울혈성 심부전 환자의 자가관리 이행, 가족 지지 및 우울 (Self Care Compliance, Family Support, and Depression in Patients with Congestive Heart Failure)

  • Purpose: The purpose of this study was to examine self care compliance, family support, and depression in patients with congestive heart failure. Method: The participants were 105 outpatients with congestive heart failure. Data were collected using a questionnaire survey on self care compliance, family support, and depression. The collected data were analyzed using the SPSS Win 10.0 program. Result: 1. The total score for Self care compliance was 78.34 out of a possible 120 and was significantly high for people in ages between 70-79 years and people who were not employed. Exercise compliance was significantly high in men and classifications I & II of the NYHA class. Compliance with smoking and alcohol cessation were significantly high in women. Medication compliance was significantly high in the group which had experience with hospitalization. Family support was 31.75 out of a possible 40 and showed significant differences depending on spouse status and religion. Depression was 30.18 out of a possible 64 and was significantly high in the group whose educational level was above college graduation or who were under 65 years of age. Conclusion: The results suggest that nurses have to emphasize smoking and alcohol cessation for men, exercise for women and total self care compliance for patients under 65 years of age.

급성관상동맥증후군 남성 환자의 흡연과 스트레스에 영향을 미치는 요인 (Predicting Factors of Smoking and Emotional Stress among Male Patients with Acute Coronary Syndrome)

  • Purpose: This study was conducted to identify the factors that predict a current smoking behavior and higher emotional stress among male patients with acute coronary syndrome (ACS). Methods: The study was approved by an institutional review board from a university hospital, 2010. A face to face interview using questionnaires was performed with 185 first-time ACS male patients who were undertaken a percutaneous coronary intervention at a cardiovascular care unit. Data were analyzed using SPSS/WIN 15.0. Results: About 54% of the study subjects were currently smoking. The current smokers had dyslipidemia and reported bad eating habits compared to the non-smokers. The current smokers were younger, living alone, and reported lower perceived benefit on smoking cessation than the non-smokers, and 15% of them did not consider quitting (precontemplation stage). Smoking status was not significantly related to emotional stress. Logistic regression analysis revealed that being employed including professional or labor increased the odds of current smoking four or three times compared to the non-employed or retired. Low income or dyslipidemia also increased the likelihood of current smoking 2.8 and 2.1 times, respectively. Blue collar workers or heavy drinkers had 2.9 and 2.8 times more risks of having higher level of stress. Conclusion: An occupational background and health habits should be considered to develop an effective educational strategy for smoking cessation and stress reduction among male patients with ACS.

남자 흡연자의 금연의도 관련 요인: 호남지역 13개 지역 2010 지역사회 건강조사 (Factors Associated with Intention to Quit Smoking among Male Smokers in 13 Communities in Honam region of Korea: 2010 Community Health Survey)

  • Objectives: This study is to identify factors associated with intention to quit smoking among male adult smokers in 13 communities of Korea. Methods: Data from 2010 Community Health Survey (CHS) in 13 communities of Honam region, Korea, were analyzed. Multiple logistic analysis method was used to determine the influence of selected factors on the intention to quit smoking among male smokers. Results: Residential area, smoking amounts per day, exposure to secondhand smoking, past attempts to quit, walking activity, brushing teeth after lunch, and stress were found to be independently associated with intention to quit smoking. Socio-demographic factors including age, marital status, educational level, and monthly income were not associated. Conclusions: Identifying factors associated with intention to quit smoking provides possibilities for shaping effective policies and programs for increasing smoking cessation in Korea.

금연에 관한 성인남성의 인식 및 금연동기 조사 (Understanding Male Adult's Awareness and Motivation of Non-smoking)

  • 본 연구는 흡연자와 비 흡연자의 금연결심에 대한 인식구조를 알아보고, 금연 교육기관과 금연 교육자의 선택 및 흡연자의 금연 동기부여 관련사항을 알아보고자 시행된 연구이다. 흡연자와 비 흡연자는 금연 정책 인식에 차이가 있었고, 금연 교육기관의 선택에서는 보건소가 높았으며, 금연 교육자는 내과의사의 비중이 높았으나 인식차이는 없었다. 일반적 사항에 따른 금연 동기부여 간 관련성에서는 폐암사진, 치과치료, 담배 값 인상, 금연 구역지정, 금연 광고 등이 흡연자의 금연 결심 원인(동기)와 금연의 동기부여 간의 결과에서 통계적으로 유의하게 나타났다[P=.000].

비스포스포네이트로 인한 하악 및 상악골에 발생한 골괴사에 대한 임상적 연구 (CLINICAL STUDY OF BISPHOSPHONATE-INDUCED OSTEONECROSIS OF MANDIBULAR AND MAXILLARY BONE)

  • Bisphosphonates are compounds widely used in the treatment of various metabolic and malignant bone disease. Recently, an association between bisphosphonate use and a rare dental condition termed 'osteonecrosis of the jaw(ONJ)' has been reported. Bisphosphonate-related osteonecrosis of the jaw(BRONJ) is rare, but serious, side effect of bisphosphonate therapy in affected patients. It is characterized by poor wound healing and spontaneous intra-oral soft tissue break down, which lead to exposure of necrotic maxillary and mandibular bone. We reviewed 11 patients of BRONJ visited Ajou University Hospital Dental clinic from May 2007 to November 2008. The management of the patients included cessation of bisphosphonate therapy and various surgical restorative procedures and conservative care there after. Aggressive debridement is contraindicated. A new complication of bisphosphonate therapy administration, osteonecrosis of jaws, seems to be developing. The improved results after cessation of the medication should make clinicians reconsider the merits of the rampant use of bisphosphonates, while further investigation is needed to completely elucidate this complication.

