Comparative Study About Academic Thoughts of Xu Lingtai and Yoshimasu Todo (II) - Analysis of the Cause of Similarities and Differences in their Academic Thoughts - (서영태(徐靈胎)와 길익동동(吉益東洞)의 학술사상 비교 연구 (II) - 학술 사상이 같고 다른 원인에 대한 분석 -)

  • In the 18th century, Xu Lingtai (徐靈胎) and Yoshimasu Todo (吉益東洞) were medical revolutionaries. They emphasized researches about synthesis of formulae, efficacy of medication and observation and then classification of clinical phenomena, so they assumed a modern scientific character. But, there were clear differences between their academic thoughts. In this paper, we examine the causes of difference in three fields, i.e. traditional culture, viewpoints of talented people and academic personality. The first, difference was due to traditional culture. Chinese medicine has a long history and heavy traditional culture. Yin-Yang (陰陽) theory, Five Phase(五行) theory, Viscera and Bowels (臟腑) theory and Meridian and Collateral (經絡) theory stemmed from everyday practice, and Chinese people learn these theories from experience and observation. From the standpoint of Chinese people, particularly scholarly doctors [儒醫] such as Xu Lingtai, it was easy to debate medical theories. In contrast, Japanese traditional culture didn't have as long a history as China. Thus as a necessity, it was harder to disseminate traditional Chinese medicine theories in Japan. Yoshimasu Todo simplified it by cutting out the superfluous traditional Chinese medicine theory, so at that time it must have been shocking to the Japanese medical world's trends. The second, difference was due to viewpoints of talented experts. From the standpoint of Xu Lingtai, above all, medicine is just a learning, only a kind of technique, even more not a means of living. Xu Lingtai was concerned with the appearance of very talented experts such as 'great man' (偉人), and 'exceptional man' (奇士) who carried out medical research. Instead of cultivating a few talented people, Yoshimasu Todo tried to produce a large number of clinicians quickly who could treat ordinary people. The third was due to personality difference. As Xu Lingtai threw away Confucianism and studied medicine in his youth, although he had a critical attitude, he was always mild-mannered. Yoshimasu Todo always had a clearly critical and rebellious nature. Personality influenced their literary spirit and learning style, so although both advocated reactionism, the academic thought of Xu Lingtai was reformative and mild, while that of Yoshimasu Todo was revolutionary and fierce. Xu Lingtai and Yoshimasu Todo had considerably similar research domains and academic thought, so it is proper for them both to serve as examples for making a comparative study of medical history in China and Japan in 18th century.

The Evaluation of Clinical Prognostic Factors for the Surgically Treated Unstable Ankle Fractures (불안정성 족근관절 골절의 수술적 치료 결과에 영향을 미치는 임상적 예후 인자에 대한 분석)

  • Purpose: The purpose of this study is to analyze the clinical prognostic factors which may affect the postoperative clinical results of the unstable ankle fractures. Materials and Methods: This study is based on 62 unstable ankle fractures treated by open reduction and internal fixation from May 1994 to June 2000, with a minimum follow-up period of 12 months(range: 13 months-7 years 3 months). The 62 patients were average 39.1 years old with male: female ratio of 41:21. Based on Lauge-Hansen classification, the supination-external rotation type was the most common with 36 (58.1%) cases. The clinical results was assessed by American Orthopaedic Foot and Ankle Society(AOFAS) functional scale. The sex, age, body weight, trauma-operation interval, operation time, cause of injury, fracture type were statistically analyzed as the possible postoperative clinical prognostic factors. Results: Postoperative AOFAS functional scale was average 82.1 points with 22(35.5 %) cases excellent, 12(19.4%) good. 16(25.8%) fair and 12(19.4%) cases poor results. The age and the operation time were found to be statistically significant factors affecting the prognosis(p<0.001). The sex, weight, trauma-operation interval factors did not significantly affect the clinical results. The pronation-external rotation type showed better clinical tendency among the fracture types, but without the statistical significance. Conclusion: The surgically treated unstable ankle fractures in patients whose age was above 41 years old or operation time exceeding 90 minutes showed significantly poor clinical results.

