Purpose: This descriptive study compared the perceived parental stress levels between parents with very low birth weight infants (VLBWIs) and nurses in the neonatal intensive care unit (NICU). Methods: In total, 83 parents of VLBWIs and 78 NICU nurses were enrolled. Data were collected with the Parental Stress Scale (PSS) and analyzed using the t-test and analysis of variance in SAS version 9.4. Results: The average PSS score was 3.31 among parents and 3.45 among nurses. The stress score was significantly higher among nurses with children (t=2.46, p=.016) and senior nurses (t=2.12, p=.037). There was a significant difference in the stress score according to parents' education (t=3.29, p=.002) and occupation (F=3.14, p=.049) in the sights and sounds subscale. Mothers had significantly higher stress scores than fathers in the parental role alterations subscale (t=2.32, p=.023). Parental stress scores were higher than those perceived by nurses in the infant's appearance and behaviors subscale for breathing patterns (t=2.95, p=.004), followed by jerky/ restless behavior (t=2.70, p=.008). Conclusion: Nurses should provide explanations to parents of VLBWIs in order to reduce parental stress about the appearances and behavior of VLBWIs. This is more important than aspect of the NICU environment and education about parental roles.

Purpose: To understand the essentials of rearing conflict experience by three shift nurses in advanced general hospitals. Methods: The design was a qualitative research of phenomenology. Participants were 7 shift nurses working in advanced general hospitals who were rearing young children. Data were collected individually through in-depth interview on their life experiences. Data were analyzed by Colaizzi's phenomenological methodology. Results: Eighteen themes were drawn from 256 meaningful experiences and these themes were integrated to six theme clusters. The most influencing themes were 'Regret that I cannot satisfy even the slightest wish', 'Fail to care for kids', and 'Mutual feeling to care giver between appreciation and inconvenience'. Other themes were as follows: 'Body and mind are broken', 'The need for a three-shift system to support nurses who are rearing children', 'Doing my best for work and child rearing'. Conclusion: The nature of three-shift nurses working in advanced hospital and caring kids is explained as 'lives with conflict' between work and home. This study suggests it is necessary to establish a 24-hour care center for 3-shift nurses to keep working while rearing their children.

Purpose: This study was conducted to identify risk factors in hospitalized children, and to develop and validate a fall-risk assessment tool for hospitalized children. Methods: A retrospective chart review was performed at one university children's hospital, and an analysis was done of the characteristics of all patients who fell during a 44-month period (n=48). These patients were compared with another 149 hospitalized children who did not fall. Results: Significant predictors of falls as identified in a multivariate logistic regression analyses were age of less than 3 years old, neurological diagnosis including epilepsy, children's dependency of ADL, physical developmental delay, multiple usage of fall-risk-increasing drugs. The respective odds ratios ranged from 2.4 to 7.1 with 95% confidence interval (p<0.05). Accordingly, defining patients with either 5 risk factors as fall-prone hospitalized children provided a sensitivity of 93.6% and specificity of 16.2%. Conclusion: The results show that this tool has an acceptable level of sensitivity to assess the risk factors of fall in hospitalized children even though the specificity was low, suggesting that this tool may enable nurses to predict the risk level of childhood falls, and develop preventive strategies against pediatric falls in children's units.
Purpose: This study was conducted to identify the role and role burden of caregivers with hospitalized children. Methods: The participants included 149 main caregivers from two pediatric hospitals whose children have been hospitalized for at least 2 days at children's hospital. The data were collected from August 1 to september 30, 2013 using self-report questionnaires of checklist type for caregivers' role and Likert scale for role burden. Results: Role for hospitalized children showed that personal hygiene was the highest, and consultation with nurses and doctors was the lowest in total frequency per day. Of the role for the caregivers themselves and family, personal hygiene was the highest, and home management was the lowest. The degree of role burden perceived by hospitalized children's caregivers was the slightly high. Of the categories, emotional burden was the highest and dependent burden was the lowest. There were differences by caregivers' age, children's age, and children's health status. Conclusion: The caregivers performed various roles, especially more frequently on personal hygiene and nutrition for their hospitalized children and themselves and family, and had slightly high role burden. These results may contribute to develop nursing interventions that can help adapt on hospitalization of children and their caregivers.

Purpose: This study was to develop a factor-type patient classification system for general nursing unit based on nursing needs (KPCS; Korean patient classification system for nurses). Method: We reviewed workload management system for nurses(WMSN) of Walter Reed Medical Center, Korean patient classification system for ICU, and nursing activities in nursing records and developed the first version of KPCS. The final version KPCS was evaluated via validity and reliability verifications based on panel discussions and data from 800 patient classifications. Content validity was performed by Delphi method and concurrent validity was verified by the correlation of two tools (r=.71). Construct validity was also tested by medical department (p<.001), patient type (p<.001), and nurse intuition (p<.001). These verifications were performed from April to October, 2008. Results: The KPCS has 75 items in classifying 50 nursing activities, and categorized into 12 different nursing area (measuring vital sign, monitoring, respiratory treatment, hygiene, diet, excretion, movement, examination, medication, treatment, special treatment, and education/emotional support). Conclusion: The findings of the study showed sound reliability and validity of KPCS based on nursing needs. Further study is mandated to refine the system and to develop index score to estimate the necessary number of nurses for adequate care.

Purpose: This study was performed to develop a valid and reliable Pediatric Patient Classification System (PPCS). Methods: The study was conducted in a children's hospital which included various ward settings. Content validity was analyzed by Delphi method and to verify intraclass correlation reliability, 7 nurse managers and 29 staff nurses classified 216 patients. To verify construct validity, the staff nurses classified 216 patients according to PPCS comparing differences by age, days of stay, type of stay and medical department. Results: The developed PPCS has 12 categories, 55 nursing activities and 80 criterions. High agreement among nurses (r=.90) suggested substantial reliability. Construct validity was verified by comparing differences in age, days of stay, type of stay and medical department (p<.05). The entire patient group were classified to four groups using PPCS. Conclusion: The findings suggest that PPCS would be a useful tool for estimating nursing demands related to medications and the complexity of pediatric patients.

