Tensile Behavior and Cracking Patterns of Fiber-Reinforced Cementless Composites According to Types of Superplasticizers (고성능 감수제 종류에 따른 섬유보강 무시멘트 복합재료의 인장거동 및 균열 패턴)

  • The purpose of this study is to investigate experimentally the effects of type of superplasticizer on tensile behavior and cracking pattern of alkali-activated slag-based cementless composite. Three mixtures were prepared according to type of superplasticizer, and the compressive strength and tension tests were performed. Test results showed that differences of tensile strength, tensile strain capacity, and toughness of composites were up to 28.1%, 39.1%, and 66.2%, respectively, according to type of superplasticizer, although fiber balling or poor dispersion of fibers in fresh composites was not observed. It was also observed that the type of superplasticizer influenced number of cracks and maximum fiber bridging stress.

  • Reverse total shoulder arthroplasty (RTSA) emerged as a new concept of arthroplasty that does not restore normal anatomy but does restore function. It enables the function of the torn rotator cuff to be performed by the deltoid and shows encouraging clinical outcomes. Since its introduction, various modifications have been designed to improve the outcome of the RTSA. From the original cemented baseplate with peg or keel, a cementless baseplate was designed that could be fixed with central and peripheral screws. In addition, a modular-type glenoid component enabled easier revision options. For the humeral component, the initial design was an inlay type of long stem with cemented fixation. However, loss of bone stock from the cemented stem hindered revision surgery. Therefore, a cementless design was introduced with a firm metaphyseal fixation. Furthermore, to prevent complications such as scapular notching, the concept of lateralization emerged. Lateralization helped to maintain normal shoulder contour and better rotator cuff function for improved external/internal rotation power, but excessive lateralization yielded problems such as subacromial notching. Therefore, for patients with pseudoparalysis or with risk of subacromial notching, a medial eccentric tray option can be used for distalization and reduced lateralization of the center of rotation. In summary, it is important that surgeons understand the characteristics of each implant in the various options for RTSA. Furthermore, through preoperative evaluation of patients, surgeons can choose the implant option that will lead to the best outcomes after RTSA.

The influencing factors for the strength enhancement of composite materials made up of fine high-calcium fly ash

  • The aim of the study was to establish the influence of particle size, chemical and phase composition of fine microspherical high-calcium fly ash (HCFA), as well as superplasticizer content on the strength of cementless composite materials based on 100% HCFA and mixtures of HCFA with Portland cement (PC). For the initial HCFA fractions, the particle size distribution, chemical and quantitative phase composition were determined. The compressive strength of cured composite materials obtained at W/B 0.4 and 0.25 was determined at a curing time of 3-300 days. For cementless materials, it was found that a change in the particle size d90 from 30 ㎛ (fraction 3) to 10 ㎛ (fraction 4) leads to an increase in compressive strength by more than 2 times. Compressive strength increases by at least another 2.2 times with the addition of Melflux 5581F superplasticizer (0.12%) and at W/B 0.25. The HCFA-PC blends were investigated in the range of 60-90% HCFA and the maximum compressive strength was found at 80% HCFA. On the basis of 80% HCFA-20% PC blend, the samples of ultra-high strength (108 and 150 MPa at 28 and 100 days of hardening) were obtained with the addition of 0.3% Melflux 5581F and 5% silica fume. The quantitative phase composition was determined for composite materials with a curing age of 28 days. It has been established that in a sample with ultra-high strength, a more complete transformation of the initial phases of both HCFA and PC occurs as compared to their transformation separately.

