Clinical Report of Baliao Electroacupuncture Treatment on a Stroke Patient Suffering from a Neurogenic Overactive Bladder (뇌경색을 동반한 신경인성 과민성 방광 환자의 배뇨장애 팔료혈 전침 치료 1례)

  • Objective: The purpose of this case study is to report the effectiveness of Baliao acupoints electro-acupuncture treatment on a neurogenic overactive bladder induced by stroke. Method: A patient suffering from middle cerebral artery infarction was diagnosed with a neurogenic overactive bladder and treated by Korean medicine, including electro-acupuncture on Baliao acupoints six times per week. The evaluation was performed by monitoring urinary frequency and using the Korean versions of the Overactive Bladder Symptom Score (OABSS) and Urinary Incontinence Quality of Life (I-QoL). Results: After 27 days of electro-acupuncture treatment, the OABSS score improved from 6 to 3. The I-QoL total score improved from 60 to 87. The patient also showed a decrease in nocturia frequency. Conclusion: This case suggests that electro-acupuncture on Baliao acupoints can be effective for overactive bladders due to supra-pontine lesions.

  • Objectives : This study had been done to evaluate the effectiveness of DITI about hemiparesis in 55 patients with cerebral vascular disease who admitted to Dong-seo oriental hospital from November. 10. 1999 to October. 13. 2000, Methods : 55 stroke patients who had hemiparesis not only numbness nor quadriparesis were taken picture with DITI to measure infrared streamed from body's surface. And we compared DITI' results with hemiparesis to evaluate the effectiveness of DITl. Results : There was significant results between DITI and hemiparesis. Above 80%, we could find abnormality in the DITI pictures, which were exactly corresponding to symptom of hemiparesis. Conclusions : We thought that we can use DITI' results as a evaluation of stroke patients with hemiparesis.

A Case of Ruptured Peripheral Aneurysm of the Anterior Inferior Cerebellar Artery Associated with an Arteriovenous Malformation : A Less Invasive Image-Guided Transcortical Approach

  • A 47-year-old man presented with a subarachnoid hemorrhage (SAH) and right cerebellar hematoma was referred for evaluation. Cerebral angiography revealed a distal anterior inferior cerebellar artery (AICA) aneurysm associated with an arteriovenous malformation (AVM). Successful obliteration and complete removal of the aneurysm and AVM were obtained using transcortical approach under the guidance of neuronavigation system. The association of a peripheral AICA aneurysm and a cerebellar AVM by the same artery is unique. The reported cases of conventional surgery for this disease complex are not common and their results are variable. Less invasive surgery using image-guided neuronavigation system would be helpful and feasible for a peripheral aneurysm combining an AVM of the posterior fossa in selective cases

  • In 123 examples of admission patients at Dongshin university oriental medical hospital circulatory internal medicine, we sequently investigated serum lipid, at admission state, 2th and 4th weeks. The results were as follows; 1. In the case of 123 stroke patients, only 12cases were related to Hyperlipemia. So stroke was not related to Hyperlipemia significantly. 2. According to sex, woman has more significant results in the T-cholesterol and LDL-cholesterol. 3. According to the living Zone, Triglyceride of the city zone group washigher than that of the contryside zone significantly. 4. According to the history of drinking and smoke, the patients who have the history of drinking and smoke had a significant remark in T-cholesterol and LDL-cholesterol. 5. In the case of patients like eating meat, it was significant gain of HDL-cholesterol. 6. In the patients who had past history of Hypertension, the value of Triglyceride is significantly low, and sequently as blood pressure decreased, serum lipid value decreased too. 7. In the patients who had past history of D.M., the value of Triglyceride is significantly high, and sequently as blood sugar value decreased, serum lipid value decreased too. 8. In the case of patients treated He-Ne laser, Triglyceride was decreased significantly. 9. In the case of treating the stroke patients, serum lipid had non-significant decrease value.

Change of Depression According to Physical Therapy in Stroke Patients (물리치료적용에 따른 일부 뇌졸중환자의 우울증 변화)

  • Objectives: This study was to identify the effect of physical therapy and related factors on depression in stroke patients. Methods: Questionnaires were completed by 46 stroke patients at physical therapy room in Busan Dongeui Medical Center from September 1999 to April 2001. The self·rating depression scale(SDS)was used to assess the differences for depression score in patients who had suffered a stroke. The design of this study was follow-up study between initial physical therapy and one month after initial. Results: A little decreased score(-0.17) was seen in patients treated with physical therapy in difference of depress between initial physical therapy and one month after initial, but there was no significant difference. In gender, male had -0.42 decreased score, female. on the contrary, had +0.09 increased score. Depress score for 50 years group was decreased(-2.08), but below 50 years and above 60 were increased in age (+1.25 and +0.27, respectively). There were no significant difference in weight, educational level, religion, side of paresis, and diabetes. Significant variables related to difference of depress in multiple regression analysis were weight(p=0.0082)and diagnosis(cerebral infarction, hemorrhage)(p=0.0473, R2=0.4241). Conclusions: This study was indicated that the more weight was a higher, the higher difference of depress score was. Difference of hemorrhage patients was smaller than that of infarction patients.

