지연된 유방암 진단을 예측하는 정신사회적 요인 : 부부관계기능의 역할 (Psychosocial Factors Predicting Delayed Diagnosis of Breast Cancer : The Role of Marital Relationship Functioning)

  • 연구목적 2001년 이후로 유방암은 한국에서 가장 흔한 여성 암이 되었다. 진행 유방암으로 진단받을 고위험군을 확인하는 것은 유방암 사망률을 줄이기 위한 효과적인 방법 중 하나라고 할 수 있다. 본 연구의 목적은 유방암을 진단 받은 시점에서 진행 유방암과 연관된 사회인구학적, 건강행위 건강특성, 임상적 그리고 정신사회적 요인을 조사하며, 진행 유방암을 예측할 수 있는 요인을 밝히는 데 있다. 방법 경북대학교 병원에서 처음으로 유방암을 진단받고, 수술을 시행한 후 '유방암 환자들을 위한 스트레스 클리닉'에 자문 의뢰되어 정신의학적 면담이 시행된 219명의 환자를 대상으로 하였다. 기본 사회인구학적 자료를 비롯하여, 자가진단 및 유방촬영술의 규칙적 시행여부, 진단 당시의 병기, 유방암 발견 경위 등을 조사하였다. 정신사회적 요인으로는 경제적 및 가족적 부담, 부부관계기능, 가족관계기능 정도 등을 반구조화된 면담을 통해 파악하였다. 교차분석과 로지스틱 회귀분석으로 조기 유방암 환자군과 진행 유방암 환자군 간에 변수를 비교하고 진행 유방암 환자군을 예측할 수 있는 요인을 확인하였다. 결과 유방암을 진단받은 시점에서 120명(54.8%)이 진행 유방암으로 확인이 되었다. 단변량분석을 통하여 조기 유방암을 진단받은 환자군과 진행 유방암을 진단받은 환자군 사이에 통계적으로 유의한 차이가 난 변수는 다음과 같았다. 건강행위 및 특성 변수로는 자가 검진행위(p<0.000), 매년 유방촬영술 시행여부(p<0.000)이었다. 암 관련 임상적 특징으로는 종양 발견경위(p<0.000), 첫 증상의 양상(p<0.000), 첫 증상 발견부터 치료까지 소요된 기간(p<0.000)이었다. 정신사회적 요인으로는 부부관계기능(p<0.000)과 가족관계기능(p=0.000), 과도한 경제적 가족적 부담(p=0.018)이었다. 로지스틱회귀분석을 통하여 매년 비진행 유방암에 영향을 미치는 독립인자로는 매년 유방암 선별을 위해서 실시하는 유방촬영술을 비정기적으로 시행하거나(OR=7.431 ; 95% CI 2.407-22.944), 전혀 시행하지 않는 경우(OR=25.299 ; 95% CI 7.855-81.482)였으며 정신사회적 요인으로는 기능 부전적 부부관계(OR=4.772 ; 95% CI 2.244-10.145)가 유일했다. 결론 본 연구를 통하여 진행 유방암 진단의 위험인자로 널리 알려진 비정기적 검진을 재확인하였으며, 이외에도 정신사회적 요인으로서 '비기능적 부부관계기능'을 밝혀냈다. 이러한 결과는 유방암을 조기에 진단하는데 전통적으로 인정되어 왔던 사회인구학적, 임상적 요인 이외에도 정신사회적 요인이 중요한 영역임을 시사한다.

세계 현대 판화속의 일본 현대판화의 국제성 연구 (Study on Internationality of Japanese Modern Print in the World Print)

