고령자들의 저작능력 및 잔존치수와 사회적 활동과의 관계 (Ability for chewing a social activity and connection with the life function of a senior citizen)

  • The purpose of this study is to widen the subject of research to the whole social activity, which is divided into the following four categories in the previous study on "Reality of social activity of senior citizen": 1. work 2. social participation and volunteer activity 3. learning activity 4. individual activity, and to clarify which category is most related to the masticatory ability. There were 57korean subjects in the survey: aged 65 through 85 (73.8$\pm$5.5), 81 women: aged 65 through 85 (74.3$\pm$5.3), a total of 138: aged 65 through 85 (74.1$\pm$5.4), in College for senior citizen of A city, Buddhist College for senior citizen of A city and senior citizen's welfare facility of A city who gave consent to the research The Japanese subject were 38 men: aged 65 through 85(75.3$\pm$5.4), 54 women: aged 65 through 85(75.5$\pm$5.2), a total of 92: aged 65 through 85(75.4$\pm$5.2) in women's division of F city Federation of Senior Citizens' Club, Otsuka division of B ward Federation of Senior Citizens' Club and S ward Federation of Senior Citizens' Club who gave consent to the research. The major findings were as follows: A social activity, individual activity were active in a social activity domain on a day so that people of Japanese advanced age had you, and writing ability was high, and, as for freezing, learning activity was active for people of Korean advanced age in a social activity domain so that writing ability was high.

융복합 시대 기구(비위관)사용여부가 경직형 뇌성마비 아동의 섭식능력과 조음능력에 미치는 영향 (The effects of feeding and articulation abilities in children with Spastic Cerebral Palsy with and without nasagastric tube in the era of fusion and convergence)

  • 본 연구는 경직형 뇌성마비 아동 30명을 대상으로 출생직후 섭식문제를 해결하기 위해 사용되는 비위관이 이후 아동의 섭식발달에 어떠한 영향을 주는지 살펴보았다. 2015년 7월 1일부터 8월 30일까지 뇌성마비 아동을 대상으로 조음능력을 평가하고 주양육자를 통한 섭식능력에 대한 설문조사를 실시하였다. 또한 섭식능력이 조음능력에 미치는 영향에 대해 살펴보았다. 수집된 자료는 SPSS 20.0을 사용하여 분석하였다. 연구결과 비위관의 사용여부는 조음정확도에 유의미한 차이를 보였다. 또한 섭식능력을 평가하는 문항 중 유동식 섭취가능여부, 섭식을 위한 특별한 방법과 도구사용여부, 깨물어 씹기 가능여부는 이후 조음정확도와 관련이 있는 것으로 나타났다. 본 연구 결과 초기 섭식능력의 일부 영역은 이후 조음능력에 영향을 미치는 것으로 보이며 예방적 차원의 접근과 조기개입이 적극적으로 이루어져야 할 것으로 생각된다.

한국 노인의 객관적·주관적 저작능력 평가에 영향을 미치는 요인 (Factors affecting objective and subjective masticatory ability assessment of Korean elderly people)

  • Objectives: The purpose of the study was to investigate factors affecting the assessment of objective and subjective masticatory ability in the elderly, and to evaluate masticatory ability assessment more accurately. Methods: A total of 112 participants were recruited after oral examination in senior citizen welfare facilities. The participants' masticatory ability was evaluated objectively (Mixing ability index; MAI), and subjectively (Key food intake ability; KFIA). Participants' general characteristics and oral health-related variables were also recorded. Based on masticatory ability assessment, participants were classified as either high or low. IBM SPSS Statistics Ver.23.0 was used for all analyses, including descriptive statistics, Chi-square test, Mann-Whitney U test, Spearman rank correlation, and Logistic regression analysis. Results: Higher masticatory ability was positively correlated with higher scores on MAI and KFIA. Additionally, there was a significant positive correlation between MAI and KFIA. When analyzing factors affecting objective and subjective masticatory ability assessments, Functional tooth units (FTUs) were revealed as a related factor. In subjective masticatory ability assessment, oral moisture, difficulty in chewing, and the Geriatric Oral Health Assessment Index (GOHAI) were also influential factors. Conclusions: In order to accurately assess masticatory ability, it is necessary to use both objective and subjective measures. Additionally, to improve the masticatory ability in the elderly, treatment should be provided to improve overall oral health and satisfaction.