The purpose of this study was to find out the effects of education for prevention of child sexual abuse (rape, sexual assault, sexual harassment etc.) of children in elementary school students. Incidences of sexual abuse in Korea is ranked 3rd in the world. Sexual abuse of children, particularly, is one of the most serious crimes that affect both the victimized children and their family. The issue related to sexual abuse of children has been ignored, and there is a lack of systematic education to prevent sexual abuse of children either for children or their parents. Furthermore, lack of knowledge and poor attitude of teachers limit their capability to provide education for sexual abuse prevention to the children. The education for teachers may change their knowledge of the sexual abuse and their attitude toward it and improve their capability in helping children. 223 Korean elementary school students participated in this study. Research design was One group pretest-posttest design. The education given to the subjects included 4 hours through the multimedia CD-ROM on the prevention of child sexual abuse. The education program was developed based on a survey. School- aged- children, parents, and teachers were interviewed to reveal their educational needs based on their experiences related to sexual abuse. This program includes such as safety education, crisis management, resource person(s), and phone numbers of available hospital. Data was collected before, during and after the education was given to the subjects, to assess their knowledge and attitude toward child sexual abuse. Two instruments were used in this study. Using Cronbach's ALPHA validity and reliability of 2 instruments were tested. The analysis of the data was done with Window SPSS 10.0 for descriptive statistics, t-test and ANOVA. The results of this analysis were as follows : 1. After education, the score of school nurses knowledge was higher than before(t= -4.677, p=0.000). 2. After education, the score of school nurses attitude was not higher than before (t=0.131, p=0.896). The findings from this study will provide an intervention strategy for prevention of sexual abuse in children.
Purpose: This study aimed to describe nurses' perceived needs and barriers to pediatric palliative care (PPC). Methods: Mixed methods with an embedded design were applied. An online survey was conducted for nurses who participated in the End-of-Life Nursing Education Consortium- Pediatric Palliative Care (ELNEC-PPC) train-the-trainer program, of whom 63 responded. Quantitative data were collected with a survey questionnaire developed through the Delphi method. The 47 items for needs and 15 items for barriers to PPC were analyzed with descriptive statistics. Qualitative data were collected through open-ended questions and analyzed with topic modeling techniques. Results: The mean scores of most subdomains of the PPC needs were 3.5 or higher out of 4, and those of PPC barriers ranged from 3.22 to 3.56, indicating the items in the questionnaire developed in this study properly reflect each factor. The needs for PPC were divided into 4 categories: "children and adolescents," "families," "PPC management system," and "community-based PPC." Meanwhile, PPC barriers were divided into 3 categories: "healthcare delivery system," "healthcare provider," and "client." The keywords derived from the topic modeling were perception, palliative, children, and education for necessities and lack, perception, medical care, professional care providers, service, and system for barriers to PPC. Conclusion: In this study, by using mixed-methods, items of nurses' perceived needs and barriers to PPC were identified, categorized, and weighted, and their meanings were explored. For the stable establishment of PPC, the priority should be given to improving perceptions of PPC, establishing an appropriate system, and training professional care providers.

Purpose: The purpose of this study was to evaluate the effect of illustrated children's book in providing information about tonsillectomy to improve post tonsillectomy pain control and compliance to treatment for preschoolers. Methods: The design of the study was a quasi-experimental, post-test only design. Subjects were preschoolers aged between 3 and 7 years old who were admitted to a hospital for tonsillectomy. A group of 25 children were assigned to an experimental group, and another 25 were assigned to a control group. The data were analysed using SPSS 12.0 program. The homogeneity between two groups was examined using $x^2-test$. The hypotheses testing was conducted using t-test. Results: The experimental group had lower pain score than control group at 8 hour after tonsillectomy (p<.05). Pain score was also lower in the experimental group after 24 hour after the surgery (p<.05). The experimental group had higher pain questionnaire score than control group at 8 hours after surgery, however the difference was not significant. The experimental group showed better compliance to treatment than the control group (p<.05). Conclusion: Providing illustrated children's book for the purpose of providing information about tonsillectomy was effective intervention for the control of post tonsillectomy pain and improvement of compliance to treatment for preschoolers. Illustrated children's book could be applied as a nursing intervention to relieve post tonsillectomy pain and improve compliance to treatment for preschoolers.

Purpose: This study was performed to verify reliability and validity of the Korean Pediatric Patient Classification System (KPPCS) and estimate nursing time conversion index. Methods: The study was conducted in 9 children's hospital which included various areas and size of bed settings. To verify intraclass correlation reliability and construct validity, staff nurses and nurse managers of 21 wards classified 575 patients according to KPPCS comparing differences by age, days of stay, type of stay and medical department. Direct and indirect nursing time of 575 patients were measured by 284 nursing staffs by stopwatch observation and self reports for 24 hours. Results: KPPCS has 12 categories, 55 nursing activities and 80 criterions. High agreement among nurses (r=.91, p<.001) suggested substantial reliability. Construct validity was verified by comparing differences in age, days of stay, type of stay and medical department (p<.05). The correlation of nursing time and classification score was also statistically significant (r=.59, p<.001). The nursing time conversion index was 10.78 minutes per 1 classification score. The entire patient group were classified to four groups using KPPCS. Conclusion: The findings suggest that KPPCS would be a useful tool for estimating nursing demands related to the complexity of pediatric patients.
