  • Purpose: This study aimed to identify the effects of the mother-medical staff partnership on mothers' condition management ability for children with chronic allergic diseases. Methods: A total of 109 Korean mothers caring for a child with a chronic allergic diseases, identified according to the allergic march, such as food allergy, atopic dermatitis, allergic rhinitis, and allergic asthma, were recruited from the pediatric department of a general hospital in Seoul through convenience sampling. Data were collected using structured self-reported questionnaires from August 1, 2017 to October 31, 2017. and analyzed by descriptive statistics and multiple regression using SPSS version 22.0. Results: The Mother-medical staff partnership had a statistically significant effect on mothers' condition management ability for children with chronic allergic diseases (p<.05). Among the general characteristics, satisfaction with nursing services had a statistically significant effect on mothers' condition management ability (p<.05). Conclusion: In conclusion, strategies to support children with chronic allergic diseases, as well as their caregivers, should consider the mother-medical staff partnership as part of a family-centered approach.

  • Chronic pelvic pain in women is a very annoying condition that is responsible for substantial suffering and medical expense. But dealing with this pain can be tough, because there are numerous possible causes for the pelvic pain such as urologic, gynecologic, gastrointestinal, neurologic, or musculoskeletal problems. Of these, musculoskeletal problem may be a primary cause of chronic pelvic pain in patients with a preceding trauma to the low back, pelvis, or lower extremities. Here, we report the case of a 54-year-old female patient with severe chronic pelvic pain after a transcutaneous electrical nerve stimulation (TENS) accident that was successfully managed with image-guided trigger point injections on several pelvic stabilizing muscles.

  • Chronic eosinophilic pneumonia is an idiopathic condition characterized by chronic infiltration of the lung with eosinophils, weight loss, dyspnea, and pulmonary infiltration. Recently, we have experienced one case of chronic eosinophilic pneumonia presenting as the acute respiratory failure. A 34-year-old man was admitted to the hospital with one month's history of coughing, dyspnea, mucoid sputum, weight loss and one day's history of subcutaneous emphysema. The radiographic lung lesion and dyspnea rapidly progressed to a critical condition of acute respiratory failure. The combination of blood eosinophilia, lung infiltration on the chest x-ray, sign of acute rapiratory failure, rapid response to steroid therapy, and the finding of the transbronchial lung biopsy permit the diagnosis of chronic eosinophilic pneumonia.

Colorectal carcinoma and chronic inflammatory demyelinating polyneuropathy: is there a possible paraneoplastic association?

  • A plethora of paraneoplastic syndromes have been reported as remote effects of colorectal carcinoma (CRC). However, there is a dearth of data pertaining to the association of this cancer with demyelinating neuropathies. Herein, we describe the case of a young woman diagnosed with chronic inflammatory demyelinating polyneuropathy (CIDP). Treatment with intravenous immunoglobulins and prednisone did not improve her condition, and her neurological symptoms worsened. Subsequently, she was readmitted with exertional dyspnea, lightheadedness, malaise, and black stools. Colonoscopy revealed a necrotic mass in the ascending colon, which directly invaded the second part of the duodenum. Pathologic results confirmed the diagnosis of locally advanced CRC. Upon surgical resection of the cancer, her CIDP showed dramatic resolution without any additional therapy. Patients with CRC may develop CIDP as a type of paraneoplastic syndrome. Clinicians should remain cognizant of this potential association, as it is of paramount importance for the necessary holistic clinical management.

  • Chronic cough is a common problem that can be refractory to medical treatment. Nonpharmaceutical management of chronic cough has an important role in well selected patients. This review article outlines the history of chronic cough management, current approaches to speech pathology management of the condition and new modalities of nonpharmaceutical treatment. There is a need for further research into nonpharmaceutical options with well described randomised control trials.

Posterior reversible encephalopathy syndrome caused by presumed Takayasu arteritis

  • Takayasu arteritis (TA) is a chronic inflammatory disease of unknown etiology that affects mainly the aorta, main aortic branches, and pulmonary arteries. Diverse neurological manifestations of TA have rarely been reported in children. Posterior reversible encephalopathy syndrome (PRES) is a neuroradiological condition that presents with headache, seizure, visual disturbances, and characteristic lesions on imaging. Inflammatory condition and severe hypertension in TA can cause PRES. We report of a 5-year-old girl with presumed TA who presented with PRES and chronic total occlusion in the renal artery. The findings on magnetic resonance imaging suggested PRES. Left nephrectomy was performed for total occlusion of the left renal artery, and the confirmatory diagnosis of TA was based on the pathologic findings of the renal artery.

