Skin malignancy initially misdiagnosed as a benign epidermal cyst

  • Skin cancer, which often occurs as a result of skin exposure to ultraviolet light radiation, usually presents with characteristic abnormal features, such as ulcerative lesions, irregular morphology, bleeding, and excessive growth. Therefore, skin cancer rarely resembles a benign tumor on visual inspection. Nonetheless, squamous cell carcinoma and basal cell carcinoma with nodular or polypoid features can have a similar appearance to that of benign tumors, meaning that they are sometimes misdiagnosed as benign. As benign and malignant tumors have some overlapping features, clinicians sometimes use additional imaging techniques such as ultrasonography to improve the accuracy of the diagnosis because even a malignant tumor that externally resembles a benign tumor generally has internal morphological features characteristic of malignancy, such as invasion and irregular borders. However, these imaging tools also have limitations, and punch or excisional biopsy can be needed if malignancy cannot be completely ruled out. Herein, we report a case of skin malignancy initially misdiagnosed as a benign epidermal cyst based on external visual inspection and ultrasonography.

Isolated temporalis muscle metastasis of renal cell carcinoma

  • Isolated head and neck metastasis of renal cell carcinoma (RCC) is relatively rare and metastasis to the temple area is very rare. Here, we present the case of a 51-year-old man who was diagnosed with RCC 2 years earlier and had a contralateral metastatic temple area lesion. The patient who was diagnosed with renal cell cancer and underwent a nephrectomy 2 years ago was referred to the plastic surgery department for a temple mass on the contralateral side. In the operative field, the mass was located in the temporalis muscle with a red-to-purple protruding shape. Biopsy of the mass revealed a metastatic RCC lesion. Computed tomography imaging showed a lobulated, contoured enhancing lesion. Positron emission tomography/computed tomography imaging showed high-fluorodeoxyglucose uptake in the right temporalis muscle. The patient underwent wide excision of the metastatic RCC including the temporalis muscle at the plastic surgery department. Skeletal muscle metastasis of head and neck lesions is extremely rare in RCC. Isolated contralateral temporalis muscle metastasis in RCC has not been previously reported in the literature. If a patient has a history of malignant cancer, plastic surgeons should always consider metastatic lesions of head and neck tumors. Because of its high metastatic ability and poor prognosis, it is very important to keep this case in mind.

Recent advances in spatially resolved transcriptomics: challenges and opportunities

  • Single-cell RNA sequencing (scRNA-seq) has greatly advanced our understanding of cellular heterogeneity by profiling individual cell transcriptomes. However, cell dissociation from the tissue structure causes a loss of spatial information, which hinders the identification of intercellular communication networks and global transcriptional patterns present in the tissue architecture. To overcome this limitation, novel transcriptomic platforms that preserve spatial information have been actively developed. Significant achievements in imaging technologies have enabled in situ targeted transcriptomic profiling in single cells at single-molecule resolution. In addition, technologies based on mRNA capture followed by sequencing have made possible profiling of the genome-wide transcriptome at the 55-100 ㎛ resolution. Unfortunately, neither imaging-based technology nor capture-based method elucidates a complete picture of the spatial transcriptome in a tissue. Therefore, addressing specific biological questions requires balancing experimental throughput and spatial resolution, mandating the efforts to develop computational algorithms that are pivotal to circumvent technology-specific limitations. In this review, we focus on the current state-of-the-art spatially resolved transcriptomic technologies, describe their applications in a variety of biological domains, and explore recent discoveries demonstrating their enormous potential in biomedical research. We further highlight novel integrative computational methodologies with other data modalities that provide a framework to derive biological insight into heterogeneous and complex tissue organization.

신세포암으로 오인된 다방성 낭종 형태의 신장의 유상피 혈관근지방종: 증례 보고 (Multiloculated Cystic Type Renal Epithelioid Angiomyolipoma Mimicking Renal Cell Carcinoma: A Case Report)

  • 신장의 유상피 혈관근지방종은 혈관근지방종의 드문 변이형으로 현저한 유상피세포로 구성되어 있다. 유상피 혈관근지방종은 일반적으로 종양 내부의 출혈과 다양한 정도의 괴사 혹은 낭성 변화를 동반한 크기가 큰 불균일한 연조직 종괴로 나타난다. 우리는 64세 여성에서 발견된 신세포암으로 오인된 다방성 낭종 형태의 신장의 유상피 혈관근지방종에 대해 발표하려 한다. 낭종 내 다량의 출혈, 조영 전 검사에서 고음영의 벽과 격막, 그리고 비대된 종괴 내혈관들이 신세포암을 감별하는 데 있어 도움이 될 수 있다.