뇌졸중 환자의 구강인두 기능회복을 위한 감각 자극의 효과 (The Effects of Sensory Stimulation for Ingestion in Dysphagic Patients with Stroke)

  • The purpose of this study is to identify the effects of a nursing intervention using sensory stimulation in dysphagic patients. Quasi-experimental with a nonequivalent control group pretest-posttest design was used. 15 patients of each group were assigned for this study, who were hospitalized in the oriental medicine hospital of K. University. Specific stimulation plans were devised based on a dysfunctional area of the subjects. For each modality, subjects were systematically stimulated for 30 minutes, in the experimental group prior to each meal and 3 times per day for a week. The data were analyzed by SPSS PC program using $X^2$-test, Wilcoxon Signed Rank test and Mann-Whitney U test. The results of this study are as follows : 1. The tongue control score of the experimental group is greater than that of the control group(u=52.0, p=.007). 2. The score of chewing ability, lip control, swallowing ability of the experimental group is greater than that of the control group. But no difference is statistically seen between the experimental group and control group. Therefore, this study shows that sensory stimulation using icing, quick stretch, etc., was effective in improving or oropharyngeal function in patients with dysphagia.

서울지역 여자 노인의 영양실태와 식생활 형태 - 노인대학을 중심으로 - (Nutritional State and Dietary Behavior of the Free-Living Elderly Women)

  • The issue of health and nutritional wellbeing becomes one of the utmost concerns for the welfare of the elderly in a society of increased aging populations. This study was conducted to assess nutritional state and to identify possible dietary factors that might influence health and nutritional state of the free living elderly women. Two hundred sixty two elderly women, aged 60-90 who are actively participating in a continuing education program for the elderly in Seoul area, were interviewed using a dietary habit questionnaire and a 24-hour recall. Anthropometric measurements such as heights, weights, body mass index, and tricep skinfolds and dental status were significantly different between the two age groups, <75 and ${\geq}$ 75 years of age. Sixty eight percent of the subjects were using nutritional supplements regularly, in the descending order of vitamin-mineral supplements(40%), botanicals(27.6%), tonic drinks(16%), and health foods(13.2%). For hot beverages, our study subjects drank coffee (33.6%) and green tea (17.9%) frequently, followed by doongulae, job's tears, citron and kyulmyung tea. With regard to micronutrients, vitamin C intake was found to be lower in the group aged over 75 compared to that below 75 (p<0.05). Calcium intake was significantly higher among the elderly with good dental status followed by the groups with gingivitis, denture, and missed teeth with poor chewing abilities (p<0.05). The amount of personal spending money was found to be a significant contributing factor (p<0.05) to the energy and nutrient intake in an individual. The results of the present study can be applied to the identification of the possible factors that might intervene the aging process, to the planning of the nutrition education program, and to the development of health food products for the promotion of health and nutrition of the elderly women.

치매와 구강 건강의 관계 (Association between dementia and oral health)

  • According to the burst of aged people, researchers have focused on aging-related diseases. Cognitive impairment including Alzheimer's disease (AD), one of the representative diseases related to aging, has no treatment option until now. Recently, it has been revealed that systemic inflammation plays a fundamental role in the pathogenesis of AD. Previous studies have suggested the association between poor oral health and cognitive impairment. Poor oral health can cause dental caries, chronic periodontitis, multiple tooth loss, and poor chewing ability, etc. Especially, periodontitis is a well-known chronic inflammatory disease and affects cognitive impairment directly and indirectly by inflammatory products mediators. Therefore, reduction of pathogenic microbial burden and inflammatory products by treating periodontitis can be a therapeutic modality to prevent cognitive impairment or to slow down the progression of it. Future studies are necessary to elucidate the causal relations and plausible mechanisms between poor oral health and cognitive impairment.

