한국과 독일 방문요양의 등급별 급여 비교 연구 (A Comparative Study on the Care Services According to Care Levels in the Korean and German Home Care System)

  • Purpose: This study aims to classify multiple forms of home care services from a service user's perspective. By comparing the German system to the Korean one, this study seeks to find better ways of enhancing client-centered care. Methods: The data of 121 home care users were collected from six home care centers that had under the management of nursing managers for more than five years. The researcher used a German instrument to estimate the level of care. Results: High correlation was found between German and Korean assessment criteria (r=0.81, p<.001). However, compared to their German counterparts, Korean home care providers allocated more time towards provision of domestic help per daily visit. German home care providers allocated more time towards services relating to provision of support for physical activities, including personal hygiene. It was confirmed that the level of care in Korea does not correlate with either the categories of home care services and duration. Conclusion: Therefore, care services should be offered according to long-term care needs. This study suggests a need for systematic care aimed at strengthening the client's autonomy which should be properly planned, implemented, and managed.

발반사요법 교육프로그램이 당뇨병 환자의 발관리에 미치는 효과 (The Effects of a Foot-Reflex-Massage Education Program on Foot Care in Diabetic Patients)

  • Purpose: This study was conducted to evaluate the effects of foot care education program using foot-reflexo-massage in diabetic patients. Method: A convenience sample of non-equivalent control group time series design was used. It provided foot care education to diabetic patients through small book for both group. For the experimental group, foot-reflexo-massage was taught by a researcher and research assistants. Analysis was done by Repeated Measured ANOVA. Result: There was significant increase in foot care knowledge, self care behavior, between the experimental group and the control group over three different times. There was significant in skin temperature and pulse of foot over three different times and interaction by groups or over time, but there was no significant difference between groups. There was no significant difference in blood flow volume and capillary filling time over three different times, between groups, but there was interaction by groups or over time. But there was significant difference in discrimination in change of dosalis pedis artery blood flow. Conclusion: Findings indicate that this study may contribute to develop nursing intervention for foot care of diabetic patients.

네일 샵의 서비스 품질 요인에 대한 평가 (Evaluation of Service Quality Factors on the Nailcare Shop)

  • The purpose of this study was to examine the differences in the service quality evaluation on the skill and expert, the shop facilities and atmosphere, the time and access, and the price and benefit of a nail care shop by age, income, spendings of appearance and a nail care, and a regular nail care. The survey was performed with questionnaires and the subjects were was 240 consumers who used the nail care. The data were analyzed by frequencies, F-test, T-test, Factor Analysis, one way ANOVA. The results of the study were as follows: Service quality evaluation were classified four factors- the price and benefits, the skill and expertness, the facilities and atmosphere, the time and access. There were no statistically significant differences in the price and benefits by age, income, spendings of appearance and a nail care, and a regular nail care, but there were statistically significant differences in the skill and expertness, the facilities and atmosphere, and the time and access. The service quality on the time and access was evaluated as high by the consumer in the 30s, with more income, and with high appearance and nail care spendings. The service quality on the shop facilities and atmosphere was evaluated as high by the consumers with high nail care spending. The consumers with a regular nail care evaluated the service quality on the skill and expertness, the time and access, the shop facilities and atmosphere as high.

노동인가, 돌봄인가, 여가인가? -전일제 임노동자 맞벌이 부모의 시간배분에 대한 젠더 영향과 학력의 조절효과- (Work, Care or Leisure? - The Effects of Gender and Education on Time Allocation of Full-time Working Parents)

