Aim: To report epidemiologic and anatomoclinical transitions of inflammatory breast cancer (IBC) in Tunisia. Materials and Methods: Data including clinico-pathological data for 208 cases of T4d or PEV 3 non-metastatic breast cancer diagnosed between 2005 and 2010 were collected from patient records. Chi2 and Z tests were used to compare variables with two Tunisian historical series and a series about Arab-American patients. Results: Thirty three percent of our patients had their first child before 23 years of age and 56% had their menarche before 12 years, 75% never receiving oral contraception. Obesity was observed in 42% of women and IBC occurred during pregnancy in 13% of cases. Tumor grade was II-III in 90% of cases, HR was negative in 52%, HER2 was over expressed in 31% and invasion of more than 3 axillary nodes occurred in 18% of patients. We observed a pCR rate of 19% after neoadjuvant treatment (anthracyline-taxane used in 79%, trastuzumab in 27% ). Compared to historical Tunisian series (since 1996), IBC epidemiology remained stable in terms of median age, menopausal status and obesity. However we observed a significant decrease in median clinical tumor size and number of positive axillary lymph nodes. Comparison to IBC in Arab-Americans showed a significant difference in terms of median age, menopausal status, positivity of hormonal receptors and educational level. Conclusions: Our assessment of epidemiologic transition showed a reduction of clinco-pathological stage of IBC, keeping the same characteristics as compared to Tunisian historical series over a period of 14 years. Features seem to be different in Arab-American patients, probably related to migration, "occidentalization" of life style and improvement in socio-economic level.
This study analysed marital comflicts, related factors, and adjustments to those conflicts described by urban Korean women. The purpose was to discover directions for resolution that would enhance the helpfulness of volunteers or prfessionals working with women in counselling settings. The investigator interviewed 20 married women, all mothers and housewives living in Seoul, attending marriage encounter programs and the Lifeline telephone counselling service. An interview schedule using six open-ended questiona guided the data colleciton which took place between May 31 and December 29, 1988. Content analysis of the data revealed, in order of reported frequency, the following areas of conflict: relationships between husband wife, relationships with children, relationships between the wife and her mother-in-Law, unsatisfied sexual needs, the husband's extra marital relationships, lack of self actualization and finacial matters. Analysis of the data suggested the following factors were related to the conflicts: reasons for marriage, traditional views about marriage and divorce including to accept a marriage arranged by the parents, the husband's traditonal attitude toward the sex role of the wife, the husband's patriarchical authoritarianism, and his attitude toward the wife;s empolyment outside the home. The women thought these conflicts severely affected their relationships within the famaily, particularly with their children. Literature suggests links with such poor parent-child-family relationships and adolescent or young people's social problems. Volunteers or professionals working with married women in counselling situation need to be conscious of the possibility of these conflict during assessment, and direct their guidance toward such conflict resolution. The researcher makes several suggestions which include the need for the husband to give emotional supports to his wife, to recognize the worth of housework, to share the work of the home, to fine ways to enhance the wife's self-actualization and to ease dreary housework routines by prviding modern appliances.
Purpose: This study aimed to develop a scale to measure variables related to alcohol drinking prevention behavior in early elementary school, based on the theory of planned behavior. Methods: A scale was developed to measure variables related to alcohol drinking prevention behavior. Initial items for direct evaluation were constructed through a literature review, and those for belief-based indirect measure were generated through interviews with 30 second- and third-grade elementary school students. The collected data from 286 third-grade elementary school students were then subjected to item analysis, exploratory and confirmative factor analysis, criterion-related validity testing, and internal consistency assessment. Results: The final scale consisted of 35 items. Intention, attitudes, subjective norms, and perceived behavioral control explained 82.7% of the variance; behavioral beliefs, normative beliefs, and control beliefs explained 65.6% of the variance; and evaluation of outcome, motivation to comply, and power of control beliefs explained 72.8% of the variance. The confirmatory factor analysis indicated that the theoretical models had a satisfactory goodness of fit. Criterion-related validity was confirmed between the direct evaluation variables and the indirect measure variables (attitudes r=.64, p<.001; subjective norms r=.39, p<.001; perceived behavioral control r=.62, p<.001). Cronbach's α was .89 for the direct evaluation variables and .93 for the indirect measure variables. Conclusion: The scale developed in this study is valid and reliable. It could be used to measure and explain variables related to alcohol drinking prevention behavior in early elementary school.