  • 일본의 대중 미술로서의 판화는 에도시대(江戶時代, 1603~1867)의 우키요에 판화부터 시작 되었다고 할 수 있다. 3인이 공동으로 제작하는 우키요에(浮世繪) 판화는 메이지시대(明治時代, 1868~1912)이후 새로운 인쇄 기술의 발달과 서구적 예술의 도입으로 인해 점차 쇠퇴하였다. 그 이후 다이쇼시대(大正時代 1912~1926)가 되면서는 야마모토 카나에(山本 鼎)가 ${\ll}$명성${\gg}$지에 발표 했던 <어부(漁夫)>라는 작품을 계기로 한 창작 판화 운동이 일어났다. 창작 판화는 복제 우키요에 판화와는 달리 한 사람이 원화부터 인쇄까지를 스스로 제작하는 것으로, 작가가 주체가 되어 자신만의 작품을 창작한다는 것에 중점을 둔 것이었다. 이전까지의 복제판화에 대해 반성하고 판화의 예술성을 인식하고자 한 이 운동을 시작으로 점차 많은 작가들이 판화 작품을 제작하였고 전시회를 통해 발표하였다. 사실 1900년대 전후만 해도 일본에서 판화는 서양화나 조각 등에 비해서는 낮은 평가를 받았었다. 그럼에도 불구하고 1950년대부터 다수의 일본 판화가들이 국제 전람회에서 수상한 사실들은 국제적으로 큰 화제가 되었고, 일본에서도 1957년에 첫 국제 미술제인 동경 국제 판화 비엔날레가 개최되는 계기가 되었다. 그 이후 점차 일본에서도 판화에 대한 인식을 새롭게 하면서 예술의 한 장르로 인정받게 되었다. 그러나 1970년대에는 미술을 표현하기 위한 매체로서의 판화의 성격이 강해지면서, 판화가 독립된 장르가 아닌 현대 미술의 표현 양식의 하나로 종속되는 것이 아닌가 하는 우려가 일어나기도 했다. 그러나 1980년대에 이르러서는 판화가들과 대학 교육 차원에서의 다양한 노력을 통해 위기를 뛰어넘고 하나의 예술 장르로써 확고히 자리를 잡았다. 그리고 전통적인 기법과 현대적인 표현 방식을 접목하여 새롭고 완성도 있는 작품을 제작하는 판화가들이 등장하여 일본에서 뿐만 아니라 세계적으로도 활약 했다. 현재까지도 지속적인 해외 활동과 판화에 대한 연구, 신진 판화가들의 양성을 통하여 일본의 현대 판화는 세계적으로 크게 인정받고 있다. 본 연구에서는 아시아를 대표하는 일본의 현대 판화가 발생되었던 역사적인 과정을 되짚어보고, 국제적인 위상을 갖게 된 이유에 대해서도 살펴보았다.

Progress of Composite Fabrication Technologies with the Use of Machinery

  • A Macroscopic combination of two or more distinct materials is commonly referred to as a "Composite Material", having been designed mechanically and chemically superior in function and characteristic than its individual constituent materials. Composite materials are used not only for aerospace and military, but also heavily used in boat/ship building and general composite industries which we are seeing increasingly more. Regardless of the various applications for composite materials, the industry is still limited and requires better fabrication technology and methodology in order to expand and grow. An example of this is that the majority of fabrication facilities nearby still use an antiquated wet lay-up process where fabrication still requires manual hand labor in a 3D environment impeding productivity of composite product design advancement. As an expert in the advanced composites field, I have developed fabrication skills with the use of machinery based on my past composite experience. In autumn 2011, the Korea government confirmed to fund my project. It is the development of a composite sanding machine. I began development of this semi-robotic prototype beginning in 2009. It has possibilities of replacing or augmenting the exhaustive and difficult jobs performed by human hands, such as sanding, grinding, blasting, and polishing in most often, very awkward conditions, and is also will boost productivity, improve surface quality, cut abrasive costs, eliminate vibration injuries, and protect workers from exposure to dust and airborne contamination. Ease of control and operation of the equipment in or outside of the sanding room is a key benefit to end-users. It will prove to be much more economical than normal robotics and minimize errors that commonly occur in factories. The key components and their technologies are a 360 degree rotational shoulder and a wrist that is controlled under PLC controller and joystick manual mode. Development on both of the key modules is complete and are now operational. The Korean government fund boosted my development and I expect to complete full scale development no later than 3rd quarter 2012. Even with the advantages of composite materials, there is still the need to repair or to maintain composite products with a higher level of technology. I have learned many composite repair skills on composite airframe since many composite fabrication skills including repair, requires training for non aerospace applications. The wind energy market is now requiring much larger blades in order to generate more electrical energy for wind farms. One single blade is commonly 50 meters or longer now. When a wind blade becomes damaged from external forces, on-site repair is required on the columns even under strong wind and freezing temperature conditions. In order to correctly obtain polymerization, the repair must be performed on the damaged area within a very limited time. The use of pre-impregnated glass fabric and heating silicone pad and a hot bonder acting precise heating control are surely required.