Bipolar Hemiarthroplasty in Unstable Intertrochanteric Fractures with an Effective Wiring Technique

  • Purpose: Bipolar hemiarthroplasty has recently been acknowledged as an effective option for treatment of unstable intertrochanteric fracture. Trochanteric fragment nonunion can cause postoperative weakness of the abductor muscle and dislocation; therefore, reduction and fixation of the fragment is essential. The purpose of this study was to perform an evaluation and analysis of the outcomes of bipolar hemiarthroplasty using a useful wiring technique for management of unstable intertrochanteric fractures. Materials and Methods: A total of 217 patients who underwent bipolar hemiarthroplasty using a cementless stem and a wiring technique for management of unstable intertrochanteric femoral fractures (AO/OTA classification 31-A2) at our hospital from January 2017 to December 2020 were included in this study. Evaluation of clinical outcomes was performed using the Harris hip score (HHS) and the ambulatory capacity reported by patients was classified according to Koval stage at six months postoperatively. Evaluation of radiologic outcomes for subsidence, breakage of wiring, and loosening was also performed using plain radiographs at six months postoperatively. Results: Among 217 patients, five patients died during the follow-up period as a result of problems unrelated to the operation. The mean HHS was 75±12 and the mean Koval category before the injury was 2.5±1.8. A broken wire was detected around the greater trochanter and lesser trochanter in 25 patients (11.5%). The mean distance of stem subsidence was 2.2±1.7 mm. Conclusion: Our wiring fixation technique can be regarded as an effective additional surgical option for fixation of trochanteric fracture fragments during performance of bipolar hemiarthroplasty.

Comparison of Biomechanical Stability of Custom-made Hip Implants using Finite Element Analysis (스템 길이에 따른 환자맞춤 인공고관절의 역학적 안정성 비교)

  • Designing a morphologically well-fitted hip implant to a patient anatomy is desirable to improve surgical outcomes since a commercial ready-made hip implant may not well conform to the patient joint. In this study, biomechanical stability of patient-specific hip implants with two different stem lengths was compared and discussed using a 3D finite element analysis (FEA). The FEA results in this study showed that an increase in stem length brings about more the peaked von-Mises stress (PVMS) in the prosthesis and less in the femur. However the decrease in von-Mises stress in the femur causes stress shielding phenomenon that usually leads to considerable bone resorption. Although, in biomechanical stability point of view, this work recommends the use of smaller stems, the length of stem must be determined by considering both the von-Mises stress and the stress-shielding phenomenon.

  • In orthopedics, hip arthroplasty is the operation that replaces damaged hip joint to artificial joint. In hip arthroplasty, quite better result can be achieved if robot is applied to machine cavity in bone, especially when cementless stem is used. So several kinds of robots were introduced for hip arthroplasty, but they used MRI, CT Scan, vision analysis and real time tracking of bone position for registration of robot. To overcome shortage of conventional robot surgery, gauge based registration method was proposed and small robot was designed. In this method, small robot is mounted on femur, and its position is determined by gauge registration method. Operation procedure was performed on model femur and result was analyzed. This robotic hip surgery system is expected to more adaptable in operation room.

  • Precast concrete produced in the industry is advantage that easy to manage, and it save construction period in the field. The specimens according to the type of activator for AAS(Alkali-Activated Slag) mortar cured in an autoclave. The specimens of AAS mortar with sodium was shown the high rate of increase of the compressive strength.

  • Clinically, proximal bone resorption in the femur is frequently seen postoperatively on the follow up X-rays after total hip replacement(THR). We developed the finite element model of cementless THR. The model is two dimensional side plate model, whereby the three dimensional structural integrity of the bone can be accounted for by a separate two dimensional mesh, a side plate. The subject of this article is the development and application of this two dimensional side plate FEM to study the reverse effect of the various degree of bone resorption of femur after THR. The results of this study indicates that two dimensional side plate model is good and simple alternative to complex three dimensional model and the severity of the proximal bone resorption has the effect of more increasing stress on the cortex at the level of femoral stem tip.

  • As a result of physical characteristics of using meta-kaolin and alkali accelerator, it was found that higher curing temperature density is favorable to strength development at early age and the higher the age is, the higher, most of the compressive strength gets. Also, I was shown that more than atmospheric curing, steam curing was favorable for development of compressive strength. When the temperature of curing temperature was higher, most of the compressive strengths were higher. Thus, based on this study, it was understood that environmental-friendly chemically combined concrete using meta-kaolin and alkali accelerator can be utilized without using cement.
