Objective: To investigate dimensional changes in regional pharyngeal airway spaces after premolar extraction in bimaxillary skeletal protrusion (BSP) patients according to vertical skeletal pattern, and to further identify dentoskeletal risk factors to predict posttreatment pharyngeal changes. Methods: Fifty-five adults showing BSP treated with microimplant anchorage after four premolar extractions were included in this retrospective study. The subjects were divided into two groups according to the mandibular plane steepness: hyperdivergent (Frankfort horizontal plane to mandibular plane [FH-MP] ≥ 30) and nonhyperdivergent groups (FH-MP < 30). The control group consisted of 20 untreated adults with skeletal Class I normodivergent pattern and favorable profile. Treatment changes in cephalometric variables were evaluated and compared. The association between posttreatment changes in the dentoskeletal and upper airway variables were analyzed using linear regression analysis. Results: The BSP patients showed no significant decrease in the pharyngeal dimensions to the lower level in comparison with controls, except for middle airway space (MAS, p < 0.01). The upper airway variable representing greater decrease in the hyperdivergent group than in the nonhyperdivergent group was the MAS (p < 0.01). Posttreatment changes in FH-MP had negative correlation with changes in MAS (β = -0.42, p < 0.01) and inferior airway space (β = -0.52, p < 0.01) as a result of multivariable regression analysis adjusted for sagittal skeletal relationship. Conclusions: Decreased pharyngeal dimensions after treatment in BSP patients showed no significant difference from the normal range of pharyngeal dimensions. However, the glossopharyngeal airway space may be susceptible to treatment when vertical dimension increased in hyperdivergent BSP patients.
Purpose: The purpose of the study was to assess the validity of three-dimensional (3D) facial scan taken with facial scanner and digital photo wrapping on the cone-beam computed tomography (CBCT). Materials and Methods: Twenty-five patients had their CBCT scan, two-dimensional (2D) standardized frontal photographs and 3D facial scan obtained on the same day. The facial scans were taken with a facial scanner in an upright position. The 2D standardized frontal photographs were taken at a fixed distance from patients using a camera fixed to a cephalometric apparatus. The 2D integrated facial models were created using digital photo wrapping of frontal photographs on the corresponding CBCT images. The 3D integrated facial models were created using the integration process of 3D facial scans on the CBCT images. On the integrated facial models, sixteen soft tissue landmarks were identified, and the vertical, horizontal, oblique and angular distances between soft tissue landmarks were compared among the 2D facial models and 3D facial models, and CBCT images. Result: The results showed no significant differences of linear and angular measurements among CBCT images, 2D and 3D facial models except for Se-Sn vertical linear measurement which showed significant difference for the 3D facial models. The Bland-Altman plots showed that all measurements were within the limit of agreement. For 3D facial model, all Bland-Altman plots showed that systematic bias was less than 2.0 mm and 2.0° except for Se-Sn linear vertical measurement. For 2D facial model, the Bland-Altman plots of 6 out of 11 of the angular measurements showed systematic bias of more than 2.0°. Conclusion: The facial scan taken with facial scanner showed a clinically acceptable performance. The digital 2D photo wrapping has limitations in clinical use compared to 3D facial scans.
The purpose of this study was to investigate the pretreatment and posttreatment dentofacial characteristics of non-extraction patients with Class I malocclusion. And to compare this result with matched non-orthodontic normal occlusion and Class I premolar extraction patients. Such comparison might help identify morphologic characteristics of the non-extraction patients. Initial and final cephalometric evaluation were compared in a sample of 22 patients with Class I malocclusions treated in non-extraction manner with edgewise appliance and MEAW The mean age of the total population was 14 years 9 months and the average treatment time was 2 years 8 months. 32 landmarks were located and digitized on each cephalogram. From these landmarks, 24 linear and angular dimension were obtained. Student's t-test were used to compare the pretreatment - posttreatment results, Nonextraction - Normal groups, and nonextraction Extraction groups. Significance was predetermined at $p{\leq}0.05$. The results were as follows. 1. Before treatment, the mean value of the $ODI\;was\;69.9^{\circ},\;APDI\;was\;82.1^{\circ},\;CF\;was\;152^{\circ},\;EI\;was\;152^{\circ}$ in the non-extraction groups. 2. The skeletal pattern of the non-extraction groups were similar with non-orthodontic normal groupsr, but the non-extraction groups had larger interincisal angle. 3. Comparison between groups treated with and without extraction indicated at pretreatment, the extraction groups had more protrusive lips, smaller interincisal angle, and EI. 4. After treatment, there was no significant changes in the skeletal pattern of the non-extraction groups, but uprighting of the maxillary and mandibular first molar and decrease of the interincisal angle were seen.
