체중현수 트래드밀 훈련이 뇌졸중 환자의 보행에 미치는 영향 (The Efficacy of Treadmill Training with Body Weight Support on Ambulation with Stroke Patients)

  • The propose of the study was to evaluate the efficacy of the partial body weight support during treadmill training on the ambulation in elderly with chronic stroke. Fourteen hemiplegic volunteers participated and were divided into an experimental and control groups. In the experimental group, the body weight support during treadmill training was performed 3 times per week for 6 weeks. In the control group, usual treadmill training was applied. Before and after experiments, temporal-spatial gait parameters were measured. The date of 14 patients who carried out the whole experimental course were statistically analyzed. The results of the study were : 1. In the comparison of gait velocity before and after experiment, the gait velocity was significantly increased in the experimental group and the control group(p<.05). In the comparison of difference of the gait velocity between groups, there was not, significant difference between the experimental group and the control group(p>.05). 2. In comparison of gait cadence before and after experiment, the gait cadence was significantly increased in both groups(p<.05). In the comparison of difference of the gait cadence between groups, there was not significant difference between the experimental group and the control group(p>.05). 3. In the comparison of step length before and after experiment, the step length was significantly increased in the experimental group and the control group(p<.05). In the comparison of difference of the step length between groups, there was not significant difference between the experimental group and the control group(p>.05). 4. In the comparison of single support time asymmetry before and after experiment, the single support time asymmetry was no significant difference between groups(p>.05). In the comparison of difference of the single support time asymmetry between groups, there was not significant difference between the experimental group and the control group(p>.05). 5. In the comparison of step length asymmetry before and. after experiment, the step length asymmetry was not significant difference between the experimental group and the control group(p>.05). In the comparison of difference of the single step length asymmetry between groups, there was not significant difference between the experimental group and the control group(p>.05).

모야모야병(moyamoya disease) 환자의 전신마취 하 치과치료: 증례보고 (DENTAL MANAGEMENT OF A PATIENT WITH MOYAMOYA DISEASE UNDER GENERAL ANESTHESIA: CASE REPORT)

  • 본 증례는 다수의 치아우식증을 주소로 내원한 모야모야병 환자의 전신마취 하 치과치료에 대한 보고이다. 모야모야병은 치과치료 동안 상당히 주의를 필요로 하는 다양한 전신질환과 관련이 있다. 여러 과의 의사들과 협진이 필요하고, 치과 예방치료에 초점을 맞추면서 적절한 시기에 치료하는 것이 중요하다. 모야모야 환자에서 울음과 과호흡은 저칼륨혈증을 일으킬 수 있고, 대뇌 혈관 수축 효과를 일으킬 수 있다. 치과치료 시 뇌졸중 발생을 예방하기 위해서 통증과 불안을 조절하는 것이 매우 중요하다. 비협조적이거나 매우 어린 모야모야병 환자에게 치과치료를 하기 위해서는 전신마취가 필요할 수 있다.

경추근막통증증후군에 대한 초음파 유도하 약침 치료 3례 증례보고 (Case Report on Three Patients with Cervical Myofascial Pain Syndrome Showing Improvement with Ultrasound-Guided Pharmacopuncture: Proposal of the 'SLS Triangle' Treatment Protocol)

  • Objectives : Cervical myofascial pain syndrome (C-MPS), a key aspect of Visual Display Terminals (VDT) Syndrome, leads to acute and chronic pain due to muscle shortening, potentially causing headaches and movement disorders. This study examines the effectiveness of Pharmacoacupuncture at the intersection of the Splenius Capitis (SC), Levator Scapulae (LS), and Serratus Posterior Superior(SPS) muscles under ultrasound guidance, known as the "SLS triangle" in treating C-MPS. Methods : Three patients (aged 30 to 37) with acute C-MPS presented at two Korean medicine clinics with severe neck pain and restricted cervical rotation. The evaluation of symptoms before treatment, immediately after treatment, and one day after treatment was conducted using the cervical rotation Range of motion (ROM) and the Numeric Rating Scale (NRS) for cervical pain. Treatments included Acupuncture, Cupping therapy, Chuna manipulation, Physical Therapy, and Pharmacopuncture (Jaha-geo extract, 2cc at the SLS triangle). Results : All three patients showed significant improvement post-treatment. Immediately after treatment, Patient 1's NRS improved from 7 to 2, Patient 2's NRS improved from 4 to 1, and Patient 3's NRS improved from 6 to 2. Directly after the procedure, there were no significant adverse reactions except for one patient experiencing temporary dizziness. When the patients were followed up by phone 24 hours later, no adverse reactions were reported. Conclusions : This study demonstrates that ultrasound-guided pharmacopuncture can significantly reduce pain and improve cervical rotation in patients with cervical myofascial pain syndrome. The findings suggest this non-invasive treatment is both safe and effective, warranting further research through larger-scale clinical trials to validate its efficacy and establish standardized protocols.