Improved Direction of Arrival Estimation Based on Coprime Array and Propagator Method by Noise Power Spectral Density Estimation (잡음 파워 스펙트럼 밀도 추정을 이용한 서로소 배열과 프로퍼게이터 기법 기반의 향상된 도래각 추정 기법)

  • We propose an improved direction of arrival (DoA) estimation algorithm based on co-prime array and propagator method. The propagator method with co-prime array does not require singular value decomposition (SVD) requiring much less computational complexity but exhibiting somewhat worse performance in comparison with MUSIC based on co-prime array. We notice that one cause of the performance degradation was in the avoidance of the usage of the diagonal elements of the signal autocorrelation matrix that contains the noise power spectral density. So we propose an algorithm with the diagonal elements of the signal autocorrelation matrix based on the fact that the noise power spectral density can be estimated using noise observation over a long period of time. We observe, through simulations, that the proposed scheme in this paper improves the performance, with 4 times more computational requirement, by signal-to-noise ratio of 1.5dB and by DoA resolution of $0.7^{\circ}$ at the detection probability of 95% compared with the previously introduced co-prime array propagator scheme, resulting in performance much closer to that of co-prime array-based MUSIC scheme.

  • Raman spectra exhibit differences in intensity depending on the measuring equipment and environmental conditions even for the same material. This restricts the pattern recognition approach of Raman spectroscopy and is an issue that must be solved for the sake of its practical application, so as to enable the reusability of the Raman database and interoperability between Raman devices. To this end, previous studies assumed the existence of a transfer function between the measurement devices to obtain a direct spectral correction. However, this method cannot cope with other conditions that cause various intensity distortions. Therefore, we propose a classification method using linear intensity calibration which can deal with various measurement conditions more flexibly. In order to evaluate the performance of the proposed method, a Raman library containing 14033 chemical substances was used for identification. Ten kinds of chemical Raman spectra measured using three different Raman spectroscopes were used as the experimental data. The experimental results show that the proposed method achieves 100% discrimination performance against the intensity-distorted spectra and shows a high correlation score for the identified material, thus making it a useful tool for the identification of chemical substances.

Study on Effects of Population Stratification on Haplotype Trend Test in Case-Control Studies (환자-대조군 연구에서 인구집단 층화가 일배체형 경향성 검정에 미치는 영향)

  • Population stratification can cause spurious associations between genetic markers and disease locus. In order to handle this population stratification in haplotype-based case-control association studies, we added population indicators as covariates to the haplotype trend regression model proposed by Zaykin et al. (2002). We investigated through simulations how both population stratification and measurement error in the estimation of true population of each individual affect type I error probabilities of the association tests based on both Zaykin et al.'s (2002) model and the proposed model. Based on those results, in the situation that there exists population stratification but there is no error in population classification of each individual, our proposed model does satisfy a type I error probability whereas Zaykin et al.'s (2002) model does not. However, as the measurement error increases, a type I error probability of our model correspondingly becomes larger than a nominal significance level. It implies that as long as uncertainty in the estimation of true population of each individual still remains, it is nearly impossible to avoid false positive in case-control association studies based on haplotypes.