Attention deficit hyperactivity disorder (ADHD) is a relatively common condition of childhood onset and is of significant educational and social concern in elementary school. Investigating the determinants of teachers' attitudes and behavior and their relative importance is crucial for improving teaching practices and initial teacher education for effective inclusion of ADHD children. Based on two hundred and twenty seven survey responses from elementary school teachers, having experienced in dealing with ADHD students, this paper examines the age, gender, career and training-hours effects on the knowledge, attitude, stress and inclusive education involving ADHD children. The regression analysis shows that the gender influences the level of stress and that the training hours has some impact on the knowledge about ADHD at 5% significance level. However, the number of ADHD children experienced, age, gender, career and training-hours were identified as the non-significant predictors of teaching behavior in relation to ADHD children. As a conclusion, a brief discussion concerning the critical needs of elementary school teachers is also presented.
The purpose of this study is to tender as the basic data for practice of environment on the consciousness and practice of life environment on technical teacher in middle school and colleage students. For this purpose, the data were collected by using questionnaire distributed to 102 technical teacher in middle school and 103 male colleage students in Jan. and Aug., 2000. The major findings of this research are as follows; 1. It was found that the teachers had consciousness in industrial environment pollution but the students has the causes of house or car. All of the subjects set to the environment information through T.V. programs and the efficacy is highest. Also the teachers set to the environment information through environmental organization and books. The teachers had concern of high level and higher than the students on the life environment. Specially the awareness of life environment was upper to the average and higher than students. 2. The assessments of the earth life environment was 69.17(M), the oceanic environment was highest and water environment was lower. The assessments of the Korea life environment was 60.67(M) were lower than the assessments of the earth life environment, all items were lower than earth lie environment. Generally leachers has positive assessments to earth and Korea life environment than male students. 3. The teachers have high practice apparence on the life environment than the students. Also the teachers had upper to the average on the practice degree, and the practice degree lower than practice awarences. But all items on practices degree was higher than student's.
This study was performed to assess the nutrient intake and dietary diversity of Korean children and adolescents using the Korean dietary pattern index developed in previous studies. For this study, 6,462 children and adolescents aged 7~18y who participated in the dietary intake survey (24h recall method) of the 2007~2013 KNHANES were sampled. The food items included in the Korean dietary index were jusik-ryu, guk/tang-ryu, gui/jjim-ryu, namul-ryu, yeomjangchaeso-ryu, jang-ryu and mitbanchan-ryu. All the subjects and both age groups (7~12y, 13~18y) were divided into quartiles. According to the results of this study, the range of the Korean dietary pattern score was 0~58 for all of the subjects and also in the 13~18 age group, and was 0~52 in the 7~12y age group. When the pattern score for each food group was compared across the quartiles of the Korean dietary pattern score, in all the subjects as well as in the 7~12y and 13~18y age groups, the pattern score for jusik-ryu and yeomjangchaeso-ryu was highest in Q1~Q4. or all the food groups, the mean pattern score was highest in Q4. These results suggested that the Korean dietary pattern score is highly associated with jusik-ryu including rice and yeomjangchaeso-ryu including kimchi. Accordingly, it is considered necessary to develop an index that reflects the characteristics of Korean cuisine and, at the same time, assesses the nutritional status and food consumption tendency of Korean children and adolescents.

This study was performed to explore the family support. the quality of marital relationship, and health promoting behaviors of the mature gravidas. and to validate the relationship between them. From November, 1999 to June. 2000. mature gravidas older than 35(N=82) were recruited from K University Hospital located in Seoul and Kyungki Province. Korea. Data was collected using a self-rating method and semi-structured interview. The questionnaires were: Perceived Social Support from Family developed by Procidano & Heller (1983). Quality of Marital Index by Norton (1983). and Health Promoting Life-style Profile by Walker, Sechrist, & Pender (1987), Interviews were conducted using the interview guide with open-ended questions related to health concerns and health promoting behaviors. Data was analyzed using a pc-SAS program for quantitative data. The levels of the perceived family support and the quality of marital relationship were relatively high. The relationship between perceived family support and quality of marital relationship, and health promoting life-style behaviors was statistically significant. Six themes emerged as primary concerns for the mature gravidas during childbearing: 'Concerns related to the fetal wellbeing', 'Concerns related to fetal education', 'Concerns related to the maternal physical health', 'Concerns related to the maternal emotional health', 'Concerns related to safe delivery' and 'Concerns related to child-rearing'. Nursing implications for practice include expanding prenatal assessment inventories to address the concerns and support system of mature gravidas.

