간식 개발을 위한 뇌성마비 아동의 식품섭취 실태 (Food Behavior and Growth of Cerebral Palsy Children - A Study for the Development of Snack)

  • 본 연구는 뇌성마비 아동의 성장발달에 기여할 간식개발을 위한 기초자료를 수집하기 위해 만 $1{\sim}7$세의 뇌성마비아동과 어머니 각각 99명을 대상으로 뇌성마비 아동의 신체발육과 식습관, 영양섭취상태, 간식섭취실태를 조사한 결과는 다음과 같았다. WLI(Weight-Length Index)를 기준으로 비만도를 판정한 결과 저체중아비율은 45.5%, 정상아는 45.5%, 과체중아는 6.0%, 비만아는 3.0%로 나타났다. Waterlow 분류법을 이용하여 영양불량상태의 정도를 분류 해 본 결과 정상인 아동이 76.6%, 쇠약인 아동이 10.3%, 성장부진인 아동이 8.0%, 성장부진과 쇠약인 아동이 5.0%로 나타났다. 아동들의 영양섭취실태를 알아보기 위해 아동들이 식사 섭취한 내용을 한국인 일일 영양권장량과 비교분석한 결과 아연과 엽산의 섭취량 비율(%RI)은 모든 연령대에서 권장섭취량에 미달하였다. 권장섭취량의 75% 이하를 섭취하는 아동의 비율은 엽산(76.8%)과 아연(76.8%), 열량 (59.8%), 칼슘(52.4%), 철(52.4%)의 경우 50% 이상이었고, 권장섭취량의 125% 이상을 섭취하는 아동들의 비율이 단백질은 85.4%, 비타민 B6은 53.7%로 나타났다. 어머니들이 주로 구입하는 시판간식은 우유 및 유제품(43.5%)과 과일류 (33.3%)가 많았고 시판 간식에 대해서는 첨가물(보존료, 색소)을 가장 문제점으로 생각하고 있었다. 아동들을 위해서 성장발달에 도움이 되는 간식(50.5%)이 새로 개발되기를 가장 바라고 있어서 이에 대한 요구도가 가장 높았으며, 새로개발되기를 바라는 간식의 종류로는 떡류(47.5%)와 과자류 (24.2%) 및 빵류(22.3%)를 선호하였다. 본 연구결과를 통하여 뇌성마비 아동은 정상아동에 비해 성장이 느리고 영양소섭취량도 현저히 낮은 것을 알 수 있었다. 본 연구대상 아동들이 현재의 식품 섭취량으로만 계속 섭취한다면 정상적인 성장을 할 수 없을 것이다. 따라서 뇌성마비 아동과 같이 저영양 상태에 있는 아동의 성장발달과 건강증진을 위한 간식제품의 개발이 시급한 것으로 보인다. 새로 개발될 뇌성마비 아동을 위한 간식모형 도출결과 간식제품은 권장섭취량과 비교했을 때 가장 섭취가 부족한 것으로 나타난 아연과 엽산을 보충하여 성장발달에 보조역할을 할 수 있고 아동들의 저작기능을 향상시키고 손에 잡기 쉬운 형태인 떡의 형태가 가장 적합한 형태로 도출되었으므로 향후 이러한 간식 개발에 대한 연구가 필요하다. 본 연구의 제한점은 뇌성마비 아동의 식사 섭취 조사의 어려움으로 인하여 일일의 식품섭취량만을 조사하여 보고한 것이라 할 수 있다. 그리고 아연과 같은 미량영양소의 영양 상태를 정확하게 측정할 수 있는 생화학적 조사가 더 이루어져야 할 것이다.

뇌성마비 환아 어머니의 사회적 지지와 부담감 (Burden and Social Support of Mothers with Cerebral Palsy Children)

  • Purpose: The purpose of this study was to identify the level of burden and social support of mothers with Cerebral Palsy (CP) children, and to determine whether providing social support is an effective strategy to relieve the burden of those mothers. Method: From October to November 2007, the data were collected from 63 mothers with CP children who were admitted and treated for CP at Y rehabilitation center in Seoul. The data were analyzed with t-test, ANOVA, Scheffe's test, Pearson's Correlation Coefficient, and Stepwise Multiple Regression using SPSS 12.0. Results: Burden of mothers was related to the Gross Motor Function Classification System (GMFCS) of the CP children. Social support was negatively correlated and GMFCS was positively correlated with burden of mothers. Social support and GMFCS was statistically significantly related with burden of mothers with CP children by multiple regression analysis. Conclusion: There was a significantly negative correlation between social support and burden. It demonstrated that social support, an important factor relating to burden imposed on mothers of CP children, can be considered as an intervening variable which can contribute to decrease of burden. In addition, applying developmental screening measures are needed to decrease disability severity.