마우스 뇌의 구조적 연결성 분석을 위한 분석 방법 (Analytical Methods for the Analysis of Structural Connectivity in the Mouse Brain)

  • 자기공명영상(MRI)은 뇌의 구조적 및 기능적 연구에서 핵심 기술로 필요성이 증가하고 있다. Tractography 분석을 이용하는 뇌지도(Connectome)는 MRI를 통해 뇌의 구조적 연결성을 확인하고 연결성의 변동성을 이용해 질병 병리학에 대한 이해를 높이는 방법으로 인간을 대상으로 활발한 연구가 진행되고 있다. 하지만 마우스 같은 작은 동물의 경우 분석 방법의 표준화가 부족하고 영상에 대한 정확한 전처리 전략 및 아틀라스 기반 신경 정보학에 대한 과학적 합의가 없다. 또한, 인간의 뇌에 비해 마우스의 뇌는 매우 작기 때문에 높은 해상도를 갖는 영상을 획득하는 것에도 어려움이 있다. 연구에서는 구조적 영상과 확산 텐서 영상을 이용해 구조 영역 세분화를 포함한 구조적 연결성 분석을 가능하게 하고 마우스 뇌 데이터를 처리하는 Allen Mouse Brain Atlas 기반 영상 데이터 분석 파이프라인을 제시한다. 각 분석 방법은 마우스 뇌 영상 데이터의 분석을 가능하게 하고 이미 인간 영상데이터로 검증된 소프트웨어를 이용해 신뢰성을 가질 수 있게 하였다. 또한, 연구에서 제시되는 파이프라인은 복잡한 분석 과정과 다양한 기능들 중 마우스 Tractography에 필요한 기능들을 정리하여 사용자가 효율적으로 데이터 처리를 하는데 최적화되었다.

Surface Plasmon Resonance Imaging Analysis of Hexahistidine-tagged Protein on the Gold Thin Film Coated with a Calix Crown Derivative

  • A surface plasmon resonance (SPR) imaging system was constructed and used to detect the hexahistidine-ubiquitin-tagged human parathyroid hormone fragment (His$\sub$6/-Ub-hPTHF(1-34)) expressed in Escherichia coli. The hexahistidine-specific antibody was immobilized on a thin gold film coated with ProLinker$\^$TM/ B, a novel calixcrown derivative with a bifunctional coupling property that permits efficient immobilizaton of capture proteins on solid matrices. The soluble and insoluble fractions of an E. coli cell lysate were spotted onto the antibody-coated gold chip, which was then washed with buffer (pH 7.4) solution and dried. SPR imaging measurements were carried out to detect the expressed His$\sub$6/-Ub-hPTHF(1-34). There was no discernible protein image in the uninduced cell lysate, indicating that non-specific binding of contaminant proteins did not occur on the gold chip surface. It is expected that the approach used here to detect affinity-tagged recombinant proteins using an SPR imaging technique could be used as a powerful tool for the analyses of a number of proteins in a high-throughput mode.

Incidental finding of an extensive oropharyngeal mass in magnetic resonance imaging of a patient with temporomandibular disorder: A case report

  • In this report, we describe the incidental finding of an oropharyngeal mass in a patient who presented with a chief complaint of temporomandibular pain. The patient was initially evaluated by an otorhinolaryngologist for complaints of headaches, earache, and sinus congestion. Due to worsening headaches and trismus, he was further referred for the management of temporomandibular disorder. The clinical evaluation was uneventful except for limited mouth opening (trismus). An advanced radiological evaluation using magnetic resonance imaging revealed a mass in the nasopharyngeal/oropharyngeal region. The mass occupied the masticatory space and extended superioinferiorly from the skull base to the mandible. A diagnostic biopsy of the lesion revealed a long­standing human papilloma virus (HPV­16)­positive squamous cell carcinoma of the oropharynx. This case illustrates the need for the timely radiological evaluation of seemingly innocuous orofacial pain.

유전자 발현 영상기법 (Imaging Gene Expression)

  • The rapid progress of molecular genetic methods over the past two decades has necessitated the development of methods to detect and quantify genetic activity within living bodies. Reporter genes provide a rapid and convenient tool to monitor gene expression by yielding a readily measurable phenotype upon expression when introduced into a biological system. Conventional reporter systems, however, are limited in their usefulness for in vivo experiments or human gene therapy because of its invasive nature which requires cell damage before assays can be performed. This offers an unique opportunity for nuclear imaging techniques to develope a novel method for imaging both the location and amount of gene expression noninvasively. Current developments to achieve this goal rely on utilizing either reporter enzymes that accumulate radiolabeled substrates or reporter receptors that bind specific radioligands. This overview includes a brief introduction to the background for such research, a summary of published results, and an outlook for future directions.

Extranodal Malignant Lymphoma Concurrent Involving Female Urethra and Cervix: a Case Report

  • Extranodal lymphoma presents in almost one-third of all non-Hodgkin lymphoma cases. The gastrointestinal tract, skin, and central nervous system are common sites of involvement, whereas the urethra and cervix are very rare. To the best of our knowledge, this is the first report on concurrent extranodal involvement of the uterine cervix and urethra. We report imaging findings of malignant lymphoma involving urethra and cervix concurrently in a 79-year-old female patient with literature review. The magnetic resonance imaging showed huge intermediate to high signal intensity mass on T2 weighted images and strong homogeneous enhancement in uterine cervix and periurethral area, but no surrounding architectural disruption.
