A HISTOLOGIC STUDY ON THE RESPONSES OF PULP IN EXPERIMENTAL TOOTH MOVEMENT OF WHITE RATS (흰쥐의 실험적 치아이동시 치수의 반응에 관한 조직학적 연구)

  • The author investigated on the responses of pulp and dentine following tooth movements. The material consisted of fifty-four intact teeth from twenty-seven adult white rats. The half of the teeth were employed as controls and the other half served as experimental group. These teeth were moved with forces ranging from 30 grams to 120 grams for from 3 to 7 days. All these were extracted immediately after the force was relieved. The main pulp changes in the experiment were vacuolization of the pulp tissue and circulatory disturbances. The magnitude of the force had an important role. In addition to these changes, the resorption in dentine and cementum was observed, which was related to the magnitude of the force and the duration of experiment.

A STUDY ON THE INITIAL TISSUE CHANGES TO INTERMITTENNT FORCE IN DOGS (성견에서 전자력을 이용한 간헐적 교정력에 의한 치아이동시 초기 조직학적 변화에 관한 연구)

  • The purpose of this study was to help examining force system, optimal treatment, and prevention of relapse. Orthodontic force was generated by electromagnets, therefore duration could be freely controlled, and applied for 4 days in dogs. Force magnitude was 200gm and 50gm. duration was contious and intermittent. Intermittent duration was divided into 2 kinds of types in on/off, 1 mininute/1 minute, 10 seconds/1 minute. The results were as follows: 1. In the intermittent group to which force was applied for 10 seconds and ceased for 1 minute, osteoclast was not observed. 2. In the intermittent group to which 200gm of force was applied for 1 minute and ceased for 1 minute, there was blood circulation in periodontal space on pressure side, but mild hyalinized zone was observed. 3. More number of total osteoclast was counted in the pressure side of continous group to which 200gm of force was applied than in any other groups. 4. More number of frontal osteoclast was counted in the pressure side of intermittent group to which 200gm was applied for 1 minute and ceased for 1 minute than in any other groups. 5. On tension side, less periodontal space widening, more new bone and secondary cementum formation were observed in the intermittent group to which force was applied for 1 minute and ceased for 1 minute than in the continous group.

FLORID OSSEOUS DYSPLASIA OF THE JAWS (상, 하악골에 발생된 개화성골이형성증)

  • Few cases of florid osseous dysplasia has been described as a condition that characteristically affects the jaws. It usually manifests as multiple radiopaque masses distributed throughout the jaws. Confusion exists about the relationship of florid osseous dysplasia, gigantiform cementoma, chronic sclersing osteomyelitis, sclerosing osteitis or multiple enostosis. Authors experienced a case of florid osseous dysplasia of the jaws in 52-year-old female on the basis of clinical, radiographic and histopathologic findings. The characteristic features are as follows : 1. In clinical examination, there was no clinical sign and symptoms except extracted area. And there was no facial asymmetry. 2. Radiograms show round or lobular dense radiopaque masses surrounded by radiolucent bands in lower molar teeth area bilaterally. And slight increased radiopacities in maxillary molar teeth area bilaterally. There was no expansion or thinning of buccal and lingual cortical bones. There is no displacement or resorption of involved teeth. In right side of mandible, mandibular canal is displaced inferiorly due to mass. 3. Photomicrograms show densely mineralized sclerotic acellular masses with empty lacunae. Pattern is suggestive of cementum, although it could be considered sclerotic bone. In the periphery, lesion consisting of moderately cellular fibrous tissue in which globular calcified products are deposited.

  • Damage to adjacent teeth is one of the various complications that may occur during implant placement and is often the result of improper direction during fixture placement or excessive depth of placement. In general, if detrimental symptoms, such as reaction to percussion in damaged teeth, mobility, and pulp necrosis, are not present, osseointegration should be observed at follow-up. In three cases, the possibility of root damage due to an implant fixture placed too close to each adjacent tooth was perceived on radiographs. However, in all of these cases, there were no clinical symptoms or radiographic changes present in the tooth, and the implants did not exhibit decreased stability or peri-implantitis. Therefore, we can carefully predict that the implant fixture close to the adjacent tooth did not invade the cementum of the root, and therefore did not produce the suspected pulpal damage or periradicular symptoms. In this study, we considered both the implant status as well as the adjacent tooth.

A SEM STUDY ON THE ADAPTATION OF ESTHETIC RESTORATIVE MATERIALS TO TOOTH STRUCTURE IN CLASS V CAVITIES (V급와동에 충전한 심미성 수복재의 치질과의 접합도에 관한 주사전자현미경적 연구)

  • The purpose of this study was to evaluate the adaptation of light cured glass ionomer cement and composite resin using all- etch technique to tooth structure. In this study, class V cavities were prepared on the buccal surfaces of 10 extracted human premolar teeth with cementum margin and teeth were randomly assigned 2 groups of 5 teeth each. The cavities of glass ionomer cement group were filled with the light cured glass ionomer cement(Fuji II LC) and the cavities of composite resin group were filled with the light cured composite resion(P - 50) using all- etch technique with All- Bond 2. The restored teeth were stored in 100 % relative humidity at $37^{\circ}C$ for 48 hours. And then, the roots of the teeth were removed with the tapered fissure bur and the remaining crowns were sectioned occlusogingivally through the center of restorations. Adaptation at tooth - restoration interface were assessed occlusally, gingivally, and axially by scanning electron microscope. The results were as follows : 1. The adaptation to enamel walls of composite resin restorations using All - Bond 2 showed better than glass ionomer restorations. 2. The adaptation to gingival and axial walls of glass ionomer restorations showed better than composite resin restorations using All - Bond 2. 3. In both groups, occlusal margins of restorations showed better adaptation than gingival margins of restorations.