  • 이 연구는 전일제 임노동자인 맞벌이 부모의 유급노동, 자녀돌봄, 여가에 대한 시간배분을 젠더와 학력 요인을 중심으로 살펴본 것이다. 연구 초점은 전일제로 일을 하는 부모의 일, 돌봄, 여가시간에 대한 배분이 전통적인 성역할 이념에 의해 영향을 받는지 여부와, 시간배분에 대한 젠더 효과가 고학력일 경우에 완화되는지 여부이다. 이에 유급노동시간, 자녀돌봄시간, 여가시간의 비중 각각에 대한 젠더 효과와, 이에 대한 학력의 조절효과 존재 여부를 살펴보았다. 분석 결과, 시간배분에 대한 젠더 효과와 학력의 조절효과에 대한 가설은 부분적으로 검증되었다. 우선 어머니임은 시간 배분에서 유급노동시간 비중을 줄이는 유의미한 요인이 아니지만, 돌봄시간 비중을 높이고, 여가시간 비중을 줄이는 유의미한 영향 요인이다. 즉, 젠더는 생산영역 시간인 노동시간 비중에는 유의미한 영향을 미치지 않지만, 가족시간인 돌봄시간, 개인적 시간인 여가시간 할당에는 유의미한 영향을 미친다. 한편 학력은 돌봄시간 비중과 여가시간 비중에 대한 젠더 영향에 조절효과를 갖는다. 고학력일수록 부모의 시간 배분에서 돌봄시간 비중의 젠더 격차는 감소한다. 고학력일수록 여가시간 비중의 젠더 격차는 오히려 커진다. 이에 학력 증가는 양성 모두의 자녀돌봄 참여에 대한 인식을 제고하여 돌봄시간 배분에 대한 젠더 격차를 줄일 수 있지만, 가부장적 성역할 인식을 전면적으로 변화시켜 부모의 일상을 일-삶 균형 제고의 방향으로 포괄적으로 변화시킬 것으로 기대하기 어렵다.

일 대학병원 호스피스 병동 입원 환자의 간호활동시간 측정과 원가산정 (Determination of Cost and Measurement of nursing Care Hours for Hospice Patients Hospitalized in one University Hospital)

  • This study was designed to determine the cost and measurement of nursing care hours for hospice patients hostpitalized in one university hospital. 314 inpatients in the hospice unit 11 nursing manpower were enrolled. Study was taken place in C University Hospital from 8th to 28th, Nov, 1999. Researcher and investigator did pilot study for selecting compatible hospice patient classification indicators. After modifying patient classification indicators and nursing care details for general ward, approved of content validity by specialist. Using hospice patient classification indicators and per 5 min continuing observation method, researcher and investigator recorded direct nursing care hours, indirect nursing care hours, and personnel time on hospice nursing care hours, and personnel time on hospice nursing care activities sheet. All of the patients were classified into Class I(mildly ill), Class II (moderately ill), Class III (acutely ill), and Class IV (critically ill) by patient classification system (PCS) which had been carefully developed to be suitable for the Korean hospice ward. And then the elements of the nursing care cost was investigated. Based on the data from an accounting section (Riccolo, 1988), nursing care hours per patient per day in each class and nursing care cost per patient per hour were multiplied. And then the mean of the nursing care cost per patient per day in each class was calculated. Using SAS, The number of patients in class and nursing activities in duty for nursing care hours were calculated the percent, the mean, the standard deviation respectively. According to the ANOVA and the $Scheff{\'{e}$ test, direct nursing care hours per patient per day for the each class were analyzed. The results of this study were summarized as follows : 1. Distribution of patient class : class IN(33.5%) was the largest class the rest were class II(26.1%) class III(22.6%), class I(17.8%). Nursing care requirements of the inpatients in hospice ward were greater than that of the inpatients in general ward. 2. Direct nursing care activities : Measurement ${\cdot}$ observation 41.7%, medication 16.6%, exercise ${\cdot}$ safety 12.5%, education ${\cdot}$ communication 7.2% etc. The mean hours of direct nursing care per patient per day per duty were needed ; 69.3 min for day duty, 64.7 min for evening duty, 88.2 min for night duty, 38.7 min for shift duty. The mean hours of direct nursing care of night duty was longer than that of the other duty. Direct nursing care hours per patient per day in each class were needed ; 3.1 hrs for class I, 3.9 hrs for class II, 4.7 hrs for class III, and 5.2 hrs for class IV. The mean hours of direct nursing care per patient per day without the PCS was 4.1 hours. The mean hours of direct nursing care per patient per day in class was increased significantly according to increasing nursing care requirements of the inpatients(F=49.04, p=.0001). The each class was significantly different(p<0.05). The mean hours of direct nursing care of several direct nursing care activities in each class were increased according to increasing nursing care requirements of the inpatients(p<0.05) ; class III and class IV for medication and education ${\cdot}$ communication, class I, class III and class IV for measurement ${\cdot}$ observation, class I, class II and class IV for elimination ${\cdot}$ irrigation, all of class for exercise ${\cdot}$ safety. 3. Indirect nursing care activities and personnel time : Recognization 24.2%, house keeping activity 22.7%, charting 17.2%, personnel time 11.8% etc. The mean hours of indirect nursing care and personnel time per nursing manpower was 4.7 hrs. The mean hours of indirect nursing care and personnel time per duty were 294.8 min for day duty, 212.3 min for evening duty, 387.9 min for night duty, 143.3 min for shift duty. The mean of indirect nursing care hours and personnel time of night duty was longer than that of the other duty. 4. The mean hours of indirect nursing care and personnel time per patient per day was 2.5 hrs. 5. The mean hours of nursing care per patient per day in each class were class I 5.6 hrs, class II 6.4 hrs, class III 7.2 hrs, class IV 7.7 hrs. 6. The elements of the nursing care cost were composed of 2,212 won for direct nursing care cost, 267 won for direct material cost and 307 won for indirect cost. Sum of the elements of the nursing care cost was 2,786 won. 7. The mean cost of the nursing care per patient per day in each class were 15,601.6 won for class I, 17,830.4 won for class II, 20,259.2 won for class III, 21,452.2 won for class IV. As above, using modified hospice patient classification indicators and nursing care activity details, many critical ill patients were hospitalized in the hospice unit and it reflected that the more nursing care requirements of the patients, the more direct nursing care hours. Emotional ${\cdot}$ spiritual care, pain ${\cdot}$ symptom control, terminal care, education ${\cdot}$ communication, narcotics management and delivery, attending funeral ceremony, the major nursing care activities, were also the independent hospice service. But it is not compensated by the present medical insurance system. Exercise ${\cdot}$ safety, elimination ${\cdot}$ irrigation needed more nursing care hours as equal to that of intensive care units. The present nursing management fee in the medical insurance system compensated only a part of nursing car service in hospice unit, which rewarded lower cost that that of nursing care.

