  • The pain clinic at the Chonbuk National University Hospital opened in 1983. We analyzed 3,212 patients who visited the clinic during the 10 years from 1983 to 1993. The patients visited total of 11,628 times. The most frequent age group was in the fifties. About an half of the patients complained of low back pain, and the epidural block was most frequently given as a treatment. We need more personnels involving in pain management and more equipments in diagnosing and treating the patients with chronic pain. Understanding and cooperation among departments under the basis of closer interdepartmental relationship would be essential for the further development of the pain clinc.

  • Multiple sclerosis is an autoimmune demyelinating disorder of the nervous system. The ocular manifestation includes optic neuritis, internuclear opthalmoplegia and nystagmus. Upbeat nystagmus is a rare manifestation of multiple sclerosis. We report a patient with relapsing multiple sclerosis who presented with upbeat nystagmus from a circumscribed lesion in the caudal medulla.

Confirmation of Schizandrin as a Marker Compound in Jangsu Omija Powder (HPLC를 이용한 기능성 원료 지표성분으로서 장수 오미자 분말 중 Schizandrin의 분석법 개발)

  • We studied the development of schizandrin as a marker compound in Jangsu Omija. Schizandrin was validated for its LOD (limit of detection), LOQ (limit of quantitation), precision, accuracy, and recovery by HPLC relative to Omija powder. It showed a high linearity in the calibration curve with a coefficient of correlation ($R^2$) of 0.9990. The LOD and LOQ were 0.2 ${\mu}g/mL$ and 0.5 ${\mu}g/mL$, respectively. The intra- and inter-day precision of schizandrin calibration standards was 0.06~0.66% and 0.13~1.19%, respectively, and the intra- and inter-day accuracy of schizandrin was 98.35~103.30% and 98.35~103.00%, respectively. Overall, schizandrin was validated through our analytical methods as a marker compound in Jangsu Omija.

  • Hypoxia has been shown in many tumors because of a reduced oxygen condition. A useful approach to detect hypoxia is to use molecular imaging. Positron emission tomography (PET), one of the biomedical molecular imaging tools, is the most common non-invasive technique for providing information about physiological and biological events such as diseases. In order to use the PET technique for healthcare, promising molecular probes such as PET tracers required. [$^{18}F$]Fluoromisonidazole ([$^{18}F$]FMISO) is the most widely used in PET tracers for hypoxia. In this review, major developments of the synthetic method of [$^{18}F$]FMISO are discussed.

  • Music therapy had been widely used in the area of psychiatry. However recently, participation of psychiatrist has been decreased, due to recent trends of emphasizing biological psychiatry and rapid effectiveness of pharmacotherapy. A 46 years old male PTSD patient was admitted to psychiatric ward. He has suffered from PTSD symptoms, such as intrusive distressing recollection of his past traffic accident, social avoidance, insomnia and depression. Music therapy, which is conducted by a psychiatrist and a music therapist, was applied. This case suggests the positive role of music therapy in treatment of PTSD and emphasizes the importance of participation of psychiatrist, as an organizer during music therapy.

  • Dissection of the middle cerebral artery (MCA) is less frequent compared with dissection of the vertebrobasilar system or carotid artery. Recently, high-resolution cross sectional MR imaging (HRMRI) has emerged as a potential technique for atherosclerotic plaque imaging in MCA, We introduce the findings of HRMRI in a 56-year-old woman with traumatic MCA dissection, HRMRI showed an intimal flap and tapered pseudolumen with intra-luminal hemorrhage, We performed stent deployment about MCA dissection after failed medical treatment Three months later, there was no in-stent restenosis and no further neurological deficit were noted.

Cystic Meningiomas

  • Cystic Meningiomas are rare tumor. There is a clear prevalance in infants, according for $10{\sim}19%$ of all intracranial meningiomas in this age group, compared with only $2{\sim}4%$ in adults. But, reports of cystic meningioma have been increased in frequency since the introduction of computed tomography[CT] and magnetic resonance image[MRI]. Authors report two cases of cystic meningioma in adults. Radiological finding showed extraaxial mass with cystic component. Authors performed total surgical resection of the tumor mass and of the entire cystic component with cystic wall in both patients. Postoperative histopathological diagnosis of the tumor mass was a transitional meningioma in both patients. Both patients showed favorable prognosis and no recurrence findings in follow up MRI.

  • Weakness of the dorsiflexor muscles of the ankle or toe, referred to as foot drop, is a relatively common presentation. In most cases, foot drop is caused by a lower motor neuron disease such as peroneal peripheral neuropathy, L4-5 radiculopathic sciatic neuropathy, or polyneuropathy. Although upper motor neuron lesions can present as foot drop, the incidence is very rare. Here, we report an extremely rare case in which foot drop was the only presenting symptom of cerebral infarction.

  • Upper extremity replantation is relatively less commonly performed than finger or hand replantation. We have experienced one case of forearm replantation and one case of upper arm replantation. After the replantation, limb volume at the biceps brachii muscle level below the replantation level appeared to be appropriate, however, the motor function of the muscles and the sensitivity were disappointing. For replantation of forearm and upper arm, restoration of the motor function and sensitivity of the extremity below the amputation level as well as the morphologic reconstruction have to be considered.
