A Study on Local Irritation Test of Bee Venom for Herb-Acupuncture (약침용(藥鍼用) 봉독액(蜂毒液)의 국소독성시험(局所毒性試驗)에 관(關)한 연구(硏究))

  • Pursuant to the Medical Product Safety Administration Guidelines for safety assessment of Korean bee vonom for herb-acupuncture, rabbits were used for skin, eye balls, subcutaneous and muscle irritation test. The results were as follows; 1. The skin irritation test of bee venom for herb-acupuncture did not produce any irritation reactions, when the skin was covered with bee venom. 2. The eye irritation test produced moderate to severe stimulating reactions, once the eye mucous membrane was contacted with bee venom. 3. The subcutaneous and muscle irritation test of bee venom showed such local inflammatory reactions as death of cells, infiltration of inflamed cells, dropsical swelling, and congestion, once injected under the skin and in the muscle. As the results of the study on bee venom for herb-acupuncture indicate that severe reactions in such tests as eye balls, subcutancous and muscle irritation exist in the laboratory, more professional caution should be taken in clinical application of the therapy. In addition, further study on subacute, chronic toxicity and allergy reactions should be pursued.

Tracheogastric Fistula : A Rare Complication of Total Laryngopharyngoesophagectomy with Gastric Pull-up Procedure (기관-위 샛길 : 전인두후두식도 절제술후 위를 이용한 재건 후 발생한 드문 합병증 1예)

  • Tracheo-gastric fistula is a very rare late complication of total laryngopharyngoesophagectomy with gastric pull-up procedure. It usually occurs after transhiatal esophagectomy, but it has only rarely been reported after total laryngopharyngoesophagectomy with gastric pull-up procedure. Chronic irritation and gastric ulcer may be the cause of tracheogastric fistula. To prevent fistula formation, active management of gastric ulcer and avoidance of mucosal irritation are necessary. We report a case of a tracheogastric fistula which occurred two years after total laryngopharyngoesophagectomy with gastric pull-up procedure that maybe occurred by chronic irritation and gastric ulcer.

Rare Giant Upper Lip Epidermal Cyst in a Patient Wearing a Denture

  • Epidermal cysts are intradermal or subcutaneous cystic tumors that frequently occur in the face, scalp, neck, and body trunk. Acquired cases of epidermal cyst commonly occur as a result of various surgical operations, chronic irritation, or trauma, all of which may trigger the occurrence of the invagination of squamous epithelium. A 57-year-old man presented with a palpable mass $7cm{\times}2cm$ in size in the upper lip. The patient had a 3-year history of wearing a denture to restore missing bilateral maxillary central and lateral incisors, accompanied by inflammatory findings on the buccal mucosa due to chronic lip irritation. The resected oval-shaped cyst had a size of $5.5cm{\times}3.0cm{\times}2.5cm$, and it was an encapsulated mass with a well-defined margin. The histopathology was typical of epidermal cyst. This case of a rare giant upper lip epidermal cyst in a patient wearing a denture may be of interest to clinicians.

Reactive Hyperplasia of Intraoral Soft Tissue by Mechanical Irritation (기계적 자극에 의한 구강내 반응성 증식)

  • Intraoral soft tissue can easily be injured by weak mechanical irritation. Each symptom by irritation is various, but most of patients show chronic inflammatory lesion. The fibroma is the most frequent disease found in intraoral area followed by pyogenic granuloma, epulis fissuratum, palatal papillomatosis, and epulis granulomatosum. The inflammatory hyperplasia by mechnical irritation is easily different from other disease, but this shows similar to several benign and malignant tumors required differential diagnosis. By microscopic feature, the lesions is divided by granulatory stage, mixed stage, and fibrotic tissue stage. The inflammatory hyperplasia is differently treated by each clinical features.: only removal of cause or, and excisional biopsy is/are required. This is the cases report of reactive hyperplasia of intraoral soft tissue by mechanical irritation that suggests various treatments of each cases.