Background: For children with cleft palates, surgeries at a young age are necessary to reduce feeding or phonation difficulties and reduce complications, especially respiratory tract infections and frequent sinusitis. We hypothesized that dexmedetomidine might prolong the postoperative analgesic duration when added to bupivacaine during nerve blocks. Methods: Eighty patients of 1-5 years old were arbitrarily assigned to two equal groups (forty patients each) to receive bilateral suprazygomatic maxillary nerve blocks. Group A received bilateral 0.2 mL/kg bupivacaine (0.125%; maximum volume 4 mL/side). Group B received bilateral 0.2 mL/kg bupivacaine (0.125%) + 0.5 ㎍/kg dexmedetomidine (maximum volume 4 mL/side). Results: The modified children's hospital of Eastern Ontario pain scale score was significantly lower in group B children after 8 hours of follow-up postoperatively (P < 0.001). Mean values of heart rate and blood pressure were significantly different between the groups, with lower mean values in group B (P < 0.001). Median time to the first analgesic demand in group A children was 10 hours (range 8-12 hr), and no patients needed analgesia in group B. The sedation score assessment was higher in children given dexmedetomidine (P = 0.03) during the first postoperative 30 minutes. Better parent satisfaction scores (5-point Likert scale) were recorded in group B and without serious adverse effects. Conclusions: Addition of dexmedetomidine 0.5 ㎍/kg to bupivacaine 0.125% has accentuated the analgesic efficacy of bilateral suprazygomatic maxillary nerve block in children undergoing primary cleft palate repair with less postoperative supplemental analgesia or untoward effects.
Background: Work and work environment have a critical influence on adolescent workers' health. They are subjected to more risks than adults. The aim of this study is to examine psychological health outcomes in adolescent workers in the areas of depression, somatization, anxiety, hostility, and negative self-concept, and to investigate any related factors. Methods: This is a descriptive and cross-sectional study. Research samples were collected from adolescent workers between 15 and 18 years old attending a 1-day mandatory education course at vocational training centers, working 5 days per week in small enterprises. Data were collected using the following instruments: Brief Symptom Inventory, Multidimensional Scale of Perceived Social Support, and Descriptive Characteristics of Children's Assessment Form. Results: The investigation covers 837 young workers, of whom 675 were males and 162 were females. The majority of the families had low incomes (68.1%). Overall, 33.5% of the adolescents had been hospitalized because of health problems. Their average weekly working hours were $78.1{\pm}10.7$. Almost 50% of adolescent workers scored above the mean average in the Brief Symptom Inventory, indicating serious pschological health symptoms. Those who scored high for hostility, depression, negative self-concept, anxiety, and somatization were between 45.4% and 48.9% of the sample. Logistic regression analysis was conducted to determine the underlying factors: a perception of "feeling very bad" health conditions was 2.07-fold whereas the rate of "no annual leave" was 0.73-fold, and both were found to be effective on psychological problems. Conclusion: In this study, it seems likely that psychological health problems are the result of multiple adverse factors including working conditions, annual leave, and health considerations.

Aplastic anemia (AA) is a serious hematologic disease characterized by hypocellular bone marrow and deficient production of erythrocytes, granulocytes and platelets. Serious complications such as uncontrolled bleeding and bacteremia can occur. A case of severe AA are presented with dental considerations. A 4-year-old boy had been referred from Seoul National University Hospital for dental examination before the hematopoietic stem cell transplantation (HSCT). Treatments were planned under general anesthesia, due to the poor compliance. Following medical consult, dental treatments were performed after platelet transfusion and antibiotic prophylaxis. Postoperatively, neither significant bleeding nor complictation was observed. On the time of the treatment planning. the anesthesiologist and dentist should perform a complete hematological assessment. It is imperative not only platelet counts but also other leukocyte counts are under safe boundaries. It is mandatory to follow strict aseptic precautios for all anesthetic and surgical maneuvers. In severe thrombocytopenic patients, platelet transfusion should be considered. Also, it is recommended to establish a good oral hygiene.
