Factors influencing the community care satisfaction of the urban elderly focusing on the outreach community health service in Seoul (서울시 찾아가는 동주민센터 방문건강관리 사업의 만족도 영향요인)

  • Purpose: This study aimed at identifying the factors affecting the service satisfaction of urban elderly, focusing on the outreach community health service in Seoul, and suggesting policy directions for the successful implementation of community care. Method: Individuals aged 65 and 70 who used the outreach community health service from July 2017 to June 2019 were eligible for the survey. A total of 2,028 people were sampled using a proportional allocation method for each autonomous district in the survey which covered 25 districts. A multi-level logistic regression analysis was conducted, taking into account the individual's socioeconomic level, health status, type of service provided, and the healthcare-related environment and service provision period of the autonomous district. Result: The results revealed that the health status of the urban elderly, the type of services provided (health screening, linkage to community health center and clinic/hospital, medical checkup results counseling, frailty evaluation), and personal experience of the service were the major factors associated with the satisfaction with the outreach health services. Conclusion: The development of customized health services based on the close relationship between visiting nurses and the elderly may be considered to promote a sustainable community health care model.

A Study on the Harmfulness of Silicon Oxide Dust and Measures for the Work Environment Improvement in Construction Sites (건설현장에서 발생하는 산화규소분진의 유해성 및 작업환경 개선대책에 관한 연구)

  • Purpose: Although the working environment is measured at the construction site, only a few noise and vibration that are typically exposed for each process are performed without measuring the working environment by segmenting the exposed harmful factors. Therefore, it is intended to find the harmfulness of silicon oxide dust, which is most exposed at construction sites, and the complementary points of improvement measures currently being implemented at construction sites. Method: The status was analyzed using the actual condition survey report issued by the Korea Occupational Health Corporation and the Korea Occupational Health Association and data from the work environment measurement institution, and compared and analyzed with the rules on work environment measurement of the Occupational Safety and Health Act. Result: The harmfulness of silicon oxide dust was identified and improvement measures were derived. Conclusion: It is expected that occupational diseases against silicon dust can be reduced if the harmfulness of silicon oxide dust at construction sites is derived and improvement measures are actively applied at the site.

Improvement of Low Bone Mineral Density Treated with Jeopgol-tang in a Middle-Aged Man: A Case Report (중년 남성에서 접골탕 투여 후 개선된 골밀도에 관한 증례 보고)

  • Bone mineral density (BMD) is a major diagnostic marker for bone health. A 44-year-old male had BMD of 0.81 g/cm2 (Z-score: -3.1) in lumbar spine scan and 0.54 g/cm2 (Z-score: -2.7) for femoral neck from regular medical checkup in Apr 2020. He had no other specific medical conditions except hyperlipidemia and alcohol was a single risk factor for fracture according to Fracture Risk Assessment Tool. After he was diagnosed with liver-kidney deficiency and treated for 20 weeks with Jeopgol-tang originally patented for promoting fracture recovery, lumbar spine BMD increased by 13.6 % (0.92 g/cm2, Z-score: -2.1) and femoral neck BMD by 22.2% (0.66 g/cm2, Z-score: -1.8) compared with those of Mar 2020. Herbal medicine treatment for tonifying liver and kidney to improve BMD warrants further investigation.

Factors Affecting the Health Behavior Compliance of Middle-aged Women (중년여성의 건강행위이행 영향 요인)

  • This study is a descriptive research study to promote health behavior compliance and use it as basic data for the program development of nursing intervention by identifying the factors affecting middle-aged women's health behavior compliance. It was collected and analyzed 162 copies of the data for a total of 10 weeks from January 17 to March 28, 2022, targeting middle-aged women aged 50 to 59 living in D and B. As a result of the study, economic status(medium) had the highest impact on health behavior compliance, followed by economic status (low), self-efficacy, perceived health status, married status(married), and regular health checkup(yes). Therefore, it is needed promoting to get regular health checkups, increasing self-efficacy with financial support, and exploring the ways and the development of programs that can improve perceived health status in order to improve middle-aged women's health behavior compliance.

