농촌 노인의 나트륨 섭취에 기여하는 음식 분석 (Dishes Contributing to Sodium Intake of Elderly Living in Rural Areas)

  • Sodium is a necessary element for the body. Excessive intake of sodium is known as one of the risk factors for chronic diseases. Recently, increasing numbers of people in Korea are suffering from chronic diseases. Major causes of deaths were chronic degenerative disease with the rising aging population. Especially, the population of rural areas is growing older fast. In rural areas, it is known that under nutrition and high sodium intake were major nutritional problems. For sodium intake, there were some studies about contributing food items. They were not enough to show diets relate to sodium intakes. Thus, this study analyzed dishes contributing to sodium intakes of elderly living in rural areas. Dietary intakes using "the 24hour recall method" were used. For the analysis for sodium intakes, "Can-pro3.0" was used. Ranking of dishes by contributions of sodium intakes were Korean cabbage, kimchi (19.6%), seasoned soybean paste (5.3%), soybean paste (4.6%), soybean paste soup dried radish leaves (3.5%), hot pork and kimchi stew (3.4%) in order. Ranking of dish groups by sodium intakes was kimchies (28.3%), soup and hot soups (22.8%), stews and casseroles (9.7%), seasonings (8.2%),and seasoned vegetables (6.0%) in order. One-dish meals among cooked rice, wheat noodles among noodle and mandu, soups using the soybean paste, stews using soybean paste and kimchi, salted fish among grilled foods, stir-fried anchovy among stir-fried foods, seasoned spinach, and Korean cabbage kimchi contributed to sodium intakes. As the nutrition deficiency of the elderly living in rural areas could be a problem, and excessive sodium intakes is threatening to their health, it is needed for the senior citizens to have adequate knowledge for diets containing less sodium. And recipes for healthy food and nutrition education based on their diets are needed.

요추 황색인대의 미세구조 ; 내층과 외층의 차이 (Ultrastructural Differences between Inner and Outer Layers of Human Lumbar Ligamentum Flavum)

  • Objectives : In lumbar spine surgery it is observed that the ligamentum flavum(LF) is bilayered, and the inner layers can be left in situ to prevent peridural adhesion in open lumbar disc surgeries. The purposes of this study are to investigate ultrastructural differences between the inner and outer layer of lumbar LF by electron microscopic examination, and to see whether these differences are, if present, more prominent in chronic degenerative lumbar spinal disorders as compared with acute lumbar disc diseases. Methods : Biopsy specimens of LF were obtained from nine patients undergoing lumbar spine surgery, five of them for degenerative spinal stenosis and four for acute disc herniation. During the surgery the outer layers of LF were carefully dissected from the inner layer, and four pieces($1{\times}1{\times}1-mm$) of biopsy samples were made from each layer. These were examined with electron microscope for the morphologies and the contents of the elastic and the collagen fibers. Results : The outer layer of LF showed elastic fiber degeneration as evidenced and decreased fiber content, while the inner layer was relatively preserved in both cases of degenerative spinal stenosis and acute disc herniation. The ultrastructural changes of the layers were more evident in the outer layer. Conclusion : With these observations the authors believe that the LF degeneration may occur mainly in the outer layer, and that this fact may aid in making the rationale for using the inner layer as physiologic barrier to prevent peridural adhesion in open lumbar disc surgeries.

치료용 레이저를 이용한 통증치료 경험 (The Clinical Experiences with Laser Therapy in Pain Patients)

  • Two hundred patients with acute and chronic pain were treated with a low power laser and 115 patients among them were divided into several groups by their pathology and evaluated their response rate to the laser therapy was evaluated through follow-up study. 1) The ages of patients were between the early twenties and late sixties, and there was no differences between sexes. 2) Degenerative spondylosis and chronic lumbar sprain were the most common diseases among those patients. 3) The average duration of therapy was about 16 days and response to the therapy appeared from the fourth day of laser therapy. 4) Acute lumbar sprain and acute spinal compression fracture showed rapid response to laser therapy. 5) The spinal pathology group was the most common at 37.5% of cases and the response rate to laser therapy was the lowest at 58.7%. 6) The articular pathology group occupied 24.6% and the response rate was the highest at 81.3%. 7). The response rate of the posttraumatic and postsurgical pathology group was 76.5%. 8) The response rate of the tendinous and sports pathology group was 75%. 9) The response rate of the miscellaneous group was 66.7%. 10) The mean response rate of all patients was 71.6%.

걷기운동 프로그램이 농촌주민의 신체적 건강에 미치는 효과 (The Effects of Walking on the Physical Health of Residents in Rural Areas)

  • Purpose: The Purpose of this study was to analyze the effect of walking on physical health such as body composition, blood pressure, blood glucose and blood lipids for residents in rural areas. Method: Data were collected from 109 residents at 4 community health centers and during the 12 weeks' period between May and July, 2007 with quasi-experimental pre/post-test design. The data were processed with SPSS Win 12.0. Result: 69.5% of the subjects had chronic degenerative diseases such as hypertension, arthritis and diabetes mellitus. Also, 52.9% had overweight and 14.7% had excessive obesity. There were significant positive changes in BMI, blood pressure, blood glucose and LDL among obese residents. However, there was no significant difference in total cholesterol, triglycerides and HDL among blood lipids. Conclusion: This study showed that a 3 months' walking program had positive effects on physical health and it should be continued.

