  • 최근 노인관련 요양 문제는 이미 사회적 이슈로 떠오르면서 각 가정에서의 돌봄이 곤란한 어르신들이 요양원, 재가복지센터, 주간보호센터, 노인전문병원 등에서 요양보호사의 도움이 절대적으로 필요한 일상이 되었다. 무엇보다도 장기요양서비스를 이용하는 어르신 대부분은 노인성 질환 또는 치매를 앓고 있다. 이는 각 가정에서의 돌봄이 어려운 어르신들에게 직접서비스를 제공 하는 종사자인 요양보호사들은 다양한 치매어르신들의 문제행동을 안고 현장에서의 돌봄을 제공한다. 따라서, 요양보호사의 직무 관련 스트레스에 미치는 영향력 파악과 이와 더불어 직무스트레스가 치매어르신의 문제행동과 종사자의 소진 간의 매개 효과가 있는 지 규명하는 연구들이 활발하게 이루어 짐으로 이를 규명한 내용을 토대로 대안 마련을 위한 유익한 제언들이 필요하다.

Factors associated with healthcare utilization for infant falls in South Korea: a cross-sectional online survey

  • Purpose: Falls are a common cause of unintentional injuries in infants. This study was conducted to examine the patterns of healthcare utilization following infant falls in South Korea. Methods: This cross-sectional descriptive study utilized an online survey designed to gather information regarding the general characteristics of parents and infants, fall-related variables, and healthcare use. Results: The most serious falls identified by parents occurred at an average infant age of 6.97 months. Most fall incidents took place indoors (95.7%), and many occurred under the supervision of caregivers (68.0%). Following the fall, 36.4% of the participants used healthcare services. Logistic regression analysis revealed that healthcare use following an infant fall was significantly associated with being a firstborn child (odds ratio [OR]=5.32, 95% confidence interval [CI], 2.19-15.28) and falling from a caregiver's arms (OR=4.22; 95% CI, 1.45-13.68). Conclusion: To prevent and decrease the frequency of infant falls, improvements are needed in both the domestic environment and parenting approaches.

A Longitudinal Study on the Burdens of Caregivers in Families with Stroke Patients (뇌졸중 환자를 돌보는 가족간호자의 부담감에 대한 종적연구)

  • This longitudinal study examines the burdens and related factors for stroke patient's primary family caregivers. Sixty-one patients treated for stroke at a general hospital in Chung-nam province and family member caregivers participated in this study from July 28, 1998 to August 1, 1999. Family caregivers' burdens were assessed by the burden scale originally developed by Zarit(1980) and Novak & Geust(1989) and modified by Oh's(1993) for use in Korea. The burden scale instrument consists of five subscales.: financial burden, social burden, physical burden, dependency burden, and emotional burden. Repeated ANOVA and Stepwise multiple regression were used in the data analyses. The results were as follows: The burden mean score was 3.23 in the hospital, 3.26 after hospital release, and 3.27 in the home environment. In the hospital, a patient's degree of cognition, and social support for family caregivers were significant factors affecting the sense of burden felt by family caregivers. After hospital release, the significant factors affecting the sense of burden were the degree to which stroke patients could participate in daily living activities, social support for family caregivers, and changing to a second caregiver. In the home environment, the most significant factor affecting the sense of burden was social support for family caregivers.

Technical and Commercialization Status of Urine and Feces Disposal Systems (자동배설처리기 기술 및 상품화 현황)

  • Urine and/or feces disposal systems are expected to replace the diapers currently used for urine and feces disposal for the elderly with urinal and/or fecal incontinence. The are designed to detect the urine and/or feces, cleanse the excremental body organs with water transported from a cleansing water container in the main body, suction them into a fluid waste storage container for future disposal, and dry the area with hot air supplied also from the main body. These systems thus could relieve the chores of a caregiver and could also enhance the patient's hygiene. We reviewed in this paper the detection systems of urine and feces, the main components of the urine and feces disposal systems, and tried to classify the systems currently available in the market.

Factors Influencing the Burden Felt by Main Family Caregivers of Elderly Patients with Brain and Spinal Diseases (뇌.척추질환 노인 환자 주 가족수발자의 부담감에 영향을 미치는 요인)

  • Purpose: This study of this study was to identify factors influencing the burden of main family caregivers who take care of elderly patients with brain and spinal diseases. Methods: This was conducted as descriptive research and data were collected from 255 main family caregivers who were taking care of elderly patients with brain and spinal diseases from 4 hospitals in Daegu and Gyeongbuk Province. Stepwise-multiple regression was used to identify the influencing factors of burden felt. Results: As the score of burden felt by the main family, economic, social, physical, interdependent and emotional burdens were high in order. Factors influencing burden felt by main family care givers taking care of elderly patients with brain and spinal diseases were changed relation with patient after hospitalization, daily life ability, marital status, education and family caregiver's personality (explanatory power of 24.6%). Family caregivers felt a heavier burden when their relation with the patient was changed negatively or when the patient's activity of daily living was low. Conclusion: Based on these results, we need to develop coping measures and interventional programs for reducing the burden felt by the main family caregivers of elderly patients with brain and spinal diseases.

