기침으로 초래된 단순 늑골골절 1례 (Case Report of a Simple Rib Fracture Caused by Coughing)

  • Violent or severe persistent coughing is rarely associated with rib fracture. We report a rare case of a cough-induced rib fracture in a patient without any traumatic history or any other underlying disease. A 32-year-old female presented to the emergency department complaining of having had right-sided pleuritic chest pain for 5 days. She had a background of an 8-week coughing illness. A posteroanterior view of the chest radiograph showed no definite fractured line. Chest computed tomography revealed a subtle break cortical line of the 7th rib in the right-sided chest wall. Early identification of a cough-induced fracture of the rib by using computed tomography may avoid unnecessary further work-ups including laboratory examination and may lead to appropriate discharge instructions including rest and reassurance. The clinical presentations and radiologic findings of rib fractures caused by coughing are presented along with a review of the literature.

교통성 폐격리증 -1례 보고- (Communication Bronchopulmonary Malformation)

  • We have experienced a case of communicating bronchopulmonary malformation. A 35-year-old female patient was admitted for coughing with sputum. About eight years ago, she had treated for bronchiectasis with medication. Chest computed tomography revealed esophagopulmonary communication with upper esophageal dilatation and bronchiectasis of the left lower lobe. A about 3 mm diameteded abnormal feeding vessel from descending thoracic aorta to the left lower lobe was detected at operation. Division and closure of the communication between the esophagus and left lower lobe was performed. Communicating bronchopulmonary malformation is the rare form of pulmonary sequestion and chest computed tomography is one of the useful diagnostic methods for pulmonary sequestion.

  • Computed tomography (CT) plays an important supplementary role in the evaluation of patients with heart disease. CT can be used to evaluate the aorta, pulmonary artery, pulmonary vein, cardiac chambers, coronary artery, valves and systemic veins (superior vena cava, inferior vena cava and hepatic veins). The "Learning Objectives" describe the normal anatomy and typical pathological conditions seen on axial scans and reformatted images from CT in patients with heart disease, focusing focus on frequent, fatal, and rare but characteristic diseases encountered in routine practice.

  • Purpose: This was conducted to assess the need for conventional radiographs of the thoracolumbar spine for routine screening of trauma patients who revealed no spinal trauma on chest and abdominal computed tomography (CT). Methods: We performed a retrospective review of the medical records of trauma patients who underwent conventional radiographs of the thoracolumbar spine following a chest and abdominal CT that revealed no spinal trauma. Results: Two hundred seventy-five trauma patients underwent conventional radiographs of the thoracolumbar spine following a chest and abdominal CT that revealed no spinal trauma. In 274 of the cases, the thoracolumbar spine series was also negative. Conclusion: CT of the chest and abdomen is an adequate evaluation of the thoracolumbar spine in trauma patients who require routine thoracolumbar spine screening, making subsequent conventional radiographs of the thoracolumbar spine unnecessary.

  • 추후 관찰이 가능하였던 26례의 환자들이 평균 9개월의 짧은 생존기간을 보이는 것으로 보아 환자들에게 고통을 적게 주고 효과적인 비용의 검사를, 즉 흉부 방사선 사진, 복부 초음파, 흉부 전산화 단층 촬영, 복부-골반 전산화 단층 촬영, 기관지 내시경, 소화기 내시경 등의 순서로 진단적 접근을 시도하는 것이 원발병소를 찾는데 도움이 될 것으로 사료된다.

Spontaneous Disappearance of a Pericardial Cyst: Case Report and Literature Review

  • Pericardial cysts are rare benign anomalies generally discovered as incidental findings on radiographic images. Rarely, pericardial cysts cause symptoms and may lead to complications. A 56-year-old woman presented to the emergency department for mild chest pain. A cardiovascular and respiratory examination revealed no abnormalities, while a chest X-ray and subsequent thoracic computed tomography (CT) showed a pericardial cyst. The patient refused both percutaneous treatment and thoracic surgery. Three years later, a thoracic CT scan showed that the pericardial cyst had disappeared. Although the spontaneous resolution of these lesions is rare, this article highlights the possibility of conservative management in select cases.

Analysis of Aspiration Risk Factors in Severe Trauma Patients: Based on Findings of Aspiration Lung Disease in Chest Computed Tomography

  • Purpose: The present study will identify risk factors for aspiration in severe trauma patients by comparing patients who showed a sign of aspiration lung disease on chest computed tomography (CT) and those who did not. Methods: We conducted a retrospective review of the Korean Trauma Data Bank between January 2014 and December 2019 in a single regional trauma center. The inclusion criteria were patients aged ≥18 years with chest CT, and who had an Injury Severity Score ≥16. Patients with Abbreviated Injury Scale (AIS)-chest score ≥1 and lack of medical records were excluded. General characteristics and patient status were analyzed. Results: 425 patients were included in the final analysis. There were 48 patients showing aspiration on CT (11.2%) and 377 patients showing no aspiration (88.7%). Aspiration group showed more endotracheal intubation in the ER (p=0.000) and a significantly higher proportion of severe Glasgow Coma Scale (GCS) (p=0.000) patients than the non-aspiration group. In AIS as well, the median AIS head score was higher in the aspiration group (p=0.046). Median oxygen saturation was significantly lower in the aspiration group (p=0.002). In a logistic regression analysis, relative to the GCS mild group, the moderate group showed an odds ratio (OR) for aspiration of 2.976 (CI, 1.024-8.647), and the severe group showed an OR of 5.073 (CI, 2.442-10.539). Conclusions: Poor mental state and head injury increase the risk of aspiration. To confirm for aspiration, it would be useful to perform chest CT for severe trauma patients with a head injury.