  • Neuroprotective strategies have been appeared to be effective in a variety of stroke models. One of the major focuses has been related to the activities of estrogen. $17\beta$-estradiol valerate(EV) has been reported to exert neuroprotective effects when administered before an ischemic insult. The purpose of this study was to determine whether EV can protect against brain injury via estrogen receptor. Chronic and acute pretreatment can reduce the ischemic damage of focal cerebral ischemia in OVX rat, indicating that EV may be a new therapeutic class of drugs to prevent neuronal damage associated with cerebral ischemia. RNAs were extracted from the hippocampus of ovariectomized female rat with or without EV. Differential gene expression profiles were revealed(Bone morphogenetic protein type 1A receptor, Protein disulphide isomerase, cytochrome bc-1 complex core P, thiol-specific antioxidant protein). RT-PCR and in situ hybridization were used to validate the relative expression pattern obtained by the cDNA array. 대칭형 상지 운동기구를 이용한 손목 운동 시 뇌 활성도 패턴 (Brain Activation During the Wrist Movement Using Symmetrical Upper Limb Motion Trainer)

  • We developed a symmetrical upper limb motion trainer for chronic hemiparetic subjects. This trainer enabled the practice of a forearm pronatio $n^ination and wrist flexion/extension. In this study, we have used functional magnetic resonance imaging(fMRI) with the developed symmetrical upper limb motion device, to compare brain activation patterns elicited by flexion/extension wrist movements of control and hemiparetic subject group. In control group, contralateral somatosensory cortex(SMC) and bilateral cerebellum were activated by dominant hand movement(Task 1), while bilateral movements by dominant hand(Task 2) activated the SMC in both cerebral hemispheres and ipsilateral cerebellum. However, in hemiparetic subject group, contralateral supplymentary motor area(SMA) was activated by unaffected hand movement(Task 1), while the activation of bilateral movements by unaffected hand(Task 2) showed only SMA in the undamaged hemisphere. This study, demonstrating the ability to accurately measure activation in both sensory and motor cortex, is currently being extended to patients in clinical applications such as the recovery of motor function after stroke.ke.

  • 손가락 운동은 뇌에 가장 큰 영향을 미친다고 알려져 있으며, 손의 기능은 운동과 감각의 복합 기능을 가지고 있기 때문에 식사하기, 옷 입기 등 일상생활을 하는데 있어 반드시 필요하다. 본 연구는 만성 뇌졸중 환자를 대상으로 손가락 재활치료를 위해 환자의 손동작 인식을 위한 'Real Sense'와 게임 개발 엔진인 'Unity3D'를 연동하여 게임을 개발하는 것을 목적으로 한다. 제안하는 게임은 활동성을 부가함으로써 손가락 재활이라는 특정 목적을 달성하는 기능성 게임이며, 주어진 과제를 단계별로 나누어 진행하도록 하여 난이도를 설정하였다. 우리는 환자들의 게임 참여도를 높이기 위해 딱딱한 화면이 아닌 친숙한 게임형식으로 구성하여 환자들이 지루함 없이 자발적으로 재활치료를 할 수 있도록 도움을 주며, 환자들은 우리의 게임을 이용하며 손가락을 균형 있게 사용함으로써 뇌 활동을 향상시킬 수 있다. 기존의 재활치료는 환자가 직접 병원을 가야하는 불편함과 가격이 비싼 재활 치료 기계를 사야하는 반면에 본 연구에서는 비교적 저렴하고 가벼운 'Real Sense'를 이용하여 시간과 공간에 얽매이지 않고 재활치료를 할 수 있도록 하였다.

가정용 게임기를 이용한 재활운동이 뇌졸중 장애인의 보행 능력에 미치는 효과 (The Effects of Rehabilitation Exercise Using a Home Video Game (PS2) on gait Ability of Chronic Stroke Patients)

