The improvement of long-term care service in Korea through the review of Australian aged care system (호주의 장기요양 시스템 고찰을 통한 우리나라 장기요양서비스 개선 방향)

  • Objectives: In order to cope with the quality and the substantiality issues in long-term care for the elderly, we should have a wider view of long-term system components based on the understanding of health care organizations, management services, support for care providers and beneficiaries, education of the workforce, and management of finance and resources. Methods: For resolving the issues raised and offering guidance in the area of long-term care, we reviewed 20 reports and documents of the government and government-related institutions using the Internet home pages of the Australian government and the related organizations in the health care sector. These organizations are undergoing a huge system reform to implement consumer-directed care since 2015, in the areas of service, resources, finances, organization, and management. Results and conclusions: The study outcomes can have some implications for the long-term care system in Korea based on the differences in the service components. The results can provide basic information for improving the long-term care service, and can have several other implications for long-term care in Korea.

  • The United States has a unique health care system, which is unlikely any other health care systems in the world. The major part of basic functional components of the system -financing, insurance, delivery, and payment- is in private hands. A market-oriented economy invites the participation of numerous private entities that are interested in carrying out the key functions of health systems. Due to this central feature, U.S.health care is not delivered through a network of interrelated components designed to work together coherently. For lack of standardization, the various components of the system fit together only loosely. The involvement of numerous players in the key functions leads to duplication, overlap, inadequacy, inconsistency, and waste, which add to the complexity and also make the system inefficient. Hence, cost containment remains an elusive goals. Moreover, the system falls short of delivering equitable services to all americans, though consumption of health care services is the largest in the world. On the other hand, United States leads the world in the latest and the best in medical technology, medical training, and research. It offers some of the most sophisticated institutions, products, and processes of health care delivery. This article discuss the characteristic features of the U.S. health care system. and its performance, trying to seek its implication on Korean health care system.

  • Health care conditions in Korea are gradually improving along with the economic and social development. However, the volume of disease is still great, especially in rural areas. This study attempts, therefore, to initiate a comprehensive proposal of rural health care delivery system. The proposal is constructed three parts, problem of health care system, medical cost, medical education system. The proposal consist of the following components: I. The health care system 1. health sub-center is required to be locate in "Myun" the basis administrative unit of local government for delivering primary health care. But, in the viewpoint of medical economics, the primary health care is operated cautiously. 2. Health center is desirable to provide health services in coordinating the health sub-center and other private health institution. 3. The secondary health care is performed in regional combination hospitals, and the attitude that doctors accomodate this system is required. II. The medical cost, Insurance In the expenditure of medical care, the method of a third person's payment is required absolutely. III. The medical education system. 1. The medical education system (process) is changed from the medical education to regional doctor education. 2 In the nurse education system. nursing technical high school is resurrected.

  • Recently, the Ministry of Health and Welfare has emphasized the necessity to build community care, and the interest of health care- long-term care-social welfare system continues. In order to examine the future vision of long-term care for elderly people, which can be a core system in establishing community models for elderly people in the future, we will implement aging in place as a national policy project. It is meaningful to analyze in depth the case of Japan's policy implementation through the amendment process of long-term care insurance system. The purpose of this study is to examine the concept definition, operating system, and major promotion process of the Integrated Care System in Japan and to suggest policy implications for the future Korea long-term care insurance system. As a result of reviewing the operation status and revision process of the long-term care insurance system for establishing the Integrated care system in Japan, it is necessary to clarify the basic principles and construction of community care system, diversification of residence type and upgrading work, establishment of linkage network of health care-long term care-social welfare, expansion of support for family and self-mind.