뇌졸중 흡연노인의 금연의도 예측요인 (Predictors of Intention to Quit Smoking in Elderly Smokers following a Stroke)

  • Purpose: This study was done to identify factors based on the theory of planned behavior that predict smoking cessation in elderly smokers following a stroke. Method: Between March and May 2013, data related to intention, attitude toward the behavior, subjective norm, and perceived behavioral control, were collected through questionnaire from 223 elders who smoked and who had a stroke. Data analysis was performed using PASW (SPSS) 18.0 program, and included t-test, one-way ANOVA, Pearson correlation coefficients, and multiple stepwise liner regression analysis. Results: Results showed a positive correlation between intention and attitude toward the behavior (r=.494, p<.001), subjective norm (r=.390, p<.001), and perceived behavioral control (r=.189, p=.005). These variables such as perceived behavior control, subjective norm, number of cigarettes per day, and usefulness of quitting smoking explained 50% of intention to quit smoking. Conclusion: The results of this study indicate that there is a need to develop smoking cessation nursing strategies for elderly smokers following a stroke in order to provide effective counseling on subjective norm and perceived behavioral control.

Factors associated with Intermittent and Light Smoking among Korean High School Students: Intermittent and Light Smoking among Korean Adolescents

  • Purpose: The purpose of this study was to identify factors associated with intermittent and light smoking among Korean high school students. Methods: In this cross-sectional study, we employed secondary data from the 2015 Korea Youth Risk Behavior Web-Based Survey, and used the biopsychosocial model as a framework. The analysis was performed using the data of 2,851 high school students who smoked. We defined intermittent and light smoking as smoking on 1 to 29 days in a 30-day period and no more than 10 cigarettes per day. A logistic regression analysis using the complex samples procedure was conducted. Results: Among all the participants, 1,231 (43.2%) were intermittent and light smokers. Factors significantly predicting intermittent and light smoking were gender and grade (biological factors); subjective stress (psychological factor); and mother's smoking, sibling's smoking and academic achievement (sociocultural factors). Conclusion: In smoking cessation programs, health care providers both at school and in the community should consider the unique biological, psychological, and sociocultural characteristics of intermittent and light smoking behavior among high school students.

The Status and Future Challenges of Tobacco Control Policy in Korea

  • Tobacco use is the most important preventable risk factor for premature death. The World Health Organization (WHO) Framework Convention on Tobacco Control (FCTC), the first international public health treaty, came into force in 2005. This paper reviews the present status of tobacco control policies in Korea according to the WHO FCTC recommendations. In Korea, cigarette use is high among adult males (48.2% in 2010), and cigarette prices are the lowest among the Organization for Economic Cooperation and Development countries with no tax increases since 2004. Smoke-free policies have shown incremental progress since 1995, but smoking is still permitted in many indoor public places. More than 30% of non-smoking adults and adolescents are exposed to second-hand smoke. Public education on the harmful effects of tobacco is currently insufficient and the current policies have not been adequately evaluated. There is no comprehensive ban on tobacco advertising, promotion, or sponsorship in Korea. Cigarette packages have text health warnings on only 30% of the main packaging area, and misleading terms such as "mild" and "light" are permitted. There are nationwide smoking cessation clinics and a Quitline service, but cessation services are not covered by public insurance schemes and there are no national treatment guidelines. The sale of tobacco to minors is prohibited by law, but is poorly enforced. The socioeconomic inequality of smoking prevalence has widened, although the government considers inequality reduction to be a national goal. The tobacco control policies in Korea have faltered recently and priority should be given to the development of comprehensive tobacco control policies.

건강신념모델을 이용한 청소년 흡연 행위 예측 (Predicting Adolescents' Smoking Behavior Using Health Belief Model)

  • This study was conducted to explain adolescents' smoking behavior using Health Belief Model. A cross-sectional study design was used to analyze characteristics and factors that influence smoking behavior among Korean adolescents. A total of 1.535 adolescents attending 21 high schools in one district of Seoul participated in the study. The results indicated that perceived susceptibility, severity, benefits, and barriers were significant in predicting smoking behavior of adolescents. Likewise, the degree of juvenile delinquency, gender, the amount of pocket money, and having smoking parent(s) and friend(s) significantly affect smoking behavior of adolescents. Based on the study results, it is recommended to incorporate HBM components in smoking cessation programs, and to include parents and friends in smoking prevention and cessation programs for high school students in Korea.

사업장 규모별 금연 및 절주 프로그램 공급 실태 및 수유 (A Study on the Supply and Demands for Worksite Health Promotion Programs)

  • This study investigates the worksite resources and programs for health promotion services, especially in areas of smoking cessation and acohol-reducing. Health promotion program is important for employees and employers to promote their productivity and enhance their quality of life. To explain the worksite health promotion programs, a three-stage survey model was established and 111 worksites filled up the questionnaire. Stages of the model included the supply status of worksite health programs, attitudes to get rid of health risks, and behavioral intentions to provide health promotion programs in the near future. The results of this study are as follows. First, the facility and personnel for health promotion services are not equipped sufficiently in the middle-sized worksites. Second, provided programs are not good enough in both quantity and quality, because most worksites provide inefficient and low-cost programs. Third, worksites provide the programs such as advertisement, education materials especially in large-sized worksites, but not in middle-sized worksites. Therefore, worksites need to be supported with a public institution for efficient programs and continuing legal and systematic support for middle-sized worksites should be emphasized.