An Index to Assess Overwork-Related Adverse Effects on Employees Under the Occupational Safety and Health Act in Taiwan

  • Background: The present study aimed to digitally evaluate the risk of overwork-related adverse effects (OrAEs) among employees from various occupational categories in Taiwan. Methods: Anonymous data of employees from seven companies/factories providing occupational health services were analyzed. The studied population comprised 5505 employees, and the data analyzed included employment duration, working hours, shift work schedules, and health checkup results. The risk for OrAEs was assessed by an index, Karo index (0-4, the larger the value, the higher the risk for OrAEs) obtained using a risk matrix made up of cardiocerebral and occupational risk factors. Karo index values of 3 and 4 were categorized as at high risk for OrAEs (h-OrAEs). Results: The 5505 employees had an average employment duration of 8.5 years and a mean age of 39.4 years. The prevalence rates for h-OrAEs of the seven companies/factories ranged from 3.9% to 34.2%. There were significant differences in prevalence rates for h-OrAEs between employees of retail stores and high-tech manufacturing factories. Multivariate analysis results indicated that workers of high-tech manufacturing factories had significantly higher risk for h-OrAEs compared with retail store workers. Conclusion: In terms of satisfying health risk management and legal requirements in Taiwan, the newly issued Karo index, which covers a wide range of occupational risk factors, can serve as an assessment and a warning tool for managing the risk of OrAEs in workplaces. To reduce risks for h-OrAEs, active and prudent control of cerebrocardiovascular risks and working hours is recommended.

A Case Report of Osuyubujaijung-tang on the Recurrence of Breast Cancer in a Patient Complaining of Coldness and Dizziness (유방암 재발 환자의 장궐 및 현훈이 오수유부자이중탕으로 호전된 치험 1례)

  • A 63-year-old female patient was admitted to the hospital with complaints of coldness in the head and right femur, persistent vertigo, dry mouth, vexation, general weakness, anorexia, and hot flashes. After being diagnosed with left-sided ductal carcinoma in 2017 and undergoing partial resection, she was recommended radiotherapy, chemotherapy, and hormone therapy, but she refused and did not receive any treatment. At a checkup in November 2022, the left-sided invasive ductal carcinoma recurred. She underwent partial resection and was scheduled to receive radiotherapy two months later. During hospitalization, the patient received traditional Korean medicine treatment, including Korean herbal medicine (Osuyubujaijung-tang), acupuncture, moxibustion, and cupping. Subjective symptom changes were evaluated daily. At discharge, the patient's symptoms improved, and her condition varied with the presence of Aconitum carmichaelii. This study suggests that "Osuyubujaijung-tang" cared the coldness and vertigo caused by "Han-dam (寒痰)" in patients with Soeumin visceral syncope pattern (少陰人 臟厥症) and that A. carmichaelii played a major role in this outcome.

The relationship between shift work pattern and thyroid stimulating hormone in female workers

  • Background: Shift work is known to cause changes in the circadian rhythm of the human body and adversely affect not only physical health but also mental health. Some studies have demonstrated the correlation between shift work and thyroid stimulating hormone (TSH), a hormone that changes according to the diurnal rhythm, but few studies have reported the different TSH levels according to the shift work type. This study aimed to investigate changes in TSH according to the shift work type. Methods: This study included 1,318 female workers who had a medical checkup at a university hospital in Changwon from 2015 to 2019. Shift work types were classified as non-shift work, regular 2 shifts, and irregular three shifts, and a TSH ≥ 4.2 mIU/L was defined as abnormal. A general linear model (GLM) was used to compare the TSH levels and the risk of subclinical hypothyroidism in each year, and a binary logistic analysis was performed using a generalized estimation equation (GEE) to compare the risk of subclinical hypothyroidism over the 5-year period. Results: Of the 1,318 participants included in this study, 363, 711, and 244 were non-shift, two-shift, and irregular three-shift workers, respectively. In the GEE analysis, after adjusting for age, body mass index, smoking, and alcohol consumption, the odds ratios (ORs) were 1.81 (95% confidence interval [CI]: 1.15-2.86; p = 0.011) in 2 shifts and 2.02 (95% CI: 1.23-3.32; p = 0.006) in irregular three shifts, compared to non-shift. Conclusions: Our results showed that shift work had a higher risk of subclinical hypothyroidism than non-shift work and that there was a significant difference in the risk of subclinical hypothyroidism according to the shift work type. These findings suggest that the shift work type can be considered in future thyroid function tests and evaluations.