  • The demand for long-term care is continuously on the rise as number of elders among the population increases. Due to the rapid growing demand for long-term care in Korea, there have been discussions for the introduction of new long-term care system into Korean society. The purpose of this study in to analyze changes of elderly care facilities in Japan after the beginning of long-term care system in 2000. The functional and architectural changes of Japanese facilities were researched to be used as references for predicting changes in Korean facilities. In Japan, after the execution of long term care system, the alterative aspect of facilities is divided into some categories; in sum, the change of environment for long term care, the effort to spread specialized facilities in a whole community, and the tendency to complex a variety of function of facilities such as an facility for home care service, and the like.

  • It is widely known that patients' utilization pattern for medical care facilities and the patientflow are influenced by multi-factors, such as demographic characteristics, structural characteristics of society, socio-psychological characteristics(value, attitude, norms, culture, health behavior, etc.), economic characteristics(income, medical price, relative price, physician induced demand, etc.), geographical accessibility, systematic characteristics(health care delivery system, payment methods for physician fees, form of health care security, etc.), and characteristics of medical facilities(reliability, quality of medical care, convenience, kindness, tec.). This study was conducted to research the mechanism of patient-flow according to changes of health care system(implementation of national health insurance, health care referral system and regionalization of health care utilization, etc.) and characteristics of medical facilities(ownership of hospital, characteristics of medical services, non-medical characteristics, etc.). In this study, the fact could be ascertained that the patient-flow had been influenced by changes of health care system and characteristics of medical facilities.

  • This study is aimed both to define a conception of Health Care Systems and to suggest Desirable Reorganization Directions in Korea. The Desirable Reorganization Directions of Health Care System in Korea is as follows ; 1. The Health Care System of Free Market System has to reorganize step by step for the Directions of National Health System. 2. The Health Care System has to reorganize with local socite as the center of local community. Especially, Health Sub-Center should be reorganized to provide Compresensive Health Care, so that the Sub-Center consist at least 15 members of health workers including a chief of governmental office. 3. The Health Care System has to reorganize for the Directions responding problems of the Elder, New Medical Technology Development, and Health Information System.

  • The purpose of this thesis is to lay groundwork for the development of emergency care system in metropolitan area. It compares the performance and outcome of the current system with foreign counterparts and investigates the changing aspects of future medical environment. Emergency medical system can be divided into two parts of both pre-hospital care, which refers to the emergency measures taken before arriving at a hospital, and hospital care that is given within a hospital. Pre-hospital care includes on-the-spot expedients, information system and delivery system, whereas hospital care is related to the classification and specialization of medical care facilities. This research focuses on the evaluation of the performance of a rescue party, which is part of pre-historical care system. As a result, it provides valuable material for the development of the emergency medical system in Seoul.

Effects of the Child Care Accreditation System : Successful Experiences of Participants (평가인증에 참여한 보육시설 종사자들의 경험을 통해 본 보육시설 평가인증제도의 효과)

  • This study analyzed the influences of child care accreditation system on the participants' successful experiences. Analysis of 20 protocols made by the participants showed three dimensions of effects (1) reinforcement of their speciality within the field of childcare, (2) psychological satisfaction; e.g. self-confidence and self-fulfillment, and (3) positive perspectives on child care accreditation. Conclusions were that child care accreditation is an effective system as long as it helps the participants achieve successful experiences. These data will be useful for understanding and publicity of the recently established child care accreditation system. Future research on this subject is needed not only to activate childcare study but also to contribute towards acceptance of the new system.

  • Purpose: This comparative study analyzed information systems including manpower, contents of service, clients, and costs among four nations. Methods: A literature review of relevant publications from Korea, United States, Japan, and Germany supported the use of several plans to activate home nursing care services under the long-term care insurance system in Korea. Results: Korean home nursing care services require quality improvements. The results indicated that a rule is necessary that rations simple visiting service and home nursing care services under the long-term care insurance system, that an integrated management system for elderly care is required, and that the revised delivery of services needs to establish and reflect on various factors to estimate the value in a home visiting nursing care cost system. Conclusion: The data should be valuable in establishment of home nursing care services under the long-term insurance system in Korea.
