  • Chronic obstructive pulmonary disease (COPD) is characterized by persistent airflow limitation that is usually progressive. It is a major cause of morbidity and mortality worldwide, leading to substantial and increasing economic and social burden. Palliative care for COPD patients aims to reduce symptoms and exacerbations and improve exercise tolerance and quality of life. It is difficult to make a prognosis for COPD patients due to the variable illness trajectory and advanced care of patients. However, severity of breathlessness, assessment of lung function impairment, and frequency of exacerbations can help to identify palliative care needs and determine effective methods to mitigate symptoms, which is discussed in this paper. In these patients, it is recommended to provide individualized palliative care along with curative/restorative care at the onset of COPD symptoms. Before launching a palliative care system in Korea, it is necessary to prepare pulmonary rehabilitation resources, patient-centered communication, timely palliative responsiveness, and a program for effective advanced care planning. A multidisciplinary approach involving collaboration with not only the respiratory and palliative care teams but also primary care offers a new model of care for these patients and should be considered with a priority.

  • PURPOSE: To solve the problems of chronic obstructive pulmonary disease (COPD) patients complaining of muscle fatigue and having limited motor abilities, this study provided the clinical basic data for pulmonary rehabilitation by examining the effects of High Low-Frequency Neuromuscular Electrical Stimulation (NMES) on the respiratory muscle strength, quadriceps muscle activity, and life quality. METHODS: Samples were collected from 20 COPD patients and placed randomly in a low-frequency group (n=10) and high-frequency group (n=10). For a pretest, the respiratory muscle strength (MIP, MEP), quadriceps muscle activity (LF, VM, VL), and life quality (SGRQ) were measured. After applying NMES to each group for 30 minutes at a time, five times a week, for four weeks, a posttest was conducted in the same way as the pretest. RESULTS: Both groups showed significant differences in the respiratory muscle strength, quadriceps muscle activity, and quality of life within the groups and there were significant differences in the quadriceps muscle activity between the groups. CONCLUSION: The four-weeks NMES positively influenced the respiratory muscle strength and life quality by enhancing the function of the quadriceps and exercise tolerance. In particular, high-frequency(75 Hz) NMES was more effective than low-frequency (15 Hz) NMES. This result can be an alternative means to improve the physical functions of COPD patients clinically in the future.

  • Purpose: This study is not a fragmentary study on characteristics of respiratory synergist when breathing, however it was intended to determine the effect of currently available respiratory exercise and to provide basic clinical information through investigation of oxygen demand and respiratory synergist that mobilizes for respiration during application of respiratory exercise. Methods: Experimental group I was selected from second grade of severity classification of GOLD, which has the highest percentage among patients with COPD, and experimental group II was selected from third grade of severity classification as a clinical sampling. After respiration pursing up lips and diaphragm respiration exercise were mediated together for six weeks, activity of respiratory muscles and oxygen saturation were measured and analyzed. Results: In comparison of change of respiratory synergist and oxygen saturation, activity of respiratory synergist in sternocleidomastoid muscle and scalene muscle showed a meaningful decrease in experimental group I. And, in comparison of change of respiratory synergist and oxygen saturation, activity of respiratory synergist in rectus abdominis muscle showed a meaningful increase in experimental group II. In comparison of change of respiratory synergist and oxygen saturation, activity of respiratory synergist in sternocleidomastoid muscle, scalene muscle, and rectus abdominis muscle showed a meaningful difference between experimental groups. Conclusion: Respiratory synergists work mainly as agonist of chest and upper limbs. Therefore it is very important to lower mobilization of respiratory synergist when breathing. It is considered that a multilateral approach and continued clinical research for improvement of respiratory function for patients with COPD will be needed in the future.

  • Background : The fact that only 10-20% of chronic cigarette smokers develop chronic obstructive pulmonary disease (COPD) reflects the presence of genetic factors associated with the susceptibility to COPD. Recently, it was reported that the surfactant protein A increases the secretion of matrix metalloprotease 9, which degrades extracellular matrices of the lung, through a Toll-like receptor 2 (TLR2). In this context, possible role of TLR2 in the pathogenesis of COPD was postulated, and a functional dinucleotide repeat polymorphism in intron II of TLR2 was evaluated for any association with COPD. Method : Male patients with COPD and male smokers with a normal pulmonary function were enrolled in this study. The number of Guanine-Thymine repeats in intron II of the TLR2 gene were counted. Because the distributions of the repeats were trimodal, the alleles were classified into three subclasses, 12-16 repeats: short (S) alleles; 17-22 repeats: medium length (M) alleles; and 23-27 repeats: long (L) alleles. Result : 125 male patients with COPD and 144 age- and gender-matched blood donors with a normal lung function were enrolled. There were no differences in the distribution of each allele subclass (S, M and L) between the COPD and control group (p=0.75). The frequencies of the genotypes with and without each allele subclass in the COPD and control group were similar. Conclusion : A microsatellite polymorphism in intron II of TLR2 gene was not associated with the development of COPD in Koreans.

