Quantification of Immunoglobulin G against Trypanosoma cruzi in Individuals with Chronic Chagas Disease Treated with Nifurtimox and Evaluated in Prolonged Follow-Up

  • In the indeterminate chronic period of Chagas disease (ChD) the treatment has not been conclusive, because the serological negativization requires many years. This study aims to evaluate the efficacy of nifurtimox (NF) in the treatment of chronic ChD in prolonged follow-up by serological techniques of indirect immunofluorescence assay (IFA) and enzyme-linked immunosorbent assay (ELISA) IgG comparing 2 groups of patients, treated and non treated. Mann-Whitney test was performed for ELISA and IFA, with significant difference between the groups (P<0.05). IgG levels were lower in individuals treated compared with untreated patients, indicating chemotherapeutic efficacy in prolonged follow-up.

Does Community Participation Work? : A Study on the Impact of Health Care Reform in Chile

  • Community participation in health has been praised as a new way of improving health inequality in developing countries for many decades. This paper is an attempt to evaluate community participation programs in health focusing on two intercultural health hospitals in IX Region of Chile. After exploring the process of program building and its impact on the quality of service, this paper concludes that a community participation program with stronger participation resulted in higher patient satisfaction. The author expects such finding to contribute to more comprehensive understanding of the impact of participation in health programs.

Descended Mouth Corner: An Ignored but Needed Feature of Facial Rejuvenation

  • For years, the gold standard in facial rejuvenation has been the face lift. However, exploring new, less complex procedures for achieving the same goal is currently drawing interest. Rejuvenation of the perioral area is a difficult task for plastic surgeons because of the minimal effect that face lift procedures have over this region and the lack of published material on the subject. In this article, the descended mouth corner anguloplasty technique is presented. It is a 20-minutes lift technique that can correct this typical feature of the ageing mouth. The authors have treated 71 patients using the technique with consistently good results, with just one requiring revision. They conclude that this procedure by itself and in combination with other small operations or even a full face lift can rejuvenate the ageing face.

New Species and New Record of Genus Chrysothrix (Chrysotrichaceae, Arthoniales) from South Korea and Chile

  • The genus Chrysothrix is very common around the world and easy to be recognized by its bright yellowish granular thallus. In this study, investigations of lichen mycota in South Korea and Chile from 2010 to 2017 have been done, and some Chrysothrix specimens were collected, based on the morphological and chemical study on these specimens, five species were confirmed in this study, including one new species from Chile, Chrysothrix chilensis D. Liu & J.-S Hur, and one new record from South Korea, C. xanthina (Vain.) Kalb. Meanwhile, detail description and illustration for each species were present in this study.

의료보험 민영화가 여성의 건강에 미치는 영향 : 칠레의 사례를 중심으로 (Lessons from Chile: The Impact of Privatization of Health Insurance on Women's Health)

  • Chile has been the first country in Latin America which has built a two-tiered health care system by partially privatizing the health insurance sector. Despite the intial decrease of health expenditure, more researches now show that health inequality within the Chilean health sector has been augmented with privatization of its insurance system. To explore such inequality, this article looks into the impact of privatization of health insurance on women's health. The author argues that privatization has intensified medicalization of women's body and, consequently, it worsened women's health in Chile. This article contributes to a more comprehensive understanding of market-oriented health care reform by linking it with medicalization process.

Extended use of P504S Positive Primary Circulating Prostate Cell Detection to Determine the Need for Initial Prostate Biopsy in a Prostate Cancer Screening Program in Chile

  • Background: To determine the frequency of primary circulating prostate cells (CPC) detection according to age and serum PSA levels in a cohort of men undergoing screening for prostate cancer and to determine the diagnostic yield in those men complying with the criteria for prostate biopsy. Materials and Methods: A prospective study was carried out to analyze all men evaluated in a hospital prostate cancer screening program. Primary CPCs were obtained by differential gel centrifugation and detected using standard immunocytochemistry using anti-PSA, positive samples undergoing a second process with anti-P504S. A malignant primary CPC was defined as PSA+ P504S+, and a test positive if 1 cell/4ml was detected. The frequency of primary CPC detection was compared with age and serum PSA levels. Men with a PSA >4.0ng/ml and/or abnormal rectal examination underwent 12 core prostate biopsy, and the results were registered as cancer/no-cancer and compared with the presence/absence of primary CPCs to calculate the diagnostic yield. Results: A total of 1,117 men participated; there was an association of primary CPC detection with increasing age and increasing serum PSA. Some 559 men underwent initial prostate biopsy of whom 207/559 (37.0%) were positive for primary CPCs and 183/559 (32.0%) had prostate cancer detected. The diagnostic yield of primary CPCs had a sensitivity of 88.5%, a specificity of 88.0%, and positive and negative predictive values of 78.3% and 94.9%, respectively. Conclusions: The use of primary CPCs for testing is recommended, since its high negative predictive value could be used to avoid prostate biopsy in men with an elevated PSA and/or abnormal DRE. Men positive for primary CPCs should undergo prostate biopsy. It is a test that could be implemented in the routine immunocytochemical laboratory.

