학습장애를 가진 초등학교 남학생의 자아상 개념과 우울-불안 특성 조사 (SURVEY OF SELF-CONCEPT AND DEPRESSION-ANXIETY OF THE ELEMENTARY SCHOOL BOYS WITH LEARNING DISABILITIES)

  • 학습장애를 가진 86명의 초등학교 남학생과 52명의 정상적인 초등학교 남학생의 자아상과 우울, 그리고 상태-특성 불안에 관하여 피어스-해리스 자아상 척도, 소아우울척도(이하 CDI), 상태-특성불안 척도(이하 STAI)를 이용하여 조사하였다. 피어스-해리스 자아상 척도의 총점에서는 두 군 사이에 차이가 발견되지 않았으나, 지적 및 학업 상태 항목과 신체적 외모, 행복과 만족 소척도에서는 두 군 사이에 의미 있는 차이가 관찰되었다. 소아우울척도의 총점과 그 소척도 중 부적합함, 무쾌감증, 부정적인 자기 존중 항목에서 학습장애 아동이 정상 아동보다 더 높은 점수를 나타내었다. 상태불안은 정상 아동보다 학습장애 아동에서 높게 보고되었으나 특성 불안에 대해서는 차이를 보이지 않았다. 정상 아동에서는 나이에 따른 자아상과 우울척도, 불안 척도의 의미 있는 변화가 관찰되지 않았으나, 학습장애 아동에서는 피어스-해리스 자아상 척도와 그 소척도의 대부분이 통계적으로 의미 있게 감소하였고, 소아우울척도의 부정적 감정과 무쾌감증, 부정적 자기 평가, 상태 불안, 특성 불안 등은 연령과 의미 있는 양의 상관관계를 나타내어서 학년이 올라갈수록 자아상은 저하되고 우울과 불안을 느끼는 강도와 범위가 증가하는 양상을 보였다. 두 군 모두에서 소아우울척도는 피어스-해리스 자아상 척도의 총점과 그 소척도들과 의미 있는 역의 상관관계를 나타내었고, 상태불안과 특성불안과는 정의 상관관계를 나타내었다. 조사 결과 학업과 관련된 자아상의 문제와 자신이 남들과 당당히 맞설 수 있는 자신감의 문제가 학습장애에서 더 많이 나타났고, 나이가 들수록 자아상이 저하되며 불안과 우울은 증가하는 것으로 나타났다. 회귀분석 결과 학습장애 아동의 자아상을 설명하는 변수들로는 나이와 자아상 척도의 행동문제, 지적 및 학업상태, 불안, 인기도, 행복과 만족, 그리고 소아우울척도의 부적합함, 대인관계문제, 부정적 자기-존중 및 상태 불안인 것으로 드러나서 학습 장애 아동의 자아상은 학업 문제와 자신을 또래와 비교하여 겪는 스트레스로 인해 부정적인 영향을 받는다는 것이 확인되었다. 결론적으로 학습장애를 가진 초등학교 남학생은 정상 초등학교 남학생보다 자아상이 낮고 우울과 불안을 더 많이 느끼며, 이런 차이는 학년이 올라갈수록 커졌다. 학습장애에서 우울증과 불안 장애가 많이 동반되기 때문에 임상에서 학습장애 아동을 치료할 때 정서 장애의 동반 여부를 확인하고 이를 적극적으로 치료하는 것이 중요한 과제이다.

  • 소아, 청소년기에 발생하는 양극성장애는 주의력결핍/과잉행동장애, 행동장애 뿐만 아니라 불안장애도 중복진단 비율이 높다. 불안장애 중 강박장애, 사회공포증, 공황장애, 분리불안장애가 많이 공존한다고 보고 되었지만 불안장애와 양극성장애 발생사이의 시간적 관련성에 관한 전향적이면서 믿을만한 연구는 드물다. 본 저자는 양극성장애에 선행한 불안증상에 대해 항우울제를 투여하였을 때 발생한 조증삽화 2예를 보고하고자 한다. 기분변화의 과거력이나 양극성장애의 가족력이 없어도 선행된 불안증상에 대해 항우울제를 사용할 때는 조증삽화의 가능성을 염두에 두고 조심스럽게 처방해야 될 필요성이 있으며 향후 소아, 청소년 양극성장애와 불안장애 사이의 시간적 연관성과 약물학적 치료 지침에 대해 포괄적이며 전향적인 연구가 필요할 것이다.