호흡 및 조음기관 훈련 프로그램이 뇌성마비아동의 말 산출 기초능력에 미치는 효과 (The Effect of Respiration and Articulator Training Programs on Basic Ability of Speech Production in Cerebral Palsy Children)

  • Cerebral palsy children represent abnormal vocalization pattern caused by respiration problem and paralyzed oral motor muscle that are the basics of speech production. Thus, this study examined the effect of respiration and articulator training programs on the basic ability of speech production in CP children. The subjects of this study were 4 children with 3 of spastic CP and 1 of ataxia CP. The respiration and articulator program was conducted in 30 sessions for 30 minutes each. Pre-test was administered twice before the program, ongoing test was administered every 5 session during the period of experiment, and post-test was administered twice. The program included speech production such as respiration training, lips, jaw, cheek, and tongue exercise, and velopharyngeal training, and related articulator training. The following results were obtained. First, all subject children were less than 5 seconds in maximum phonation time before the experiment and 2 were improved by more than 4$\sim$5 seconds during the experiment, but 2 had relatively low rising width. Second, while children with less than 30dB before the experiment became bigger in strength during the experiment, children with more than 35dB before the experiment showed a minor change. Subject child 4 had lower vocal strength in the post-test period. Finally, although each subject had individual difference in syllable diadochokinetic ability, the function was improved and the number of repetition in one respiration was also increased.

Characteristics of the Abdominal and Neck Flexor Muscles of Children with Cerebral Palsy

  • Purpose: The purpose of this study was to compare the activities of the abdominal and neck flexor muscles of children with and without cerebral palsy (CP) while lifting the head in a supine position. Methods: The subjects were eight children with CP and eight children without the disease. The activities of the external abdominal oblique (EO), internal abdominal oblique (IO), rectus abdominis (RA), sternocleidomastoid (SCM), and RA/SCM muscles were collected by surface electromyography (EMG) when the children lifted their heads. A Mann-Whitney U test was used to compare the activity of each muscle during the head-lifting exercise. Statistical significance was accepted at p<0.05. Results: The activities of the EO, IO, and RA, and RA/SCM muscles differed significantly between the children with and without CP, but there was no significant between-group difference in the activity of the SCM muscle. Conclusion: These findings suggest that the abdominal muscles are not employed as much in the activities of children with CP compared to those without the disease. Additionally, those with CP were more dependent on the neck flexor muscle during the head-lifting exercise in a supine position.

Comprehensive Physiotherapy Approaches for Children With Cerebral Palsy: Overview and Contemporary Trends

  • Cerebral palsy (CP) is a prevalent neurodevelopmental disorder characterized by motor and postural impairments caused by central nervous system dysfunction. It significantly impacts children's daily functioning and quality of life. Physical therapy is a crucial intervention for children with CP that aims to improve motor skills and functional abilities. This study aimed to provide a comprehensive overview of holistic physical therapy approaches methods specifically designed for children with CP and examine recent research trends and their implications for optimizing outcomes in this population. This study employed a narrative review approach, conducting a comprehensive examination of the current literature pertaining to physical therapy methods for children with CP. The review encompassed studies exploring assessment techniques, evidence-based interventions, and innovative approaches in the field. It was discerned that encompassing physical therapy strategies, which encompass individualized treatment plans, evidence-based interventions, and the integration of innovative techniques, yield a favorable influence on the motor skills and functional capacities of children with CP. This review synthesizes the current knowledge on effective physical therapeutic strategies for children with CP. Furthermore, this review highlights the need for continued research and innovation in the field of pediatric physical therapy for CP.

뇌성마비 환아 한의치료 기술에 대한 전문가 설문 및 심층 면접 연구 (The Experts Survey and In-depth Interview on the Korean Medicine Treatments for the Cerebral Palsy Children)

  • Objectives This study aims to investigate current status of the Korean medicine treatment on the cerebral palsy (CP) via interviewing the doctors who have expertise in CP children. Methods From June 2015 to May 2016, the surveys and in-depth interviews were conducted on the fifteen Korean medicine doctors. Results Based on the surveys and interviews, widely-used treatments were body acupuncture and herbal decoction. Scalp acupuncture, intradermal acupuncture and manipulation were also used frequently. Experts stated Korean medicine could be more effective in younger patients, patients with cognitive or language problem. They also stated that Korean medicine has its advantage on strengthening the body to help grow and develop in CP children. However, the absence of guideline related to Korean medicine treatment in CP children as well as high cost are the problems. Finally, the issues regarding Korean medicine treatment in CP children, based on the in-depth interviews, emerged when western medicine is used simultaneously, when having seizure during treatment. Further research is needed for better understanding. Conclusions This study provided current status of the evidence-based Korean medicine treatment in CP children. However, further studies about the addressed issues are needed.