노년기 의료서비스 이용이 삶의 만족도에 미치는 영향: 만성질환 유무의 조절효과를 중심으로 (The Effects of Medical Service Utilizations on Life Satisfaction among the Elderly: Focusing on the Moderating Effects of the Presence of Chronic Illnesses)

  • 현재 우리나라가 경험하고 있는 급속한 인구고령화 현상은 노년층 사이의 만성질환 및 의료서비스 이용의 증가로 연결된다. 의료서비스 이용은 궁극적으로 서비스 소비자들의 삶의 만족도에 영향을 준다. 하지만 노년기의 의료서비스 이용이 삶의 만족도에 미치는 영향이나 만성질환의 유무에 따라서 의료서비스 이용이 삶의 만족도에 미치는 영향이 상이한지에 대해서는 알려진 바가 많지 않다. 본 연구는 노년기 의료서비스 이용이 삶의 만족도에 미치는 영향에 대해서 살펴보고, 의료서비스 이용이 삶의 만족도에 미치는 영향이 만성질환 유무에 따라서 상이한지를 살펴보고자 한다. 이러한 목적을 달성하기 위해서 한국복지패널 3차년도 조사에 참가한 65세 이상 노인 3,944명을 대상으로 구조방정식(Structural Equation Modeling)과 다중집단 구조방정식분석(Multi-Group SEM)을 하였다. 관련변수들을 통제한 상태에서 주요결과는 다음과 같다. 노인 전체적으로는 외래서비스 이용이 많을수록 삶의 만족도 수준은 낮고, 입원일수가 많을수록 삶의 만족도 수준이 높은 반면, 건강검진 서비스 이용수준은 만족도와 유의한 관련성이 없었다. 하지만 의료서비스가 만족도에 미치는 영향은 만성질환의 유무에 따라서 상이하였다. 만성질환이 있는 사람들은 입원서비스 이용 수준이 삶의 만족도에 영향을 주지 않는 반면, 없는 사람들은 입원일수가 많을수록 삶의 만족도가 높았다. 질병이 있는 사람들은 건강 검진서비스 수준이 삶의 만족도에 영향을 미치지 않는 반면, 질병이 없는 사람들은 건강검진 서비스를 많이 받은 사람들의 만족도 수준이 낮았다. 외래서비스 이용이 만족도에 미치는 영향은 질명유무에 따라서 차이가 없었다. 이러한 결과는 노년기 의료서비스 이용이 삶의 만족도에 영향을 준다는 것과 의료서비스 이용이 만족도에 주는 영향이 만성질환이 있는 노인들과 없는 노인들 사이에 차이가 있을 수 있다는 점을 보여준다. 연구 결과를 토대로 이론적 실천적 함의에 대해서 논의하고 향후 연구의 방향에 대해서 제안하였다.

Effects of elastic band orthosis (aider) on balance and gait in chronic stroke patients

  • Objective: The objective of this study was to investigate the feasibility and effects of balance training using a newly developed elastic band orthosis (aider) for improvement of mobility and balance in chronic stroke patients. Design: Cross-sectional study. Methods: Ten patients with chronic hemiplegia participated in this study. There were six males and four females; two patients had right hemiplegia and eight had left hemiplegia. This study investigated the effect of the elastic band orthosis on balance and gait ability compared with bare foot condition. Gait parameters were measured using the opto-gait system for analysis of the spatial and temporal parameters of walking in stroke patients. In addition, balance ability in stroke patients was evaluated using the Timed Up and Go (TUG) and Berg Balance Scale (BBS). Results: This study investigated the effect of the elastic band orthosis on balance compared with bare foot condition. The TUG and BBS showed significant improvement with use of the elastic band orthosis (p<0.05). Use of the Elastic band orthosis resulted in significantly improved velocity, cadence, less-affected step length, less-affected stride length, and less-affected single limb support in stroke patients (p<0.05). Conclusions: We demonstrated a significant improvement in dynamic balance and gait ability in chronic stroke patients using the elastic band orthosis. This orthosis may aid in prevention of spastic foot drop, leading to improvement of walking ability.

황련아교탕(黃連阿膠湯) 투여로 호전된 만성 동전 모양 습진 1례 (A Case of Chronic Nummular Dermatitis Improved by Hwangryunagyo-tang)

  • Objectives: This study aims to report a case of chronic nummular dermatitis improved by Hwangryunagyo-tang. Methods: The patient in this study was a 15-year-old female, who visited our hospital with symptoms such as redness, erythema, papule, excoriation, scale, exudation, crusting, lichenification, and pruritus of the extremities and trunk. Considering that the skin lesions were coin-shaped and persisted for more than 6 months, the patient's symptoms were diagnosed as chronic nummular eczema. Based on the patient's symptoms and overall condition, Hwangryunagyo-tang was prescribed. Results: Approximately 4 months after administration of Hwangryunagyo-tang, the exudate disappeared, and overall skin symptoms improved, from objective score 33.7 to 6.6, leaving only slight scale, dryness, and pigmentation. As the pruritus of the skin lesion decreased from numeric rating scale 8 to 1, the patient's sleeping condition also improved from numeric rating scale 9 to 0. Conclusion: This study is significant in that the symptoms of chronic nummular dermatitis were effectively improved after administration of Hwangryunagyo-tang.

Successful Treatment of Abdominal Cutaneous Entrapment Syndrome Using Ultrasound Guided Injection

  • There are various origins for chronic abdominal pain. About 10-30% of patients with chronic abdominal pain have abdominal wall pain. Unfortunately, abdominal wall pain is not thought to be the first origin of chronic abdominal pain; therefore, patients usually undergo extensive examinations, including diagnostic laparoscopic surgery. Entrapment of abdominal cutaneous nerves at the muscular foramen of the rectus abdominis is a rare cause of abdominal wall pain. If abdominal wall pain is considered in earlier stage of chronic abdominal pain, unnecessary invasive procedures are not required and patients will reach symptom free condition as soon as the diagnosis is made. Here, we report a case of successful treatment of a patient with abdominal cutaneous nerve entrapment syndrome by ultrasound guided injection therapy.