개선된 가변형 능동모델을 이용한 흉부 컴퓨터단층영상에서 폐 실질의 분할 (Image Segmentation of Lung Parenchyma using Improved Deformable Model on Chest Computed Tomography)

  • 흉부 컴퓨터단층영상에서의 얻어진 폐 영상은 볼륨과 형태 등의 정량적인 정보들로서 진단과 수술 계획 등에 있어서 필연적 정보를 제공한다. 일반적인 영상분할은 이미지를 구성 요소영역이나 목적물에 따라 나누는 방법이다. 그러나 재분할을 하는 단계에서 최종영상은 에너지 최소화를 해결하는 정도에 의존하며, 분할은 응용대상의 관심 영역에서 객체나 물체의 경계에서 정지하게 된다. 가변형 능동모델은 컴퓨터 비젼, 영상처리 분야에서 광범위하게 사용되고 있다. 또한 영역 분할은 현재까지 많은 연구가 되고 있으며, Xu에 의해서 GVF라는 새로운 형태의 외부힘이 제안되고 있다. 본 논문에서 제안하는 알고리듬은 흉부 컴퓨터단층영상에서 실질을 자동 분할하기 위해서 에너지 최소화 방법을 사용하고, 영역분할을 위해 개선된 가변형 능동모델을 제안한다. 알고리듬은 정확한 영역분할을 위해서 기존 방법과 다른 개선된 외부힘을 정의하는 것이다. 임상의 실험은 흉부 컴퓨터단층영상에서 진단에 필요로 하는 폐 실질의 분할이 성공적인 결과를 나타내었다.

흉부 전산화 단층촬영을 이용한 혈흉의 정량분석 (Quantitative analysis of hemothorax by computed tomography)

  • Computed tomography[CT is an effective technique for the evaluation of the thorax following blunt trauma. To evaluate multiply injured 30 patients who were diagnosed as hemothorax in emergency room, computed tomography of thorax was done. The thickness of slice was one centimeter and the entire pleural cavity from the apex to the costophrenic angle was included in the evaluation. Integration and addition of the hemothorax area for each CT slice was made and amount of blood in the pleural cavity was estimated. The slice which showed largest area of hemothorax was selected and the height and width of the hemothorax area were measured. The number of slices which showed radiographic evidence of hemothorax was counted. Regression analysis was done and measured amount of hemothorax, the height and width of the hemothorax area for each slice and number of slices were put as variables. And following equation was derived. V=108.3A-0.8B-7.4C+84.7 [R2=0.74 [ V: amount of hemothorax, A: height, B: width, C: number of slices Total amount of blood from thoracic drainage was compared to the measured amount by computed tomography and the relation between the two values was statistically significant.[p=0.001 In conclusion, quantitative estimation of size of hemothorax was possible by the above equation and the process was very helpful for determination policy of treatment of individual patient.

흉부 외상 환자에서 일반흉부촬영과 비교한 흉부단층촬영의 진단적 유용성 (Diagnostic Usefulness of Computed Tomography Compared to Conventional Chest X-Ray for Chest Trauma Patients)

  • Purpose: Early diagnosis and management of therapeutic interventions are very important in chest trauma. Conventional chest X-rays (CXR) and computed tomography (CT) are the diagnostic tools that can be quickly implemented for chest trauma patients in the emergency department. In this study, the usefulness of the CT as a diagnostic measurement was examined by analyzing the ability to detect thoracic injuries in trauma patients who had visited the emergency department and undergone CXR and CT. Methods: This study involved 84 patients who had visited the emergency department due to chest trauma and who had undergone both CXR and CT during their diagnostic process. The patients' characteristics and early vital signs were examined through a retrospective analysis of their medical records, and the CXR and the CT saved in the Picture Archiving Communication System (PACS) were examined by a radiologist and an emergency physician to verify whether or not a lesion was present. Results: Pneumothoraxes, hemothoraxes, pneumomediastina, pulmonary lacerations, rib fractures, vertebral fractures, chest wall contusions, and subcutaneous emphysema were prevalently found in a statistically meaningful way (p<0.05) on the CT. Even though their statistical significance couldn' be verified, other disorders, including aortic injury, were more prevalently found by CT than by CXR. Conclusion: CT implemented for chest trauma patients visiting the emergency department allowed disorders that couldn' be found on CXR to be verified, which helped us to could accurately evaluate patients.