A Study on Health Care Utilization Rates by Gender and Age: Focusing on Data from the 17th Wave of Korea Welfare Panel (2023) (성별 및 연령대별 보건의료 이용율에 관한 연구 : 한국복지패널 17차 웨이브(2023) 자료를 중심으로)

  • This study aims to analyze the secondary data of disease distribution and medical service behavior according to gender and income by using the 17th wave data of the Korea Welfare Panel, which is being distributed in April 2023. Data for 7,865 people of raw data generated using the R language were collected, and among them, missing values (NA, - 2,012) were analyzed for 5,853 people. For analysis, average income by health status and gender, relationship with chronic diseases, outpatient visits to medical institutions by gender/age group, type of medical institution used by age group, and annual health checkup usage rate by gender/age group were examined. Through this, the medical utilization rate was higher in men than in women, and the utilization rate of hospitals and clinics was high.

Association between shift work and inflammatory markers in workers at an electronics manufacturing company

  • Background: Shift work is known to be associated with cardiovascular disease (CVD). It has been found that inflammatory reactions are involved in the onset and progression of CVD. Therefore, the purpose of this study was to investigate the association between shift work and inflammatory markers. Methods: Among workers at an electronics manufacturing company, 2,329 workers who had a health checkup from January 2019 to December 2019 were targeted. The general and biochemical characteristics of daytime workers and shift workers were compared through the Independent-test and the χ2 test. Through multiple linear regression analysis, the association with shift work and inflammatory markers was investigated. Through multiple logistic regression analysis, the association with shift work and high inflammatory markers Results: The mean total leukocytes, neutrophils, monocytes, lymphocytes of shift workers were significantly higher than those of daytime worker. The mean high-sensitivity C-reactive protein (hs-CRP) of shift workers was also higher than that of daytime workers but not significantly. In multiple linear regression, shift work was associated with increase of total leukocyte count (β = 0.367, p < 0.001) and hs-CRP (β = 0.140, p = 0.005) after adjusting for all variables. In multiple logistic regression analysis, shift work showed 2.27 times risk of high leukocyte count and 1.8 times risk of high hs-CRP level compared to daytime work after adjusting for all variables. Conclusions: This study confirmed that shift work is associated with high inflammatory markers. Considering that high inflammatory markers is independent indicator of CVD, the association between shift work and high inflammatory markers may help to understand the CVD risk of shift workers.

Association of Serum Vitamin D with Insulin Resistance and Beta Cell Function in Korean Health Checkup Examinees (건강검진 수검자에서 혈청 Vitamin D 수준과 인슐린저항성 및 β-세포 기능과의 관련성)

  • Objectives: This study was conducted to examine the association of serum Vitamin D with insulin resistance and ${\beta}-cell$ function in Korean health checkup examinees. Methods: This study subjects were 374 healthy adults (199 males, 175 females) over the age of 20, who visited a general hospital medical center located in Haenam-gun, Jeollanam-do. To find the association of Vitamin D with HOMA-IR and $HOMA-{\beta}$, the used statistical analysis were ANOVA and ANCOVA. Results: Of the study subjects, the level of serum Vitamin D defined by deficient group, insufficient group and sufficient group was 38.5%, 48.1% and 13.4%, respectively. According to the level of serum Vitamin D, the mean values of HOMA-IR were $1.92{\pm}1.08$ in sufficient group, $1.99{\pm}1.04$ in the insufficient group and $2.91{\pm}1.05$ in deficient group and there were statistically significant different(p<0.001). The mean values of $HOMA-{\beta}$ were $84.69{\pm}1.07$ in sufficient group, $78.41{\pm}1.04$ in the insufficient group and $80.48{\pm}1.04$ in deficient group, and there were not significant. As a result of ANCOVA, adjusted mean of HOMA-IR were statistically significant different (p<0.001), but those of $HOMA-{\beta}$ were not statistically significant according to the level of serum Vitamin D. Conclusion: The insufficient level of serum Vitamin D was relatively high in healthy adults who live in rural area, and it was found that HOMA-IR significantly increased when Vitamin D was deficient. To prevent insulin resistance or diabetes, it is necessary to provide sufficient information related to sufficient production of Vitamin D such as Vitamin D supplement, sun exposure, food intake and etc.
