영상의학과 의사들을 위한 실리콘 유방 보형물 관련 합병증의 이해 (Understanding Silicone Breast Implant-Associated Complications for Radiologists)

  • 미용적 성형 및 유방암 수술 후 재건의 목적으로 사용되는 실리콘 보형물의 사용이 국내외에서 크게 증가함에 따라 진료 영역에서 실리콘 보형물 삽입술을 받은 환자들을 어렵지 않게 접하게 되었다. 기존에 알려져 있던 보형물의 파열이나 구축과 같은 합병증 외에 최근에는 유방 보형물 연관 역형성 대세포 림프종과 같은 악성 종양과의 연관성도 보고되면서 보형물 관련한 영상 검사가 증가하고 있다. 이러한 상황에서 영상의학과 의사들은 보형물 삽입술을 받은 환자에 대해 어떤 검사가 필요하고 어떤 영상 소견이 보형물 관련 합병증을 시사하는지에 관한 충분한 지식을 갖추고 있어야 할 것이다. 본 종설에서는 영상의학과 의사들이 알아야 하는 실리콘 보형물의 다양한 합병증과 이들의 영상 소견에 대해 다루고자 한다.

페리틴 리포터 유전자를 발현하는 백서 중간엽 줄기세포의 특성과 자기공명영상 연구 ($In$ $vitro$ MRI and Characterization of Rat Mesenchymal Stem Cells Transduced with Ferritin as MR Reporter Gene)

  • 목적: 백서 중간엽 줄기세포에 페리틴 유전자를 형질 도입시켜 생물학적 특성의 변화 유무를 평가하고, 자기공명영상에서 신호강도의 차이를 확인해보고자 하였다. 대상과 방법: 백서 중간엽 줄기세포에 렌티바이러스를 이용하여 사람유래 재조합 페리틴과 녹색형광단백질 유전자의 과발현을 유도하였다. 페리틴 유전자가 발현된 백서 중간엽 줄기세포의 증식성과 생존능을 분석하기 위해 MTT 어세이를 수행하였으며, 유세포 분석을 수행하여 중간엽 줄기세포의 표면 마커 발현을 평가하고, 세포 내 철 함량을 측정하고 프러시안 블루 염색을 시행하여 철 축적능력을 분석하였다. 세포 팬텀을 이용하여 9.4 T 자기공영영상 기기를 이용하여 검출가능성을 평가하였다. 결과: 페리틴과 녹색형광 유전자는 백서 중간엽 줄기세포에서 안정적으로 발현되었다. 페리틴 유전자의 과발현으로 인해 백서 중간엽 줄기세포의 생물학적 특성 (증식능력, 생존능, 표면마커)은 영향을 받지 않았다. 페리틴을 발현하는 중간엽 줄기세포에서 철의 축적능력이 증가된 것이 확인되었고, T2 이완 시간은 유의하게 감소하였다. 결론: 줄기세포 치료 연구에서 자기공명 리포터 유전자 페리틴은 자기공명영상법을 이용하여 중간엽 줄기세포를 비침습적으로 가시화 할 수 있고 이를 이용하여 생체추적이 가능할 것으로 기대된다.

Comparison of Monoexponential, Biexponential, Stretched-Exponential, and Kurtosis Models of Diffusion-Weighted Imaging in Differentiation of Renal Solid Masses

  • Objective: To compare various models of diffusion-weighted imaging including monoexponential apparent diffusion coefficient (ADC), biexponential (fast diffusion coefficient [Df], slow diffusion coefficient [Ds], and fraction of fast diffusion), stretched-exponential (distributed diffusion coefficient and anomalous exponent term [α]), and kurtosis (mean diffusivity and mean kurtosis [MK]) models in the differentiation of renal solid masses. Materials and Methods: A total of 81 patients (56 men and 25 women; mean age, 57 years; age range, 30-69 years) with 18 benign and 63 malignant lesions were imaged using 3T diffusion-weighted MRI. Diffusion model selection was investigated in each lesion using the Akaike information criteria. Mann-Whitney U test and receiver operating characteristic (ROC) analysis were used for statistical evaluations. Results: Goodness-of-fit analysis showed that the stretched-exponential model had the highest voxel percentages in benign and malignant lesions (90.7% and 51.4%, respectively). ADC, Ds, and MK showed significant differences between benign and malignant lesions (p < 0.05) and between low- and high-grade clear cell renal cell carcinoma (ccRCC) (p < 0.05). α was significantly lower in the benign group than in the malignant group (p < 0.05). All diffusion measures showed significant differences between ccRCC and non-ccRCC (p < 0.05) except Df and α (p = 0.143 and 0.112, respectively). α showed the highest diagnostic accuracy in differentiating benign and malignant lesions with an area under the ROC curve of 0.923, but none of the parameters from these advanced models revealed significantly better performance over ADC in discriminating subtypes or grades of renal cell carcinoma (RCC) (p > 0.05). Conclusion: Compared with conventional diffusion parameters, α may provide additional information for differentiating benign and malignant renal masses, while ADC remains the most valuable parameter for differentiation of RCC subtypes and for ccRCC grading.

