만성두통 환자에서 한방 치료 후 인영혈 부위의 뇌혈류에 대한 연구 (Study on Cerebral Blood Flow in In-Young(ST9) after Treatment on Chronic Daily Headache Patients)

  • Objectives : This study was performed to change of blood flow in In-Young(ST9) after treatment on chronic daily headache patients. Methods : Monitoring of TCD was examined in 10' s chronic daily headache patients before and after treatment. Mean velocity flow, systolic velocity and pulsatility index were analyzed from TCD at In-Young(ST9). The patients was laid for 20 minutes before treatment and also for treatment. Results : The results showed a significant(p<0.05: Paired T-test) decrease in mean velocity flow and systolic velocity. but there were no significant differences in pulsatility index. Conclusions : These results suggest that there were significant change on blood flow in In-young of chronic daily headache patients.

Persistent headache and chronic daily headache after COVID-19: a prospective cohort study

  • Background: Little is known about the frequency and impact of the persistent headache and about the incidence of chronic daily headache (CDH) after coronavirus disease 2019 (COVID-19). The aim of this prospective cohort study was to assess the incidence, risk factors, characteristics, and impact of CDH in patients with COVID-19. Methods: In the first stage, 288 patients were interviewed by telephone after the acute phase of COVID-19. Subsequently, 199 patients who presented headache were reinterviewed at least one year after COVID-19. Headaches that persisted beyond the acute phase of COVID-19 for three or more months and presented frequency ≥ 45 days over the first three months were considered to be CDH. Results: One hundred and twenty-three patients were included, 56% were females; median age: 50 years (25th and 75th percentile: 41;58). The headache persisted beyond the acute phase of COVID-19 in 52%, and 20.3% had CDH (95% confidence interval: 13.6-28.2). Individuals who previously had headaches and who had headaches of greater intensity during the acute phase were at higher risk of developing CDH. The group with CDH included more females, greater impact of headache, more persistence of headache beyond the 120th day of COVID-19 and less throbbing headache than did the other individuals whose headache persisted. Conclusions: Patients who had COVID-19 had a high incidence of CDH. Previous headache and greater intensity of headache were associated with higher risk of CDH.

New daily persistent headache with isolated sphenoiditis in children

  • Isolated sphenoid sinusitis is a rare disease in children, and its symptoms are often nonspecific and confusing. Rarely, severe headache can be the first or only symptom of isolated sphenoid sinusitis. New daily persistent headache (NDPH) is a form of chronic daily headache that may have features of both migraines and tension-type headaches. NDPH is difficult to diagnose and requires a multifaceted approach. Here, we report on a 10-year-old boy and an 11-year-old girl who both presented with typical NDPH symptoms. These patients had no nasal symptoms or signs of infection. Neither nonsteroidal anti-inflammatory drugs nor topiramate had any effect on the headaches. Their neurological and ophthalmological examinations were normal. The results of routine blood work, including thyroid function tests, inflammatory markers, complete blood count, tests for viral infection, and a metabolic panel, were normal. A brain magnetic resonance imaging scan showed isolated sphenoid sinusitis. Both patients' symptoms resolved completely after approximately 1 month of oral antibiotics for sinusitis.

만성매일두통 환자에 대한 홍화 약침 치료 : 예비연구 (The effect of Carthami-Semen acupuncture on chronic daily headache: A pilot study)

  • Objectives : This pilot study is designed to reveal the effect of Carthami-semen acupuncture treatment on chronic daily headache. Methods : 1. All the voluntary subjects were diagnosed as CDH. 2. Carthami-semen acupuncture is applied 2 times a week for 4 weeks. 3. The patients were assessed by Headache impairment test score(HIT), Short Form 36 (SF-36) Health Survey, and Headache Free Days(HFD) before and after 4 weeks treatment, and two weeks the treatment finished. Results : The result were as follows ; 1. HIT score decreased from $65.3{\pm}5.5$ to $53.1{\pm}9.4$, and furthermore declined to $49.5{\pm}8.0$ after 2 weeks the treatment finished. 2. HFD increased from 19.1% to 31.5% during the treatment time. Moreover it showed continuous effectiveness after 2 weeks the treatment finished. 3. SF-36 showed significant improvement on all of the QOL items. Conclusions : This pilot study suggests that Carthami-semen acupuncture treatment has an effect on the CDH patients.

