Synergistic Inhibition of Aronia melanocarpa and Moringa oleifera Seed Extract on Experimental Atopic Dermatitis (아로니아 및 모링가 종자 복합물의 항아토피 상승효과)

  • Atopic dermatitis is a chronic, relapsing inflammatory skin disease. This study aimed to investigate the therapeutic benefits of Aronia melanocarpa (AM) and Moringa oleifera seed extract (MO) on experimental atopic dermatitis. We examined the effects of AM or MO and their combination on 2,4-dinitrochlorobenzene (DNCB)-induced atopic dermatitis in BALB/c mice as well as tumor necrosis factor $(TNF)-{\alpha}$ and interferon $(IFN)-{\gamma}-stimulated$ HaCaT keratinocytes. Mice were orally treated with extract during repeated application of DNCB to shaved dorsal skin. Our results show that treatment with AM and MO in combination reduced histological manifestations such as epidermal hyperplasia and inflammatory cell infiltration. Furthermore, it significantly decreased skin thickness and serum immunoglobulin E (IgE) level compared to the AM or MO alone treated group. Combined extract of AM and MO suppressed expression of $TNF-{\alpha}/IFN-{\gamma}-induced$ T helper 2 (Th2) chemokines such as thymus and activation-regulated chemokine and macrophage-derived chemokine. To sum up, combination of AM and MO suppressed the inflammatory response and serum IgE as an indicator of several allergic diseases in DNCB-induced experimental atopic dermatitis and Th2 chemokine expression in HaCaT cells. This result suggests that combination of AM and MO could be a valuable strategy to improve atopic dermatitis.

  • Atopic dermatitis, also called congenital fever, is a allergic eczema of chronic itching disease. It is a recurrent and familial disease and appears on a wide age group from infant to adult. It is very common, and the ratio of occurrence is about $9{\~}l2\%$ of a child. However. it is showing trend of continuous increase by social and natural environment, food culture, and life style, recently. The human skin plays a barrier role against a physical and chemical stimulus from external environment. According to the latest study, the decreased amount of ceramide in horny layer impairs the bier function and moisture-maintaining function of skin in atopic dematitis patient. Ceramide is a kind of the sphingolipid in which a fatty acid is connected to sphingosin. Ceramide constitutes about $40\%$ of total lipid between keratinocytes and has the function of defense wall and building regular structure to suppress moisture vaporization in horny layer. In horny layer of skin a comified cell is composed of multi-layer structure of a brick shape, and, as for this cornified cell, it is strongly connected by ceramide, cholesterol, and free fatty acid. Here, we described the effects of a cream containing ceramide on the recovery of skin harrier function of atopic dermatitis patient. The safety and efficacy of latex and liquid formula were evaluated as cosmetics for atopic dermatitis. The latex products was composed of intercellular lipid components-ceramide, cholesterol, and free fatty acid-to restore skin barrier function in atopic dermatitis patients. The liquid one contained beta-glucan, magnolia extracts, and licolice extracts, which have skin immunomodulatory and anti-inflammatory effects. It is also confirmed that their possibility on new cosmetic market of atopic dermatitis.

A Clinical Study on the Patients with Nasal Polyps (비용(鼻茸)환자에 대한 임상적 연구)

  • Objectives : The purpose of this clinical report was to investigate the general characteristics of the patients with nasal polyps and estimate the efficacy of the oriental medical treatment on nasal polyps. Methods : We performed a clinical analysis of 50 patients who visited Kabsan oriental clinic due to nasal polyps. We inquired into distribution of age, sex, operation history related to nose, grade of nasal polyps, efficacy of treatment, etc. Results and Conclusions : 1. The sex ratio was 2.3 : 1. The average age was 40.02 years. 2. In the nasal operation history, inexperienced patients were 35 cases and experienced patients were 15 cases. 3. In the region of nasal polyps, there were 35 cases of bilateral types of nasal polyps, which is mon than in unilateral types. In the number of nasal polyps, multicentric types of 26 cases were similar in the number with solitary type of 24 cases. 4. In the predisposing or concomitant diseases, chronic sinusitis was the most frequent followed by allergic rhinitis, chronic rhinitis, septal deviation chronic otitis media, asthma, tonsilitis and allergic dermatitis. 5. In the main symptoms, nasal obstruction was the most important. In the concomitant symptoms, patients had frequent rhinorrhea, smell disturbance, postnasal drip, headache, etc. 6. The average treatment period was 3,58 months and the average number of treatment was 12.94 sessions. 1. In the amount of herbal medicine, 2 and 3 Ji(劑) were the most. The most frequently used herbal medicine to treat was Hyangbujamogeuntanggagam(香附子茅根湯加減) 8. In the change of grade after treatment, Grade I was the most. In the efficacy of treatment, 'good' was the most, followed by 'excellent' and 'uneffective'.

