Advanced peri-implantitis cases with radical surgical treatment

  • Purpose: Peri-implantitis, a clinical term describing the inflammatory process that affects the soft and hard tissues around an osseointegrated implant, may lead to peri-implant pocket formation and loss of supporting bone. However, this imprecise definition has resulted in a wide variation of the reported prevalence; ${\geq}10%$ of implants and 20% of patients over a 5- to 10-year period after implantation has been reported. The individual reporting of bone loss, bleeding on probing, pocket probing depth and inconsistent recording of results has led to this variation in the prevalence. Thus, a specific definition of peri-implantitis is needed. This paper describes the vast variation existing in the definition of peri-implantitis and suggests a logical way to record the degree and prevalence of the condition. The evaluation of bone loss must be made within the concept of natural physiological bony remodelling according to the initial peri-implant hard and soft tissue damage and actual definitive load of the implant. Therefore, the reason for bone loss must be determined as either a result of the individual osseous remodelling process or a response to infection. Methods: The most current Papers and Consensus of Opinion describing peri-implantitis are presented to illustrate the dilemma that periodontologists and implant surgeons are faced with when diagnosing the degree of the disease process and the necessary treatment regime that will be required. Results: The treatment of peri-implantitis should be determined by its severity. A case of advanced peri-implantitis is at risk of extreme implant exposure that results in a loss of soft tissue morphology and keratinized gingival tissue. Conclusions: Loss of bone at the implant surface may lead to loss of bone at any adjacent natural teeth or implants. Thus, if early detection of peri-implantitis has not occurred and the disease process progresses to advanced peri-implantitis, the compromised hard and soft tissues will require extensive, skill-sensitive regenerative procedures, including implantotomy, established periodontal regenerative techniques and alternative osteotomy sites.

Pediatric Inguinal Hernia Surgery 2,230 Cases Performed with Ketamine and Lidocaine (케타민과 리도카인으로 시행한 소아 탈장 수술 2,230명)

  • Ketamine is a safe and effective drug for pediatric anesthesia, sedation and analgesia. We hoped to identify that surgeons could operate a pediatric hernia with the ketamine anesthesia without general anesthesia. The study was a consecutive case series of 2230 inguinal hernia patients aged 1 months to 17 years in a Joo's day-surgical clinic during 11-year period. The patients had pediatric inguinal hernia surgery without general anesthesia under the day-surgery system. We retrospectively analyzed the medical record of patients who were registered with the Diagnosis Related Group (DRG) system. All patients received ketamine (5mg/kg) and atropine (0.01mg/kg) intramuscularly before surgery. After anesthesia, we injected 1~2% lidocaine (Less than 5ml) subcutaneously at the site of incision and started operation. The surgical method was the high ligation method of the hernia sac.) In total 2230 patients, male were 1756 and female were 474. 2076 patients were a unilateral inguinal hernia at the time of surgery and 154 were bilateral hernia patients. Less than three months, depending on the age of the patients was 391, and less than 12 months the patient was 592 people (26.5%). After surgery, there were no accidents or long term complications associated with ketamine anesthesia. We think the surgeon can safely do the pediatric inguinal hernia surgery using ketamine and lidocaine without anesthesiologist through 11 years of our surgical experiences.

  • Korean universities have selected candidates through admission officer system since 2009. However, the universities now have to settle several problems that they faced at the first stage of the system. Therefore, this research, taking the admission screening data of J University as examples, aims to discuss how differently the weight of the screening factors appears depending on the subjects related to college entrance, such as parents, teachers, and admission officers. The research indicates that the subjects have different perspectives about entrance screening requisites. Parents and teachers more value the student record that is a countable indicator than the letter of self-recommendation that is a uncountable indicator. However, it also indicates that admission officers take attitude against parents and teachers.

Categorical time series clustering: Case study of Korean pro-baseball data (범주형 시계열 자료의 군집화: 프로야구 자료의 사례 연구)

  • A certain professional baseball team tends to be very weak against another particular team. For example, S team, the strongest team in Korea, is relatively weak to H team. In this paper, we carried out clustering the Korean baseball teams based on the records against the team S to investigate whether the pattern of the record of the team H is different from those of the other teams. The technique we have employed is 'time series clustering', or more specifically 'categorical time series clustering'. Three methods have been considered in this paper: (i) distance based method, (ii) genetic sequencing method and (iii) periodogram method. Each method has its own advantages and disadvantages to handle categorical time series, so that it is recommended to draw conclusion by considering the results from the above three methods altogether in a comprehensive manner.

  • 'Personal Healthcare Records' (PHR) is a service for providing individual clinical data to patients. PHR service should be useful for the patient and healthcare service providers. This study has aimed at not only providing patients' clinical data but also developing optimized healthcare service for every patients. The research has been conducted as 3 phases: formal case analysis, caregivers interviews and patients interviews. The patients interviews were limited to cancer patients. As results, 3 key functions have been developed. First, it offers patient's clinical pathway as a personalized medical treatment scheduler. Second, it supplies Question & Answer board on online. Last, it supports patients to input their healthcare record. This Myongji PHR service has 3 months of pilot test on web and mobile application(android version). For further commercialized PHR service, the standardization for clinical pathway registration and user convenience need to be considered.

  • The present with in this country, rock bolt installation, shotcrete, and concrete lining in construction order has become accepted as a general tunnelling method in NATM. On the other hand Unlined tunnelling method, which was developed by many countries near Scandinavian Peninsula with hard rocks comparatively, has recently been introduced all over the world, and numerous studies about that have been being devoted to domestic tunnels. Unlined tunnelling method has been developed on the basis of the permanently unsupported openings, and general 7 requirements for them were suggested by Nick Barton. There are no case record about these conditions for Q-system in this country. Therefore, input parameters for Q-system under these conditions were applied to general road tunnel cross-section and numerical analyses for each condition were executed with UDEC-BB, Distinct Element Method.