만성 긴장성 두통을 호소하는 소음인(少陰人), 태음인(太陰人) 환자 3례 (Approaching Chronic Tension-type Headache through Sasang Constitutional Therapy: A Series of 3 Cases who were classified as the Soeum Type and Taeeum Type)

  • Objectives We report a series of 3 chronic tension-type headache cases who were constitutionally typed as the Soeum type, Taeeum type and treated with Dangguibaekhaoguanjung-tang, Chungshimyeonja-tang or Chunghyulganggui-tang. Methods In treating 1 patient who was constitutionally typed as the Soeum type, Case 1 fell into the category of the Taeeum symptomatology, 2 patients who were constitutionally typed as the Taeeum type, Case 2 and Case 3 fell into the category of Dry-Heat symptomatology. Each patient was given Sasang medication according to protocol: Dangguibaekhaoguanjung-tang to Case 1, Chungshimyeonja-tang to Case 2, Chunghyulganggui-tang to Case 3. On admission day and approximately 2 weeks later, patients evaluated their pain and quality of life using SF-36 and HIT-6. The patients symptoms were observed daily and NRS scores were recorded daily. Results and Conclusions 2 Patients(Case1, Case2) showed improvement in their NRS scores, SF-36 scores and HIT-6 scores. 1 patient(Case3) showed improvement in NRS scores, but worse SF-36 scores and no improvement in HIT-6 scores. Treating chronic tension-type headache based on Sasang Constitutional therapy was found to be effective for managing pain. The treatment protocol suggested in this investigation should be explored and verified in future studies.

만성두통환자에게 적용한 비약물적 치료가 통증행동에 미치는 영향 (Effects on pain behavior in non-medicinal treatment applied to chronic headache patients)

  • Objectives : This study is a random-controlled post-design for confirmation of headache degree, quality of life, and satisfaction to therapeutical effect on chronic headache adults after management of non-medicinal treatment (acupuncture therapy and stellate ganglion block therapy). Methods : 51 clinical experiment participants were gathered and through a questioutaire patients who experienced headache for more than 4 hours a day and more than 15 days per month were qualified as chronic headache patients. The qualified patients were classified in to two groups, acupuncture group (AT group, n=28) and stellate ganglion block group (SGB group, n=23). Treatment was applied 2 times a weeks for 4 weeks. The effects of both groups were analyzed using VAS scores, BPI (Brief Pain Inventory) and the satisfaction degree to the therapy. Results : 1. The recognized score of the headache of AT group and SGB group was reduced indicating the degree of the headache was released. 2. The recognition at damage to the quality of life was reduced post therapy of AT group and SGB group, in which general activity, mood, enjoyment of life, personal relationship, and sleeping showed significant improvement of life quality, 3. The satisfaction degree to the therapy showed lower score than expectation to it in AT group and SGB group, however, it was not significant. Conclusions : The results showed that the four-week non-medicinal treatment (AT and SGB) in chronic headache patients was effective for reducing headache and releasing its damage in daily life, however, no difference in superiority was found. Therefore, non-medicinal treatment (AT therapy and SGB) could be utilized in chronic headache patients.

두통의 치료 (Treatment of the Headache)

  • 두통은 인류의 가장 흔한 호소 중의 하나로 임상에서 흔히 보는 장애이다. 두통은 뇌막염, 뇌출혈, 또는 뇌종양과 같은 다른 질환의 증상일수 있으나, 또한 편두통이나 군발두통 등과 같은 질병 자체로 표현된다. 일차적으로 두통 장애의 역학이나 국제 두통학회의 진단기준을 이해하고 흔치 않으나 심각한 이차적인 두통장애와 감별에 관심을 둬야 한다. 환자가 일차 두통장애의 기준에 맞으면 신경학적 진단검사의 보충이 없어도 치료를 시작한다. 두통 유형, 표현 양상, 동통기간과 강도 등에 따라 진통소염제나 혼합진통제, 혈관작용의 항편두통 약물 또는 신경이완제나 corticosteroid등을 선택한다. 편두통의 빈도와 강도에 따라 예방치료가 보통 4~6개월간 조절한다. 긴장형 두통은 발작성과 만성두통으로 구분되나 치료적으로는 급성완화와 예방치료로 시도된다. 많은 만성매일두통 환자들이 진통제나 ergotamine을 과용하고 있으며 그들의 의존성과 내재된 갈등조절, 수면장애, 우울등으로 과용된 약물의 제한이 쉽지 않다. 치료의 첫단계는 약물을 끊고 조심스럽게 대치요법을 시행한다.

