A Study on the Convergence of the Average Daily Smoking and Tooth Loss of the Elderly in Korea (우리나라 노인의 하루 평균 흡연량과 상실치아의 융합적 관련성)

  • This study was to investigate the relationship between the smoking status and the number of tooth loss according to the daily average smoking amount of the elderly in Korea using the raw data of the 6th National Health and Nutrition Survey. The study subjects were 4,037 elderly people aged 65 or older. As a result, it was found that 2.35 times more severe heavy smoking in the past, 3.01 times in the current light smoker, 3.27 times in the current heavy smoker, Respectively. This study confirmed that there is a difference in the risk of tooth loss according to the average daily smoking amount. Therefore, in order to increase and maintain the residual teeth as much as possible, it can be suggested as an objective data of smoking cessation treatment at the dental clinic and contribute to motivation.

The association of genetic polymorphism between responder and nonresponder to acupuncture in smoking cessation (금연침에 대한 반응군과 비반응군의 유전자 다형성 차이 연구)

  • Objectives : This study was designed to investigate the differences of effects in smoking cessation after acupuncture treatment and we hypothesized that the discrepancies might be caused by individual genetic differences. Methods : Acupuncture treatment was given to the subjects three times a week for the 231 healthy male Korean smokers without personal or familial history of psychiatric or neurological illness. We evaluated for differentiate responder and non-responder who showed more than 50% decrease in the cigarette consumption or the desire for smoking were regarded as responder, and less than 25% decrease in the cigarette consumption or the desire for smoking were regarded as non-responder, respectively. Allele and genotype frequencies of the Taq1 A polymorphism of dopamine D2 receptor (DRD2) gene were compared in 231male smokers. Chai-square analyses were performed to test for an interactive effect between the DRD2 Taq1 A allele. Results : The allele frequencies and genotype distributions of DRD2 gene among the smokers (n = 231) showed significant the differences in their genotype distributions. The responder and non-responder showed the difference in genotype distribution with a prevalence of A1 allele. A slightly positive association of DRD2 Taq1 A1 genotypes with smoking was observed. Conclusions : This experiment results indicate that the present of DRD2 allele genotype showing significant difference in the genotype distributions between responders and non-responders could be explained by the difference in the genetic effect of DRD2 A1 allele.

CP-690550 Treatment Ameliorates Established Disease and Provides Long-Term Therapeutic Effects in an SKG Arthritis Model

  • Although pathogenesis of human rheumatoid arthritis (RA) remains unclear, arthritogenic T cells and downstream signaling mediators have been shown to play critical roles. An increasing numbers of therapeutic options have been added for the effective control of RA. Nevertheless, there is still a category of patients that fails treatment and suffers from progressive disease. The recently developed immunosuppressant CP-690550, a small molecule JAK kinase inhibitor, has been implicated as an important candidate treatment modality for autoimmune arthritis. In this study, we evaluated the therapeutic effect of CP-690550 on established arthritis using an SKG arthritis model, a pathophysiologically relevant animal model for human RA. CP-690550 treatment revealed remarkable long-term suppressive effects on SKG arthritis when administered to the well-advanced disease (clinical score 3.5~4.0). The treatment effect lasted at least 3 more weeks after cessation of drug infusion, and suppression of disease was correlated with the reduced pro-inflammatory cytokines, including IL-17, IFN-${\gamma}$, and IL-6 and increased level of immunoregulatory IL-10.

Plasma Prolactin, Blood Metabolites and Yield and Composition of Milk during Early Lactation in Goats Following Administration of Bromocryptine

  • Six crossbred goats in their 2nd or 3rd lactation, were administered bromocryptine at 5 mg/day during early lactation of 15-20 days (period I) and thereafter again at an interval of 13 days, bromocryptine was given for 5 days (period II). Blood samples were collected before (-5, -4, -3, -2, -1) during (1, 2, 3, 4, 5) and after (+1, +2, +3, +4, +5) administration of bromocryptine in both the periods of study. In period I, administration of bromocryptine resulted in a decrease in milk yield to the extent of 16..8% in comparison to before treatment, and 28.5% after the cessation of treatment. The glucose content of blood increased (p<0.01) as the milk yield decreased without any change in NEFA concentration. During period II of bromocryptine treatment the milk yields did not change in spite of a decline in prolactin level, perhaps the effect of previous treatment was prolonged. A decline in protein and lactose content of milk after bromocryptine treatment in both the periods of study, when prolactin level also declined suggests a role of prolactin in protein synthesis and also a depressing effect on lactose synthesis.