소장 질환별 방사선학적 검사의 적용에 관한 연구 - 소장조영술과 복부골반전산화단층촬영의 결과 분석을 중심으로 - (Application of Radiological Study by Small Bowel Disease - Comparison of Diagnostic Results of Small Bowel Series and Abdominal Pelvic Computed Tomography -)

  • 목 적 : 소장조영술과 복부골반 CT검사의 진단 결과를 비교함으로써 방사선학적 영상의 특징 파악 및 질환에 따른 적절한 방사선학적 검사법을 선택하는데 기초자료를 제공하고자 한다. 대상 및 방법 : 1999년부터 2000년까지 소장조영술과 복부골반 CT검사를 병행하여 시행한 환자 284명을 대상으로 하여, 각각의 진단 소견을 병명별로 분류하였다. 소장조영술은 250 ml 황산바륨 현탁액(W/V 40W/V%, Methylcellulose+$BaSo_4$)을 경구로 복용시키고, Rt. recumbent position을 취하게 한 후 약 10분 후에 Carboxymethylcellulose solution(0.5%) 600 ml 복용시켰다. 20분, 40분, 1시간 정규촬영을 하고, 조영제가 회맹판(I-C valve)을 통과하면 투시 하에 저격촬영을 시행하였다. 복부골반전산화단층촬영은 검사 1시간 전에 $BaSO_4$+Methylcellulose 1.5 W/V% 450 ml를 복용시키고 검사직전 450 ml 추가로 복용시켰다. 요오드 성분의 비이온성 조영제 2.5 ml flow/sec로 주입하고, 조영제 주입 후 72초 지연 촬영을 하였다. 촬영은 간 상연에서부터 치골결합 부위까지 continuous scan을 시행하였으며, 검사 protocol은 Helical thick 5 mm, picth 1.375 : 1, speed 27.50, increment 5 mm, 120 kV, 245 mA로 하였다. 결 과 : 소장조영술에서는 정상(Normal)이 131예(46.1%), 감염성 장질환(Inflammatory bowel disease ; 이하 IBS)이 65예(22.9%), ischemia+ileocolitis+vasculitis 22예(7.7%), obstruction+stricture 21예(7.4%)의 순으로 나타났고, 복부골반 CT검사에서는 정상이 103예(36.3%) IBS가 65예(22.9%), wall thickening+lymphadenopathy가 42예(14.8%), fluid collection이 17예(6%)의 순으로 나타났다. 소장조영술과 복부골반 CT검사에서 동일하지 않은 결과로 진단된 경우는 소장조영술에서 정상으로 진단된 132명(46.1%) 중 14예(10.6%), 복부골반 CT검사 결과에서는 wall thickening+lymphadenopathy와 IBS로 진단되어 차이를 보였으며, 복부골반 CT검사 결과 정상으로 진단된 103예(36.3%) 중 5예(5.8%)가 소장조영술에서 ischemia+ileocolitis+vasculitis로 진단되었다. 정상으로 진단 된 235예 중 37예(15.9%)가 서로 다른 결과로 진단되어 통계적으로 유의한 차이를 보였다(P<0.05). 결 론 : 본 연구에서 특정질환에 특정검사의 진단결과가 우수하게 나타나는 것으로 조사되었으므로 방사선학적 검사의 진단적 민감도를 높이기 위해서는 처방 및 검사에 있어서 질환에 따른 적절한 검사법의 선택이 무엇보다도 중요하리라 사료된다.

Virtual Digital Test Pattern Method를 이용한 CR 시스템의 영상처리 특성 분석 (Analysis of Image Processing Characteristics in Computed Radiography System by Virtual Digital Test Pattern Method)

  • 연구 목적은 virtual digital test pattern method을 이용하여 REGIUS 150 CR 시스템의 각 LUT별 프로세싱 커브를 도출하고, Dry Imager의 특성을 파악하여 CR의 영상처리 방법을 분석하는 것이다. 실험은 먼저, CR의 화상데이터에 맞게 virtual digital test pattern 파일을 만들고, 인위적으로 입력하여 virtual digital test pattern step의 변화를 통해 각 LUT(THX, ST, STM, LUM, BONE, LIN)별 프로세싱 커브를 구한다. 그리고 하드카피 이미지를 통해 Dry Imager의 프로세싱 커브도 도출한다. 마지막으로, CR 본체의 영상처리 파라미터들을 변화시켜 각각의 특성을 알아낸다. 실험 결과를 살펴보면, 입 출력 값을 통해 CR 시스템의 입력 다이나믹 레인지의 LUT별 프로세싱 커브를 구하였다. 프로세싱 커브는 모두 곡선으로 이어져 있었으며(LIN mode 제외), 각 커브마다 조금씩 차이를 보였다. 또한 Dry Imager의 프로세싱 커브는 중간부분에만 선형성이 나타났다. 그리고 프로세싱 파라미터의 G value는 기울기와, S value는 감도의 shift량과 관련된다는 것을 알 수 있었고, 선량에 따라 S value가 비례하여 변화한다는 것 또한 유추해 낼 수 있었다. 결론적으로 CR 시스템은 각 회사별로 내부 처리가 다르고, 구조 또한 복잡하기 때문에 내부 영상처리의 파악이 어렵다. 하지만 본 연구에서 사용한 virtual digital test pattern method는 CR 시스템의 내부처리 구조와 영상 파라미터의 변화에 따른 특성을 추정할 수 있으므로, 이 방법을 통해 타 장비의 CR 시스템과 Dry Imager의 전체적인 영상처리 특성을 비교, 분석 가능할 것이다.