A Study on Economical Efficiency Evaluation of Semiforcing under Structure Watermelon Cultivators (반촉성 시설수박 재배농가의 경제적 효율성 분석)

  • Technical efficiency of semiforcing watermelon growers is 0.8248 on average, and distributed between 0.6744 and 0.9268. The result showed that semiforcing watermelon growers had by 18% of technical inefficiency and could be assumed that increasing technical efficiency could induce watermelon production more increase. Consequently, if growers' technical efficiency were improved while other environments were constant, watermelon production could be increased. Following the results from the inefficiency effect model, all assumption coefficient such as growers age etc, are significant at 10% level. Estimate of dispersion parameter ${\gamma}$ is 0.89, which confirms those differences between practical output and frontier output were derived from the technical efficiencies among growers. Differences of production system between high and low level growers in production efficiency were showed at side altitude, ventilation and heat-retaining in section of facilities and automation, soil test and calcium application in section of environment management, transplant preparation and duration of pollination in section of crop management and shipment place, sorting degree and management record analysis in section of business management respectively. As a result of analyzing consulting data by using standard diagnosis table of watermelon cultivation under structure which cultivated on semi-forced watermelon growers, gap between high and low level growers was 7.0 points in facility automation section, 7.1 points in environment section, 8.8 points in crop management section and 13.6 points in business management section, respectively, which were the biggest one among them. In case of excluding information-related items from the evaluation index of business management section, changes of business achievement are to occur. Therefore, it is recommended for us to review the standard diagnostic table of watermelon cultivation under structure by dividing evaluation index of management section into management and information.

A Review on Nominal Validity of Seokgatab and Dabotab (석가탑(釋迦塔)과 다보탑(多寶塔)의 명칭적인 타당성 검토)

  • The names of Seokgatab and Dabotab are almost generalized among the people along with Bulguksa the most well-known temple in Korea. But, despite this generalized situation, the names of Seokgatab and Dabotab have not secured the clear standpoint till now. It is because the names of Seokgatab and Dabotab can be found only in the 18C documents like and . More complicatedly, in the record of the Goryeo sarira case found during the dismantling of Seokgatab, the name of Seokgatab was 'Mugujeonggwangtab' or 'Seoseoktab'. This paper is to secure the nominal validity of Seokgatab and Dabotab to remove the confusion due to their names and to raise the efficiency of studies related with Bulguksa in the future. First, their names and the problems related with them were summarized in Chapter 2. Based on this critical consciousness, Chapter 3 deals with the tragedy of Asadal related with the creation of the stone pagodas and its symbolic re-interpretation. In this process, we can see that there could be a viewpoint of the great comprehension in them through the completion of 'the Buddhist Nation'. Then, the architectural features of Seokgatab and Dabotab were analyzed in the ritual view point of . Finally, in Chapter 4, on the basis that the viewpoint of one specific sect can not be predominantly applied in the construction of Buddhist land, it is held that Avatamsaka Idea and Saddharma-pundarika Idea can coexist without any conflict. It is shown in the fact that Seokgatab and Dabotab can make harmony together in the Avatamsaka Idea while keeping their names. With these efforts, we can clearly see that the peculiar titles of Seokgatab and Dabotab can be said to secure sufficient validity.

Wireless Electrocardiogram Monitoring System for Small Animals Based on IEEE 802.15.4 (IEEE 802.15.4에 기초한 작은 동물을 위한 무선 심전도 감시 시스템)

  • Electrocardiogram (ECG) is a graphical record of the voltages generated during the heart beats, and is very useful in observing the status of the heart of the human or animals. In this paper, an ECG monitoring system is developed for the small animals, such as the rat. This system is very useful in monitoring the side effect of drugs. Since small animals have faster heart rate than the human case, the monitoring system has a faster sampling frequency and processing speed. Furthermore, for a realtime monitoring of the small moving animals, the system is composed of a small wireless ECG module, which is based on the IEEE 802.121.4 wireless physical layer. The developed wireless ECG system can successfully monitor the ECG of small animals, such as the rat.

Significance of Blood Group and Social Factors in Carcinoma Cervix in a Semi-Urban Population in India

  • Background: To assess the significance of social factors as risk factors for carcinoma cervix and to determine the significance of blood group to prevalence of carcinoma cervix in a semi-urban population of Kolar, Karnataka, India. Materials and Methods: One hundred cases of carcinoma cervix were included in the study, along with 200 females of the same ages considered as controls. Case details were collected from the hospital record section regarding social factors and blood groups and the data were analyzed by descriptive statistical methods. Results: Blood group B showed the highest number of cases (55 cases) followed by blood group O (29 cases) in carcinoma cervix which was statistically significant (p<0.001). Age of marriage between 11 to 20 years showed highest number of carcinoma cervix cases (77 cases) and this also was statistically significant (p<0.001). Patients with rural background were 75 (p=0.112, odds ratio: 1.54), parity of more than or equal to two constituted 96 cases (p=0.006, odds ratio: 4.07) and Hindu patients were 95 in number (p=0.220, odds ratio: 1.89). Conclusions: Blood group B and age of marriage between 11 and 20 years were significantly associated with carcinoma cervix in our population. Region of residence, parity and religion presented with a altered risk for carcinoma cervix.