Histopathological Analysis of Irritation Fibroma Occurred in Young Male Gingiva: A Case Report

  • Irritation fibroma is a reactive hyperplastic lesion caused by chronic stimuli with low intensity in the oral cavity. Irritation fibroma is common in middle-aged females but it may also occur at any age and sex. Clinical characteristics of irritation fibroma are similar to other reactive lesions or benign tumors, therefore, histological examination is essential to make an accurate diagnosis. This case report presents two cases of irritation fibroma occurred on the gingiva in young males. Two male patients in their 20s and 30s of age visited the clinic for the evaluation and treatment of painless gingival overgrowth in the anterior region. Clinically, the lesions were well-defined and firm, with similar color and texture to the adjacent normal gingiva. Excisional biopsy under local anesthesia was conducted with a scalpel, and the lesions were completely removed. Histopathologically, connective tissue consisting of dense collagen bundles, proliferation of fibroblasts and minor infiltrated inflammatory cells were observed. Based on the clinical and histopathological findings, the diagnosis of irritation fibroma was confirmed in both cases.

Survey on Effect and Satisfaction of Korean Medicine Distillate Nasal Spray of Patient with Chronic Rhinitis (만성 비염 환자의 한방코분무제에 대한 효과 및 만족도 조사)

  • Objectives : This study was performed in order to investigate an immediate improvement effect of nasal obstruction after having patient with chronic rhinitis use nasal spray in nasal cavity after distilling Korean medicine, satisfaction for its fragrance and irritation and its effect continuance time. Methods : A questionnaire was performed for an immediate therapeutic effect of nasal spray 1, 2 and 3 weeks later after using spray by targeting 13 patients who used Korean medicine distillate spray for over 3 weeks among the patients who visited Sangji University Korean Medicine Hospital and whose symptoms were determined as chronic rhinitis and for whom Korean medicine distillate spray was prescribed. 3 weeks later after using spray, a questionnaire was performed for fragrance, irritation and continuance effect of Korean medicine. Immediate therapeutic effect being felt right after spraying was also surveyed and symptom scores was expressed from 10 to 0 and on the assumption that nasal obstruction symptom just before spraying was 10 points, symptom score after spraying was surveyed. In case of evaluation of satisfaction, satisfaction for fragrance and irritation of nasal spray was evaluated by scale based on full score of 5 point from 'very satisfied=5', 'satisfied=4', 'average=3', 'unsatisfactory=2' and 'very unsatisfactory=1', respectively. Effect continuance time after spraying was surveyed by dividing it into within 5 minutes, within 5-10 minutes, within 10-30 minutes and over 30 minutes. All the data was statistically analyzed by Friedman Test that is non-parametric test of Paired t-test and descriptive statistics. Results : Total surveyed patients were 13 persons including 6 males and 7 females and total average age was represented to be $27.1{\pm}17.7$ years old. Score of immediate nasal obstruction improvement effect at 1st week was $6.3{\pm}2.4$ points, at 2nd week $5.7{\pm}2.5$ points and at 3rd week $5.5{\pm}2.5$ points and when comparing 1st week, 2nd week and 3rd week statistically, p value was 0.047<0.05 and from this, it could be seen that the longer spray was used, the more was improvement effect represented with slight significance. In case of fragrance and irritation, both of its score was represented to be high as average $4.0{\pm}1.08 $points and average $4.4{\pm}0.8$ points, respectively. Effect continuance time below 10 minutes was represented in 8 persons who accounted for 61.5%. Conclusions: Immediate improvement effect of nasal obstruction being felt by the patients by using Korean medicine nasal spray could be confirmed and by having the patients use such spray for 3 weeks continuously, it could be realized that such effect was sustained and satisfaction for fragrance or irritation of Korean medicine nasal spray was significant. However, rather short sustained time was considered to be unsatisfactory and this shortcomings is required to be improved in the future.

