Risk Factors for Developing Large Emboli Following Carotid Artery Stenting

  • Objective : The introduction and development of the embolic protecting device (EPD) has resulted in a decreased rate of stroke after carotid artery stenting (CAS). The authors performed a retrospective study to investigate the risk factors for developing large emboli after CAS which can lead to ischemic events. Methods : A total of 35 consecutive patients who underwent CAS between January 2009 and March 2012 were included in this study. Patients were divided into two groups including those with small emboli (group A; grade 1, 2) and those with large emboli (group B; grade 3, 4). The size and number of emboli were assigned one of four grades (1=no clots, 2=1 or 2 small clots, 3=more than 3 small clots, 4=large clots) by microscopic observation of the EPD after CAS. We compared demographic characteristics, medical history, and angiographic findings of each group. Results : Thirty-five patients underwent CAS, and technical success was achieved in all cases. Twenty-three patients were included in group A and 12 patients in group B. Our results demonstrated that advanced age [odds ratio (OR) 1.24; 95% confidence interval (CI) 1.01-1.52; p=0.044] and smoking (OR 42.06; CI 2.828-625.65, p=0.006) were independent risk factors for developing large emboli after CAS. Conclusion : In patients with carotid artery stenosis treated with CAS, advanced age and smoking increased the number and size of emboli. Although use of an EPD is controversial, it may be useful in CAS in patients with risk factors for large emboli in order to reduce the risk of ischemic events.

  • CRISPR/Cas9 genome editing systems have been established in a broad range of eukaryotic species. Herein, we report the first method for genetic engineering in pyogo (shiitake) mushrooms (Lentinula edodes) using CRISPR/Cas9. For in vivo expression of guide RNAs (gRNAs) targeting the mating-type gene HD1 (LeA1), we identified an endogenous LeU6 promoter in the L. edodes genome. We constructed a plasmid containing the LeU6 and glyceraldehyde-3-phosphate dehydrogenase (LeGPD) promoters to express the Cas9 protein. Among the eight gRNAs we tested, three successfully disrupted the LeA1 locus. Although the CRISPR-Cas9-induced alleles did not affect mating with compatible monokaryotic strains, disruption of the transcription levels of the downstream genes of LeHD1 and LeHD2 was detected. Based on this result, we present the first report of a simple and powerful genetic manipulation tool using the CRISPR/Cas9 toolbox for the scientifically and industrially important edible mushroom, L. edodes.

CSE1L/CAS, a microtubule-associated protein, inhibits taxol (paclitaxel)-induced apoptosis but enhances cancer cell apoptosis induced by various chemotherapeutic drugs

  • CSE1L/CAS, a microtubule-associated, cellular apoptosis susceptibility protein, is highly expressed in various cancers. Microtubules are the target of paclitaxel-induced apoptosis. We studied the effects of increased or reduced CAS expression on cancer cell apoptosis induced by chemotherapeutic drugs including paclitaxel. Our results showed that CAS overexpression enhanced apoptosis induced by doxorubicin, 5-fluorour-acil, cisplatin, and tamoxifen, but inhibited paclitaxel-induced apoptosis of cancer cells. Reductions in CAS produced opposite results. CAS overexpression enhanced p53 accumulation induced by doxorubicin, 5-fluorouracil, cisplatin, tamoxifen, and etoposide. CAS was associated with $\alpha$-tubulin and $\beta$-tubulin and enhanced the association between $\alpha$-tubulin and $\beta$-tubulin. Paclitaxel can induce G2/M phase cell cycle arrest and microtubule aster formation during apoptosis induction, but CAS overexpression reduced paclitaxel-induced G2/M phase cell cycle arrest and microtubule aster formation. Our results indicate that CAS may play an important role in regulating the cytotoxicities of chemotherapeutic drugs used in cancer chemotherapy against cancer cells.

A Survey of the Serotype Distribution of Streptococcus mutans Isolated from Dental Plaques of Caries-active and Caries-free Subjects (치아우식증환자와 치아정상인(齒牙正常人)의 치태(齒苔)에서 분리(分離)한 Streptococcus mutans의 혈청형(血淸型) 분포(分布)에 관(關)한 조사(調査) 연구(硏究))

