한국의 병원 구조에서 QA 팀을 어떻게 구성하고 운영 할 것인가? (How to organize and manage hospital QA according to specific structures of a general hospital in Korea?)

  • 한국의 병원 구조에서는 우리 실정에 맞게 QA를 조직하고 운영하여야 한다. QA 실은 원장 직속 기구로 두고, QA 위원회의 자문을 받는다. 의료 보험과 QA의 근원지 국가들에서는 보험 심사실이 없고, 심사 업무를 QA실에서 하고 있다. 심사실과 QA 실이 이미 따로 설치된 병원에서는 이들을 통합하고, QA 실을 신설하고자 하는 병원은 심사실을 QA 실로 전환시킨다. 심사 간호사회가 이를 적극적으로 찬성하고, 이를 수용 할 준비를 하고 있다.

간호사의 직무만족에 관한 논문 분석 (Analysis of Studies on Job Satisfaction among Korean Nurses)

  • The purpose of this study was to identify patterns of and trends in studies of job satisfaction among Korean nurses as selected from master theses whose research had covered studies from the period $1972{\sim}1997$. These studies were analyzed for publication date, research design, theoretical framework, research questions, and research hypotheses, subjects, sampling methods, measurement tools, statistical analyses, job satisfaction-related variables, and suggestions for further research. The findings of the analysis were as follows: 1) The number of studies on the subjects of job satisfaction has increased enormously since the 1980s. 2) In research design, 73.8% of the studies involved correlation research. 3) Conceptual frameworks, research questions, and hypotheses were not involved in 88.8%, 76.3%, and 68.% of the studies respectively. 4) With respect to subjects, 53.7% involved head nurses, charge nurses, and staff nurses. 5) As sampling methodology, questionnaires were employed in all the studies. 6) Measurement instruments developed by Slavitts et al.(1978) and Stamps et al.(1978) were used in 27 studies and 24 studies, respectively. 7) For the statistical analysis, ANOVA(30.7%), t-test(23.7%), and correlation(23.7%) were used. 8) The number of job satisfaction-related variables were eight items and 36 sub-concepts. 37.5%(30 studies) employed applied variables related to organizational characteristics. 9) 93.7% of the studies made suggestions for further research. On the basis of the above findings, the following recommendations were made: 1) Qualitative research should be performed to identify variables related to job satisfaction. 2) Research on developing interventions is needed to elevate job satisfaction. 3) Development of measurement tools is needed to evaluate the job satisfaction of Korean nurses. 4) Correlation studies are needed to identify the relationship between job satisfaction of nurses and outcomes to patients.

학교금연교육프로그램 현황 및 문제점: 부산광역시 중.고등학교를 중심으로 (School-based Educational Programs for Anti-tobacco Use at Busan Middle and High Schools)

  • Purpose: The purpose of this study was to describe the current status of managing educational programs for tobacco use prevention at middle and high schools located in Busan. Methods: Using a survey method with a 29-item self-administration questionnaire, the data were collected from January 10 through February 18, 2005, from school nurses as well as teachers major in healthcare of 227 participant schools. Results: A strategic plan for school-based educational programs of tobacco use prevention was mainly developed by a school nurse at middle school and a teacher in charge of school discipline at high school. The educational programs for tobacco use prevention were implemented for approximately 40% of schools in Busan with the budget ranged from 110,000 won to 200,000 won a year. Eighty five percepts of the schools had anti-tobacco regulations and guidelines in place. Twenty two percents of the middle and high schools collected the data regarding adolescents' smoking status periodically. Over 70% of schools provided tobacco use prevention education programs as a part of regular semester curriculum. Conclusions: For making the management of educational programs for tobacco use prevention effective and consistent with government's anti-smoking policy, schools need (a) development of tobacco management guideline and regulations with participation of teachers, students, and parents, (b) clear job and role descriptions for personnel to plan and implement anti-tobacco programs, (c) compilation of the budget enough to implement the programs, (d) periodically assessment of adolescents' smoking status, and evaluation of the programs to monitor whether those are effective for adolescents' health promotion; and (e) to make an effort to less range of quality in educational programs for tobacco use.

이민 임산부의 스트레스와 분만경험에 대한 현상학적 연구 (A phenomenologic study on the stresses and the experiences of pregnant women and postpartum mothers who had immigrated to the United States)

  • Koreans are one of the fastest growing immigrant groups in America. Adjsting to life in foreign country produces a great deal of stress. Differences in culture, language, expectation and social behavior can lead to misunderstandings. The pregnancy and delivery event is one of maturational crisis in life cycle. The purpose of this research was to understand the structure of the lived experience of pregnant women and postpartum mothers who had immigrated to the United States. The research question was "What is the structure of the experience of pregnant women and postpartal mothers?" The sample consisted of 16 women registrated at the Obstetrics and Gynecology Department of one local clinic in Hawaii. The unstructured interviews were carried out from Jnuary 5, through January, 30, 1994. They were audio-recorded and analyzed using Van Kaam's method. There are different views on the causative factors of stress. Maladjustement to the immigrant life, spousal conflicts, anxiety related to bringing up the bay and conflicts between mother-in-law and daughter-in-law are considered to be related factors. The experiences of pregnant women over the period of the pregnancy can be varied and can include change of body image, emotional and physical change. The experiences of postpartum mothers over the postpartum period can be varied and can include postpartum depression emotional irritability, fear related to bringing up the baby and disappointment with husband. Positive experiences over the period of pregnancy and postpartum were the strengthen-ing beliefs. Sources of support were, first, spouse then mother and faith. Support was also received from the physician in charge and through self-control. The nurse, by providing empathetic support, should be a person with whom they can express their feelings and share their experiences.

