Distribution of Pseudomonas-Derived Cephalosporinase and Metallo-β-Lactamases in Carbapenem-Resistant Pseudomonas aeruginosa Isolates from Korea

  • The emergence of carbapenem resistance among Pseudomonas aeruginosa is an increasing problem in many parts of the world. In particular, metallo-$\beta$-lactamases (MBLs) and AmpC $\beta$lactamases are responsible for high-level resistance to carbapenem and cephalosporin. We studied the diversity and frequency of $\beta$-lactamases and characterized chromosomal AmpC $\beta$lactamase from carbapenem-resistant P. aeruginosa isolates. Sixty-one carbapenem-resistant P. aeruginosa isolates were collected from patients in a tertiary hospital in Daejeon, Korea, from January 2011 to June 2014. Minimum inhibitory concentrations (MICs) of four antimicrobial agents were determined using the agar-dilution method. Polymerase chain reaction and sequencing were used to identify the various $\beta$-lactamase genes, class 1 integrons, and chromosomally encoded and plasmid-mediated ampC genes. In addition, the epidemiological relationship was investigated by multilocus sequence typing. Among 61 carbapenem-resistant P. aeruginosa isolates, 25 isolates (41.0%) were MBL producers. Additionally, 30 isolates producing PDC (Pseudomonas-derived cephalosporinase)-2 were highly resistant to ceftazidime (MIC50 = $256{\mu}g/ml$) and cefepime (MIC50 = $256{\mu}g/ml$). Of all the PDC variants, 25 isolates harboring MBL genes showed high levels of cephalosporin and carbapenem resistance, whereas 36 isolates that did not harbor MBL genes revealed relatively low-level resistance (ceftazidime, p < 0.001; cefepime, p < 0.001; imipenem, p = 0.003; meropenem, p < 0.001). The coexistence of MBLs and AmpC $\beta$-lactamases suggests that these may be important contributing factors for cephalosporin and carbapenem resistance. Therefore, efficient detection and intervention to control drug resistance are necessary to prevent the emergence of P. aeruginosa possessing this combination of $\beta$-lactamases.

Isolation and Identification from Korean Olive Flounders (Paralichthys olivaceus) Showing Abnormal Swimming Behavior, and Sstudy of Antibiotic Susceptibility (이상유영 증상 국내산 양식 넙치에서의 슈도알테로모나스 분리·동정 및 항생제 감수성 연구)

  • Olive flounders which were cultured in commercial fish farm showed abnormal swimming behavior in November 2020. Then, gradual mortality was observed in the fish farm. During the diagnosis, bacterial strain KNCFKW-PN1 was isolated from the kidney of the dead fish. Based on the sequence of gyrase B subunit gene, KNCFKW-PN1 was proved to be Pseudoalteromonas nigrifaciens, showing 99.59% nucleotide identity with that of P. nigrifaciens LMG 2227T. According to the result of antibiotic susceptibility test, P. nigrifaciens KNCFKW-PN1 showed intermediate resistance to ciprofloxacin, and was resistant to ampicillin, cefepime, cefotaxime, ceftazidime, and amikacin. This is the first report of the isolation of multiple-antibiotic-resistant P. nigrifaciens from olive flounder.

Stability of Paclitaxel with Cephalosporines in $0.9\%$ Sodium Chloride Injection and $5\%$ Dextrose Injection During Simulated Y-Site Administration (Y-Site 동시투여 동안 $0.9\%$ 생리식염수와 $5\%$ 포도당 용액에서의 Paclitaxel과 Cephalosporines의 안전성 연구)

  • Paclitaxel과 cephalosporines(제 1세대인 ceftezole sodium과 cephradine, 제2세대인 cefamandole sodium과 cefmetazole sodium, 제3세대인 cefoperazone sodium과 cefotaxime sodium 그러고 제4세대인 cefepime hydrochloride)을 $5\%$포도당주사액 그리고 $0.9\%$ 염화나트륨주사액과 함께 Y-Site 장치를 써서 환자에게 주입할 때 paclitaxel의 안정성에 관하여 연구하였다. Paclitaxel 0.3 mg/ml 및 1.2 mg/ml과 cephalosporines 20 mg/m을 각각 1 : 1로 혼합한 후 0, 1, 2, 4, 12시간 시점에서 paclitaxel의 농도를 HPLC로 분석하였다. 방해물질에 의한 분석오차를 줄이기 위해 분석법을 여러 상태에서 확인하였으며, 각 농도에서 3차례씩 실험하였고 각 샘플은 2차례 반복하여 HPLC로 분석하였다. 분석전에 각 시료의 투명도, 색의 변화, 침전상태 및 pH를 검사하였다. Paclitaxel 0.3 mg/ml 및 1.2 mg/ml와 cephalosporines 20 mg/ml를 각각 혼합하였을 때 12시간 동안 안정하였으며, 주사액의 혼탁이나 색의 변화 및 침전은 나타나지 않았으며 pH도 변하지 않았다.