베트남 홍케 지역 사금의 산상과 생성연구 (Morphology, Mineralogy and Genetic Implication of Placer Gold from the Huongkhe Area, Vietnam)

  • 베트남 홍케 광화대의 동도 및 호아하이 지역으로 부터 채취된 사금은 일반적으로 세립질로서, 동도 지역 사금은 호아하이 지역에 비하여 상대적으로 큰 입도와 낮은 분급도의 경향성을 보여준다. 이들 사금 입자의 산출형태는 장경과 단경의 비에 의하여 spherical, subprismoidal, prismoidal 및 irregular로 분류 할 수 있으며, spherical form이 ${\approx}75%$인 호아하이 지역에 비하여 동도 지역 사금 업자들의 형태는 다양하게 관찰된다. 이러한 산상에 의하면, 동도지역에 비하여 호아하이지역 사금의 이동거리 (또는 시간)가 걸었던 것으로 추정된다. 이들 사금은 그 화학조성에 의하여 electrum (type I, fineness=568~931), amalgam (type II, fineness=671~927), native gold (type III, fineness=923~999) 등으로 분류된다. Type I은 그 산출특정에 따라 상대적으로 낮은 함은량 (11~39 atomic % Ag)을 갖는 type IA와, 상대적으로 높은 함은량 (40~58 atomic % Ag)을 갖는 type IB로 세분되며, 이는 기원광상산 electrum의 화학조성을 보여주는 것으로 사료된다. Type II는 주로 호아하이지역에서 산출되며, 함수은량은 동도지역의 사금이 낮은 경향을 보여준다. Type III는 주로 type I 또는 type II 사금업자의 가장자리에 산출한다. 이들 type III의 gold-rich rim은 이동과 풍화과정중 산화환경에서 야기된 self-electrorefining과 silver의 preferential dissolution에 기인된 것으로 사료된다. 홍케지역 사금중 적어도 일부 높은 함수은량을 보이는 경우는 함금-은 천열수 광상과 유사한 환경에서 생성된 기원광상으로부터 유래되었을 것으로 추정된다.

안정동위원소 비를 이용한 하구 갯벌에 있어서 저서 무척추 동물의 유기물 기원의 공간적 특성 (Spatial Characteristic in Food Sources for Benthic Invertebrates in an Estuary Tidal flat: Carbon and Nitrogen Stable Isotope Analyses)

  • 일본 나나키다 하구 갯벌에 있어서 탄소 질소 안정동위원소를 이용해 먹이 연쇄의 공간적 가변성에 대해 조사했다. 서로 다른 특징을 갖고 있는 갯벌에서 잠재적인 유기물(육상식물, 해양 입자성 유기물, 저서 부착 미세조류 및 하구 입자성 유기물), 퇴적유기물 및 저서 무척추 동물(Nuttallia olivacea and Nereidae)에 대한 샘플링을 행했다. 본 연구의 목적은 좁은 공간적 가변성에 따른 Nuttallia olivacea와 Nereidae의 먹이원을 결정하는 것이다. 육상식물(${\delta}^{13}C=-26.6{\pm}0.76$, ${\delta}^{15}N=2.7{\pm}0.31$) 과 하구 입자성 유기물(${\delta}^{13}C=-25.5{\pm}0.13$, ${\delta}^{15}N=5.2{\pm}0.46$)은 저서 부착 미소조류${\delta}^{13}C=-16.3$, ${\delta}^{15}N=6.2$)와 해양 입자성 유기물(${\delta}^{13}C=-19.6{\pm}0.08$, ${\delta}^{15}N=8.9{\pm}1.70$)의 탄소 질소 안정동위원소비 보다 낮았다. 퇴적물의 탄소 안정동위원소 비는 -27.4~-22.8‰ 나타냈으며, 하구에서 하천 방향으로 갈수록 낮은 탄소 안정동위 원소비를 나타냈다. 저서 무척추 동물의 탄소 질소 안정동위원소비는 각각 -22.8~-18.4‰, 8.1~11.9‰ 범위를 나타냈다. 이러한 결과와 더불어 혼합 모델을 이용해 저서 무척추 동물의 먹이원의 기여율을 추정한 결과 해양 입자성유기물과 저서 부착 미세조류의 기여율은 높았지만, 육상식물과 하구 입자성 유기물의 기여율은 비교적 낮았다. 이러한 저서 무척추 동물의 먹이 기여는 각 장소마다 계절 및 물리적 환경 요소의 영향을 받는다고 사료된다.