한 농촌보건지소에서의 보건관리 실태에 관한 고찰 -수동면 보건지소의 조직과 인력을 중심으로- (A Study on Status of Health Care for Community Residents in a Rural Health Subcenter - With special Reference to Organization and Man Power of Su Dong Myun Health Subcenter -)

  • In order to find out status of health care for community residents with special reference to organization and man power in a rural health subcenter, a study was carried out, through analyzing the data, operated by health subcenter during 1973~1990 in a rural area, Su Dong-Myun, Nam Yang Ju-Gun, Kyung Gi-Do, Korea. The following results were obtained : l) The Number of Population in Su Dong-Myun is decreasing and household is increasing chronologically. 2) In 1989, the characteristics of population composition rate in Su Dong-Myun were shown : the groups of Age, 65 and over was high rate(9.8%) and 0~4 was low rate(5.3%) decreasing chronologically. 3) Since 1972, services of Su Dong-Myun health subcenter have been carried in the 3 functions(medical treatment, health care management and clerical work) with supports of Ewha Woman's University through the participation of community residents organization. 4) The Number of Su Dong-Myun health subcenter personnel in 1990 was 5 persons(public health doctor l, public health dentist l, health worker l, dental technician l and assistant nurse l) and of these, health worker who must have the huge charge of health care management, has been worked at Myun-office as a public official, in condition, decreased from 3 in 1980 to 2 in 1981 and from 2 to 1 in 1985. 5) Health service Activities of Su Dong Myun health subcenter obtained good results in it's achievement during 1975-1985, but since 1986, it has been in condition of lower stepping. 6) Since 1977, annual medical utilization rate showed decreasing tendency such as 900 per l,000 population in 1977, 846 in 1979, 723 in 1981, 343 in 1973 and 34l in 1987. 7) A proposal : (l) Organization of health subcenter must be unified and systematized by government, so that health subcenter can carry out primary health care for community residents through responsibilities and authorities. (2) Teaching programs in educational process must be reorganized, according to periodical request. considering relative importance to primary health care in health care needs of community residents.

메르스 환자 간호에 대한 간호사의 경험 (Nurses' Experience of Middle East Respiratory Syndrome Patients Care)

  • 본 연구의 목적은 메르스 환자 간호에 대한 간호사의 실제적인 경험을 파악하기 위함이다. 본 연구에 참여한 대상자는 S시 1개 상급종합병원에서 메르스 환자 간호에 참여한 간호사 9명이며, 자료 수집을 위해 심층 면담이 수행되었다. 자료수집 기간은 2015년 8월부터 2016년 3월까지였다. 수집된 자료는 Colaizzi의 현상학적 방법에 따라 절차에 맞게 분석하였다. 도출된 5개의 범주는 다음과 같다. '1. 신종 감염병 전염 위험성에 따른 불안과 부담', '2. 메르스 환자 간호사라는 이유로 느끼는 사회적 고립', '3. 준비되지 않은 치료환경 문제에 부딪힘', '4. 힘겨운 메르스 환자 간호과정을 이겨냄', '5. 신종 감염병 대책 마련에 대해 성찰함'이었다. 이와 같은 결과는 메르스 환자를 포함한 다양한 감염병 환자를 담당하는 간호사의 부정적 영향 뿐 아니라, 긍정적인 경험을 도출했다는데 의의가 있다. 이를 바탕으로 심리사회적 부정적 영향을 감소시키고, 긍정적인 적응을 향상시키는데 근거로 활용될 수 있을 것이며, 간호사를 포함한 보건의료 종사자가 신종 감염병 환자를 간호하기 위한 인력, 부서 및 실무지침 마련이 필요하다고 생각된다.

의학적 충고에 반한 퇴원의 특성과 퇴원결정 요인에 관한 연구 - 사회사업가의 개입사례와 역할을 중심으로 - (A Study on the Characteristics of DAMA(Discharge Against Medical Advice) Case and Causal Factors of DAMA - Perspective of Medical Social Worker's Role and Intervention -)