Factors Associated with Use and Types of Multiple Concurrent Care and Education Arrangements in Early Childhood in South Korea

  • This paper explores factors associated with multiple concurrent care and education arrangements in early childhood in South Korea. It draws on a subsample from the Korean Longitudinal Survey of Women and Families. Results show that about one-fifth of the families utilized multiple arrangements for their first preschool child. The primary non-parental option in multiplicity was nurseries or kindergartens. Home care and education options such as home study materials were found to be most prevalent secondary non-parental option, followed by services at private institution as well as relative care. Children's age, care cost, non-parental care time and time constraints were found to be positively related to the incidence of multiplicity for the first preschool child while family income and cost constraints were negatively associated. As a secondary option in the multiplicity, services at private institutions increased with children's age, care cost and when the grandparents live far from the parents' house. Home care and education utilization was found more among mothers with low degree of time constraints. Utilization of relative support decreased with the children's age and meant lower care cost and increased with mother's employment, fathers' education, family income, cost constraints and when grandparents live nearby. This paper, based on the associated factors, suggests how issues of children enrichment and constraints might be related to the multiplicity.

  • 본 연구는 우리나라 어린이집에서의 다양한 유아를 위한 보육(장애아보육, 다문화가정 유아보육, 영아보육, 시간연장형보육)의 발전방안을 모색하는 데 그 목적을 두고 있다. 이를 위해 먼저 어린이집에서의 장애아 보육과 통합보육 및 다문화가정 보육과 영아 보육의 현황 및 정책과 이에 따른 연구동향에 대해 살펴본 후, 문제점 및 개선점을 제시하고, 발전방안을 제안하였다. 이러한 기초자료 제공 및 관련 논의를 통해 장애아 보육과 특수보육 등의 다양한 보육 대상자를 위한 포괄적 서비스 제공을 도모함으로써 보육의 질적 수준을 제고하는 데 기여하고자 한다.

생체신호를 통한 헬스케어 디바이스 기술 동향 연구 (A Study on the Trend of Healthcare Device Technology by Biometric Signal)