Usefulness of cyclic thermal therapy and red blood cell scintigraphy in patients with chemotherapy-induced peripheral neuropathy

  • Background: Pharmacological and non-pharmacological therapies have been used to treat patients with chemotherapy-induced peripheral neuropathy (CIPN). However, the effect of therapies in cancer patients has yet to be investigated comprehensively. We hypothesized that cyclic thermal therapy would improve blood flow and microcirculation and improve the symptoms driven by CIPN. Methods: The criteria of assessment were blood volume in region of interest (ROI) in the images, and European Organization for Research and Treatment of Cancer-Quality of Life Questionnaire-Chemotherapy-Induced Peripheral Neuropathy 20 questionnaire scores. The blood volume was quantified by using red blood cell (RBC) scintigraphy. All patients were treated 10 times during 10 days. The thermal stimulations, between 15° and 41°, were repeatedly delivered to the patient's hands. Results: The total score of the questionnaires, the score of questions related to the upper limbs, the score of questions closely related to the upper limbs, and the score excluding the upper limbs questions was decreased. The blood volume was decreased, and the variance of blood volume was decreased. During cooling stimulation, the blood volume was decreased, and its variance was decreased. During warming stimulation, the blood volume was decreased, and its variance was decreased. Conclusions: We suggest that cyclic thermal therapy is useful to alleviate CIPN symptoms by blood circulation improvement. RBC scintigraphy can provide the quantitative information on blood volume under certain conditions such as stress, as well as rest, in peripheral tissue.

Primary Intracranial Squamous Cell Carcinoma in the Brain Stem with a Cerebellopontine Angle Epidermoid Cyst

  • Primary intracranial squamous cell carcinoma is extremely rare, with most cases arising from a preexisting benign epidermoid cyst. We report a rare case of primary intracranial squamous cell carcinoma in the brain stem with a cerebellopontine angle (CPA) epidermoid cyst. A 72-year-old female suffered from progressive left hemiparesis, difficulty in swallowing, and right hemifacial numbness. Diffusion-weighted magnetic resonance imaging revealed a high signal intensity (SI) lesion in the CPA region and an intra-axially ring-enhanced cystic mass in the right brain stem with low SI. Whole-body positron emission tomography showed no evidence of metastatic disease. The histological findings revealed a typical epidermoid cyst in the CPA region and a squamous cell carcinoma in the brain stem. We speculate that the squamous cell carcinoma may have been developed due to a chronic inflammatory response by the adjacent epidermoid cyst. The patient underwent a surgical resection and radiotherapy. After 12 months, she had no evidence of recurrence.

소세포폐암에저의 $^{18}F-FDG$ PET의 임상 이용 (Clinical Application of $^{18}F-FDG$ PET in Small Cell Lung Cancer)

  • This review focuses on the clinical use of $^{18}F-FDG$ PET in small cell lung cancer. For initial staging of small cell lung cancer, $^{18}F-FDG$ PET appears to be better than conventional staging methods. $^{18}F-FDG$ PET seems to be potentially useful for detecting recurrence, restaging and therapy response assessment in small cell lung cancer. However, due to small number of literatures, the role of $^{18}F-FDG$ PET in small cell lung cancer requires further investigations.

  • 한선종은 땀샘에서 기원하는 종양으로 전형적으로 진피 및 피하층에 위치한다. 악성 한선종은 매우 드물다. 그리고, 보통 영상 검사 없이 수술적 절제 치료를 시행한다. 악성 한선종의 초음파 소견도 매우 드물게 보고되어 있다. 저자들은 악성 한선종의 초음파 소견을 보고하고자 한다.

Role of Non-Thermal DBD Plasma on Cell Migration and Cell Proliferation in Wound Healing

  • Plasma technology isbeing developed for a range of medical applications including wound healing. However, the effect of plasma on many cells and tissues is unclear. Cell migration and cell proliferation are very important biological processes which are affected by plasma exposure and might be a potential target for plasma therapy during wound healing treatment. In this study, we confirmed the plasma exposure time and incubation time after plasma treatment in skin fibroblast (L-929 cells) to evaluate the optimal conditions forplasma exposure to the cell in-vitro. In addition, we used a scratch method to generate artificial wound for evaluating the cell migration by plasma treatment. Where, the cells were treated with plasma and migration rate was observed by live-cell imaging device. To find the cell proliferation, cell viability assay was executed. The results of this study indicate the increased cell proliferation and migration on mild plasma treatment. The mechanisms for cell migration and cell proliferation after plasma treatment for future studies will be discussed.

An integrated elastomer substrate with a lens array and pixel elements for three-dimensional liquid crystal displays

  • In this paper, a concept of an integrated elastomer substrate for a three-dimensional (3D) liquid crystal display based on the integral-imaging method is presented. The elemental lens array and columnar spacers were integrated into one of the two substrates, an elastomer substrate, through an imprinting process. The integrated elastomer substrate was capable of maintaining the uniform liquid crystal (LC) cell gap and promoting homeotropic LC alignment without any surface treatment. The monolithic approach reported herein will provide a key component for 3D displays with enhanced portability through a more than 40% weight reduction compared with the conventional integral-imaging method.

Basic principles and applications of $^{18}F$-FDG-PET/CT in oral and maxillofacial imaging: A pictorial essay

  • A combination of positron emission tomography (PET) with $^{18}F$-labeled fluoro-2-deoxyglucose ($^{18}F$-FDG) and computed tomography ($^{18}F$-FDG-PET/CT) has increasingly become a widely used imaging modality for the diagnosis and management of head and neck cancer. On the basis of both recent literature and our professional experience, we present a set of principles with pictorial illustrations and clinical applications of FDG-PET/CT in the evaluation and management planning of squamous cell carcinoma of the oral cavity and oropharynx. We feel that this paper will be of interest and will aid the learning of oral and maxillofacial radiology trainees and practitioners.