Seismic Performance of High-rise Concrete Buildings in Chile

  • Chile is characterized by the largest seismicity in the world which produces strong earthquakes every $83{\pm}9years$ in the Central part of Chile, where it is located Santiago, the capital of Chile. The short interval between large earthquakes magnitude 8.5 has conditioned the Chilean seismic design practice to achieve almost operational performance level, despite the fact that the Chilean Code declares a scope of life safe performance level. Several Indexes have been widely used throughout the years in Chile to evaluate the structural characteristics of concrete buildings, with the intent to find a correlation between general structural conception and successful seismic performance. The Indexes presented are related only to global response of buildings under earthquake loads and not to the behavior or design of individual elements. A correlation between displacement demand and seismic structural damage is presented, using the index $H_o/T$ and the concrete compressive strain ${\varepsilon}_c$. Also the Chilean seismic design codes pre and post 2010 Maule earthquake are reviewed and the practice in seismic design vs Performance Based Design is presented. Performance Based Design procedures are not included in the Chilean seismic design code for buildings, nevertheless the earthquake experience has shown that the response of the Chilean buildings has been close to operational. This can be attributed to the fact that the drift of most engineered buildings designed in accordance with the Chilean practice falls below 0.5%. It is also known by experience that for frequent and even occasional earthquakes, buildings responded elastically and thus with "fully operational" performance. Taking the above into account, it can be said that, although the "basic objective" of the Chilean code is similar to the SEAOC VISION2000 criteria, the actual performance for normal buildings is closer to the "Essential/Hazardous objective".

Evidence for obtaining a second successive semen sample for intrauterine insemination in selected patients: results from 32 consecutive cases

  • Objective: The goal of this study was to compare the semen parameters of two successive samples obtained within an interval of less than 60 minutes from patients planning to undergo intrauterine insemination (IUI) whose first samples exhibited low semen quality. Methods: Thirty-two consecutive patients were enrolled in the study. On the day of IUI, the semen analysis of the samples initially presented by all patients met at least two of the following criteria: sperm concentration $<5{\times}10^6/mL$, total sperm count $<10{\times}10^6$, progressive sperm motility (a+b) in the native sample <30%, and total motile sperm count (TMSC) $<4{\times}10^6$. A successive semen sample was obtained no more than 60 minutes after the first sample. Results: Compared to the first sample, the second exhibited significantly (p<0.05) improved sperm concentration, TMSC, progressive motility, and vitality. Regarding TMSC, the most critical parameter on the day of IUI, 23 patients (71.8%) improved it, while nine (28.2%) displayed poorer outcomes. Conclusion: In defined cases, requesting a second successive ejaculate on the day of insemination may result in a high percentage of cases in an improvement of the quality of the sample.

한-칠레 자유무역협정이 부산항 물동량에 미치는 영향분석 - 대 칠레 수입 컨테이너화물 중심 - (An Analysis on the Impact of Korea-Chile FTA on Busan Port - Focusing on the Inbound Container from Chile -)

  • 자유무역협정은 국가 또는 지역간에 교역의 장벽이 되는 관세를 폐지하는 것으로서 궁극적으로 해상운송 및 항만물동량에 영향을 미친다. 한국은 2004년 처음으로 칠레와 자유무역협정을 체결하였고 그 이후 양국간 교역이 크게 증가한 것으로 나타났다. 그러나 동 협정이 해상운송과 항만물동량에 미치는 여향을 분석한 연구는 극히 미흡하다. 이에 본 연구는 한-칠레 자유무역협정이 부산항에서 처리하는 대칠레 수입 컨테이너 물동량에 미치는 영향을 분석하는 것을 주목적으로 한다. 이를 위하여 2000년부터 2011년까지 물동량, GDP, 관세율, 해상운임 등을 포함하는 횡단면 자료와 시계열 자료가 결합된 패널자료를 수집하여 중력모형을 적용하였다. 분석 결과 GDP와 관세율은 통계적으로 유의하게 나타났으나 해상운임 변수는 통계적 유의성과 모수 부호에 일관성을 보이지 못하였다.

The effect of water status on productive and flowering variables in young 'Arbequina' olive trees under limited irrigation water availability in a semiarid region of Chile

  • The intense drought affecting olive production in Northern Chile underscores the need to research non-traditional irrigation strategies to obtain the best crop performance. Accordingly, this study aimed to obtain preliminary data to guide future research on this topic. Different water replenishment levels on crop evapotranspiration ($ET_c$ ; 13.5, 27.0, 40.5, and 54%) were established in a young orchard, cv. Arbequina, from the end of fruit drop (EFD) to full bloom in the next season. We evaluated the influence of plant water status (${\Psi}_{stem}$ ) and crop load, considered as function of fruit number divided by trunk cross-sectional area, on reproductive and productive variables using multiple linear regressions. Our results show that crop load and ${\Psi}_{stem}$ measured from EFD to harvest affected yield components. Nevertheless, ${\Psi}_{stem}$ had the strongest influence on fruit size, pulp development, oil accumulation, and yield. Oil content and yield were reduced by 54% and 50% for each MPa, respectively, from ${\Psi}_{stem\;EFD-H}$ -1.8 MPa, an effect that intensified as crop load increased. During the period of flower development (September-November), the number of flowers per inflorescence and percentage of perfect flowers were reduced when ${\Psi}_{stem}$ was less than -2.0 MPa. These preliminary results showed that bud differentiation, inflorescence and flower formation are highly sensitive to water deficit.