In orthodontic field, it is very important to understand the normal growth. Such an understanding can be derived from observation of normal growth in various samples from childhood to adulthood, and this builds a foundation from which growth abnormality or variation can be defined. Thus, a broad data collection of normal children, as well as basic study reviewing such data become necessary. The relationship between the mean values of cephalometric measurements in Growth and Development was studied among the groups(boys and girls) of Korean chidren from the ages 6-years to 16-years. 220 boys 170 girls were chosen as subjects : Cephalometric X-ray were taken for 3 years and hard tissue analysis on McNamara and Ricketts Analysis which was divided into measurements of 5 parts(Cranial base, Cranial base and Maxilla & Mandible, Maxilla and Mandible, Mandible, Dental measurements). The relationship of craniofacial growth was studied. The following conclusions were obtained: 1. There were statistically significant differences in anterior cranial base between the two sexes of 14 and 15-year grouips. 2. In comparison of growth amounts among different age groups, statistically singnificant difference in Posterior facial height exhisted among $10\~11,\;12\~13\;and\;14\~15$ year-old interval groups. This pattern increased with aging. 3. Na perpendicular to A showed earlier growth peak in females(11-12years) than males(12-13years). When horizontal measurements of point A and Pogonion are compared, mandibular growth appeared to be greater. 4. Maximum growth peak of cranial base and mandible was earlier in females (11-12years) than males(13-14 years). 5. Upper central incisor flared out with aging, and there were increases in lower incisor to A-Pg, and lower incisor inclination There was significant difference between the two sexes in 10-year-old group.

This study aimed to evaluate the nasopharyngeal and oropharyngeal dimensions of the patients with skeletal class II division 1 or division 2 patterns during the pre-peak, peak, and post-peak growth periods for comparison with a skeletal class I control group (79 for pre-peak, 40 for peak, 40 for post-peak). Total 159 lateral cephalograms (70 for skeletal class I, 51 for skeletal class II, division 1, and 38 for skeletal class II, division 2) were selected. The growth of anteroposterior dimension of the pharyngeal airway were statistically significant among growth periods. The dimension for the nasopharyngeal and oropharyngeal airway space was the smallest in the division 1 skeletal class II group followed by class II division 2 and skeletal class I.
This study was performed to establish Korean soft tissue cephalometric norms, to compare the norms between sexes and between races, and then to suggest a guideline to execute orthodontic diagnosis and treatment planning for dentofacial deformities in Korean. Young Korean adults were selected. They were 27 males $(23.8{\pm}2.6-year-old)$ and 20 females $(22.5{\pm}1.7-year-old)$ who had harmonious facial balance. After taking lateral cephalograms under the natural head position which is widely known as a highly reliable and reproducible reference position, films were traced and analysing factors were measured as introduced by Arnett et at. Comparisons were done between male and female groups and between Korean and Caucasian groups using unpaired t-test. From the results it was concluded that Korean male had generally thicker lower facial soft tissue and smaller nasolabial angle, longer facial height, deeper facial depth, and more protrusive lower face than Korean female. From the comparison with Caucasian norms adopted from the research of Arnett et al., both Korean male and female showed longer facial lengths generally except less exposed maxillary incisor, and shorter facial depth than Caucasian counterparts. Also, both races showed similar horizontal position of maxillary structures from TVL (true vertical line), but there were more significant anteroposterior differences of marilla-mandible in Korean groups. These results mean Korean had relatively more retruded mandibular structures from the reference line, TVL. Individuals who had harmonious facial balance showed similar facial angle, more or less 170 degree, regardless of sexes or races.