  • Objectives : DAMA cases were analyzed to examine what the main casual factors of DAMA were and how to deal with these cases effectively in hospital with the DAMA interdisciplinary team including medical social worker whose role is to perform psycho-social assessment, family counsel, to evaluate family's DAMA need. Patients and Methods : The content analysis of medical record and social work record were reviewed in 37 cases referred by medical doctor to DAMA team. These cases were reported by patients' self discharge request or family's request for discharge from September 1998 to February 2000. The DAMA team consists of Assistant Director of Hospital as team leader, medical staff in-charge, social worker, QI nurse, other staff members who are not involved in direct treatment for patient, and administrative clerk. Results : The results of content analysis are as follows : 1) The most causal factors of DAMA consist of combination of more than 2 factors. 2) The major decision-maker is revealed to be son and daughter of patient. 3) In 59.4% of cases, family was not informed of patients' prognosis, alternatives, the consequence of DAMA at all. 4) In cases of DAMA report, the rapid intervention of social worker is carried out. Conclusion : In this study, we propose the interdisciplinary team approach to make decision legitimately and ethically for DAMA. The suggestions from this study are as follows : 1) To deal with DAMA case properly, the interdisciplinary team approach should be considered. 2) The criteria for DAMA case should be formed carefully. For the explicit selection of DAMA case, preliminary system for high-risk patient screening is recommended. 3) The medical social worker is available for the psycho-social problems of the patient once family members. For the effective family counselling, discharge planning and nursing home placement, the participation of medical social worker should be mandatory.

기혼 여성간호사의 직장-가정 갈등과 사회적 지지가 재직의도에 미치는 영향 (Impact of Work-Family Conflict and Social Support on Retention Intention among Married Female Nurses)

  • 본 연구는 기혼 여성간호사를 대상으로 재직의도에 대한 직장-가정 갈등과 사회적 지지의 영향을 확인하기 위하여 시도되었다. 연구 대상자는 C시의 종합병원에 근무하는 기혼 여성 간호사 156명이며 자료수집은 2017년 9월 1일부터 15일까지 하였다. 자료는 SPSS 23.0을 이용하여 빈도, t-test, one-way ANOVA, 피어슨 상관계수, 다중회귀분석을 하였다. 연구결과 재직의도에 영향을 주는 요인은 시간갈등, 동료지지, 자녀수 한명 이상, 책임간호사, 상사지지 순이었으며 이들의 설명력은 37.3%로 나타났다(F=11.25, p<.001). 본 연구에서 시간갈등은 음의 영향을, 동료지지와 상사지지는 양의 영향을 재직의도에 미치는 것을 보여주었다. 이러한 결과는 재직의도를 높이는 새로운 전략개발에 도움이 될 것이다. 기혼 여성간호사의 재직의도에 영향을 미치는 다른 요인을 확인하기 위한 후속연구가 필요하다.

초등학생들의 집단따돌림에 관한 연구 (Study on Bullying among Primary School Students)

  • This study was conducted with 512 students in 4 primary schools located at the inner and the outskirt of K city, Northern Kyungsang Province from November 6, 2001 to November 27, 2001(during 3 weeks). This study was a research about the difference of a bullying degree and tendency of both teasing and teased students. The purpose of the study was to help nursing teachers in charge of student consultation and treatment. The findings are as follows: First, for to whom students confess their worries, students in lower grade talked to their parents while students in higher grade talked to their close friends. The higher grader, the higher rate in solving their problems by themselves. For how many students are teased by their peers in groups, 52.0% students answered 1 or 2 students in a class. For the reasons of bully, 37.0% higher graders answered, very selfish and putting on airs after 29.6% lower graders answered, ugly and bad at speaking. Second, for perception of peer relation, 81.3% students participating in the study answered, very important. For a question whether students satisfy present relation with their friends by gender, 53.7% boys were satisfied while 34.6% girls were satisfied. Third, for difference in bullying by gender, boys mainly teased others by using words and physical power(p<0.000) while girls estranged others. When it goes to teased students, it was the case: boys were bullied physically(p<0.000) and linguistically while girls were hurt by estrangement. Having nothing direct to do with school achievement, nursing teacher should make good use of such an advantage that students tend to easily confess their worries to them and take care of psychological aspects of students in treatment. Furthermore, it needs to include contents of bully in the regular heath care education.

신규간호사와 프리셉터가 인지하는 프리셉터의 교수효율성 비교 (A Comparison of Preceptors' and New Graduate Nurses' Perception of Teaching Effectiveness)

  • Objective: The purpose of this study was to compare preceptors and new graduate nurses on their perception of preceptor teaching effectiveness. Methods: The participants were 90 new nurses and 90 preceptors who worked in A medical center. The data were collected from July 1 to September 30, 2008. A cross-sectional descriptive survey was done using a structured questionnaire. Results: New nurses' perception ($4.07{\pm}.44$) of the preceptor teaching effectiveness was significantly higher than perception of the preceptors ($3.57{\pm}.37$). Fifty five percent of new graduate nurses reported a lack of coherence in the preceptor's practice guidelines. For 5 causal factors classified on the teaching effectiveness, the factor of 'Professional knowledge and ability' showed the highest score, but 'Interpersonal and communication skill' got lowest score for both group. Preceptors responded that they did not have enough time to teach well because of their heavy workloads. Conclusions: These results suggest that the preceptors need appropriate compensation and education opportunities, and new graduate nurses need consistent education by the teaching professionals. Therefore, it is important to give preceptors full charge of the preceptorship. Also, it will be necessary to develop education programs to enhance interpersonal and communication skill for preceptors and new nurses.
