Blood Protein Adsorption and Platelet Activation on an Ultra-hydrophilic Substrate (초친수성 표면에서 혈장 단백의 흡착 및 혈소판의 변화에 관한 연구)

  • Background: We evaluate the efficacy of ultra-hydrophilic coated bypass circuits in comparison with uncoated bypass circuits in a porcine cardiopulmonary bypass model. Material and Method: Normothermic cardiopulmonary bypass was peformed in 10 anesthetized pigs via the left atrium and ascending aorta with a centrifugal biopump. Ultra-hydrophilic coated bypass circuits wore used in 5 pigs (the study group) and uncoated bypass circuits were used for the control group. Platelet counts and platelet aggregation tests were peformed. The thrombin-antithrombin(TAT) complex level and total protein level were evaluated. Result: There were no significant changes En the platelet counts and aggregation ability of both groups. The TAT complex levels were not different between the two groups. The total protein level was significantly lower in the control group after cessation of cardiopulmonary bypass. Conclusion: The clinical effects of ultra-hydrophilic coating circuits were not remarkable, in terms of reducing inflammatory reaction and protection of platelet function. However, the effect of protection for blood protein adsorption might be acceptable.

High School Students' Knowledge and Attitude about Smoking according to Their Smoking Status (일 지역 고등학생의 흡연상태에 따른 흡연지식과 흡연태도)

  • Purpose: This study was to provide basic information for preventing high school students from smoking by examining their actual smoking condition and their knowledge and attitude about smoking. Method: The subjects were 515 students selected from six(6), high schools in Jecheon. Data were collected using a questionnaire from the 1st to the 17th of December 2002. The author prepared the tool by modifying the scale of knowledge and attitude about smoking developed by Moon(2001). Data were analyzed with SPSS/WIN program using frequencies, percentages, $x^2$-test, t-test, ANOVA. Duncan test and Pearson's correlation coefficient. Result: 1. The percentage of ex-smoking students was 29.9% of male students and 18.6% of female ones. The percentage of currently smoking students was 23.9% of male students and 3.0% of female ones. The percentage of non-smoking students was 46.2% of male students and 78.4% of female ones, 2. There were not significant differences in smoking knowledge according to smoking status in male and female students. 3. There were significant differences in smoking attitude according to smoking status in male(F=32.70, p=.000) and female(F=11.15, p=.000) students. 4. In smoking status according to general characteristics, there were significant differences according to the type of school($x^2$=32.93, p=.000), grade($x^2$=11.91, p=.018), educational level of the father($x^2$=30.52, p=.000) and the mother($x^2$=14.23, p=.027), monthly allowance($x^2$=17.19, p=.028), harmony of family members($x^2$=15.23, p=.019), school life($x^2$=26.96, p=.00l) and school record($x^2$=30.24, p=.000). 5. In smoking status according to smoking-related characteristics, there were significant differences according to experience in cigarette purchase($x^2$=150.04. p=.000), experience in errands for cigarette($x^2$=20.00, p=.000), knowledge of cigarette price($x^2$=72.52, p=.000), close friends' smoking($x^2$=107.41. p=.000) and smoking of brothers and sisters($x^2$=16.97, p=.002). 6. There were significant correlation between knowledge and attitude about smoking in the whole students(r=.337. p=.000), ex-smokers(r=.324. p=.000), non-smokers(r=.369. p=.000) but there was not a significant correlation between knowledge and attitude about smoking in current smokers(r=.128. p=.30l). Conclusion: Smoking prevention education should considered students' smoking status. Current smokers need smoking prevention education aiming to change their attitude toward smoking in order for them to reduce smoking and to practice smoking cessation.

Subhective Symptoms and Work-related Health Risk Factors in Korean Dental Laboratory Technicians (우리 나라 치과 기공사의 신체 자각 증상과 직업 관련 건강 위험 요인)