Purpose: The purpose of this study was to compare the changes in the pharyngeal airway space, tongue and hyoid bone positions according to the orthognathic surgical methods of mandibular prognathism. Methods: The subjects included 30 patients (16 males, 14 females) with the skeletal class III malocclusion. Group 1 (10 patients) underwent bilateral sagittal split ramus osteotomy (BSSRO) only; group 2 (10 patients) underwent BSSRO with genioplasty; and group 3 (10 patients) underwent BSSRO, Le Fort I osteotomy. We measured the lines between the selected upper air way, hyoid bone and tongue landmarks on the lateral cephalometric x-ray films of skeletal class III. The measurements were made preoperation, within 1 week after the operation, 3~6 months after the operation and 1 year after the operation. We compared and analyzed the measurements with matched paired t-test and independent samples t-test. Results: There were no postoperative changes in the nasopharyngeal airway space in group 3. The measurements of group 3 also increased during the follow-up period as compared to the preoperative measurements. In group 1, 2 and 3, the immediate postoperative oropharyngeal and hypopharyngeal airway spaces were decreased. In the following period, the hypopharyngeal airway space returned to the preoperative positions, but the oropharyngeal airway space was not significantly changed. The upper and lower tongue was posteriorly repositioned immediately after the surgery. During the follow-up period, the lower tongue position returned to the preoperative position, and the upper tongue position was not significantly changed. Immediately after the surgery, the B point was moved to the posterior position, and a slight anterior advancement was found in the follow-up period. Conclusion: Patients who received the mandibular setback surgery showed a decrease in the posterior airway space, and those who underwent maxillary advancement showed a significant increase of the nasopharyngeal airway space, which remained stable during the evaluation period. The change of the airway space, position of the hyoid bone and tongue did not differ according to the presence or absence of genioplasty.
The purpose of this study was to compare on masse retraction of six anterior teeth with separate canine retraction in the amount of the anchorage loss and the retraction of the anterior teeth. The subjects consisted of 30 adult female patients with Angle Class 1 malocclusions who were treated by .022' straight wire appliance with 4 first permolar extraction. They were composed of two groups. Group 1 consisted of 15 subjects, whose six anterior teeth were retracted by on masse retraction. Group 2 consisted of 15 subjects, whose canines were retracted separately. Pre-treatment and post-treatment lateral cephalometric radiographs were analyzed. All data were processed statistically with independent samples t-test, and the conclusions were as follows. 1. There was no significant difference in the amount of the anchorage loss between two groups(p>0.05). 2. There was no significant difference in the amount of the retraction of the anterior teeth between two groups(p>0.05). 3. There was a significant difference in the amount of the inclinational change of the upper incisors between two groups. It was greater in Group 2. 4. There was a significant difference in the vertical positional change of the upper incisal edges between two groups. The upper incisal edges in Group 2 were more extruded than Group 1 by about 1mm. 5. There was no significant difference in the vertical positional change of the root apex of the upper incisors between two groups(p>0.05). And there was no significant difference in the vertical positional change of the upper molar(p>0.05).

In the field of pediatric dentistry, comparison and analysis of cephalogram values of children are important fir evaluation of growth and development, and are essential to evaluate the craniofacial form and growth pattern for early diagnosis of malocclusion. For this, cephalographic norm values are important, but not many studies on the primary dentition exist. To compare the past norm values of normal occlusion in the primary dentition with current norms, preschool children, 4 to 5 years of age, with normal occlusion in the primary dentition who visited our hospital were examined. Among these children, 46 children with normal facial form and developmental status were chosen for evaluation of cephalogram values. The following results were as follows: 1. For skeletal values, the angular values showed no significant differences between males and females, and the linear values were generally greater in males than females. 2. SNA was $81.3^{\circ}$, SNB was $76.6^{\circ}$ and ANB difference was $4.7^{\circ}$. 3. The ratio for Mandibular body length to Anterior cranial base length was 0.9 : 1 for both male and female and the ratio for posterior facial height to anterior facial height was 61.4 % for male, 62.0 % for female. 4. For dental values, IMPA was $84.2^{\circ}$ and UA to SN was $90.8^{\circ}$. 5. The upper lip to Ricketts esthetic line was positioned 2.6 mm anteriorly, and the lower lip to Ricketts esthetic line was positioned 2.5 mm anteriorly.

The purpose of this study was to evaluate the skeletal and dental effects obtained by the Frankel functional regulator III in growing children with Class III malocclusions. Cephalometric changes in thirty children at the time of mixed dentition malocclusions (initial mean age, $7.9{\pm}1.1$ years; mean treatment duration, $1.5{\pm}0.8$ years) were analysed. The results were as follows : 1. The skeletal effects on the maxilla showed a significant downward displacement whereas forward displacement was not significant in comparison with the control group. 2. The skeletal effects on the mandible showed statistically significant backward and downward displacement. 3. The dental effects showed statistically significant backward movement in the mandibular incisor tip and increase of overjet The results suggested that forward displacement on the maxilla was insufficient and treatment effects were caused mainly by downward displacement of the maxilla, backward and downward rotation of the mandible, and the increase of overjet during short period.