한국인 주의력결핍-과잉행동장애 아동의 세로토닌 수송체 유전자 다형성 (The Serotonin Transporter Gene Polymorphism in Korean Attention-Deficit/Hyperactivity Disorder Children)

  • Objectives : The aim of this study was to investigate the association between Korean ADHD patients and the l/s polymorphism of serotonin transporter(5-HTTLPR). Methods : The study sample consisted of 189 Korean ADHD children diagnosed by Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version-Korean Version(K-SADS-PL), both parents of ADHD children, and 150 normal children. DNA were extracted from the blood of all samples, and genotyping was done. Based on the allele and genotype information, not only the case-control analysis between ADHD and normal children but also the family-based association test among ADHD children and their parents. Transmission disequilibrium test(TDT) were performed for family-based associated test(number of trio=113). The results of the clinical rating and neuropsychological tests were compared according to the l/s genotype of ADHD children. Results : In case-control analysis, there were no statistically significant difference of l/s gene polymorphism between ADHD and normal children in various kinds of analysis condition. In family-based association study, TDT failed to detect linkage disequilibrium between l/s gene polymorphism and ADHD in whole ADHD families. However, in the families of ADHD inattentive type only(number of trio=23), I allele was transmitted more preferentially in the proband with ADHD even if the number of families was small(${\chi}^2$=4.57, p=.032). In the analysis of the results from the clinical scales and neuropsychological tests in ADHD children, the score of the Novelty- Seeking of ADHD children with l/l genotype was significantly lower than with the other genotypes(F=3.15, p=.047), and that of Self Transcendence was significantly higher(F=4.25, p=.017). Conclusion : The results of this study suggest there were no significant genetic association between the 5- HTTLPR gene polymorphism and Korean ADHD.

한국인 자폐스펙트럼장애에서 Savant Skill과 자폐증상의 연관성 (Relationship between Savant Skills and Autistic Symptoms in Korean Patients with Autism Spectrum Disorder)

  • Objectives: This study was conducted to examine the prevalence and characteristics of savant skills and the relationship between the savant skills and autistic symptoms in Korean ASD children. Methods: 141 ASD subjects participated in this study and they were divided in to two groups based on the presence or lack of savant skills. The domain scores and total scores of the K-ADI-R, K-ASDS and SRS were used for evaluating the ASD symptoms between the groups. Results: Memory (N=47) was the most prevalent savant skill in the savant ASD group (N=60). The savant ASD group had a statistically higher mean age and IQ score than did the nonsavant ASD group. Despite their high IQ profile, the savant ASD group showed a higher restricted, repetitive and stereotype behavior score on the K-ADI-R and higher language and cognitive scores on the K-ASDS than did the nonsavant ASD group. Conclusions: These results suggest savant syndrome in ASD might be related to the severity of some subdomain of autistic symptoms even though their IQ scores were higher than nonsavant ASD patients.

소아(小兒) 행동양식(行動樣式)과 소증(素症)의 관련성(關聯性)에 관한 탐색적(探索的) 연구(硏究) (A Study for Corelation between Action Type and Physical Symptoms in Children)

  • Objective : This study was designed for find corelation between behavior and physical symptoms in child. Physical symptoms appearing related with weak viscera are selected. And behaviors related to selected symptoms are compared that of children with character type defined by jung, c. G. Method: 497 children ( 3-11 years old ) who have visited two oriental clinic ( one is in Gang-Nam Gu Seoul (282 children), the other is in Joong-Rang Gu Seoul (215 children) ) from Jan. to Jul. 2006 were invested through the questionnaires, which were filled by their parents. These questionnaires have 61 yes-no type questions about behaviors on 12 situations, 36 yes no type questions about physical symptoms in 15 categories. 6 questions expected positive answer in child with weak spleen are categorized sp-q and 6 related weak lung, lu-q, 6 related weak kidney, ki-q. Numbers of positive answers in sp-q, lu-q and ki-q are counted so-index, lu-index and ki-index each. Corelation between behavior questions and 3 symptom indexes are measured and questions shown low significance level ( under 5% in pearson corelation, two-tailed ) are selected. Result: 15 behaviors related sp-index, 12 related lu-index, 12 related ki-index are selected and analysed. Conclusion : 1. Behaviors related sp-index is similar to that of Thinking Type children Grouped by jung, c. G.. 2. Behaviors related lu-index is similar to that of Sensation Type. 3. Behaviors related sp-index is similar to that of Feeling Type.