  • Although dental laboratory technicians are prone to be exposed to various work-related health hazardous materials such as dusts, chemicals, etc., the prevalence and nature of work-related health problems of them have not been a matter of great concern in the field of occupational health service in Korea. The purpose of the present investigation was to describe a collected profile of subjective health symptoms and their attributable factors in Korean dental laboratory technicians. A questionnaire listing five groups of health symptoms and five health symptom-related factors was mailed to randomly selected 1,900 dental laboratory technicians. Among them, 1,344 dental laboratory technicians filled out the questionnaires and returnde them. Five groups of health symptoms included musculoskeletal symptom, dermal symptoms, respiratory symptoms, eys symptoms, and ear symptoms. Five health symptom-related factors were occupational environment-related health risk factors, work history, health related habits and status, use of personal protective equipment and general characteristics. Detailed parameters of health risk factors were work posture, vibration, and chemical or physical hazards such as dust, fume, vapor, solvent, light, and noise for occupational environment-related factors; work place, area, number of employees, work hours, career, work part, and work load for work history; Broca's index, hours of sleep, eating, smoking, alcohol, exercise, health examination, and self assessed health status for health habits and status; face masks, goggles, and so on for use of personal protective equipment, and; age, sex, marital status, and education for general characteristics. Before the start of main survey, a pilot survey was carried out for validity and reliability tests of the questionnaire. All the data obtained were coded and analyzed with PC/SAS 6.12 program. The prevalence of health symptoms was the highest in musculoskelton (87.3%), and followde by eyes (78.9%), respiratory organs (64.3%), ears (57.8%), and skin (52.2%) in descending order. Statistically significant risk factors by multiple logistic regression analyses were sex, health examination, self assessed health status, and hand/finger posture in musculoskeletal symptoms; sex, self assessed health status, career, acid gas, and hand contact with resin mixture in deraml symptoms; Broka's smoking, exercise, self assessed health status, and face mask in respiratory symptoms; sex, hours of sleep, self assessed health status, work hours, work load, plaster dust, inadequate lighting, and goggle in eys symptoms, and eating, smoking, self assessed health status, and work load in ear symptoms. With the above considerations in mind, prevalence of subjective symptoms among Korean dental laboratory technicians was relatively high, and they were attributable to most of the occupational environment-related factors, work history, use of personal protective equipment, health habits and status, and general characteristics. Particularly, it is suggested that health promotion programs for promoting self- assessed health status and smoking cessation, preventive measures for protection of the female technicians’health, and reducing work load be necessary, since those factors were associated with more than one subjective symptom.

Health Practices of University Students (일부 대학생들의 건강관련행위)

  • This paper analyzed health practices of students in an university in Chollabukdo Province. It attempted to study how many university students had what kinds of health practices. Furthermore, it tried to find out what are the major problems in health practice among university students. To study health practices of university students, this study sampled 1,469 students randomly. Many of the students had poor practices of teeth care. In tooth-brushing, many students (35.8%) brushed their teeth before breakfast in the morning. Most of the students (95% or more) visit dental clinic only when they had problem(s) in their teeth. For weights, there exists a great gap between perception and BMI (Body Mass Index), especially among female students. Among the female students who perceived their weights were above average, 96.4% of them were normal according to BMI. Many of the students were drinking greater amount of alcohol than safe amount suggested by the National Health and Medical Research Council of Australia. The proportion of the students drinking harmful or hazardous amount of alcohol were 79.5% of the male students and 74.9% of the female students. It suggests that health promotion fund or tax need to be levied on alcohol to frustrate unhealthful drinking. More than half (59.0%) of the male students and about 6.0% of the female students were smoking cigarettes. The smoking initiation ages were 17.8 for male and 18.2 for female students. It means that many of the students started smoking in their junior high or high schools. It implies that smoking prevention and smoking cessation programs need to be emphasized in these schools. For perception on sex, 27.8% of the male students and 60.9% of the female students thought to keep their virginity before their marriage. The female students were more conservative in perception on sex. The average number of sexual intercourses per year of male students was 17.0 times with their lovers, 7.9 times with prostitutes, and 7.0 times with maids. The female students had sexual intercourses 5.0 times with their lovers and 4.0 times with waiters per year. The proportion of students who had experience of sexual intercourse with homosexual partners was 1.0%. The finding this study was that the university students were vulnerable to poor health practices. It is suggested that public health intervention program be provided for university students to keep them in good healthy lifestyle.

Impact of Chronic Hepatitis B and Hepatitis C on Adverse Hepatic Fibrosis in Hepatocellular Carcinoma Related to Betel Quid Chewing

  • The pathogenesis of hepatocellular carcinoma (HCC) related to habitual betel quid (BQ) chewing is unclear. Risk of HCCis increased with adverse hepatic fibrosis. This study aimed to assess the impact of chronic viral hepatitis on adverse hepatic fibrosis in HCC related to BQ chewing. This hospital-based case-control study enrolled 200 pairs of age- and gender-matched patients with HCC and unrelated healthy controls. Serologic hepatitis B surface antigen (HBsAg), antibodies to hepatitis C virus (anti-HCV), ${\alpha}$-fetoprotein (AFP), and surrogate markers for significant hepatic fibrosis were measured. Information on substance-use habits was obtained with a questionnaire. By analysis of surrogate markers for hepatic fibrosis, the prevalence of significant hepatic fibrosis in patients chewing BQ was between 45.8% and 91.7%, whereas that for patients without BQ chewing was between 18.4% and 57.9%. The difference was significant (P <0.05 for each surrogate marker). Multivariate analysis indicated that cirrhosis with Child-Pugh C (odds ratio (OR) = 3.28; 95% confidence interval (CI), 1.29-8.37), thrombocytopenia (OR = 3.92, 95% CI, 1.77-8.68), AFP >400 mg/L (OR = 2.21, 95% CI, 1.05-4.66) and male gender (OR = 4.06, 95% CI, 1.29-12.77) were independent factors associated with habitual BQ chewing. In conclusion, adverse hepatic fibrosis and severe liver damage play important roles in the pathogenesis of BQ-related HCC, which could be aggravated by chronic hepatitis B and hepatitis C. BQ-cessation programs and prevention of chronic HBV/HCV infection are needed to prevent HCC related to BQ chewing.