  • 최근 개인의 생체 데이터를 수집·활용하여 적시에 효과적인 예방과 치료를 제공하는 맞춤형 의료 및 서비스가 가능하며, 사용자의 헬스케어에 활용되고 있다. 실시간 건강관리 서비스로 개발하여 스마트폰, PC 태블릿 등을 활용하여 각 개인에게 정보를 제공·수집된 데이터를 통해 경보하고 관리자에게 분석 데이터 및 결과를 알려서 양방향 커뮤니케이션을 지원하며, 이에 따른 건강관리를 구성할 수 있는 시점에 직면하고 있다. 본 연구에서는 생체신호를 통한 헬스케어 디바이스와 실시간 건강관리 시스템을 포함하는 스마트 웨어러블 헬스케어 기술 동향을 분석하여 현재 시점에서의 기술개발 특허 출원에 대비하고자 한다. 스마트 웨어러블 헬스케어 기술 관련 특허는 연구개발 집중도가 비교적 높은 것으로 조사되었다. 분석 구간 전반에 걸쳐 한국은 실시간 헬스케어 시스템에 대한 특허 활동에 집중하고 있으며, 미국 및 유럽은 생체기술을 통한 헬스케어 디바이스에 대한 특허 활동을, 일본은 생체기술을 통한 헬스케어 디바이스 및 시스템 전반에 걸쳐 특허 활동을 활발하게 하는 것으로 나타났다. 생체기술을 통한 헬스케어 디바이스와 실시간 건강관리 시스템에 대한 특허는 최근 한국에서 시장을 주도하고 있으며, 해당 기술에 대한 특허 및 시장을 확보하고 있는 것으로 나타남에 따라 스마트 웨어러블 헬스케어 기술은 시장진입 장벽이 높은 분야로 판단된다. 향후 기술을 상업화시킬 계획이 있다면 제품의 형상, 제조방법 및 기타 관련 기술 시스템에 대한 특허권을 획득하여 특허 포트폴리오를 구축하는 것이 중요하다.

호스피스 병동에 입원한 말기 암환자의 통증에 영향을 미치는 요인 : 입원 경과 시점에 따른 분석 (Factors Influencing Pain with Terminally Ill Cancer Patients in Hospice Units)

  • The purpose of this study was to investigate the impact of depression, discomfort, spirituality, physical care, and opioid use on pain with terminally ill cancer patients residing in hospice units. The convenient sample of this study consisted of 41 terminally ill cancer patients at three hospice units in university affiliated hospitals. Patients were interviewed with structured questionnaires three times at predetermined intervals: admission to the hospice unit (Time 1), one week later (Time 2), and two weeks later (Time 3). The data was collected from January 1998 to January 1999 and was analyzed using ANOVA, Pearson correlation coefficient, and multivariate multiple regression. 1. The mean age of the participants was approximately 55 years old. In terms of diagnosis, lung cancer showed the highest frequency (19.5%), followed by stomach cancer and rectal cancer (17.1%). The motive of seeking hospice unit admission was control (72. 2%), followed by spiritual care (50%), and symptom relief (38.9%). 2. Regarding the type of pain felt, the highest pain frequency the participants experienced was deep pain (55%), followed by multiple pain (25%), intestinal pain (10%), then superficial (5%) and neurogenic pain (5%). For the level of pain measured by VAS, there was no significant difference among the three time points; Time 1 (5.04$\pm$2.21), Time 2 (4.82$\pm$2.58) and Time 3(4.73$\pm$2.51). 3. There was significant change seen in spirituality and physical care in each time interval. Namely, the longer the length of admission at the hospice unit, the higher the importance of spirituality (p=0.0001) and the more the physical care the participants received (p=0.01). The opioid use at the three time points showed the following frequencies : Time 1 (75.6%), Time 2 (85.4%) and Time 3 (75.6%). 4. Regarding factors influencing pain, the pain level was significantly affected by the depression level (p〈0.01) and the opioid use (p〈0.1). These results were the most significant at the two time points (Time 1 and Time 2). At Time 3 (two weeks later), the pain level was significantly affected by the depression level (p〈0.05) and the amount of physical care the participants received (p〈0.1). In conclusion, the terminally ill cancer patients had moderate pain, were generally depressed, and were treated with opioid analgesics. As approaching death, the patients received more physical care due to increased physical symptoms experienced and they had a higher perception of the importance of spirituality. Thus, health care professionals need to provide continuous care for each of them to die comfortably physically, psycho- logically, and spiritually.

Non-Value-added Activity 개선을 통한 간호업무 효율성 향상 (Case study on improvements in non-value-added nursing activities to increase the efficiency of nursing care)

  • Background: This study investigates improvements in non-value-added nursing activities in clinical work, in order to improve the efficiency and quality of nursing activities. Methods: The study was performed as a quality improvement project at a tertiary general hospital. The nursing activities that needed improvements were categories into admission care, discharge care, supply management, diagnostic work-up related activities, and others. The nursing time and frequency of non-value-added activities were compared across nine nursing units before and after implementation of the quality improvement program. Post-implementation patient and nurse satisfaction were subsequently analyzed. Result: Post-implementation, the time spent on non-value-added nursing activities was reduced and patients and nurses were satisfied with the improvements. Discussion: Reducing non-value-added activities in nursing can increase the work efficiency and ensure time for patient care, thus improving the quality of nursing care. For further study, accurate surveys on nursing activities based on nursing time are required.
