Moyamoya Disease : A Case Report and Review of the Literature

  • Moyamoya disease is characterized by the angiographic findings of bilateral occlusion of the distal portion of the internal carotid arteries with a vascular network at the base of the brain. The typical findings on conventional angiography and magnetic resonance imaging usually confirm the correct diagnosis of moyamoya disease. We experienced a 11-year-old girl with moyamoya disease showing repeated transient ischemic attacks and we report on this case with a review of the literature.

Neuroprotective Effect of Duloxetine on Chronic Cerebral Hypoperfusion-Induced Hippocampal Neuronal Damage

  • Chronic cerebral hypoperfusion (CCH), which is associated with onset of vascular dementia, causes cognitive impairment and neuropathological alterations in the brain. In the present study, we examined the neuroprotective effect of duloxetine (DXT), a potent and balanced serotonin/norepinephrine reuptake inhibitor, on CCH-induced neuronal damage in the hippocampal CA1 region using a rat model of permanent bilateral common carotid arteries occlusion. We found that treatment with 20 mg/kg DXT could attenuate the neuronal damage, the reduction of phosphorylations of mTOR and p70S6K as well as the elevations of $TNF-{\alpha}$ and $IL-1{\beta}$ levels in the hippocampal CA1 region at 28 days following CCH. These results indicate that DXT displays the neuroprotective effect against CCH-induced hippocampal neuronal death, and that neuroprotective effect of DXT may be closely related with the attenuations of CCH-induced decrease of mTOR/p70S6K signaling pathway as well as CCH-induced neuroinflammatory process.

Surgical treatment of Takayasu's arteritis : Report of one case (Takayasu씨 동맥염의 수술치험 1예)

  • Takayasu`s arteritis is an arteritis of unknown etiology involving larger elastic arteries such as aorta and its branches, pulmonary arteries, and rarely coronary arteries. The late pathologic feature is vascular obstructive change and the resulting clinical manifestations are local ischemic symptoms such as syncope, visual disturbance, claudication of extremities, hypertension, and angina. the disease occurs predominantly in females, with the age of onset between 10 and 30 years. Recently we have experienced one case of Takayasu`s arteritis involving aortic arch and all its major branches. The patient was 36 year-old female and she was admitted because of headache, blurred vision, and easy fatigability and motor weakness of upper extremities. Aortogram revealed total obstruction of both carotid arteries at the site of its origin and partial irregular obstructive change in the innominate artery and both subclavian arteries. Bypass graft surgery using Gore-Tex grafts was performed with successful result.

  • 경동맥 내중막두께(intima media thickness: IMT)는 2000년 미국심장학회에서 관상동맥 질환이나 허혈성 뇌질환 위험도의 독립적인 인자임을 인정하여 일차 진단지표로 권장함에 따라 세계적으로 그 관심이 확산되고 있다. 최근 국내에서도 서구화되어 가는 식생활 패턴으로 인해 고혈압, 동맥경화, 뇌졸중, 관상동맥 질환 등의 심뇌혈관계 질환 발병률이 높아지고 있다. 서구의 연구결과에 의하면 경동맥 내중막 두께는 인종, 성별, 연령에 따라 많은 차이를 보이기 때문에 임상적 진단에 기초가 되는 한국형 경동맥 내숭막두께 정상치 확립이 요구된다. 경동맥의 내중막두께는 경동맥 팽대부에서 근위부 10 mm 부위의 far wall 총경동맥 부위에서 심혈관계 위험 인자와 가장 상관성이 많은 것으로 보고되고 있다.

  • B-모드 초음파를 이용한 경동맥 내중막 두께(IMT intima-media thickness) 측정은 관상동맥 질환이나 허혈성 뇌질환 위험도의 독립적인 인자로 매우 중요하다. 본 연구에서는 NTSC(national television system committee) 방식으로 경동씩 초음파 동영상신호를 획득하여 프레임으로 나누어 각각의 IMT를 측정하였다. 각각의 측정값을 시간축으로 재구성하여 시간도메인에서 IMT 의 시변동성을 분석하였다. IMT 시변동성 분석을 통하여 상대적으로 부정맥피험자가 정상피험자에 비해 IMT 평균값이 작아 위험도가 낮다고 평가 될 수 있지만 시변동성 분석으로 IMT 평균값이 작아도 혈관의 기능적 특징이 부족하여 위험도가 높다는 것을 알 수 있었다. 경동맥의 형태적 특징과 기능적 특징을 반영하는 이 분석방법은 경동맥 내중막 두께측정의 신뢰성을 향상시키며 관련질환의 위험도와 중증도를 세분화 할 수 있는 새로운 방법이 될 수 있을 것이다.