자폐스펙트럼장애에서 나타나는 퇴행의 임상적인 특징 (Clinical Characteristics of Developmental Regression in Autism Spectrum Disorders)

  • Pbjective : A significant proportion of children with autism spectrum disorders (ASD) have regression characterized by loss of previously acquired skills. The purpose of this study was to compare demographic, clinical characteristics and autism-related symptomatology of the children who have regression with children who don't have regression. Methods : The subjects with ASD and their unaffected siblings (SIB) were recruited from the Korean Autism Genetic Study Consortium. Typically developing children (TC) were volunteered from community. The subjects were administered the Korean version of Autism Diagnostic Interview-Revised (K-ADI-R) and the Korean version of Autism Diagnostic Observation Schedule (K-ADOS) to diagnose or exclude ASD. Regression was defined on the basis of K-ADI-R data. The Korean version of Vineland Adaptive Behavior Scale (K-VABS), Aberrant Behavior Checklist (K-ABC) and Social Responsiveness Scale (K-SRS) were obtained from their parents. Results : Regression occurred in 8.33% (n=14) of children with ASD (n=168). Any SIB (n=166) and TC (n=53) did not experience regression. Regression was associated with lower IQ and lower score of K-VABS. There was no difference in autism symptom severity and K-ABC, K-SRS scores, between children with ASD who experienced regression and who did not. Conclusion : Regression seems to be a distinctive feature of ASD. Regression is associated with cognitive and more general functions, rather than symptoms specific to autism.

자연경과 관찰에 의한 주의력결핍 과잉행동장애 아동에서 장기간의 약물치료가 성장에 미치는 영향 (A Study about Long-Term Effects of Attention-Deficit Hyperactivity Disorder Medication on Height and Weight in the Korean School-Aged Boys with Attention-Deficit Hyperactivity Disorder Based on Observation of Natural Course)

  • Objectives : This study was conducted naturalistically in order to observe the long-term effects of attention-deficit hyperactivity disorder (ADHD) medications on growth rates among Korean school-aged boys with ADHD. Methods : Participants in the study were boys with ADHD aged 6 to 11 years who have taken ADHD medication, methylphenidate (extended release) or atomoxetine. They attended scheduled visits monthly or bimonthly for clinical assessment with measurement of height and weight. In this study, 35 boys with ADHD (mean age at baseline=$7.90{\pm}1.77$ years ; mean age at endpoint= $12.54{\pm}1.91$ years) were included, with a mean follow-up period of 4.64 years (${\pm}1.62$ years), ranging from 2 to 9.7 years. Height, weight, and body mass index (BMI) measurements were converted to "age-corrected Z-scores" using data from Growth Charts provided by the Korean Center for Disease Control and Prevention from 2007. Results : Age-corrected endpoint growth parameters (height, weight, BMI Z-scores) did not differ significantly from the baseline values (height t=0.027 ; weight t=-0.61 ; BMI t=-1.86, in paired t-test). Especially high correlation was observed between the baseline and endpoint height Z-scores (r=0.876, p<.001), for which the coefficient of determination $r^2$ was 0.767, meaning that the amount of variability in endpoint height Z-scores explained by the baseline height Z-scores was 76.7%. Conclusion : Our results suggested that the long-term effects of ADHD medications on growth parameters to be tolerable in Korean school-aged boys with ADHD.