  • The term odontoma, first introduced by Broca, was originally applied to all odontogenic tumors. Related to the clinical and histological differences, 2 groups of odontoma were defined: compound odontoma and complex odontoma. Complex odontoma is more common in the mandible than in the maxilla, usually occuring in the mandibular premolar-molar area. And this consists of small mass of irregularly arranged enamel, dentine, cementum, and connective tissue, but tooth-like structures are not found. In our case, 34 year old man was diagnosed as the complex odontoma of maxillary sinus by the physical exam, Panex, and C.T scan. We sectioned and removed it, and then confirmed the diagnosis histopathologically. Its uncommon location and size made us report that case and review the literature concerned.

A COMPARISON OF DEMINERALIZED FREEZE-DRIED BONE AND HYDROXYAPATITE IN THE TREATMENT OF PERIODONTAL OSSEOUS DEFECTS IN DOGS (이종 탈회 건조골 및 Hydroxyapatite 제재의 골 이식이 성견 치조골 결손부 치유에 미치는 영향에 관한 비교연구)

  • This study was conducted to evaluate the healing potential of hydroxyapatite and demineralized freeze dried bone in 5 dogs. Chronic periodontitis was induced by ligating elastic wire randomized as follows. The group in which only flap operation was performed was used as control. The group in which flap operation using nonresorbable nonporous hydroxyapatite (Orthomatrix)was performed was used as experimental I. The group which flap operation using resorbable porous hydroxyapatite (Biocoral) was performed as experimental II. The group in which flap operation using demineralized freeze-dried bone was performed was used as experimental III. Thereafter dogs serially sacrificed at the 1,2,4 and 8 weeks and the specimens were prepared, and stained with Hematoxilin-Eosin stain for the light microscopic evaluation. The results of the this study were as follows : 1. Control group : progressive inflammatory cell infiltration till 4 weeks and epithelial undergrowth. 2. Group I. : epithelial undergrowth and new bone formed with fibrous margin around HA granule. 3. Group II. : no epithelial undergrowth and direct bone formation at the porous granule 4. Group III. : could not see epithelial undergrowth but obviously new cementum formation.

The SEM Observation of The Various Root Treatment Effect On Furcation Area (치근 이개부 병소의 치근처방법에 따른 주사현미경적 연구)

  • In periodontal regeneration treatment, access to the frucation area is very difficult. Thus complete removal of plaque, calculus and endotoxin is somewhat impossible. In this study, teeth that were extracted due to periodontal disease were used. The furcation area was treated with periodontal curette, ultrasonic scaler, roto bur and they observed using SEM. The result was follows 1. The group treatment with curette showed remaining plaque, the cementum existed in most of the surface and partial dentinal tubule orifice could be seen. 2. The group treatment with ultrasonic scaler showed less removalof plaque compared to curette and irregular surface could be seen. 3. The group treatment with roto bur showed cleaner surface and many dentinal tubule orifice could be seen compared to the curette and ultrasonic scaler groups. Thus when suing treatments such as bone grafting or guided tissue regeneration, it is considered that the furcation area should be treatment with Roto bur.

ONE VISIT CANAL FILLING BY USING MINERAL TRIOXIDE IN A MENTALLY RETARDED CHILD : A CASE REPORT (정신지체장애아의 Mineral trioxide aggregate를 이용한 즉일 근관 충전)

  • A-12year-old boy visited the clinic with chief complaint of anterior maxillary trauma. He was diagnosed with first degree mental retardation and cerebral disorders. By clinical and radiographic examination, intrusion of maxillary central incisors were found. The intrusion was not severe, teeth were luxated with a slight force and the prognosis was followed. After the first year, external root resorption was seen radiographically. Due to difficult behavior management, one visit root canal filling with OrthoMTA(BioMTA, Korea) which is known to generate of cementum and periodontal ligament was planned along with general anesthesia. OrthoMTA was filled from the apex to 1-2mm below cervical area and composite resin used for crown restoration. 6 months after, further resorption, discoloration and mobility was not found. This case is currently checked yearly and further research is needed for inflammatory root resorption and ankylosis.

Tooth Movement in Demineralized Area by Etchant in Rabbits

  • Purpose: Among the facilitation of tooth movement in adult orthodontic treatment methods, surgical approaches are gaining popularity but complications following mechanical bone reduction are a problem. In this study, tooth movement was observed after alveolar bone was chemically demineralized to verify whether tooth movement had been facilitated. Materials and Methods: Twelve rabbits were used. In the experimental group, the alveolar bone of the left first molar area was exposed and demineralized. Thirty seven percents phosphoric acid was applied for 5 minutes for demineralization. The opposite first molar area was used as control. Two teeth were pulled with 200 g force and 4 rabbits each were sacrificed at 3, 7, and 14 days after the force was applied. Histologic examination was done with hematoxylin and eosin and tartrate-resistant acid phosphatase staining. Result: The histologic examination results revealed more bone resorption in the demineralized area. As time passed, the number of osteoclasts increased in the compressed area. The amount of tooth movement was larger in the experimental group compared to the control group but the difference was not statistically significant. Conclusion: The demineralization with etchant resulted in limited bone resorption, more tooth movement and less damage of the cementum after applied orthodontic force.