Reparability and Surgical Indications of Rotator Cuff Tears (회전근 개 파열의 봉합 가능성 및 수술 적응증)

  • Purpose: The reparability of rotator cuff tears is very important to establish the indication of the surgical treatment of rotator cuff tears. We reviewed the factors that effect to reparability of rotator cuff tears, and considered about the surgical indication of rotator cuff tears. Materials and Methods: The reparability was considered by the surgical reparability and the postoperative healing potentials of rotator cuff. The natural history of rotator cuff tear gives us the information to make decision about the surgical treatment or the conservative treatment. Results: There are 3 general categories of factors that help predict the healing potential of a rotator cuff tear. These include surgical technique, biology, and environmental issues. Surgeon can control the surgical technique by himself. The control of biology comes from careful operative indications by age, acute trauma history, chronicity, and tear size. And the control of environmental factor can give better outcomes by cessation of smoking and nonsteroidal anti-inflammatory drug (NSAID). Conclusion: A better understanding of the natural history and the reparability of rotator cuff tear will help us to estabilish the indication of surgical treatement of rotoator cuff tears, and lead to satisfactory clinical outcomes.

A comparative study on Self-esteem, Organizational commitment, Transformational leadership of ROTC Cadets: focus on a non-smoker and smoker (학군사관후보생의 자아존중감, 조직몰입, 변혁적 리더십 비교 -비흡연자와 흡연자 중심으로)

  • This study was conducted to compare physical fitness, self-esteem, organizational commitment and transformational leadership among non-smoker and smoker ROTC cadets. This study used 425 questionnaires from male grade 3 and 4 ROTC cadets of 12 universities located in P city and G-do. Data were collected from May 1 to June 23, 2017. The data were analyzed by SPSS/WIN 23.0 using a ${\chi}^2-test$, independent t-test, and Pearson's correlation coefficient. Running 3km (t=3.26, p=0.046), self-esteem (t=1.06, p=0.003), organizational commitment (t=2.57, p<0.001) and transformational leadership (t=3.98, p<0.001) were significantly lower in the smoking group than the non-smoking group. There was a positive correlation between self-esteem, organizational commitment, and transformational leadership in both non-smokers and smokers. Therefore, to practice smoking cessation for the health promotion of ROTC cadets smokers, it is necessary to develop an effective education and intervention program that can enhance self-esteem, organizational commitment and transformational leadership.

The Diagnosis and Treatment of Osteoporosis (골다공증의 진단과 치료)

  • Osteoporosis, a disease characterized by low bone mass and microarchitectural deterioration of bone tissue leading to enhanced bone fragility and fracture risk, is a major public health problem. The diagnostic methods for osteoporosis include simple radiography, bone scan, DXA (Dual energy X-ray Absortiometry) and biochemical markers of bone turnover. Optimal treatment and prevention of osteoporosis require modification of risk factors, particularly smoking cessation, adequate physical activity, and attention to diet, in addition to pharmacologic intervention. The estrogens and raloxifene both prevent bone loss in postmenopausal women, and the estrogens probably also decrease the risk of first fracture. There is good evidence that raloxifene prevents further fractures in postmenopausal women who already have had fractures and some evidence that estrogen does as well. Bisphosphonate prevents bone loss and reduces fractures in healthy and osteoporotic postmenopausal women and in osteoporotic men as well. Risedronate is more potent and has fewer side effects than alendronate and reduces the incidence of fractures in osteoporotic women. Calcitonin increases bone mineral density in early postmenopausal women and men with idiopathic osteoporosis, and also reduces the risk of new fractures in osteoporotic women. All of the agents discussed above prevent bone resorption, whereas teriparatide and strontium increase bone formation and are effective in the treatment of osteoporotic women and men. New avenues for targeting osteoporosis will emerge as our knowledge of the regulatory mechanisms of bone remodeling increases, although issues of tissue specificity may remain to be addressed.