  • Objectives : The aim of this study was to provide basic background information on stroke by evaluating various etiological factors on the basis that the onset time of cerebral infarction varies according to its cause. Methods : We studied hospitalized patients within 4 weeks after their ictus who were admitted at Kyunghee OMC, Kyungwon OMC, or Donguk Ilsan OMC from [month] $1^{st}$, 2005 to June $30^{th}$, 2007. We compared the general characteristics of acute stroke patients according to onset time. Results : Regardless of the onset time, the distribution of cerebral infarction patterns showed high SVO. The onset time did not show significant difference for average age, height, weight, BMI, waist measurement, hip measurement, or waist/hip ratio between onset during sleep and awakening. Patients whose stroke occurred while awake were more likely to have a past history of HTN, DM, ischemic heart disease, or atrial fibrillation but did not show significant difference according to their onset time. Conclusions : The above results show that the overall tendency of acute stage cerebral infarction patients varies according to their onset time. This study was carried out on the basis of previous findings that cerebral infarctions that occurred during sleep were more likely to have been caused by cerebral thrombosis, and strokes that occurred while awake were more likely to have been caused by cerebral embolism. However, there were no statistically relevant results, so a larger study group is needed to research the tendency of stroke patients.

Characteristics of Nasal Trauma in the Implanted Nasal Prosthesis (실리콘 코높임술 후 코 부위 외상의 특징)

  • Purpose: Presently, silicone rubber is chosen most frequently for nasal augmentation. However, there is a possibility of extrusion with this material. Sometimes, noses are prone to be traumatized, and then silicone rubber has a possibility of deformity or deviation resulting in trauma. We experienced cases with complications and traumatic deformities after the augmentation rhinoplasty. Methods: A retrospective review was performed to determine the characteristics of the implanted nasal silicone prosthesis after trauma. The patients' data such as deviation of implant, shape of fracture, age and sex of the patient, time of treatment, operative methods were reviewed. From March 2001 to March 2008, this study was performed in 30 patients. The patients were 25 females and 5 males, from 24 to 60 years of age, with an average of 42. All patients had previous augmentation rhinoplasty with silicone implant. Results: All of the 30 patients were confirmed as deviation of silicone and nasal bone fractures in the facial bone CT scan. The most common cause of fracture was traffic accident. The classification of nasal trauma after augmentation was done by facial bone CT. Class I: Deviation of silicone without nasal bone fracture without extrusion(12 cases, 40%), Class II: Deviation of silicone without nasal bone fracture and with extrusion(4 cases, 13%), Class III: Deviation of silicone with nasal bone fracture and without extrusion(8 cases, 27%), Class IV: Deviation of silicone with nasal bone fracture and with extrusion(3 cases, 10%), Class V: Mild deviation of silicone with nasal bone fracture(3cases, 3%). Specially, the comminuted or trapezoid nasal fracture was confirmed in 11 cases(Class III, IV). Conclusion: The problems of silicone implant have generally been related to foreign body reactions, rigidity of the material, encapsulation, infections, and extrusion. We experienced 11 cases of comminuted or comminuted trapezoid shaped fracture below nasal implant. So, we think this phenomenon could be used in late problem of silicone implant.

Mitral Valve Repair for Mitral Regurgitation (승모판막폐쇄부전에 대한 승모판막재건술)

  • From February 1996 to May 1997, 18 patients underwent mitral valve repair for mitral regurgitation. There were 9 male and 9 female patients aged from 19 to 68 years(mean, 53). Thirteen patients were in New York Heart Association(NYHA) class III and IV. The cause of mitral regurgitation was degenerative in 12 patients, rheumatic in 5 patients and infective in 1 patient. Fifteen patients were in Carpentier's functional classification II, 2 patients in Carpentier's class III and 1 patient in Carpentier's class I. Surgical procedures included prosthetic ring annuloplasty(16 cases), rectangular resection of posterior leaflet(15 cases), chordal shortening(5 cases), triangular resection of anterior leaflet(2 cases), commissurotomy(2 cases), partial transposition of posterior leaflet(1 case). These procedures were combined in most patients. There was no operative death. These patients have been followed from 1 to 15 months, mean of 6.7 months. There was one late death resulted from low cardiac output following mitral valve replacement. The function of the repaired valve in other 17 patients has remained satisfactory during the observed interval. We consider that mitral valve repair is highly satisfactory in patients with mitral regurgitation.