  • 본 연구는 가정용 게임기를 이용한 재활 운동을 적용하여 뇌졸중 장애인의 보행능력에 효과가 있는 가를 알아보고자 하였다. 뇌졸중으로 6개월 이상 장애를 가진 32명이 연구에 참여하였고 게임운동군 16명과 대조군 16명으로 나누었다. 게임운동군은 가정용 게임기(PS2, sony)를 이용하여 1시간씩 주 3회, 6주간 실시하였다. 대조군은 운동의 적용 없이 평상시 생활을 유지하도록 하였다. 운동 전과 후에 보행능력을 측정하여 효과를 비교하였다. 통계처리 방법으로 전 후 차이를 검증하기 위하여 대응표본 t 검정을 실시하였고 대조군과의 차이 검증을 위하여 독립표본 t 검정을 실시하였다. 모든 통계적 유의 수준은 0.05로 하였다. 본 연구의 결과 보행능력에서 10m 보행속도가 유의하게 증가하였고(p<.05), 6분 보행거리도 유의하게 증가하였다(p<.05). 결론적으로 가정용 게임기를 이용한 재활운동이 뇌졸중 장애인의 보행능력에 효과가 있는 것으로 나타났다. 향후 뇌졸중 장애인에게 흥미를 유발하고 기능회복의 효과를 가진 게임형태의 재활운동이 다양하게 개발되어 적용하는 연구가 필요하다고 생각한다.

Factors Affecting Health Related Quality of Life in Korean Perimenopausal Women Using Hierarchical Regression Analysis

  • Women's menopause is a natural process that every woman must eventually experience, but changes in hormones before and after menopause can serve to produce life-threatening crises in some situations, with individual differences. Data for the study was elicited from 22,610 Korean women ranging in age from 45 to 55 years in the 2013 Korean Community Health Survey. Statistical analyses was performed using descriptive, t-test, ANOVA, and hierarchical regression analysis using SPSS IBM 20.0 version. Individual characteristics, lifestyle, history of chronic disease(s), psychological and environmental factors were included as independent and EQ5D weights of Koreans were used as dependent variables. The survey subjects consisted of a total 15,505(58.3%) in their 50s, 1,765 (66.5%) in middle and high school dropouts or graduates, 22,174 (83.3%) living in spouses and 10,534(39.6%) in wages. There was a significant difference in HRQOL among all independent variables except drinking and residential areas. Hierarchical regression analysis showed that age, smoking, obesity and other incidental disease factors (fall, angina, asthma, arthritis, osteoporosis, stroke) had a negative effect on HRQOL. The selected independent variables accounted for 22.7% of HRQOL. It is necessary to find a way to improve HRQOL of Korean perimenopausal women, focusing on the significant variables revealed by the study results.

음주 감소를 위한 정책방향 (Policies to Reduce Alcohol Consumption)

  • Recently, attention for health promotion is rising. Alcohol is widely consumed among adults. About 1/3 of people 15 years of older enjoy drinking alcohol. Some 80% of them drink a half or more bottle of soju each time. Regular excessive drinking of alcohol may cause various problems. WHO(1990) reported that social problems such as divorce, unemployment, and financial difficulties ; psychological problems such as melancholy, suicide, and drug abuse ; physical problems such as cirrhosis, lung cancer, high blood pressure, stroke, and sterilization. The patients with liver disease are estimated to be 628,000. Approximately, 12,000 persons are dying by chronic liver disease and cirrhosis each year. Among the people of 15 years or older, persons dependent on alcohol are estimated to be 1,480,000. This study suggests policies to reduce the consumption of alcohol for planning for health promotion. Limitations of sites and times of sales and designation of sellers, designation of sites prohibiting drinking, limitation of alcohol sales promotion, and restrictions on advertisement can be inaugurated. Increase of price through the raise of tax and taxation of promotion cost. Education of high risk groups such as soldiers, pregnant women, and the youth can be introduced. Provision of alternative socialization programs instead of drinking. Some approaches on target groups were suggested.

고려인삼과 당뇨병: 세포와 동물 및 인체실험을 통한 고려인삼의 당뇨병에 대한 효능 (Korean Ginseng and Diabetes: An Insight into Antidiabetic Effects of Korean Ginseng (Panax ginseng C. A. Meyer) in Cultured Cells, Animal Models and Human Studies)

  • Diabetes mellitus, commonly known as diabetes, is a group of metabolic disorders characterized by high blood sugar levels over a prolonged period. Diabetes has been found to show many acute complications such as cardiovascular disease, stroke, chronic kidney disease, foot ulcer and damage to eyes. Korean ginseng (Panax ginseng) has been traditionally known to normalize the functional deficiencies of the lung, spleen and stomach, and thus improve the secretion of body fluids, thereby quenching thirst, suggesting it to be effective in the treatment of diabetes. Experimental studies (in vitro and in vivo) have recently shown that Korean ginseng and its extracts exhibit antidiabetic effects, and also insulin secretion and sensitizing effects related to blood glucose control. Moreover, clinical trials on antidiabetic effects of Korean ginseng have been reported to show blood glucose control, improvement of insulin resistance, reduction of postprandial blood glucose level and improvement of serum lipids (TG, TC, LDL-C). These will be critically examined by means of in vitro studies, cell experiment, animal models and human trials with a focus on understanding of molecular mechanisms.