Expression of TGF-β1 and EGFR in Irritation Fibroma and Oral Leukoplakia (면역조직화학염색법을 이용한 자극성 섬유종과 구강 백반증에서의 TGF-β1과 EGFR 발현 비교 연구)

  • Irritation fibroma (IF) is the most common tumor-like oral lesion that is evolved by proliferation of collagen in response to chronic irritation. Oral leukoplakia (OL) is considered as precancerous lesion characterized by proliferation of epithelial cells due to chronic irritation, smoking and drinking. TGF-${\beta}1$ and EGFR are important factors that play an essential role in extracellular matrix remodeling during normal wound healing process. The epithelial reaction by chronic irritation may be connected with pathogenesis of IF and OL. In the present study, we examined the expression of TGF-${\beta}1$ and EGFR in the IF and OL using immunohistochemistry. We used 88 cases of IF, 44 cases of OL and 9 cases of normal oral mucosa as normal control. TGF-${\beta}1$ was decreased in the epithelium of IF and OL. As for EGFR, the epithelial cells revealed the increased positive expression in IF and OL. In case of OL, the Spearman correlation coefficient of TGF-${\beta}1$ and EGFR was -0.10 (p< 0.05), which showed weak correlation. In the fibrous tissue, TGF-${\beta}1$ was increased only in IF. The expression difference of TGF-${\beta}1$ and EGFR may be involved in the pathogenesis of IF and OL.

Irritation Fibroma Associated with Ectopic Eruption of the Maxillary Incisor (상악 중절치의 이소맹출로 인해 발생한 자극성 섬유종)

  • Irritation fibroma is a common hyperplastic lesion of the oral mucosa that can occur in response to chronic irritation or trauma. This report presents an unusual case of irritation fibroma associated with ectopic eruption of the maxillary left central incisor in a patient with Angelman syndrome. Considering the patient's medical history and cooperative ability, excisional biopsy under intramuscular sedation using midazolam was performed. The patient exhibited successful healing without lesion recurrence. It is important to perform histopathological examination followed by excisional biopsy, because other benign or malignant tumors may mimic the clinical appearance of an irritation fibroma.

Evaluation of Acute Toxicity of the Herbicide Methiozolin (제초제 Methiozolin의 급성독성평가)

  • We investigated methiozolin acute toxicity using with Sprague-Dawley rats. The results of acute oral toxicity using rats showed $LD_{50}$ value of over 2,000 mg/kg bw for methiozolin. The calculate acute dermal $LD_{50}$ value of methiozolin was over 4,000 mg/kg. The skin irritation test showed moderately irritation and weak response of eye irritation test was observed in this experimental condition. According to these results, We concluded that methiozolin was Category IV in GHS chemical classification for acute toxicity. Future, we need more chronic toxicity test for safety.

Results of Modified Chrisman-Snook Procedure for Chronic Ankle Instability (변형 Chrisman-Snook 술식을 이용한 만성 족관절 불안정성의 치료 결과)

  • Purpose: We try to retrospectively analyze the clinical results of the modified Chrisman-Snook procedure for chronic ankle instability. Materials and Methods: From November 1997 to April 2006, thirty-one patients who underwent modified Chrisman-Snook procedure for chronic ankle instability were analyzed. All patients were male and the mean age was 31 years. The follow-up period averaged 48 months. We evaluated the clinical results measured by Hasegawa method. Results: Among them, there were soldiers in 11, socker players in 6, patients who weigh more 80 kg in 5. And there were 9 patients who previously underwent modified Brostrom procedure for chronic ankle instability. The clinical results were rated as excellent in 29, fair in 2 who did not cooperate with postoperative rehabilitation program. There were complications of 2 cases of irritation of the sural nerve and recurrence respectively, 1 case of wound problem. Conclusion: Our results show that the modified Chrisman-Snook procedure is effective treatment method for patients with high-performance athlete/soldier or failed modified Brostrom procedure.
