapy Including Chuna Therapy (추나 요법을 포함한 복합 한방치료를 이용한 오스굿-슐라터 병의 치료 증례보고)

  • Objectives This case study aimed to investigate the effect of complex Korean medicine therapy and fascia Chuna therapy on Osgood-Schlatter disease. Methods Hospitalization treatment was performed on a 13-year-old Osgood-Schlatter patient who visited Woosuk University Korean Medicine Hospital. The patient complained of pain in both the tibial tuberosity and restriction on the range of knee joint movement. The results were evaluated using the visual analogue scale, knee injury and osteoarthritis outcome score, and range of motion. Results After treatment, knee pain decreased and range of movement angle improved. Conclusions This study suggests that complex Korean medicine therapy, including myofascial Chuna therapy, are effective in improving the range of motion and improving pain in early Osgood-Schlatter disease. The limitation of this study is that the subject of the study was limited to one case. Further clinical studies are required.

Chuna Manual Therapy for Functional Dyspepsia : A Systematic Review and Meta-analysis (기능성 소화불량증에 대한 추나요법의 효과 : 체계적 문헌고찰과 메타분석)

  • Objectives : To evaluate the evidence supporting the effectiveness of Chuna manual therapy for functional dyspepsia. Methods : We conducted search across the 3 electronic databases (Pubmed, CAJ and Oasis) to find all of randomized controlled clinical trials(RCTs) that used Chuna manual therapy as a treatment for functional dyspepsia. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. Results : Thirteen RCTs met our inclusion criteria. The meta-analysis showed positive results for the use of Chuna manual therapy combined with medication treatments in terms of the efficacy rate when compared to medication treatments alone. Positive results were also obtained, in terms of the efficacy rate, when comparing Chuna manual therapy combined with medication plus specific electromagnetic therapy to medication treatments alone. Conclusions : Our systematic review found encouraging but limited evidence of Chuna manual therapy for functional dyspepsia. However, to obtain stronger evidence without the drawbacks of trial design and the quality of studies, we recommend sham-controlled RCTs or comparative effectiveness research to test the effectiveness of Chuna manual therapy.

Systematic Review and Meta-analysis of Chuna Therapy for Sciatica (좌골신경통에 적용한 추나 치료에 대한 체계적 문헌 고찰 및 메타 분석)

  • This study aimed to evaluate the effects of Chuna therapy for Sciatica. We searched the following 16 online databases without a language restriction (Pubmed, Cochrane, Embase, CINAHL, Ovid, Kmbase, RISS, NDSL, OASIS, KISS, KNAL, KTKP, DBpia, CNKI, Wangfang, J-stage) to find randomized controlled clinical trials that used Chuna therapy for Sciatica. The methodological quality of randomized controlled clinical trials (RCTs) were assessed using the Cochrane risk of bias tool and meta-analysis were performed. Among 496 articles that were searched, 15 RCTs were finally selected for systematic review. 14 studies showed that Chuna therapy has positive effect on sciatica. Two studies noted that there were side effects, and the difference between the intervention group and the control group was statistically insignificant. One study noted no side effects and the rest of the study, there was no mention of side effects. Meta-analysis showed positive results for Chuna single therapy in terms of efficiency rate compared to painkiller, herb medicine excepting acupuncture. When comparing Chuna therapy plus acupuncture and acupuncture, Chuna therapy plus acupuncture had a more positive result than acupuncture in terms of efficiency rate. Cochrane Risk of Bias (RoB)evaluation method, most of the studies's selection, performance, detection and reporting bias were unclear. The studies showed that Chuna therapy can significantly effective on sciatica. However, most of the studies's Risk of Bias included in the analysis were not low enough. In the future, to prove the level of evidence of Chuna therapy, more high-quality studies will be needed.