  • An epidemiological survey was carried out to identify the serotypes of Streplococcus mutans isolated from carious lesions of 65 caries-active subjects(CAS) and sound tooth surfaces of 40 caries-free subjects(CFS). The autoclaved antigen extract was performed on each isolate, and then, serotypes of the isolates were determined in agar-gel immunodiffusion test. The results were as follows: 1. S. mutans was found in 78% of the samples of CAS, and of CFS. The difference of isolation frequency between CAS and CFS was not observed. 2. Only one serotype per single subject was detected in 61% of total samples, in remaining 39% of samples two or more serotypes were detected. 3. In 41.2% of CAS samples plural serotypes of S. mutans were found, whereas 35.5% of CFS samples showed plural serotypes distribution. 4. The most frequently identified serotype in each subject was serotype c; 69.5% of subjects harbored serotype c S. mutans. Serotype d was next most frequently isolated from subjects, comprising 23.2%. 5. Serotype c strain was found in 64.7% of CAS, 77.4% of CFS. 6. Of the isolates from CAS and CFS, serotype c was most commonly found, comprising 48.8%, serotype d was found in 16.3%, serotypes f. e, and g comprising 13.2%, 9.3%, and 7.8% respectively. Serotypes a and b were also found but in far lower frequencies(2.3%, 0.8%). 7. Serotype c strains were more found in CFS than in CAS, but serotypes d and e were more found in CAS.

Protected versus Unprotected Carotid Artery Stenting : Meta-Analysis of the Current Literature

  • Objective : To compare peri-operative any symptomatic stroke after carotid angioplasty and stenting (CAS), based on the application or absence of a cerebral protection device. Methods : A systematic literature review using PubMed, Embase, and the Cochrane Central was done across an online data base from January 1995 to October 2016. Procedures which were performed due to carotid dissection or aneurysm, procedures using covered stents or conducted in an emergency, were excluded. The primary endpoint was perioperative any symptomatic stroke within 30 days after the procedure. A fixed effect model was used in cases of heterogeneity less than 50%. Results : In the 25 articles included in this study, the number of stroke events was 326 (2.0%) in protected CAS and 142 (3.4%) in unprotected CAS. The use of cerebral protection device significantly decreased stroke after CAS (odds ratio [OR] 0.633, 95% confidence interval [CI] 0.479-0.837, p=0.001). In the publication bias analysis, Egger's regression test disclosed that the intercept was -0.317 (95% CI -1.015-0.382, p=0.358). Regarding symptomatic patients (four studies, 539 CAS procedures), the number of stroke was six (1.7%) in protected CAS and 11 (5.7%) in unprotected CAS. The protective effect against stroke events by cerebral protection device did not have a statistical significance (OR 0.455, 95% CI 0.151-1.366, p=0.160). Conclusion : The use of protection device significantly decreased stroke after CAS. However, its efficacy was not demonstrated in symptomatic patients. Routine use of protection device during CAS should be critically assessed before mandatory use.

Scanning Rayleigh Doppler Lidar for Wind Profiling Based on Non-polarized Beam Splitter Cube Optically Contacted FPI

  • A Scanning Rayleigh Doppler lidar for wind profiling based on a non-polarized beam splitter cube optically contacted FPI is developed for wind measurement from high troposphere to low stratosphere in 5-35 km. Non-polarized beam splitter cube optically contacted to the FPI are used for a stable optical receiver. Zero Doppler shift correction is used to correct for laser or FPI frequency jitter and drift and the timing sequence is designed. Stability of the receiver for Doppler shift discrimination is validated by measuring the transmissions of FPI in different days and analyzed the response functions. The maximal relative wind deviation due to the stability of the optical receiver is about 4.1% and the standard deviation of wind velocity is 1.6% due to the stability. Wind measurement comparison experiments were carried out in Jiuquan ($39.741^{\circ}N$, $98.495^{\circ}E$), Gansu province of China in 2015, showing good agreement with radiosonde result data. Continuous wind field observation was performed from October 16th to November 12th and semi-continuous wind field of 19 nights are presented.

Cost-Effectiveness of Carotid Endarterectomy versus Carotid Artery Stenting for Treatment of Carotid Artery Stenosis

  • Background: Symptomatic or asymptomatic patients with significant carotid artery stenosis (range, 70% to 99%) generally undergo either carotid artery endarterectomy (CEA) or carotid artery stenting (CAS) to prevent stroke. In this study, we evaluated the cost effectiveness of these two treatment modalities. Methods: A total of 47 patients (mean age, $67.1{\pm}9.1$ years; male, 87.2%) undergoing either CEA (n=28) or CAS (n=19) for the treatment of significant carotid artery stenosis were enrolled in this study. Hospitalization costs were subdivided into three parts, namely pre-procedure, procedure and resource, and post-procedure costs. Results: Total hospitalization costs were similar in both groups of CEA and CAS (6,377 thousand won [TW] vs. 6,703 TW, p=0.255); however, the total cost minus the pre-procedure cost was higher in the CAS group than in the CEA group (4,948 TW vs. 5,941 TW, p<0.0001). The pre-procedure cost of the CEA group was higher than that of the CAS group (1,429 TW vs. 762 TW, p<0.0001). However, the procedure and resource cost was higher in the CAS group because the resource cost was approximately three times higher in the CAS group than in the CEA group. The post-procedure cost was higher in the CEA group because hospital stays were approximately two times longer. Conclusion: The total hospitalization cost was not different between the CEA and the CAS groups. The pre-procedure cost was high in the CEA group, but the cost from procedure onset to discharge, including the resource cost, was significantly lower in this group.