Functions (Mobility, Self-care, Social Ability) and Health-related Quality of Life in Children with Cerebral Palsy

  • Purpose: The purpose of this study was to describe the functional (mobility, self-care, social ability) and health-related quality of life in children with cerebral palsy (CP). Methods: A cross- sectional survey of 202 children with CP, mean age $5.91{\pm}1.57$ years, was carried out using the Gross Motor Function Classification System (GMFCS), Gross Motor Function Measure (GMFM), Pediatric Evaluation of Disability Inventory (PEDI), and Child Health Questionnaire (CHQ). Results: The functional assessment of children with CP showed that a more severe GMFCS level was associated with lower functional abilities (p<0.05). The health-related quality of life assessment showed that psychosocial well-being was less impaired than physical well-being. The internal consistency of the three instruments was satisfactory (cronbach's ${\alpha}$>0.80). The three different scales were correlated from moderate to strong (r=0.44 to 0.92). It was also found that mobility, tone distribution, and the parents' education level exerted a significant effect on the quality of life of children with CP (p<0.05). Conclusion: These findings suggest that children with CP have reduced function and quality of life and these are influenced by various factors. However, planning and application of various task-oriented functional interventions to childhood CP may be useful.

  • Purpose : The purpose of this study was to examine the correlation between Pediatric Balance Scale(PBS) and Gross Motor Function Measurement(GMFM) with cerebral palsy(CP) children. Methods : 31 ambulatory children with CP were recruited for this study. PBS is a modified version of the Berg’s Balance Scale to access the children with motor impairments. To access the motor functions that directly influence to the functional independence of the ambulatory CP children, total scores of standing category and walking/running/jumping category of GMFM were selectively compared with PBS score by Spearman correlation coefficient analysis. Results : The results revealed high correlation between PBS and GMFM scores both in standing and walking/running/jumping categories(r=.9). Conclusions : Therefore, this study indicates that PBS can be applied not only as a tool to access balance, but also as a measurement to predict and access the level of standing and ambulatory related functions of children with CP.

뇌성마비 유형 중 편마비 아동을 위한 수정된 강제유도 운동치료의 효과에 대한 체계적 고찰 (A Systematic Review of Modified Constraint- Induced Movement Therapy in Children With Hemiplegic Cerebral Palsy)

  • 목적 : 본 연구는 체계적 고찰을 통해 뇌성마비 유형 중 편마비 아동에게 적용된 수정된 강제유도 운동치료의 프로토콜(protocol)과 중재 효과를 알아보고자 한다. 연구방법 : 검색용어는 "Constraint Induced Movement Therapy", "modified Constraint Induced Movement Therapy", "Cerebral Palsy", "Hemiplegia"를 사용하였고, 수정된 강제유도 운동치료가 처음 제시된 2001년부터 2011년 5월까지 학회지에 게재된 논문을 PubMed, Medline, Ovid를 이용하여 검색한 후 자료를 분석하였다. 결과 : 총 10개의 연구가 분석되었고, 뇌성마비 편마비 아동에게 수정된 강제유도 운동 치료를 적용한 결과 상지운동 기능 및 움직임의 질이 향상되었고, 일상생활에서 환측의 상지 사용 빈도가 증가하였음을 나타내었다. 결론 : 분석논문의 수가 적어 다양한 질적 수준의 연구를 포함한 제한점은 있지만, 아동의 특성에 맞도록 제한시간과 제한방법을 조절하고 다양한 치료 활동을 적용한다면 수정된 강제유도 운동치료가 뇌성마비 편마비 아동의 기능회복에 효과적으로 사용되어질 수 있을 것으로 기대된다.

뇌 생 마비 환아의 Botulinum Toxin A 주사 후 보행양상의 변화 (A Case Study of Botulinum Toxin A Treatment in Cerebral Palsy)

  • The purpose of this case study was to introduce botulinum toxin A injection in cerebral palsy. Spasticity can be managed using a variety of methods. Eliminating aggravating sources, promoting stretching and bracing, and positioning are the least invasive methods of treatment. Botulinum toxin A injection is a relatively recent method of spasticity management in children with cerebral palsy. A 3-year old boy was evaluated for possible botulinum toxin injection to promote left side function. The patient had left hemiparetic cerebral palsy. He walked with bilateral intoning, much worse on the left than on the right and with excessive plantar flexion on the left. Botulinum toxin A was injected into the left medial gastrocnemius, with the goals of improving quality of gait. Finally, botulinum toxin treatment of would improve the motor function and ambulatory status in cerebral palsy by hypertonicity, spasticity, dynamic contracture and athetoid movement.