  • Objectives : It is important to develop a new Chuna manual therapy as a representative of Chuna manual therapy technique at the time of KSCMM's (Korean Society of Chuna manual medicine) joining a $F{\acute{e}}d{\acute{e}}ration$ Internationale de $M{\acute{e}}decine$ Manuelle (FIMM). Therefore, Sunu manual therapy (SMT) will be introduced as a purely new Chuna manual therapy. Main subject : Sunu manual therapy (SMT) was discovered about two decade ago by a Korean Medicine doctor who wanted to develop a new manual therapy to treat a internal disease. It is very important to make the best use of four powers such as a compression force, a torsion force, a bending force, a tension force using a resonance principle and to experience a feeling of spine axis. SMT is different from Chiropractic technique and Osteopathy technique, in that SMT could treat so many disease using only one technique, but other manual therapy could utilize so many technique to treat only one disease. Discussion : Because SMT utilize a power of a moving in silience, it will take several years to totally master a SMT which will make a patient and a doctor experience a feeling of spine axis. As a feeling of spine axis is deeper, a nerve could be excited and treated by a SMT. Also as a feeling of spine axis is shallower, a muscle could be excied and treated by a SMT. Conclusions : SMT could be classed as a craniosacral chuna therapy or visceral chuna therapy to treat a internal disease considering SMT's action and purpose.

Effects of Chuna Manual Therapy after Hip Arthroplasty: A Systematic Review and Meta-Analysis of Randomized Controlled Trials (고관절 치환술 후 추나치료 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives This study was conducted to assess the effect of chuna manual therapy after hip arthroplasty. Methods We searched across 9 electronic databases (PubMed, Cochrane Library, Wangfang data, China National Knowledge Infrastructure [CNKI], Oriental Medicine Advanced Searching Integrated System [OASIS], National Digital Science Library [NDSL], Korean Medical Database [KMBASE], Koreanstudies Information Service System [KISS], Research Information Sharing Service [RISS]) to find randomized controlled clinical trials for chuna manual therapy after hip arthroplasty. Results On inclusion criteria, 11 appropriate studies were included and analyzed. The deep vein thrombosis incidence of the chuna manual therapy group was statistically lower than the conventional treatment group (p=0.0002). Chuna manual therapy significantly improved the Harris hip score compared with conventional treatment (p<0.00001). Also, chuna manual therapy combined with herbal fumigation therapy significantly elevated the hip joint function score of Harris hip score compared with the conventional therapy group (p<0.00001). Conclusions The systematic review showed that chuna manual therapy had significant effects on hip arthroplasty. Nonetheless, considering the high risk of bias and geographic bias, further research with well-designed studies is required to support the effectiveness of chuna manual therapy.

The methodological quality of each RCTs was assessed using the Cochrane Risk of Bias tool and nRCTs was assessed using the Cochrane Risk of Bias Assessment tool for Non-randomized Study tool. Results Two RCTs and three nRCTs met our inclusion criteria. The meta-analysis showed positive results for the use of Chuna manual therapy in terms of the efficacy rate when compared to medication treatments alone. Positive results were also obtained, in terms of the efficacy rate, when comparing Chuna manual therapy combined with acupoint application therapy to Chuna manual therapy alone, but was not statistically significant. Conclusions Our systematic review found encouraging but limited evidence of Chuna manual therapy for nocturnal crying. We recommend clinical trials which compare the effectiveness of Chuna manual therapy with usual care to obtain stronger evidence without the demerits of trial design.

Systematic Review of Chuna Manual Therapy for Cervicogenic Headache (경추성 두통에 대한 추나 요법의 체계적 문헌고찰)

  • Objective : The purpose of this study is to review the randomized clinical trials of Chuna manual therapy for cervicogenic headache and provide a evidence for the efficacy of Chuna manual therapy. Methods : We searched randomized clinical trials that performed Chuna manual therapy for cervical headache up to Feb. 2017 in 6 databases. Randomized clinical trials were selected according to the inclusion criteria and the data were extracted and analyzed. The risk of bias was assessed using the Cochrane Risk of Bias Criteria. Results : 16 RCTs met the inclusion criteria. The meta-analysis of 13 RCTs showed favorable results for the use of chuna manual therapy compared to drug, physical treatment. Conclusions : In 16 RCTs, we found that Chuna manual therapy was effective in cervicogenic headache. However, all RCTs are exposed to a number of bias risks. Therefore, well designed clinical trial would be needed to raise the evidence level of Chuna manual therapy.