Differential Expression of Three Novel Carbonic Anhydrases (CAs) Genes in Marine Dinoflagellate Prorocentrum minimum Against Various pH Conditions (해양 와편모조류 Prorocentrum minimum 기원 신규 탄산무수화효소(CAs) 유전자 3종의 차등 pH 대응 발현)

  • Carbonic anhydrase (CA) is a key controller of the carbon concentrating mechanism (CCM), and is known to be affected by ambient pH and CO2 compositions. Herein, we characterized three novel CAs genes (PmCA1, 2, and 3) from the marine dinoflagellate Prorocentrum minimum, and evaluated the relative expressions of the PmCAs and photosynthetic genes PmatpB and PmrbcL under different pH conditions. Each PmCA was predicted to have amino acid residues constituting the zinc binding site. With signal peptide, PmCA1 and PmCA2 were predicted to be intracellular CAs located in the cytoplasm and chloroplast membrane, respectively. On the other hand, PmCA3 was predicted to be extracellular CA located in the plasma membrane. Also, PmCA1 was classified into the beta family, and PmCA2 and PmCA3 were classified into the alpha family via phylogenic analysis. The photosynthesis efficiency of P. minimum was similar at pH 7 to 9, and decreased significantly at pH 6 and pH 10. Overall, relative gene expression levels of the three PmCAs decreased at low pH, and increased as pH increased. Photosynthesis related genes, PmatpB and PmrbcL, showed similar expression patterns to those of PmCAs. These results suggest that changes in seawater pH may affect photosynthesis and CO2 metabolism in marine dinoflagellates.

An Analysis of the Cognitive Processes of 5-Year-Old Children : A Focus on a Performance of Cognitive Assessment System Based on Gender, Monthly Age, and Tendencies towards Hyperactivity (만 5세 유아의 인지과정 특성 분석 : 성별, 월령, 과잉행동성향에 따른 CAS 수행 결과를 중심으로)

  • This study investigated the cognitive process of 5-year-old children, with a particular focus on gender, monthly age, and their tendencies towards hyperactivity through the performance of the Cognitive Assessment System (CAS; Das & Naglieri, 1997). The children with tendencies towards hyperactivity were identified based on Conners Teachers' Rating Scale (CTRS). The subjects were 75 five-year-old children in Seoul and surrounding metropolitan areas. Data were analyzed by means of descriptive statistics, an independent sample t-test, Pearson's correlation coefficient, one-way ANOVA, and by K-mean cluster analysis. Our results were as follows : (1) The CAS and CTRS' sub-factors were correlated negatively, except the positive correlation between planning factor and hyperactivity factor. (2) Girls exhibited significantly higher CAS scores in planning & sequential processing than boys. (3) The upper monthly age group (68-71 months) showed significantly higher score in terms of planning than the lower monthly age group (60-63 months). (4) The CAS scores of the children with tendencies towards hyperactivity was lower than that of normal children. (5) The CAS profile of 5-year-old children was divided into 4 groups with distinctive characteristics by means of K-mean cluster analysis.

RPS5A Promoter-Driven Cas9 Produces Heritable Virus-Induced Genome Editing in Nicotiana attenuata

  • The virus-induced genome editing (VIGE) system aims to induce targeted mutations in seeds without requiring any tissue culture. Here, we show that tobacco rattle virus (TRV) harboring guide RNA (gRNA) edits germ cells in a wild tobacco, Nicotiana attenuata, that expresses Streptococcus pyogenes Cas9 (SpCas9). We first generated N. attenuata transgenic plants expressing SpCas9 under the control of 35S promoter and infected rosette leaves with TRV carrying gRNA. Gene-edited seeds were not found in the progeny of the infected N. attenuata. Next, the N. attenuata ribosomal protein S5 A (RPS5A) promoter fused to SpCas9 was employed to induce the heritable gene editing with TRV. The RPS5A promoter-driven SpCas9 successfully produced monoallelic mutations at three target genes in N. attenuata seeds with TRV-delivered guide RNA. These monoallelic mutations were found in 2%-6% seeds among M1 progenies. This editing method provides an alternative way to increase the heritable editing efficacy of VIGE.