반하백출천마탕가미방(半夏白朮天麻湯加味方)을 투여한 두통환자(頭痛患者) 1예(例)에 대(對)한 임상보고(臨床報告) (A Clinical Study of One Patients Suffering Headache after Administering Banhabaekchulcheonma-tang)

  • Headache is a term used to describe aching or pain that occurs in one or more areas of the head, face, mouth or neck. Headache can be chronic, recurrent or occasional. The pain can be mild or severe enough to disrupt daily activities. Headache involves the network of nerve fibers in the tissues, muscles and blood vessels located in the head and at the base of the skull. Headache also can result from syndromes involving the eyes, ears, neck, teeth or sinuses. In these cases, the underlying condition must be diagnosed and treated. Recently one patient was come with headache. The patient is a 35-year-old lady with complaints of very severe headache that strike as often as several times a week. She has the throbbing pain on the right side of her head with nausea, vomiting, fatigue, dizziness, coldness of hand and leg, or ringing in the ears sometimes etc. After treatment through Banhabaekchulcheonma-tang, most of the symptoms improved. Therefore, this application of oriental medicine is reported with a plea for further investigation.

도수근막이완기법과 폼롤러를 이용한 자가근막이완기법이 만성 긴장성 두통 환자의 통증문턱값 및 신체 도식에 미치는 효과 비교 (Compairison of Effect of Manual Myofascial Release and Self Myofascial Release Technique Using a Foam Roller on Pain Thresholds and Body Schema in Subjects with Chronic Tension-type Headache)

  • Purpose: Headache is a very common disease experienced at least once in daily life, and tension-type headaches have a high and increasing prevalence. Chronic headaches can cause functional damage and huge socioeconomic impacts. This study aimed to compare the effects of myofascial release technique with manual therapy and self-myofascial release technique using a foam roller on the pain threshold and body schema in patients with chronic tension-type headaches. Methods: The study was conducted on 20 patients living in Busan with chronic tension headaches. Myofascial release technique with manual therapy was performed on the suboccipital, sternocleidomastoid, scalene, and upper trapezius muscles. The self -myofascial release technique using a foam roller was applied to the cervical and thoracic muscles. A laterality test was performed using a recognized neck application developed to evaluate body schema ability. A pressure-pain threshold test was performed using an electronic pressure algometer to compare the results before and after the myofascial release technique. Results: After applying myofascial release with manual therapy and a foam roller, the pressure-pain threshold values showed significant changes in both groups (p<.05). As a result of the laterality test, myofascial release with manual therapy and a foam roller were applied to the painful area. The values showed significant changes in both groups (p<.05), but only the group using the foam roller showed a significant difference (p<.05) in painless areas. Conclusion: The myofascial release technique with manual therapy can be the primary treatment technique for pain control in painful areas. The self-muscle release technique using a foam roller can be an effective method when there is no pain or when maintenance is needed after pain control.

대학생의 스마트폰 이용 시간과 경항통과의 관련성 연구 (The Correlation Analysis between Hours of Smartphone Use And Neck Pain in the Gachon University Students)

  • Objectives : The purpose of this study was to analysis the relationship between hours of smartphone use and neck pain in university students. Methods : A survey of 2,353 university students was conducted in Gyeonggi province and Incheon city using a self report questionnaire from April 1 to 11 2013. The questionnaire of questions regarding the hours of using smartphone, mainly used function in participant's smartphone and neck discomfort degree. Neck disability index(NDI) was used to evaluate degree of pain. The collected data were analyzed with the t-test and Kendall's tau test using the SPSS 21.0 program and R 3.1.0, respectively. Results : 1. Almost all of the survey participants(99.07 %) were smartphone users. 66.97 % of them answered that they use their smartphone daily more than 2 hours and 48.18 % of them answered that they use their smartphone 10 to 30 minutes everytime they use it. 2. The overall distribution of NDI scores was 'no disability(0~4 score, 62.92 %)', 'mild disability(5~14 score, 32.85 %)', 'moderate disability(15~24 score, 1.19 %)'. As a result of t-test, we found that the average NDI score for female students was significantly higher than the average NDI score for male students(p<0.05). 3. The Kendall's tau test revealed that total time spent daily using smartphones has a strong positive correlation with 'pain intensity', 'lifting', 'reading' 'headache', 'concentration', and 'driving'(p<0.05) in NDI. Also, time duration of one time smartphone usage has a strong positive correlation with 'pain intensity', 'lifting', 'reading', 'headache', 'concentration', 'work', and 'recreation'(p<0.05). 4. The Kendall's tau test revealed that total NDI scores have a significantly strong positive correlation with both of total time spent daily using smartphones(p<0.05) and time duration for one time smartphone usage(p<0.05). This results imply that long-time use of smartphone has a strong relationship with neck pain. Conclusions : The results of the study would be a good starting point for future studies to reduce the risks of chronic neck pain caused by smartphone usage.