Comparison of Nurses's Fatigue, Sleep Quality and Interpersonal Relationship Stress between Comprehensive Nursing Care Service Wards and General Wards (간호·간병통합서비스병동과 일반병동 간호사의 피로, 수면의 질, 대인관계 스트레스 비교)

  • Purpose: The purpose of this study was to compare the differences in fatigue, sleep quality and interpersonal relationship stress levels between nurses in comprehensive nursing care wards and in general wards. Methods: Data were collected using self-reported structured questionnaire from 14 August to 17 September 2018. The participants were 220 nurses from 6 hospitals in Seoul and Gyeonggi-do. The data were analyzed using the IBM SPSS 23.0 program, descriptive statistics, t-test, ${\chi}^2-test$, Fisher's exact test, and Pearson's correlation coefficient. Results: The subjects' fatigue, sleep quality, and interpersonal relationship stress were not significantly different between the comprehensive nursing care services wards and the general wards. The interpersonal stress with a superior was statistically higher among general wards than comprehensive nursing care services wards. The relationship stress between patient and caregiver was higher in comprehensive nursing care services wards than in general wards. Conclusion: A follow-up study is needed to develop a strategic method to reduce interpersonal stress in nurses.

A Method for Detecting Movement and Posture During Sleep Using an Acceleration Sensor of a Wearable Device (웨어러블 단말의 가속도 센서를 이용한 수면 중 움직임 및 자세를 감지하는 방법)

  • The number of patients with many complications grows with the increase of aging population. As the elders and severely ill patients spend most of their time in bed, it leads to Pressure Injuries (PI) such as bedsores. Unfortunately, there is no method to automatically detect changes in patient's posture which leads to the need for a caregiver every set of times when the patient needs to be moved. Many studies are conducted to solve this inefficient problem. Yet, these studies require costly devices or use methods that disturb patient's sleeping environment. Those methods are mostly hard to implement in practice due to these reasons. We propose a method to detect posture using a three-axis acceleration sensor from the wrist band. We developed a wearable watch that measures sleep-related data. We analyzed 40 people's sleep data with a wearable module and watch to measure their postures such as supine, left-side, and right-side. Then, we compared the classified posture from the watch with the wearable module and achieved 90% accuracy. Therefore, we concluded that only by using the wearable watch, we can detect the sleeping position without any new equipment or system to diagnose the patients without discomfort during their daily lives.

Transfer anxiety in parents of children transferred from pediatric intensive care units to general wards in South Korea: a hybrid concept analysis

  • Purpose: This study aimed to analyze the concept of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Methods: The hybrid model by Schwarz-Barcott and Kim was used to analyze the characteristics of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Results: Transfer anxiety was defined by the following attributes: 1) stress concerning the adaptation process, 2) concern about the child's condition worsening due to the parent's caregiving, and 3) involuntary changes in daily life due to the treatment. Transfer anxiety has the following antecedents: 1) uncertainty; 2) a lack of knowledge about the illness, medical devices, and caregiving; and 3) a lack of social support. It resulted in 1) caregiver burden, 2) a decrease in the capacity for coping with caregiving, 3) delays in the child's physical and psychological recovery, and 4) decreased quality of life. Conclusion: It is necessary to develop an assessment scale that considers the attributes of transfer anxiety in parents of children transferred from pediatric intensive care units to general wards. Furthermore, an effective nursing intervention should be developed to reduce transfer anxiety.

A Qualitative Case Study on Choosing Elementary Schools for the Children of Highly Educated Married Immigrant Women from China (중국 출신 고학력 결혼이주여성들의 자녀 초등학교 선택에 관한 질적 사례연구)

  • This study aims to understand the experiences of highly educated married immigrant women from China, who hold a master's degree or higher, when selecting an elementary school for their children and to explore the significance of that experience. This study included eight participants, all of whom were married immigrant women who had resided in Korea for a minimum of five years. Data was collected through in-depth interviews, which were conducted over approximately one year, from June 2022 to August 2023. The study was approached as a case study. As a result of this study, the similarities between the cases were "choices based on charactercentered education and cultural convergence", "choices for the child's future and career", and "choices influenced by the primary caregiver's use of bilingualism". The differences observed in the cases appeared in "choices based on the balance between global education ideals and economic realities", "choices based on specialized curriculum content", and "choices based on connections to higher-level schools". This study is important, as it underscores the need to conduct various research initiatives on multinational immigrant women and their children. These findings suggest that the study can contribute to the development of multicultural policies that are grounded in the actual experiences and critical awareness of married Chinese immigrant women, who constitute a substantial part of Korean society.

Impact of Dental Treatment under General Anesthesia or Intravenous Sedation on Oral Health-related Quality of Life in Pediatric Dental Patients (전신마취 또는 정주진정 하 치과치료가 소아 환자의 구강 건강과 관련된 삶의 질에 미치는 영향)

  • The present study aimed to assess the perception of a child's primary caregiver on the oral health-related quality of life (OHRQoL) of dental treatment under general anesthesia (GA) or intravenous sedation (IV-SED) in pediatric dental patients. Self-administered questionnaires were completed before dental treatment under GA or IV-SED by 52 primary caregivers of healthy pediatric patients (10 years old or younger), and 43 (84%) of these caregivers completed the same questionnaires within 6 months after treatment. The Korean version of Child Oral Health Impact Profile (COHIP) and the Family Impact Scale (FIS) were used to assess OHRQoL in the questionnaires. The scores of COHIP and FIS improved after dental treatment. Demographic variables such as gender did not affect the improvement of COHIP and FIS, and age did not affect that of COHIP. However, baseline FIS score of younger patients was better than that of older patients before dental treatment. Regardless of the treatment variables, all COHIP and FIS scores were improved after dental treatment. COHIP score improved more in cases with pulp treatments compared to those without the treatments. On the other hand, FIS score improved less in cases with posterior stainless steel crown restorations compared to those without them. Anterior esthetic restorations and anterior teeth extractions did not affect the degree of improvement. Based on the primary caregiver's perceptions, the OHRQoL of healthy pediatric patients was improved by dental treatment under GA or IV-SED.