2 CASES OF CAROTID ARTERY RUPTURE FOLLOWING TOTAL LARYNGECTOMY (후두전적출술 후에 발생한 경동맥 파열 2례)

  • 경동맥 파열은 악성 종양으로 인한 두경부 수술의 1-5%에서 발생하고 80%의 사망률과 생존자의 50%에서 신경학적 후유증을 나타낸다. 저자들은 수술전ㆍ후 방사선치료를 받은 환자 2례에서 경동맥파열을 경험하였다. 제 1례는 51세의 남자로서 후두암($T_4$NoMx)으로 후두전적출술과 6120 rad의 수술후 방사선 치료 후 좌측 악하 부위에 악성 종양의 경동맥 침윤으로 시험적 수술후 6일만에 경동맥 파열로 사망하였다. 제 2례는 51세의 남자로서 하인두암($T_3$$N_2$Mx)으로 7200 rad의 수술전 방사선치료후 악성 종양의 재발로 후두전적출술과 일측의 근치적 경부곽청술후 누공을 형성하여 수술후 14일만에 경동맥 파열로 사망하였다.

A Case of Foreign body in Parapharyngeal space (인두주위강 이물 1례)

  • Foreign bodies of the parapharyngeal space can cause severe complications such as descending suppurative mediastinitis, jugular thrombophlebitis, cavernous sinus thrombosis and carotid erosion. Therefore, early diagnosis and surgical intervention are needed to reduce morbidity and mortality. We present a case of a toothbrush as foreign body in the parapharyngeal space in 28-year-old male patient. The tooth brush was broken and the remnant of that was left in the patient's mouth. Under general anesthesia, intraoral approach was undertaken and successfully the toothbrush was removed, and then incision and drainage was done by transcervical approach. We report this case with review of literature.

Anterior Cranial Fossa Dural Arteriovenous Fistulae Presenting as Subdural Hematoma

  • Anterior cranial fossa dural arteriovenous fistulae (DAVFs) are very rare and the bleeding rate is very high, especially in the presence of leptomeningeal draining vein and aneurysmal varix formation. A 85-year-old male patient presented with subdural hematoma (SDH). Magnetic resonance image (MRI) and transfemoral carotid angiography (TFCA) disclosed DAVF at the anterior cranial fossa with bilateral arterial feeders and leptomeningeal draining vein with varix formation. The lesion was treated by simple ligation of pial connecting vein using low frontal craniotomy. In comparison with DAVFs of the other sites, the anterior cranial fossa DAVF is difficult to manage by endovascular treatment due to not only the difficulty of transvenous access but the risk of visual impairment when using transarterial route. Surgical ligation of pial connecting vein is feasible and effective treatment.

Brain Uptake and the Analgesic Effect of Oxytocin - its Usefulness as an Analgesic Agent

  • To establish the usefulness of oxytocin (OT) as an analgesic for women in delivery, the pharmacokinetic parameters and blood-brain barrier (BBB) permeability of [$^3H$] OT were obtained using an intravenous injection technique or the internal carotid artery perfusion/capillary depletion (ICAP/CDM) method. Brain uptake of OT was similar to that of sucrose, plasma space marker, indicating that OT has a poor BBB permeability. Moreover, the analgesic effects of OT injected through the jugular vein on nociception were evaluated by the tail-flick method. The antinociceptive effects of OT injected at a dose of 0.2 ${m}g/kg$or 2 ${m}g/kg$ were dose-dependent. In addition, the analgesic effects of OT on the CNS were unaffected by naloxone, a m-receptor antagonist. In a similar manner to the opioid system, OT may play a modulatory role in antinociception.