Sewer CCTV Inspection Prioritization Based on Risk Assessment (위험도 기반의 하수관로 CCTV 조사 우선순위 결정 연구)

  • Most sewer lines buried in the city are likely to be collapsed due to serious aging. Also, due to the high concentration of development and high population density and traffic, the collapse of the sewer will cause enormous social and economic damage. Therefore, proactive maintenance is required to prevent accidents caused by deteriorated sewer pipe. In order to utilize limited budget effectively, risk-based prioritization methods should be proposed that simultaneously consider the consequence of failure and the probability of failure. In this study, the method of risk-based prioritization of sewer was examined by reviewing various cases of overseas studies and applied to the urban sub-catchment. First, the impact factors that can be secured through the sewer GIS DB in Seoul were derived, and the weight, sub-criteria, and impact score of each impact factor were determined and the consequence of failure was calculated by weight sum method. In addition, the probability of failure was calculated by dividing the service life by the estimated useful life, and the consequence of failure and the probability of failure were classified into five grades by the Jenks natural breaks classification method. The prioritization method was applied to sub-catchment in the Seoul to derive a risk matrix and a risk grade. As a result, 26% of all subjects were selected as the inspection priority subjects with 4-5 risk grade. Therefore, using the risk-based CCTV prioritization methodology, it will be possible to systematically determine the objects that need investigation first.

A Study on the Weight Length Index and Dental Caries of Elementary School Students (초등학생들의 체중신장지수(WLI)와 치아우식증에 관한 연구)

  • In Korea, changes in children's diet patterns accelerate their physical growth and development: frequent snacking has been presumed to be a major cause of increasing dental caries. The present study attempts to clarify the relationship between the physical development of growing children and their tooth decay. For this purpose, 632 six-grade children in 4 elementary school located in Urban(Seoul) and Rural(Po-gok, Yang-In) were classified into three groups based on the Weight Length Index(WLI), known to reflect the nutritional conditions of school-age children, and the relationship was analyzed between each group and the variables considered to be related with dental caries. The result is as follows: The average weight and height of the male is $44.88{\pm}10.89$ kg, $148.49{\pm}7.33$ cm and female is $43.35{\pm}9.60$ kg, $149.23{\pm}6.73$ cm, respectively, which are in the similar level with the Korean Physical Standard. The classification of the children by the WLI reveals a relatively high distribution of over-weighted child ren - 212 persons, 335% of the entire population. The DMFT Index was a little high in the rural area(3.15 teeth in urban and 3.31, in rural). Among the groups of children classified by the WLI, the over-weight group have the highest DMFT index(3.69 teeth). The relationship between the frequency of taking in basic nutrients and the DMFT index is also found: the relationship is not evident in case of the foods containing rich calcium, protein, as well as fruits and vegetables. But, in the protein-rich food, higher frequency of its intake means significantly lower DMFT index in the normal-weight group of the urban children. In case of carbohydrate, higher frequency of its intake means significantly higher DMFT index in all the groups of the rural children. The DMFT index has some correlations with the relevant variables: the index has a positive correlation with the frequency of snacking, and a negative correlation with the economic status. That is, the higher the frequency of snacking is, and the lower the economic status is, the higher the DMFT index may be. In the logistic multiple regression analysis conducted with the presence of DMFT as a dependent variable, only the frequency of tooth brushing is turned to be a variable affecting the presence of either decayed, missing, or filled teeth. Based on the above result, the variables affecting the DMFT index are a time spent on eating, frequency of intake of protein and carbohydrate for a week, frequency of snacking, regular dental check-ups, preventive behaviors for oral health(fluoride gargling, tooth brusing after each meal, proper tooth brushing method). These variables have a relationship with the DMFT index, but the degree is somewhat different between the groups classified either by the region or by the WLI. Therefore, appropriate nutrition management should be conducted according to the individual's nutritional conditions when the services like nutritional education are provided based on the closely-examined characteristics of each target group. And, at the same time, oral health education should be strengthened, and its importance should also be emphasized so that people can pay attention to their own oral health.