  • Objective: Farmers are known to have high prevalence of asthma and chronic obstructive pulmonary disease(COPD). This study aims to investigate the current status of asthma and COPD in domestic and foreign farmers with the ultimate goal of raising awareness of asthma and COPD among farmers and contribute to supporting evidence for the prevention of respiratory diseases in farmers. Methods: The study utilized data from the "Farmer's Occupational Disease Survey," conducted by the Rural Development Administration, to determine the prevalence of respiratory diseases in domestic farmers. The prevalence of asthma and COPD in overseas farmers was evaluated by researching other foreign studies and articles. Results: The prevalence of asthma and COPD in domestic farmers was less than of foreign farmers'. The prevalence of asthma in domestic farmers was high as upto 8.4% and COPD upto 5.5%. Studies from Europe and America showed prevalence of asthma in farmers high as upto 14.8% and COPD upto 17.1%. Conclusions: This study conducted an investigation using the 'Survey on Occupational Diseases and Injuries in Farmers to understand the current status of asthma and COPD in domestic farmers. Due to a lack of relevant domestic research, we examined and compared with the research results on asthma and COPD among overseas farmers. Further research is necessary and preventive measures for respiratory diseases need to be developed.

  • Objectives : This study aimed to evaluate the effects of Saengmaekcheongpye-eum (SCE) on a LPS-induced COPD (chronic obstructive pulmonary disease) model. Materials and Methods : The extract of SCE was treated to A549 cells and and LPS-induced COPD mouse model. Then, various parameters such as cell-based cyto-protective activity and histopathological finding were analyzed. Results : SCE showed a protective effect on LPS-induced cytotoxicity in A549 cells. This effect was correlated with analysis for caspase 3 levels, elastin contents, protein levels of cyclin B1, Cdc2, and phospho-Erk1/2, and gene expression of TNF-${\alpha}$ and IL-$1{\beta}$ in A549 cells. SCE treatment also revealed a protective effect on LPS-induced lung injury in COPD mouse model. This effect was evidenced via histopathological findings including immunofluorescence stains against elastin and caspase 3, and protein levels of cyclin B1, Cdc2, and Erk1/2 in lung tissue. Conclusions : These data suggest that SCE has pharmaceutical properties on lung injury. This study thus provides scientific evidence for the efficacy of SCE for clinical application to patients with COPD.

  • Background: Although physical activity is known to be beneficial to lung function, few studies have been conducted to investigate the correlation between physical activity and lung function in dusty areas. Therefore, the purpose of this study is to investigate the correlation between physical activity and lung function in a Korean cohort including normal and COPD-diagnosed participants. Methods: Data obtained from the COPD in dusty areas (CODA) cohort was analyzed for the following factors: lung function, symptoms, and information about physical activity. Information on physical activity was valuated using questionnaires, and participants were categorized into two groups: active and inactive. The evaluation of the mean lung function, modified Medical Research Council dyspnea grade scores, and COPD assessment test scores was done based on the participant physical activity using a general linear model after adjusting for age, sex, smoking status, pack-years, height, and weight. In addition, a stratification analysis was performed based on the smoking status and COPD. Results: Physical activity had a correlation with high forced expiratory volume in 1 second ($FEV_1$) among CODA cohort (p=0.03). While the active group exhibited significantly higher $FEV_1$ compared to one exhibited by the inactive group among past smokers (p=0.02), no such correlation existed among current smokers. There was no significant difference observed in lung function after it was stratified by COPD. Conclusion: This study established a positive correlation between regular physical activity in dusty areas and lung function in participants.

  • This study has been conducted by a systematic review of literature and a meta-analysis in order to identify the effect of water based exercise training for COPD(Chronic obstructive pulmonary disease). Two researchers selected relevant literature, and extracted the date and assessed the quality of literature. A total of 5 studies met the inclusion criteria, and the outcome of methodological quality of the literature was not positive. According to the result of meta-analysis of water based exercise and usual care, it was considerably effective in 6 MWT and quality of life. This study introduces various types of water based exercise for COPD patients, which is considered useful for application of the program. However, it experiences difficulty to generalize due to lack of the number of literature in relation to the water based excercise.

  • Background: Chronic airway diseases, such as asthma and chronic obstructive pulmonary disease (COPD), are increasingly being treated with inhaled corticosteroid (ICS). However, ICSs carry potential infection risks, particularly nontuberculous mycobacteria (NTM). This study investigated the association between ICS use and NTM infection risk using national insurance data, particularly for individuals with chronic airway diseases. Methods: We conducted a nationwide population-based study using data from the National Health Insurance Service-National Sample Cohort in South Korea from 2002 to 2019. The cohort included 57,553 patients diagnosed with COPD or asthma. To assess the risk of NTM infection, we used Cox proportional hazards models and propensity score-based inverse probability of treatment weighting (IPTW) to ensure a balanced analysis of covariates. Results: Of the 57,553 patients (mean age 56.0 years, 43.2% male), 16.5% used ICS and 83.5% did not. We identified 63 NTM infection cases, including nine among ICS users and 54 among non-users. Before and after IPTW, ICS use was associated with a higher risk of NTM infection (adjusted hazard ratio [HR], 4.01; 95% confidence interval [CI], 1.48 to 15.58). Higher risks were significant for patients ≥65 years (adjusted HR, 6.40; 95% CI, 1.28 to 31.94), females (adjusted HR, 10.91; 95% CI, 2.24 to 53.20), never-smokers (adjusted HR, 6.31; 95% CI, 1.49 to 26.64), systemic steroid users (adjusted HR, 50.19; 95% CI, 8.07 to 312.19), and those with higher comorbidity scores (adjusted HR, 6.64; 95% CI, 1.19 to 37.03). Conclusion: ICS use in patients with chronic airway diseases might increase the risk of NTM infection, particularly in older females, never-smokers, and systemic steroid users.
