Family Phenomena and Characteristics by Social-economic Charateristics of the Family (가족의 사회경제적 특성에 따른 가족현상 및 가족특성)

  • Objectives: The purpose of this study was to identify the family phenomenon and characteristics by ICNP such as family shape, life standard. area, and development stage. Method: 1. Sample size was 115. 2. Data analysis method included frequency analysis including ratio. Results: 1. On the distribution of the family phenomena by family shape, the nuclear family showed a higher ratio on the distribution in family communication and the unhealthy life style. The nuclear family was related to children andhad a higher ratio on the inadequate care management of the sick member. The extended family showed the highest ratio on the inadequate care management of sick member. 2. On the distribution of the family phenomena by family life standard, the family with medium life standard showed a higher ratio on the inappropriate family coping, whereas the family with lower life standard appeared to have a higher ratio on the inadequate care management of the sick member. 3. On the distribution of the family phenomena by area. the large city area showed the highest ratio on the inappropriate family coping, the medium and small city area appeared to have the highest ratio on the unhealthy life style. The county area showed the highest ratio on the inadequate care management of sick member. 4. On the distribution of the family phenomena by development stage, the family in rearing period showed a higher ratio on the inappropriate family coping and the lack of family intimacy. Families with preschool children showed a higher ratio on the unhealthy life style, and families with school age children showed the highest ratio on the inappropriate family coping. Families with adolescents appeared to have the highest ratio on the disturbance in family communication, and families with launching young adults showed the highest ratio on the inadequate care management of sick member. 5. On the distribution of the family characteristics by family shape, the nuclear family showed a higher ratio on the family characteristics such as less communication chances among family members and neglect of general child rearing, whereas the third generation family appeared to have a higher ratio on the characteristics such as overburden of housewife's role and short of caring among family members. 6. On the distribution of the family characteristics by family life standard, the family of medium life standard showed higher ratio on the family characteristics such as a few communication chance among family and overburden of housewife's role, and the family of lower life standard appeared to higher ratio on the family characteristics such as short of caring among family members. 7. On the distribution of the family characteristics by area. the large city area showed a higher ratio on the family characteristics such as overburden of housewife's role and neglect of general child rearing. The medium and small city area appeared to have a higher ratio on the family characteristics such as less communication chancec among family members. The county area showed a higher ratio on the family characteristics such as short-term care among family members. 8. On the distribution of the family characteristics by development stage, the family with rearing period showed a higher ratio on the family characteristics such as neglect of general child rearing. Families with preschool children showed a higher ratio on the family characteristics such as less communication chancec among family members. Families with school age children showed a higher ratio on the family characteristics such as overburden of housewife's role. Families with adolescents appeared to have a higher ratio on the family characteristics such as less communication chances among family members. Families with launching young adults showed a higher ratio on the family characteristics such as less communication chances and short-term care among family members.