Asymptomatic Penetration of the Oculomotor Nerve by a De Novo Aneurysm Associated with Severe Atherosclerotic Stenosis of the Supraclinoid Internal Carotid Artery

  • A 70-year-old woman presented with headaches and recurrent stroke symptoms. During five years, the patient has been treated for cerebral infarction associated with severe atherosclerotic stenosis of the internal carotid artery. Three-year follow-up magnetic resonance angiography showed a tiny de novo aneurysm arising from the distal part of atherosclerotic internal carotid artery. And 5-year follow-up three-dimensional CT angiogram demonstrated a definite aneurysm enlargement as large as requiring treatment. During dissection of aneurysm, the oculomotor nerve was found to be penetrated with the growing de novo aneurysm. The authors report a case of a de novo aneurysm, which resulted from atherosclerotic stenosis of the internal carotid artery at the supraclinoid portion, that was found to be penetrating the oculomotor nerve with no ocular palsy.

The Effect of NEES on the Occurrence of Caspase-3 in the Cerebellum of Rats with Transient Global Ischemia

  • The cerebellum is known to control balance, equilibrium, and muscle tone. If the cerebellum becomes damaged, the body is unable to retain its balancing functions or involuntary muscle movement. This is why, in stroke patients, there is a high risk of functional disability, as well as a myriad of other disabilities secondary to stroke. Ischemia was induced in SD mice by occluding the common carotid artery for 5 minutes, after which blood was reperfused. Needle electrode electrical stimulation(NEES) was applied to acupuncture points, at 12, 24, and 48 hours post-ischemia on the joksamri. Protein expression was investigated through caspase-3 antibody immuno-reactive cells in the cerebral nerve cells and Western blotting. The results were as follows: The number of caspase-3 reactive cells in the corpus cerebellum 12 and 24 hours post-ischemia was significantly (p<.05) smaller in the NEES group compared to the GI group. caspase-3 expression 12 and 24 hours post-ischemia was significantly(p<.05) smaller in the NEES group compared to the GI group. Based on these results, NEES seems to have a significant effect on Caspase-3 in the cerebellum in an ischemic state at 12 and 24 hours post ischemia, NEES delays the occurrence of early stage apoptosis-inducing Caspase-3, delaying and inhibiting apoptosis. Further systematic studies will have to be conducted in relation to the application of this study's results on stroke patients.

Neuroprotective effect of modified Boyanghwano-Tang and the major medicinal plants, Astragali Radix and Salviae Miltiorrhizae Radix on ischemic stroke in rats (허혈성뇌졸중 흰쥐모델에서 가미보양환오탕(加味補陽還五湯)와 주요 구성약재인 황기(黃芪), 단삼(丹蔘)의 뇌신경보호효과에 대한 연구)

  • Objectives : In this study, the neuroprotective effects of modified Boyanghwano-Tang (mBHT) and the major medicinal plants, Astragali Radix(AR) and Salviae Miltiorrhizae Radix(SMR) were investigated in transient middle cerebral artery occlusion (tMCAO)-induced ischemic stroke of rats. Methods : mBHT(400 mg/kg) and AR(154 mg/kg) or SMR(62 mg/kg) water extract orally injected in rats after 90 min occlusion of MCA and then allow reperfusion to 24 h. Brain infarction was measured by TTC staining and the expressions of NOS isoforms and apoptotic molecules were determined in ischemic brain by Western blot. Results : The results showed that mBHT has stronger neuropreotective property through inhibitions of the PARP cleaved and caspase-3 activation in ischemic rats, and could reduced infarction volumes comparison of those of AR or SMR, respectively. While, AR extract has an angiogenic property through increasing the expressions of eNOS and VEGF, and SMR extract has a strong anti-inflammatory effects through inhibition of iNOS expression in ischemic brains. Conclusions : These results suggest that mBHT has multifactorial therapeutic advantages through anti-apoptosis, anti-inflammation and angiogenesis for ischemic stroke based on a synergistic combination of ingradients rather than monotherapy.

The Neuroprotective Effects of Carnosine in Early Stage of Focal Ischemia Rodent Model

  • Objective : This study was conducted to elucidate neuroprotective effect of carnosine in early stage of stroke. Methods : Early stage of rodent stroke model and neuroblastoma chemical hypoxia model was established by middle cerebral artery occlusion and antimycin A. Neuroprotective effect of carnosine was investigated with 100, 250, and 500 mg of carnosine treatment. And antioxidant expression was analyzed by enzyme linked immunosorbent assay (ELISA) and western blot in brain and blood. Results : Intraperitoneal injection of 500 mg carnosine induced significant decrease of infarct volume and expansion of penumbra (p<0.05). The expression of superoxide dismutase (SOD) showed significant increase than in saline group in blood and brain (p<0.05). In the analysis of chemical hypoxia, carnosine induced increase of neuronal cell viability and decrease of reactive oxygen species (ROS) production. Conclusion : Carnosine has neuroprotective property which was related to antioxidant capacity in early stage of stroke. And, the oxidative stress should be considered one of major factor in early ischemic stroke.

Stroke Recurrence in a Patient Twelve Years after Repair of a Secundum Atrial Septal Defect

  • Background: Secundum atrial septal defect (ASD) is a common congenital heart defect in adults. Patients with ASDs at high risk of cardiovascular complications undergo either surgical repair or percutaneous device closure. Case Report: We report the case of an 85-year-old male with unusual recurrent cerebral infarctions. The patient has undergone repair of secundum ASD 12 years ago. Evaluation by transesophageal echocardiography revealed a mobile mass at the patch repair site in the left atrium. The mass was surgically removed due to recurrent stroke during the anticoagulation. Conclusion: This case emphasizes the importance of regular cardiac checkup and the need to consider cardioembolic source as being part of the etiology of stroke recurrence, even if the event occurs many years after intracardiac shunt closures.