메밀복합분을 이용한 제면 특성 (Noodle Making Characteristics of Buckwheat Composite Flours)

  • To determine the optimum condition for buckwheat noodle making, we prepared noodle from com posite flours of buckwheat, wheat and corn starch, and those physical properties and sensory evalua tion were investigated. The weights of cooked noodle became lower with increasing buckwheat content level indicating that elution of its components to soup was high. Wheat flour in the composite flours probably caused to drop gelatinization temperature and maximum viscosity compared to buck wheat flours. Corn starch in the composite flours increase gelatinization temperature and maximum viscosity compared to buckwheat flours. From the result of farinogram, buckwheat flour lower water absorption ability, and increased dough development time compared to composite flours. Adhesiveness of the cooked noodle with wheat flour was decreased. Cohesiveness of the cooked noodle was decrea sed with increasing buckwheat flour level. In sensory evaluation, chewing and softness of noodle with 100% buckwheat flour was evaluated the best. In taste, BW1 and BSW1 noodles showed slightly higher preference although there was no significant differences.

우리나라 노인의 신체기능 요인과 차별경험의 관련성 연구: 노인실태조사(2014, 2017) 자료 분석 (Relationship between Physical Function Factors and Discrimination Experiences of the Elderly in Korea: The 2014 and 2017 National Survey on Living Conditions and Welfare Needs of Korean Older Persons)

  • Background: Supporting the elderly population is presented as a social issue, and it affects age discrimination, which forms a negative perception and avoids the elderly. Since age discrimination lowers the quality of life of the elderly and hinders social unity, it may be important to research related factors. This study examined the physical function as a factor that influences discrimination experiences and aimed to identify the relationship between physical function factors and discrimination experiences. Methods: In this study, we analyzed 20,225 elderly from the 2014 and 2017 survey of living conditions and welfare needs of Korean older persons, conducted nationwide among older than 65 years. Physical function factors are activities of daily living, instrumental activities of daily living, sight discomfort, hearing discomfort, chewing discomfort, and leg strength as factors of interest. We performed multivariable logistic regression that reflected survey characteristics, adjusting for socio-demographic factors, health related factors, and social-support factors. Results: The number of elderly who faced discrimination experiences was 1,175 (5.8%). The results showed that the odds ratio of facing a discrimination experience in the dependent group was significantly higher compared to the independent group when being transferring out of the room (4.05; 95% confidence interval [CI], 1.50-10.88) and difficulty in hearing (1.25; 95% CI, 1.05-1.49). Even with respect to chewing ability, which was significant in models 1 and 2, they face more discrimination experiences (1.30; 95% CI, 1.11-1.53). Conclusion: These research results indicate that an important strategy for preventing age discrimination is to understand the physical function conditions of the elderly and promote the physical functions of the elderly related to transferring, hearing, and chewing; it is necessary to develop a practical intervention plan that considers these aspects.

측두하악장애 환자에서 저작 효율 및 구강건강관련 삶의 질 평가 (Evaluation of Masticatory Efficiency and Oral Health Related Quality of Life in Temporomandibular Disorder Patients)