A Trend of Antibiogram of Escherichia Coli Detected by VITEK ESBL Test in Recently Two Years

  • Extended spectrum β-lactamase (ESBL) generated Enterobacteriaceae including Escherichia coli is responsible for resisting antibiotics, which is clinical problem. This study was performed to investigate the isolation rates of 111 strains having positive of VITEK ESBL test 111 ESBL-strains in each month and season and statistically to determine their patterns of antibiotic test. One hundred eleven ESBL-strains were collected among 1,688 strains of E. coli isolated from various clinical specimen of one general hospital in Busan during 2002 to 2003. Month rates of ESBL-strains were 0% to 13.3%, while the seasonal rates were highest at autumns during two years. The resistance to ampicillin, cefazolin, azteronam, ceftriaxone, and cefepime were 100% in 2002. In ampicillin and cefazolin the resistances were 100% during the two years. There were significant differences of cefoxitin and piperacillin/tazobactam between the two years but not significant between specimens. Four groups were divided according to the pattern of resistance and then the highest group had 93∼100% of the resistance to 8 drugs but not resistant to imipenem.

C-3 Quarternized Cephalosporin 화합물 합성 및 항균 활성

  • 4세대 cephalosporin계 약물로 개발되고 있는 cefpirome, cefepime, SCE-2787과 같은 화합물은 항균 spectrum 및 항균력에서 3세대 약물인 cefotaxime, ceftazidime보다 우수하며 구조적으로 C-3 quarternized기률 함유한 특징을 가지고 있다. 본 연구소에서는 기존에 보고된 화합물보다 개선된 약물을 찾으려는 시도로서 benzimidazole, imidazolopyridine등 기타 유사구조를 가진 C-3 quarternized 화합물 합성을 하여 왔으며, 본 연구에서는 최근에 합성된 9종의 신규 화합물에 대한 합성 및 항균활성을 보고한다. 이중 9 종의 화합물은 시험한 균주에서 일반적으로 cefpirome 보다 우수한 항균 profile을 나타내었으나 실제 신약으로의 개발 가능성 평가를 위하여 자세한 연구가 필요한 것이었다 이 화합물에 대한 확립된 실험실적 합성 공정과 물성자료를 함께 보고한다.

  • In the preceding paper, we reported the synthessis and biological properties of cephalosporins having quinoliniumthiomethyls at the C-3 position and demonstrated that quinoliniumthiomethylcephalosporins exhibited an extended antibacterial spectrum including antipseudomonal activity, especially strong antiGram-positive activity. With the aim of elaboration to optimize the antibacterial potency of the quaternary ammonium cephalosporins, we have studied the modification of substituents in the pyrrolidine ring of cefepime 1 which is characterized by its potent activity.

  • Background: Nosocomial pneumonia in an intensive care unit (ICU) is associated with a high mortality rate. Diagnosing a respiratory tract infection in critically ill patients is still difficult but detailed information for the pathogens is needed to establish an adequate antimicrobial treatment. This study examined the causative organisms and their antimicrobial resistance using bronchoalveolar lavage (BAL) from patients suspected of having pneumonia in the ICU. Methods: From January 2004 to June 2006, ICU patients with diffuse lung infiltration were prospectively enrolled. The BAL was used to diagnose the respiratory infection, with 104 > or = organisms considered a positive result. The most common organisms and their antimicrobial resistances were analyzed from the quantitative BAL cultures in the burn ICU and non-burn ICU. Results: A total 72 patients were included, 35 (M 29, F 6) in the burn ICU and 37 (M 26, F 11) in the non-burn ICU. 27 patients (77.1%) in the burn ICU and 22 patients (59.5%) in the non-burn ICU met the criteria for a positive BAL culture. The major pathogens were Staphylococcus aureus, Acinetobacter species and Pseudomonas aeruginosa. All strains (100%) of Staphylococcus aureus isolated from BAL (9 cases) were methicillin-resistant (MRSA) in the burn ICU, but 5 strains (71.4%, 7 cases) were MRSA in the non-burn ICU. Regarding Pseudomonas aeruginosa, the rate of resistance to amikacin, ciprofloxacin, cefepime, imipenem, ceftazidime, piperacillin/tazobactam in the burn and non-burn ICU ranged from 45.5% to 90% and 25% to 50%, respectively. In addition, the rate of resistance of Acinetobacter species to the above drugs in the burn and non-burn ICU ranged from 81.8% to 100% and 62.5% to 100%, respectively. Conclusions: These results are expected to provide useful guidelines for choosing the effective empirical antimicrobial therapy in patients with lung infiltrations in the burn and non-burn ICU.