  • Persistent cellular oxidative stress and enhanced lipid peroxidation (LPO) of PUFAs, leading to macromolecular damage and disruption of signaling pathways, are implicated in the development of human malignancies and other chronic degenerative diseases. LPO generates by oxidation of linoleic acid (LA) or arachidonic acid ($\omega$ -6 PUPAs) reactive aldehydes, such as trans-4-hydroxy-2-nonenal, which form etheno $\varepsilon$ -DNA adducts in a variety of human tissues and thus can contribute to diet-related cancers.(omitted)

  • The National Health Insurance Corporation (NHIC) was performed population health checkup survey program with especially two target ages, 40 years and 66 years old that were called as "life transition period" for national collective prophylaxis of cardiovascular diseases in Korea. We were investigated and compared between two age groups with the serum levels of cholesterol and triglyceride from data of the survey in Daegu province, Korea at April, 2007. Data of one hundred persons per each age (40 years and 66 years old) were collected and investigated for risk factors for chronic degenerative diseases including serum lipid concentration, health habits of drinking, smoking, exercise etc. In spite of the definite difference of life transition of the each age, the transition from adolescence to adulthood at 40 years old, and from adulthood to senescence at 66 years old, there were no significant difference of total cholesterol and triglyceride between two age groups.

Drosophila as a model for unfolded protein response research

  • Endoplasmic Reticulum (ER) is an organelle where most secretory and membrane proteins are synthesized, folded, and undergo further maturation. As numerous conditions can perturb such ER function, eukaryotic cells are equipped with responsive signaling pathways, widely referred to as the Unfolded Protein Response (UPR). Chronic conditions of ER stress that cannot be fully resolved by UPR, or conditions that impair UPR signaling itself, are associated with many metabolic and degenerative diseases. In recent years, Drosophila has been actively employed to study such connections between UPR and disease. Notably, the UPR pathways are largely conserved between Drosophila and humans, and the mediating genes are essential for development in both organisms, indicating their requirement to resolve inherent stress. By now, many Drosophila mutations are known to impose stress in the ER, and a number of these appear similar to those that underlie human diseases. In addition, studies have employed the strategy of overexpressing human mutations in Drosophila tissues to perform genetic modifier screens. The fact that the basic UPR pathways are conserved, together with the availability of many human disease models in this organism, makes Drosophila a powerful tool for studying human disease mechanisms.   • Macular degeneration and glaucoma are representative age-related retinal diseases that rank second and third in the prevalence of retinal diseases, and are a kind of degenerative neurological disease. Irreversible visual acuity and visual field damage may occur, and the number of patients is rapidly increasing as the population ages. Since this retinal disease is a chronic disease, continuous drug treatment is required. There are various drug delivery methods for treatment, but direct injection of the drug into the intravitreal is the most effective for continuous delivery of the drug over a long period of time. In order to solidify Dexamethasone, a retinal disease treatment, and insert it into the primary intravitreal, it is important to develop a technology to miniaturize the treatment and an applicator to deliver the treatment. In this study, a mold technology was developed to integrate the cannula and hub, which are one part of applicator. In addition, surface treatment was performed on the outside of the cannula to improve the bonding strength between the cannula and the hub, and the bonding strength according to each condition was analyzed through a tensile test.

과학기술지식정보서비스의 보건의료 분야 연구·개발과제: 분포와 연구비용 비중 분석 (An Analysis of Heath-Related Research and Development Registered at the National Technical Information Services)

  • With the growth of aging population in Korea, a better care of chronic and other degenerative illnesses is urgently needed. Evidences suggest that this can be achieved through incorporating a wide range of care options, expanding beyond medical interventions. The aim of this study is to analyze the distribution of publically funded research to understand if the Korean research and development funding system matches various approaches and purposes to successfully tackle the chronic care needs of an aging society. We complied the list of funded projects to be analyzed by searching the National Technical Information Service database with key words such as aging society/senescence, chronic diseases, disability, and health promotion. Most projects were based on the biomedical approach with the purpose of establishing the etiology and clinical (treatment) interventions. Health promotion projects showed a distinctive distribution with more percentage of projects based on psycho-behavioral approaches while research on chronic diseases predominantly biomedical. It would be necessary to diversify publically-funded research projects to develop effective and efficient care technologies for the future.

고액진료비 환자의 특성 비교분석 - 의료보험과 의료보호환자를 중심으로 - (Comparative Analysis on the Characteristics of High Cost Medical Users between the Health Insurance and Medical Assistance Program)

  • Background : A small number of high cost patients usually spend a larger proportion of scarce health resources. Aged, long-term care and readmitted patients usually belong to these high cost patient group. Among others, long length of stay and readmission can be reduced by checking its cause, and these are the areas needed most of quality improvement activity. Characteristics of high cost medical users between health insurance program and medical assistance program were reviewed. Methods : The inpatient claims of health insurance and medical assistance program were analyzed. Patients were divided by 6 groups; long-term, mid-term, short-term, readmitted, cancer and aged. We defined high cost patients as those who had spent one and half million won and over per 6 months. Characteristics of high cost patients for each group were reviewed. Results : medical assistance patients used much more resources than the insured members in the average hospital cost per case but less in daily hospital cost. The former had a longer length of stay and had much heavier diseases. Major diseases of both group were cancer, diseases of circulatory system and chronic degenerative diseases. Gallstone and schizophrenia were more in the insured program. However, pulmonary tuberculosis, asthma were more common among the medical assistance patients. Early readmission before 2 weeks were 28-30% of the total readmission. Readmission rate in the malignat neoplasm and renal failure were 80% and more. Q.A program should be installed to prevent unnecessary readmissions. Conclusion : Almost 30% of early readmissions and admissions due to complications and long length of stay should be reviewed carefully to keep cost down and to enhance the quality of hospital care.