  • A serious disease in a family influences the entire family member given the fact that the members closely interact with each other. Especially in terms of pediatric nursing, study on family gains importance as the need to care of families whose children with developmental disabilities and chronic disease This study was done based on The Resiliency Model of Family Adjustment and Adaptation(McCubbin, 1991) is intended to examine the stress of parents whose children suffer from cleft lip or /and cleft palate. It also helps them to cope with the stress and analyze the relationship between the stress and coping This study used Family Inventory of Life Events and Changes (FILE) and Coping Health Inventory for Parents(CHIP) for measuring family stress and coping. The two instruments are revised to fit the social and cultural environment of Korean culture. Data collection was done from April 18, 1996 to May 18, 1996 at 8 University medical centers located in Seoul. Those who answered questionnaires were 84 parents whose children have cleft lip or /and cleft palate. SPSS PC+ was used to analyze the data collotted. Programs used for data analysis were t-test, ANOVA, Pearson correlation coefficient. The study is summarized as follows .1. The average score of family stress is 10.46(percentage of the full score 24.90) and 'finance and business strains'(3.25), and 'intrafamily strains'(2.65) ranked the highest. The average score of family's coping is 1.93, which is close to the answer of' moderately helpful' and they are measured to put their utmost efforts to' intergration and cooperation of family and optimistic definition on the situation'. 2. There is no significant statistical correlation between the family stress and coping. 3. Mothers show more stress than fathers in the parts of 'illness and family care strains' and 'losses'(t〓-2.34, t〓-2.32, p<.05). 4. Fathers show more willingness to cope with the stress than mothers do in the parts of' seeking social support','self-esteem','emotional comfort' 5. Mothers are more stress than fathers in the parts of family stress and its coping with it by usual traits(t〓-2.78, p<.05). Parents with religion are measured to cope more willingly than those who are not 6. Income of a family shows positive correlationship with family coping (r〓.28, p<.05). The study shows that gender difference is significant variable in studying on family stress and coping. Mothers get more stress than fathers, which has much to do with the fact that they are in charge of raising children and keeping houseworks. Accordingly, managing family crisis and its survival can be induced by giving support for the mothers, studying fathers including the rest of the family members and giving nursing care and arbitration ; religious background is also considered to be one of the important factors in family stress , judging from the relationship between family income and family's coping, caring given to suffering children is needed on societal levels. The above considerations bring up the need to have a longitudinal study of children with congenital anomaly including cleft lip or /and cleft palate and their families about family stress and coping. Resiliency programs on family system and their effectiveness and the relationship between the enlarged families with social and cultural values reflecting Korean tradition are also needed to be studied.

THE CHARACTERISTICS OF CHILD AND ADOLESCENT INPATIENTS WITH SEVERE OBSESSIVE-COMPULSIVE DISORDER (심각한 소아 ${\cdot}$ 청소년 강박장애로 입원한 환아들의 특징)

  • Objectives : Child and adolescent onset obsessive compulsive disorder(C-OCD) is known to be associated with poor drug response, high comorbid rate and strong genetic tendencies. Till now studies for C-OCD have been very rare in Korea. We conducted this study to investigate the informations about clinical features, familial psychiatric loading, treatment profiles and course of server C-OCD from the retrospective analysis of inpatient data of Seoul National University Children's Hospital. Method : Retrospective chart review and data analysis was performed. Twenty(male 16 : female 4) patients with final C-OCD diagnosis by DSM-IV at discharge from 1994 to 2002 were found and their medical chart, psychological data, family interview data and nursing reports were collected and analyzed. Results : 1) The sex ratio of C-OCD was male dominant(4:1). 2) Phenomenological, most common obsession was pathologic doubt, contamination fear, followed by aggressive obsession, need for symmetry, sexual obsession, most common compulsion was checking and washing, followed by breathing, movement, symmetry, repetitive asking, hoarding, mental compulsion. 3) Most common comorbid diagnosis was depression. Other axis-I diagnosis associated OCD were anxiety disorder, tic disorder, conduct and oppositional defiant disorder and psychosis. 4) Regarding psychiatric familial loadings, 17 patients(85%) had relatives with psychiatric disorders, OC-spectrum disorders(OCD or OCPD) were found in 9 patients(45%). 5) The majority of patients(75%) have received SSRI and antipsychotics treatment. The response rate above 'moderate improved" by CGI was 75%. 6) During follow-up period in outpatient clinic, five patients(25%) showed continuous complete remission, 10 patients (50%) did residual symptoms with chronic course. Conclusion : This seems to be the first systemic investigation of severe pediatric OCD patients in Korea. The children & adolescents with severe OCD in inpatient-setting showed the high comorbid rate, familial psychiatric loading, and combined pharmacotherapy with antipsychotics, As for symptoms, high rate of aggressive-sexual obsession and atypical compulsions like breathing and moving was reported in this study. Severe pediatric OCD patients, however, responded well to the combined SSRI and antipsychotics regimen.