주의력결핍 과잉행동장애 아동의 약물치료 순응도에 영향을 미치는 요인 : 후향적 연구 (Factors Affecting Adherence to Pharmacotherapy in Children with Attention-Deficit Hyperactivity Disorder: A Retrospective Study)

  • Objectives: To identify the factors affecting long-term adherence to methylphenidate treatment in children with attention-deficit hyperactivity disorder (ADHD). Methods: A retrospective medical record review of 239 ADHD patients (mean age $9.3{\pm}2.6$ years, range 6.0-17.4 years) who had visited the child and adolescent psychiatry clinic at a university hospital, in Seoul, Korea from March 2005 to February 2008. Subjects were diagnosed as ADHD based on the criteria set forth in the Diagnostic and Statistical Manual of Mental Disorders 4th edition, text revision version (DSM-IV-TR) and underwent neuropsychological tests including the continuous performance test (CPT). Treatment discontinuation was defined as the last prescription date when the medication possession rate (MPR) became less than 0.80. Subjects were divided into three groups and labeled as Group I, non-adherence without pharmacotherapy, Group II, non-adherence with short-term pharmacotherapy, and Group III, adherence with long-term pharmacotherapy. Results: Ninety (37.7%) patients were grouped as non-adherent (Groups I+II) and 149 (62.3%) as adherent (Group III). The adherence group exhibited lower intelligence, higher symptom severity, and a higher number of comorbid psychiatric disorders than controls. The use of stimulants was significantly associated with long-term adherence to treatment. Additionally, the duration of interval between the date of the first visit and the date of the first prescription was positively associated with long-term adherence. Conclusion: About two-thirds of patients diagnosed as ADHD adhered to the treatment six months after the first visit. With respect to patient evaluation and the development of treatment strategies, factors affecting early drop-out and longer follow-up must be considered.

Factors on the Pathway from Trauma to Suicidal Ideation in Adolescents

  • Objectives: Suicide is the most frequent cause of death among Korean adolescents, and adolescents who have experienced trauma have an increased risk of post-traumatic stress disorder (PTSD) symptoms, depression, and suicide attempts. However, resilience and self-esteem are protective factors. We examined the effects of resilience and self-esteem on the relationship among traumatic experiences, PTSD symptoms, depression, and suicidal ideation. Methods: Middle-school students (n=403) completed questionnaires assessing traumatic experiences, PTSD symptoms, depression, suicidal ideation, resilience, and self-esteem. Path analysis was performed to investigate the mediating effects of PTSD symptoms, resilience, self-esteem, and depression on the relationship between trauma exposure and suicidal ideation. Results: Traumatic experience was positively correlated with PTSD symptoms, depression, and suicidal ideation. PTSD symptoms and depression were positively correlated with suicidal ideation. The relationship between traumatic experiences and suicidal ideation was mediated by PTSD symptoms, which had both direct and indirect effects on suicidal ideation; the indirect effect was mediated by resilience, self-esteem, and depression. Conclusion: Korean adolescents who had experienced trauma were more likely to develop PTSD symptoms, increasing their risk of depression and suicidal ideation. However, self-esteem and resilience may help protect against depression and suicidal ideation. Our findings could inform suicide prevention initiatives.

Autism and Beauty: Neural Correlates of Aesthetic Experiences in Autism Spectrum Disorder

  • Objectives: The purpose of this study was to investigate whether the neural activity of autism spectrum disorder (ASD) patients is different from that of normal individuals when performing aesthetic judgments. Methods: We recruited typical ASD patients without savant skills (ASD group, n=17) and healthy controls (HC group, n=19) for an functional magnetic resonance imaging study. All subjects were scanned while performing aesthetic judgment tasks on two kinds of artwork (magnificent landscape images and fractal images). Differences in brain activation between the two groups were assessed by contrasting neural activity during the tasks. Results: The aesthetic judgment score for all images was significantly lower in the ASD group than in the HC group. During the aesthetic judgment tasks, the ASD group showed less activation than the HC group in the anterior region of the superior frontal gyrus, and more activation in the temporoparietal area and insula, regardless of the type of images being judged. In addition, during the aesthetic judgment task for the fractal images, the ASD group exhibited greater neural activity in the amygdala and the posterior region of the middle/inferior temporal gyrus (Brodmann area 37) than the HC group. Conclusion: The results of this study suggest that the brain activation patterns associated with aesthetic experiences in ASD patients may differ from those of normal individuals.