부산에 한 중형 종합 병원 내과에서의 폐결핵 환자의 양상과 귀결 (The Characteristics and Fates of Pulmonary Tuberculosis Patients Seen at Medical Department of A Medium Sized General Hospital)

  • 연구 배경 : 보건소에서와는 달리 폐결핵 환자들의 일반적 양상과 동태, 치료결과에 대한 일반 중형 종합병원에서의 연구 보고는 별로 없어 본원에서 치료한 환자의 특성과 결과를 보고하며 또한 의료보험 정책 설정에 도움을 주기 위한 것이다. 방법 : 1989년초 부터 1990년 말까지 2년간에 부산시 동래구에 있는 중형 종합병원인 대동병원 제 4 내과에서 진료한 환자, 남자 922명, 여자 1,059명 총 1981명의 의무기록을 조사해서 폐결핵환자들에 관한 것을 분석 검토하였다. 결과: 1) 본 내과 환자 1981명중에 폐결핵 환자는 96명, 4.85%의 빈도인데 내과 환자중에서 상당히 많은 비중을 차지했다. 남자에선 7.81%, 여자에선 2.27%이었다. 초진단 초치료 환자는 61.46%, 재치료 환자는 38.54%인데, 여기엔 61.46%, 재치료 환자는 38.54%인데, 여기엔 남여 차이가 별로 없었다. 2) 연령군별 분포를 보면 노령층에서 많아지는 결핵 전국 실태조사에서와는 달리 남여가 21~40세군이 각각 45.45%와 76.67%를 차지해서 최고 빈도이고, 61세 이상군에서는 남자가 3.03%로 최저이고, 여자는 1명도 없었는데 이것은 노령에서 자타가 모두 폐결핵 진료에 무관심한 까닭이라고 생각된다. 3) 병변의 크기 분류에서 경중이 57.07%, 중등증이 48.96%, 중증이 18.75% 균양성률은 배양검사를 기준으로 했는데 양성이 37.50%, 음성이 46.88%, 균검사를 하지않은 자가 15.63% 이었다. 중증일수록 양성률이 높다는 일반견해와 일치하는 데 특기할 것은 균불감 사자가 경증에선 16.13%인데 중증에서 오히려 좀 많아서 22.22%나 되었다는 것은 주목할 일이다. 초치료자와 재치료자간의 균양성률은 각기 41.07%와 55.00%으로 약간 차이가 있었고 균불검사자는 초치료자가 17.86%로 재치료자 12.50%보다 좀 많았다. 4) 발병모양(초진단동기)은 서서히 발병한 것이 68.75%, 객혈이 9.38%, 급성폐렴양 3.13% 이고, X 선 사진 검사에서 발견된 것이 18.75%로 많았으므로 우리나라에선 여러 경우에 실시하는 폐 X 선사진검사가 결핵 조기진단에 큰 도움이 된다고 하겠다. 5) 8개월이상 진료를 계속한 환자는 초치료자의 35.71%, 재치료자의 25.00%, 경증환자에선 16.13%, 중증환자에서도 겨우 27.78%이었다. 6) 8개월이상 치료자 30명에서는 객담음성화률이 80%, X 선사진상 현저한 경쾌가 56.67%이었는데 중증환자에선 각기 60%와 20%로 저율이었고 재치료환자에서도 역시 각기 60%와 10%도 저율이었다. 결론 : 우리나라의 결핵문제는 아직도 얘우 중요하고 폐결핵 환자의 치료와 관리가 향상되기 위해서는 국가제도의 획기적 개선과 사회보장제도의 향상 그 의사들의 더 많은 노력이 필요하다고 하겠다.

Sensory Information Processing

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times.