Influencing Factors on Prenatal Attachment (임부의 산전애착에 영향을 미치는 변인)

  • This study is to investigate the main factors influencing prenatal attachment. The research method is on the basis of descriptive correlational study, and data collecting period was from 11 to 29, November, 2003. The objects of this study were 100 pregnant women of two OBGY Hospitals in Masan. Using the SPSS/WIN 10.1 program the data were analyzed by t-test, ANOVA, Pearson correlation coefficient, stepwise multiple regression analysis. The result of this study was as follows : The mean score of prenatal attachment was 2.64, the mean score of physical discomforts, 1.61, the mean score of husband-wife attachment, 3.28, the mean score of psychosocial adjustment of pregnancy, 1.82. The degree of prenatal attachment according to general characteristics was significantly difference according to religion. The degree of prenatal attachment according to obstetrical characteristics was significantly difference according to Lamaze class, Taekyo. Significant correlations of prenatal attachment was found in both psychosocial adjustment of pregnancy, Taekyo, husband-wife attachment, Lamaze class, religion. Stepwise multiple regression analysis showed that 52% of the variance in prenatal attachment was significantly accounted by psychosocial adjustment of pregnancy(34%), Taekyo(6%), husband-wife attachment(5%), Lamaze c1ass(5%), and religion(3%). Further findings of this study support the development of creative strategies to enhance positive attachment relationship for pregnant woman.

Knowledge of Nocturnal Emissions in Late Elementary School Boys in Incheon (일지역 후기 학령기 남자 아동의 몽정에 대한 지식 정도)

  • Purpose: The purpose of this study was to determine the knowledge of nocturnal emissions, or wet dreams and to provide information for developing a wet dream education program for elementary school boys. Method: The research design was a descriptive study through questionnaires. Data was collected from May 10, 2009 to May 20, 2009. A total of 197 late elementary school boys in Incheon, Korea participated in this study. Data was analyzed using the SPSS WIN 15.0 program. Result: The results of this study were as follows; Knowledge of wet dreams in late elementary school boys was low; The mean of the information about wet dreams was 14.00 on a 100 point scale; There were significant differences of knowledge regarding wet dreams according to the educational experience at 6 items and wet dreams experience at 12 items of total 21 items. This indicates that present education for wet dreams is not efficient and does not provide appropriate information when they need practical information. Conclusion: Self education learning program for wet dreams should be developed for late elementary school boys which includes different contents and time required for further study, evaluation and assessment.

Effects of Breastfeeding Interventions Program on Breastfeeding for Premature babies: A Systematic Review and Meta-analysis (미숙아를 위한 모유수유 중재 프로그램의 효과: 체계적 문헌고찰과 메타분석)

  • Purpose: This study is a systematic literature review and meta-analysis study conducted to identify the effects of breastfeeding intervention program on premature infants by integrating and analyzing the results of randomized controlled trials and non-randomized trials. Methods: The literature review process was based on the PRISMA (Preference Reporting Items for Systematic Reviews and Meta-Analyzes) guideline. Data retrieval and collection were conducted from May 25 to May 30, 2018, and the articles analyzed were all domestic papers retrieved from the database. Two reviewers independently select the studies and assessed methodological risk of bias of studies using the Cochrane criteria. The topics of breastfeeding interventions were analyzed using descriptive analysis and the effects of intervention were meta-analyzed using the R program. Results: Finally, eight papers were included in the systematic review and meta-analysis. The breastfeeding intervention program for premature infants showed a significant increase in the self-efficacy and the amount of pumping and baby's hight. Conclusion: This study has some limitations due to the few randomized controlled trials and non-randomized trials comparing breastfeeding for premature babies in Korea. Therefore, it needs to be integrated with the research conducted in other countries.