  • Purpose: The aim of this study is to report a case of oral bisphosphonate-related osteonecrosis of the jaw (BRONJ) resulting in implant failure. Methods: A patient suspected of having BRONJ was referred to the Department of Periodontology, Kyung Hee University School of Dentistry for the evaluation and treatment of exposed bone around implants. Results: The patient, who had been taking oral bisphosphonates (BPs) for about a year, was successfully treated with systemic antibiotics, chlorhexidine mouth rinse, explantation, and surgical debridement of necrotic bone. Conclusions: The results of this case suggest that a patient taking BPs orally should be treated cautiously. Appropriate management including cessation of BPs and respective dental treatment may reduce the development of BRONJ.

  • We investigated the increased risk of Clostridium difficile infection (CDI) caused by the combined use of antibiotics and an immunosuppressive drug in a mouse model. Our data showed that an approximate return to pretreatment conditions of gut microbiota occurred within days after cessation of the antibiotic treatment, whereas the recovery of gut microbiota was delayed with the combined treatment of antibiotics and dexamethasone, leading to an increased severity of CDI. An alteration of gut microbiota is a key player in CDI. Therefore, our data implied that immunosuppressive drugs can increase the risk of CDI through the delayed recovery of altered gut microbiota.

  • Lateral antabrachial cutaneous neuropathy(LACN) was diagnosed in a 42-year-old woman who developed pain and paresthesia in the left forearm after several days of heavy labor. The symptoms were resolved with conservative treatment, including cessation of heavy labor and a brief course of oral corticosteroids. But the symptoms recurred after 9 months. Those were also resolved with same treatment as the first attack. LACN is important to recognize because the symptoms may mimic the pathology of a cervical root, the brachial plexus, the radial and median nerves at the level of the elbow, and a focal idiopathic inflammatory neuritis.

  • Sleep-disordered breathing (SDB) is defined as a disturbed breathing during sleep caused by repetitive upper airway collapse. Complete collapse causes a cessation of breathing, known as obstructive sleep apnea (OSA) and snoring can arise from partial collapse. Undiagnosed and untreated OSA means recurrent intermittent hypoxemia and leads to a variety of cardiovascular disorders, disturbed neurocognition, and excessive daytime sleepiness. Various behavioral modalities have been suggested for treating snoring and sleep apnea including changing the sleep position, avoiding alcohol, and weight loss. Until now continuous positive airway pressure (CPAP) therapy is one of effective treatment for patients with OSA, but its discomfort causes less tolerance and compliance. Therefore, clinical effectiveness and convenience for oral appliance have emerged and the role of dentists has become more important in the management of OSA.

  • Psoriasis is a common, chronic, recurrent, immune-mediated inflammatory disorder of skin. Although conventional topical and systemic therapies yield remission for a while, they carry a risk of many side effects that limit the long term use of these agents and the disease recurs generally in a short period of time after cessation of therapy. An increasing number of psoriatic patients are using herbal products as a treatment to control their disease with less side effects. Clinicians should be aware of plants used in psoriasis and side effects related with them. The aim of this article is to review the different herbs used in psoriasis treatment.

The Effect of Indwelling Silk Suture Following Aspiration in the Treatment of Chronic Lateral Malleolar Bursitis (흡입 후 견 봉합사 거치를 통한 만성 족관절 외과 점액낭염의 치료)

  • Purpose: To evaluate the drainage effect of silk suture following aspiration of the bursa as an early treatment of chronic lateral malleolar bursitis. Materials and Methods: Thirteen cases, which have over two weeks of history and over one year of follow-up, were investigated. The average duration of follow-up was 16.4 months. The average symptom duration before introduction into this study was 7.8 weeks. With an aseptic technique, the aspiration of the bursa was done with 18G needle and syringe and then the insertion of silk suture through the aspiration needle was performed. The amount of drainage was identified two or three times in a week and stitch out was done at the cessation of drainage. Over one year follow-up, recurrence, infection, pain, and limitation of range of motion were investigated by telephone interview. Results: Redness around the insertion site of silk suture was found in all cases, but there was no development of active infection or recurrence. The average duration of treatment is 10.4 days. Conclusion: The drainage with silk suture following aspiration of the bursa is less invasive and very effective method in the early treatment of chronic lateral malleolar bursitis.