  • 본 연구에서는 최근 Sato 등이 고안한 mixing ability index(MAI)를 이용하여 측두하악장애 환자 중 통증이 있는 군과 무통성 관절잡음이 있는 군을 대상으로 객관적인 저작 효율을 정상인으로 구성된 대조군과 비교하고, 주관적인 평가 방법인 설문조사법 중에서 음식섭취가능지수(Food Intake Ability Index, FIA)와 저작 능력에 대한 주관적 인식도 평가(Visual Analogue Scale, VAS)를 이용하여 저작 효율을 평가하고, 구강건강과 관련된 삶의 질을 평가하기 위해 Oral Health Impact Profile(OHIP)-49K 설문조사를 시행하였다. 각 평가 방법 간의 상관성을 알아보고, 또한 통증군에 있어서는 치료 후 통증이 감소함에 따라 저작 효율의 변화 양상을 조사하여 다음과 같은 결과를 얻었다. 1. MAI, FIA, VAS는 통증군이 정상군에 비해 통계학적으로 유의하게 감소하였으며, 관절잡음군보다도 감소하여 통증군이 정상군과 관절잡음군에 비해 저작 효율이 떨어지는 것으로 나타났다(P<0.05). 관절잡음군에서는 MAI, FIA, VAS에서 정상군과 유의한 차이를 보이지 않았다. 2. 구강건강관련 삶의 질에 대한 설문지인 OHIP-49K의 경우 정상군에 비해 통증군과 관절잡음군 모두 통계학적으로 유의하게 증가하여 구강건강과 관련한 삶의 질이 떨어지는 것으로 나타났다. 3. 전체 대상자(71명)에서 객관적 저작 능력 평가 방법인 MAI와 주관적 저작 능력 평가 방법인 FIA와 VAS간에는 서로 유의성 있는 상관관계를 보였다(P<0.01). OHIP-49K에서도 MAI를 제외하고 모두 통계학적으로 상관관계가 있는 것으로 나타났다. 4. 관절잡음군에서는 FIA와 VAS에서만 통계학적으로 상관관계가 있는 것으로 나타났으며(P<0.01), 이외의 모든 항목은 상관관계가 없는 것으로 나타났다. 5. 각 평가 방법 간의 상관성에서 FIA와 VAS는 전체 대상자, 통증군, 관절잡음군 모두에서 상관관계가 있는 것으로 나타났다. 6. 통증군에서 치료 전에 비해 치료 후 통증 감소와 함께 VAS에서는 통계학적으로 유의성 있게 증가 하였다(P<0.05). 반면에 MAI에 있어서는 저작 효율이 증가하는 경향을 보였으나 통계학적 유의성은 없었다. 또한 FIA, OHIP-49K에서도 치료 전과 후 통증 감소에 따른 변화는 없는 것으로 나타났다. 이번 연구를 통하여 측두하악장애 환자에서 저작 효율을 감소시키는 주요인은 통증이며, 관절잡음은 저작 효율을 감소시키는 요인은 아니지만 삶의 질을 저하시키는 한 요인으로 작용한다는 것을 알 수 있었다. 따라서 측두하악장애 환자에서 저작 효율을 개선시키기 위해서는 우선적으로 통증을 치료하는 것이 가장 중요하며, 삶의 질을 향상시키기 위해서는 통증뿐만 아니라 관절잡음에 대한 관리도 필요할 것으로 사료된다.

국민건강영양조사에서 나타난 노인의 건강관련 삶의 질(EQ-5D)이 구강건강 문제에 미치는 영향 (Influence of Health-Related Quality of Life (EQ-5D) on Oral Health Problems in the Elderly in the National Health and Nutrition Survey)

  • 건강관련 삶의 질(EQ-5D)이 구강건강 문제에 미치는 영향을 규명하여 향후 노인 건강증진 프로그램 및 정책방향을 설정하기 위한 기초자료를 제공하고자 한다. 국민건강영양조사 제7기 원시자료를 이용하여 만 65세 이상 노인대상으로 계통추출법으로 진행 하였다. EQ-5D와 씹기 문제는 운동능력이 없는 그룹 통증불편이 있는 그룹, 불안우울이 있는 그룹에서 씹기 문제 오즈비(OR)가 증가하였다 (p<0.05). EQ-5D와 말하기 문제는 운동능력이 없는 그룹, 통증불편이 있는 그룹, 불안우울이 있는 그룹에서 말하기 문제 오즈비(OR)이 증가하였다 (p<0.05). 본 연구결과 건강관련 삶의 질 (EQ-5D)는 노인의 구강건강과 직결된다고 볼 수 있다. 따라서 노인의 EQ-5D을 증가시키기 위해서는 지역사회에서 구강보건교육 및 계속구강건강관리제도가 요구되어진다.