  • Objectives: This study analyzes the electronic medical record (EMR) data of the spine specialist oriental hospital and clinic in various regions, and reports the actual number and used cases of Chuna therapy. Methods: 2,470,772 data was extracted retrospectively from electronic medical records of all inpatients and outpatients who were treated chuna therapy at 21 Korean medicine hospitals and clinics from January 1, 2018 to December 31, 2018. The characteristics of medical treatment using chuna therapy reflect the minimum, maximum and average values of the number of hospitalized patients, length of hospitalization, frequency of hospitalization, number of outpatients, frequency of treatment and frequency of visit. Diseases were classified in the proportion of Chuna treatment according to the KCD, 7th edition. The chuna and blindness charts were derived accordingly from illness and disease of each part of the body. Results: During the study period, a total 1,342,022 inpatients and outpatients visited the study sites. The male proportion was a little higher than the females' (male: 53.7%, female: 46.3%). According to age, the 30s and 40s were more than half the total(30s: 33.0% and 40s: 20.1%). Chuna therapy was treated to more outpatients than hospitalized patients (outpatient: 83.6%, hospitalization: 16.4%), and most treatments were related to musculoskeletal illness(99.06%). Conclusions: As a result of this study, 1,342,389 chuna therapy was performed in 21 hospitals for one year. As highly demanded by the public, we look forward to ensuring national health care options and medical access when health insurance for chuna therapy is applied beginning March 2019.

  • Objective : There is no case report on the Idiopathic Conjunctive treated by CHUNA manual therapy in oriental medicine. We experienced a patient with the Idiopathic Conhunctivitis improved by CHUNA manual therapy in X-ray imagine and visual analogue scale(VAS), and we report It. Methods : During the patient with the Idiopathic Conjunctivitis was treated CHUNA manual therapy, we examined cervial X-ray and evaluated the patient's symptoms by VAS and counting eye drop. And we observed whether the change exist or not in X-ray. Result : Symptoms(Red eye, eye xerosis) was improved significally, and the X -ray image was improved. Conclusion : Conjunctivitis was generally treated steroid eye drop and conservative treatment. But CHUNA manual therapy may be more effective in the Idiopathic Conjunctivitis, if more clinical studies and researchs are accomplished.

Chinese Medicine including Chuna Manual Therapy for Osteoarthritis : A Systematic Review (골관절염에 대한 추나요법을 포함한 중의학치료의 효과 : 체계적 문헌 고찰)

  • Objectives: To evaluate the evidence supporting the effectiveness and safety of Chinese medicine, including Chuna manual therapy, for the treatment ofosteoarthritis. Methods: We conducted a search of 8 electronic databases(Pubmed, Web of Science, EMBASE, Cochrane Library, CAJ, KISTI, NDSL, and RISS) to find all randomized controlled clinical trials that discuss Chuna manual therapy forosteoarthritis till May 2018. Results: Our inclusion criteria selected 7eligible RCTs involving 633 participants. The meta-analysis of 6 studies showed positive results for the useof Chinese medicine, including Chuna manual therapy, for treating knee osteoarthritis. There was no evidence of worsening symptoms or any adverse events. Conclusions: Our systematic review found favorable evidence regardingthe effectiveness of Chinese medicine, including Chuna manual therapy, for treating knee osteoarthritis. However, our systematic review had several limitations owingto the low quality of original articles andadditionalclinical trials would be necessary for stronger evidence.