The Comparison of Self-leadership and Self-care in Primigravida and Multigravida (초임부와 경임부의 자기리더십과 자가간호 비교)

  • Purpose: This study investigates the degree of self-leadership and self-care in primigravida and multigravida. Method: For this study, the questionnaires were given to 90 primigravida, 95 multigravida who visit woman hospital in Taegu and collected during the period from 16th February to 26th March, 2004. Data were analyzed by SPSS Wins. Result: The degree of self-leadership and self-care in primigravida was higher than multigravida. The comparison of self-leadership and self-care by the demographic characteristics between primigravida and multigravida were as follows; there was statistically significant difference according to education in self-leadership; age, education, religion, occupation, economic status and type of family in thought self-leadership; age, religion and occupation in self-care. The comparison of self-leadership and self-care by the obstetric characteristics between primigravida and multigravida were as follows; there was statistically significant difference according to frequency of abortion in self-leadership, self-management and natural reward; frequency of abortion, period of pregnancy and attending of childbirth education in thought self-leadership; frequency of abortion and period of pregnancy in self-care. Conclusion: Primigravida in self-leadership and self-care would have higher than those of multigravida. Further study needs to be done to identify in primigravida and multigravida. Also it should be developed self-leadership promoting programs for multigravida.

Effects of a Breastfeeding Program for Premature Infants on Maternal Stress, Knowledge, and Breastfeeding Behaviors (미숙아 모유수유 프로그램이 미숙아 어머니의 스트레스, 모유수유 지식정도 및 실천에 미치는 효과)

  • Purpose: This study aimed to develop a breast feeding program for mothers of premature infants. The program was applied and its effect was evaluated on stress, knowledge and breastfeeding behaviors of premature infant mothers. Method: This study employed a one group pre- and post-test quasi-experimental design. The data were collected from 17 participants from August 15 to October 24, 2004, at a N1CU at D University Medical Center located in Susan, Korea. Data were analyzed by means of frequency, SD, and t-test using SPSS version 10. Result: After the program, participants significantly showed a greater level of knowledge than before the program (t=-5.750, p=.000). No statistically significant differences in stress level scores were found (t=1.453, p=.153). The breastfeeding practice rate was 94.1%: Conclusion: This educational program using a handbook and video had a positive effect on increasing knowledge and practice of breastfeeding among premature infant mothers. It is suggested that this study be replicated with a larger sample size to compare group responses.

The Effect of a Preparation Method using Hospital Play on Anxiety and Role Behaviors of Children Undergoing Tonsillectomy (병원놀이 교육이 편도선 적출술 아동의 수술 전 불안과 수술 후 환아 역할행위에 미치는 영향)

  • Purpose: The purpose of this study was to examine the effect of a preparation method using hospital play on anxiety and role behaviors of children undergoing tonsillectomy. Method: Twenty-eight children aged 5 to 7 years who were admitted for tonsillectomy were assigned to either an experimental group or a control group purposefully. The preparation method included provision of information using hospital play. The data were analyzed using Mann-Whitney test and Chi-square test. Result: There was no Significant difference in anxiety before surgery between the experimental and control groups. Also, there was no Significant difference in role behaviors on the 1st and the 2nd day of surgery. Conclusion: Even though there was no statistical significance, the result showed that the preparation method had some positive effect on children and parents. It is needed to develop systematic preparation methods for children undergoing surgery to enhance their coping.

  • The purpose for this study is to identify the effects of nonnutritive sucking and facilitated tucking as a pain management during painful heelstick procedure. This study was a repeated measurement design. Data were collected from Sep. 15, to Oct. 20, 2005. According to the criteria twenty seven high risk infants were selected from the NICU of university hospital. The behavioral state were evaluated with the PIPP(Prematures Infants Pain Profile. PIPP, heart rate, saturation were observed without versus with nonnutritive sucking and tucking care. Statistic analysis was conducted with a wilcoxon nonparametric test. The results of this study were as follows. Pina behavior responses in nonnutritive sucking and tucking cases were lower than without treatment(Z=-4.430, p=.000), Heart rate and heart rate recovery time in nonnutritive sucking and tucking cases were attenuate decrease in heart rate(Z=-2.694, p=.005) and statistical significant differences in periods (Z=-4.229, p=.000). But, Saturation was no significant differences(Z=-3.230, p=.000). In conclusion, the application of nonnutritive sucking and tucking as an pain relieg for high risk infants is nursing intervention in pain management ar heelsticks.