CTX-M-14 Producing Enterobacteriaceae Isolated from Chickens at Gyeongsang Provinces (경북지역의 닭으로부터 CTX-M-14 생성 장내세균 분리동정)

  • Antimicrobial agents have been used in poultry for treatment of bacterial infections or additives over the past half century. However, increasing antimicrobial resistance has led to selective pressure for therapeutic use in humans and made treatment of bacterial infection more difficult. In this study, we examined the prevalence of plasmid mediated antimicrobial resistant determinants for resistance to ${\beta}-lactam$, quinolone, and aminoglycoside in Enterobacteriaceae isolates obtained from chickens in Gyeongsang provinces, and correlation between the resistant genes and antimicrobial resistance rate was also assessed. A total of 43 Enterobacteriaceae isolates were recovered from 40 chickens at Gyeongsang provinces in Korea. Antimicrobial susceptibility was determined by disk diffusion method. PCR and DNA sequencing were performed to characterize the antimicrobial resistant genes. Of the 43 Enterobacteriaceae isolates tested, 2 isolates harbored $bla_{CTX-M-14}$ gene, and 2 and 5 strains contained qnrS and aac(6')-Ib-cr genes, respectively. A total of 43 isolates displayed a relatively lower susceptible rate ranging between 0.0 and 23.3% to most of the antimicrobial agents, except cefepime, ceftazidime, and cefaclor. We confirmed that plasmid mediated antimicrobial resistant determinants were distributed in Enterobacteriaceae isolates from chickens. Investigation of the genes and monitoring of antimicrobial resistance rate is required to prevent further spreading of antimicrobial resistant genes among Enterobacteriaceae isolates.

Effectiveness of cephalosporins in hydrolysis and inhibition of Staphylococcus aureus and Escherichia coli biofilms

  • Importance: Staphylococcus aureus and Escherichia coli contribute to global health challenges by forming biofilms, a key virulence element implicated in the pathogenesis of several infections. Objective: The study examined the efficacy of various generations of cephalosporins against biofilms developed by pathogenic S. aureus and E. coli. Methods: The development of biofilms by both bacteria was assessed using petri-plate and microplate methods. Biofilm hydrolysis and inhibition were tested using first to fourth generations of cephalosporins, and the effects were analyzed by crystal violet staining and phase contrast microscopy. Results: Both bacterial strains exhibited well-developed biofilms in petri-plate and microplate assays. Cefradine (first generation) showed 76.78% hydrolysis of S. aureus biofilm, while significant hydrolysis (59.86%) of E. coli biofilm was observed by cefipime (fourth generation). Similarly, cefuroxime, cefadroxil, cefepime, and cefradine caused 78.8%, 71.63%, 70.63%, and 70.51% inhibition of the S. aureus biofilms, respectively. In the case of E. coli, maximum biofilm inhibition (66.47%) was again shown by cefepime. All generations of cephalosporins were more effective against S. aureus than E. coli, which was confirmed by phase contrast microscopy. Conclusions and Relevance: Cephalosporins exhibit dual capabilities of hydrolyzing and inhibiting S. aureus and E. coli biofilms. First-generation cephalosporins exhibited the highest inhibitory activity against S. aureus, while the third and fourth generations significantly inhibited E. coli biofilms. This study highlights the importance of tailored antibiotic strategies based on the biofilm characteristics of specific bacterial strains.

Toxin Genes and Antibiotic Resistance of Food Poisoning Bacteria Isolated from Food Service Equipment in Childcare Centers (어린이집 급식설비에서 분리된 식중독 미생물의 독소 유전자 및 항생제 내성)

  • This study assessed the contamination level of food poisoning bacteria on handles of food service equipment in childcare centers to prevent food poisoning and analyzed toxin genes and antibiotic resistance of isolated strains. The isolates used in this study were collected from 101 childcare centers in Jeollanam-do. Four strains of Bacillus cereus and two strains of Staphylococcus aureus were isolated on the handles of food service equipment (refrigerators and freezers). The toxin genes of B. cereus were detected as nheA, nheB, nheC, entFM, and cytK. No toxin genes of S. aureus were detected. B. cereus showed resistance to β-lactam antibiotics, such as ampicillin and cefepime. S. aureus also showed resistance to antibiotics such as ampicillin and cefepime. Therefore, microbial safety and hygiene management, such as periodic sterilization of handles, should be strengthened to prevent food poisoning caused by cross-contamination of food service equipment handles in childcare centers.