Evidence for Adenosine Triphosphate (ATP) as an Excitatory Neurotransmitter in Guinea-Pig Gastric Antrum

  • We explore the question of whether adenosine 5'-triphosphate (ATP) acts as an excitatory neurotransmitter in guinea-pig gastric smooth muscle. In an organ bath system, isometric force of the circular smooth muscle of guinea-pig gastric antrum was measured in the presence of atropine and guanethidine. Under electrical field stimulation (EFS) at high frequencies (>20 Hz), NO-mediated relaxation during EFS was followed by a strong contraction after the cessation of EFS (a 'rebound-contraction'). Exogenous ATP mimicked the rebound-contraction. A known $P_{2Y}-purinoceptor$ antagonist, reactive blue 2 (RB-2), blocked the rebound-contraction while selective desensitization of $P_{2Y}-purinoceptor$ with ${\alpha},{\beta}-MeATP$ did not affect it. ATP and 2-MeSATP induced smooth muscle contraction, which was effectively blocked by RB-2 and suramin, a nonselective $P_2-purinoceptor$ antagonist. Particularly, in the presence of RB-2, exogenous ATP and 2-MeSATP inhibited spontaneous phasic contractions, suggesting the existence of different populations of purinoceptors. Both the rebound-contraction and the agonist-induced contraction were not inhibited by indomethacin. The rank orders of agonists' potency were 2-MeSATP > ATP ${ge}$ UTP for contraction and ${\alpha},{\beta}-MeATP\;{\ge}\;{\beta},{\gamma}-MeATP$ for inhibition of the phasic contraction, that accord with the commonly accepted rank order of the classical $P_{2Y}-purinoceptor$ subtypes. Electrical activities of smooth muscles were only slightly influenced by ATP and 2-MeSATP, whereas ${\alpha},{\beta}-MeATP$ attenuated slow waves with membrane hyperpolarization. From the above results, it is suggested that ATP acts as an excitatory neurotransmitter, which mediates the rebound-contraction via $P_{2Y}-purinoceptor$ in guinea-pig gastric antrum.

Regulatory Role of Cyclic Nucleotides in Non-Adrenergic Non-Cholinergic Relaxation of Lower Esophageal Sphincter from Dogs (개 하부식도괄약근의 비아드레날린성, 비콜린성 이완반응에 있어서 Cyclic Nucleotide의 역할)

  • The role of the lower esophageal sphincter(LES) is characterized by the ability to maintain tone and to relax allowing the passage of a bolus. It is known that LES relaxation during swallowing may be induced by the cessation of the tonic neural excitation and the activation of non-adrenergic, non-cholinergic(NANC) inhibitory neurons. Furthermore, it is generally accepted that the relaxation of the smooth muscle is mediated primarily by the elaboration of adenosine 3',5'-cyclic monophosphate(cyclic AMP) and guanosine 3',5'-cyclic mono-phosphate(cyclic GMP) via activation of adenylate cyclase and guanylate cyclase, respectively. It is thus possible that cyclic nucleotides might be a second messenger involved in neural stimulation-induced relaxation of LES, although a relationship between relaxation and changes in cyclic nucleotides after neural stimulation has not been established. The present study was performed to define the participation of cyclic nucleotides in the relaxation of LES of dog in response to neural stimulation. Electrical field stimulation(EFS) caused relaxation of the canine isolated LES strips in a frequency-dependent manner, which was eliminated by pretreatment with tetrodotoxin$(1{\mu}M)$, but not by atropine$(100{\mu}M)$, guanethidine$(100{\mu}M)$ and indomethacin$(10{\mu}M)$. The nitric oxide synthase inhibitors, $N^G-nitro-L-arginine$, $N^G-nitro-L-arginine$ methyl ester and $N^G-monomethyl-L-arginine$ inhibited EFS-induced relaxation. Additions of sodium nitroprusside, a nitrovasodilator and forskolin, a direct adenylate cyclase stimulant, caused a dose-dependent relaxation of LES smooth muscle. Effects of sodium nitroprusside and forskolin were selectively blocked by the corresponding inhibitors, methylene blue for guanylate cyclase and N-ethylmaleimide(NEM) for adenylate cyclase, respectively. Dibutyryl cyclic AMP and dibutyryl cyclic GMP caused a concentration-dependent relaxation of the LES smooth muscle tone, which was not blocked by NEM or methylene blue, respectively. However, both NEM and methylene blue caused significant antagonism of the relaxation in LES tone in response to EFS. EFS increased the tissue cyclic GMP content by 124%, whereas it did not affect the tissue level of cyclic AMP. Based on these results, it is suggested that one of the components of canine LES smooth muscle relaxation in response to neural stimulation is mediated by an increase of cyclic GMP via the activation of guanylate cyclase. Additionally, an activation of cyclic AMP generation system was, in part, involved in the EFS-induced relaxation.