What is the 'objective' differential factor of diarrhea in infancy?: Normal state versus diarrheal illness in infants with chronic frequent and loose stool

  • Purpose: This study aimed to identify 'objective' differential factors for normal frequent loose stool (NFLS) and diarrheal illness with dehydration and nutritional deficiency (DIDN) among infants with chronic frequent loose stool (CFLS). Methods: Data were analyzed from infants under 2 years of age with CFLS who had been transferred from general pediatricians. These 46 patients were divided into 2 groups (NFLS versus DIDN). Nocturnal stool was defined as evacuation between 10 pm and 6 am. Maximal stool amount/day (measured using the mother's hand) was specified as the highest score during the period of CFLS obtained by adding up each evacuation's score (range, 0-2 points). Results: There were 36 cases of NFLS and 10 of DIDN. A failure to gain weight ($P$=0.0001), fever ($P$=0.0079), colic/abdominal pain ($P$=0.0014), gross blood in stool (except allergic proctocolitis) ($P$=0.0113), nocturnal stool ($P$=0.0001), and the score of stool amount ($P$=0.0001) were found to significantly differentiate the groups. A failure to gain weight was observed in 39% of even NFLS. The frequency, mucus content, and microbiological findings of stools, as well as diaper dermatitis were not found to significantly differentiate the groups. Conclusion: NFLS was more common than DIDN in infants with CFLS. The most 'objective' differential factors were nocturnal stool and the score of stool amount (${\geq}7$ points/day).

A Case Report on Chronic Nipple Eczema with Deficiency of Qi Blood and Liver Fire Improved by Korean Medicine (기혈양허(氣血兩虛)와 간화(肝火)를 동반한 만성 유두 습진 1례)

  • Objective : This study assessed the effect of a combination of Korean medicine on a chronic nipple eczema patient complaining of stress and immunodeficiency due to long-term steroid use. Methods : Herbal medicine, moxibustion and acupuncture were performed for a year. Treatment outcomes were evaluated with DLQI(Dermatology Life Quality Index), EQ-5D-5L(The 5 level of EuroQol 5 Dimensio scale), EQ-VAS(EuroQol-Visual Analog Scale), ADSI(Atopic Dermatitis Severity Index), pruritus NRS(Numeral Rating Scale), pictures and patient's statements. Results : After the treatment, symptoms have been alleviated, the quality of life has increased, and it has been maintained without further nipple eczema for three months. Conclusion : We suggest that Korean medicine is effective for chronic nipple eczema patient.

  • Malignant catarrhal fever (MCF) is a systemic disease of ruminants caused by a gamma herpesvirus, ovine herpesvirus 2 (OvHV-2). Four 1-year old goats (Capra hircus), which were infected with MCF virus, OvHV-2, by being housed together with MCF virus-infected seep, were referred with a I-month history of chronic dermatitis. On the other hand, MCF virus-negative goats, which were isolated for negative control, had not those kinds of skin problems. Examination of the affected goats revealed generalized alopecia, patchy erythema, and superficial erosions with histologic evidence of mural folliculitis. Fungal culture tests and external parasite tests with the scraping skin samples were negative. However, polymerase chain reaction revealed the existence of MCF virus DNAs in the lesion. These results suggested that MCF virus may induce mural folliculitis and alopecia in goat.

  • Two dead and two terminally sick badgers with signs of lacrimation and nasal discharge were submitted to the Pathology Division of the National Veterinary Research Institute for necropsy. The major gross findings included chronic dermatitis and pneumonia. histologically intracytoplasmic and intranuclear acidophilic inclusion bodies consistent with Canine Distemper (CD) virus particles in lung kidney urinary bladder skin foot pad stomach and small intestine. Additionally there were diffuse bronchointerstitial pneumonia hyperkeratosis of foot pads and focal non-suppurative encephalitis. Canine distemper infection in these badgers was further confirmed by immuofluorescent technique which demonstrated CD virus-specific antigens in lung and kidney sections.

  • Transient symptomatic zinc deficiency (TSZD) is clinically similar to acrodermatitis enteropathica. The patients are experienced dermatitis on periorifice and extremities, alopecia, diarrhea, growth retardation, anemia, delayed wound healing and infection. They showed a characteristically rapid response to zinc supplement without recurrence. We report 3 cases of TSZD infants, 2 premature infants with inadequate zinc supply and 1 mature infant with chronic diarrhea.

  • Dyshidrotic eczema is a chronic vesicular skin disease due to stress, sweating and nickel dermatitis. It has the characteristic symptoms of a cluster of vesicles and frequent recurrence. We diagnosed a 63-year old male patient case with hyperkeratosis, itching and vesicles at his right foot as dyshidrotic eczema and treated him with application of herbal medicine(bo-jung-ik-ki-tang) and acupunture at Dong-su Oriental-medicine hospital in 2003. We had a good result, so we report this case.

  • The concept of traditional oriental medicine and pharmaceutical and clinical effects were studied on holyessing obtained through carbonization of wood, before practical application. The results were obtained as follows: 1. Holyessing can be thought to be effective fluid to promote physiological metabolism as heat of water(水中之火) in the respest of oriental medicine. 2. Pharmaceutical effects of holyessing were reported to be antidiabetic effect, protective effect on liver, anticancer activity, protective effect against electronic damage, thermic effect, deodouring effect and proetcive action on atrophy dermatitis. 3. Several kinds of holyessing derived from other trees except of oak chiefly used so far had better be studied such as Morus bombycis. 4. Acute and chronic toxicity, combination of holyessing with other food, artifical correction of its taste and concentration should be absolutely studied.
