산재장해인의 장기요양서비스 요구 분석 (Long-Term Care Needs Assessment of the Disabled Workers After an Industrial Injury)

  • Purpose: The objective of this study was to evaluate long-term care needs using RAI MDS-HC and MI-CHOICE among the disabled workers. Methods: Data were obtained from 45 personal care recipients with the disability of mental and nervous system, and analyzed using SAS 9.1 by applying t-test, ${\chi}^2$ test, or fisher's exact test. Results: Only 'bed mobility' and 'indoor ambulation' items of ADL and problem activity were statistically significant factors by the level of personal care benefit. By MICHOICE grouping, 20.0 percent of subjects belonged to nursing home group, 51.5 percent were home care service, 28.9 percent were intermittent personal care. Conclusion: Personal care services in industrial accident compensation insurance have been categorized with two groups according to level of disability. But our results could contribute to provide personal care service according to the long term care needs.

  • Purpose: The purpose of this study was to investigate the needs for pediatric palliative care (PPC) among parents of children with complex chronic conditions (CCCs) and to investigate differences in the needs for PPC according to their general characteristics. Methods: A cross-sectional survey was conducted between March 28 and May 18, 2018. Parents (N=96) who had a child under 18 years with a CCC were recruited. Data were analyzed using descriptive statistics, independent t-test, and one-way ANOVA. Results: The overall average need for PPC was $3.58{\pm}0.33$ out of 4.00. In terms of care for the subjects' children, the highest need was physical care, followed by psychosocial and spiritual care. In the sub-dimensions, preservation of physical function received the highest score. Of the items, the highest need was for seizure control. In terms of care for the subjects themselves, the highest need was for psychosocial care, followed by bereavement and spiritual care. In the sub-dimensions, communication received the highest score. Of the items, the highest need was for smooth communication with medical staff. Differences in needs for PPC according to participants' general characteristics were not statistically significant. Conclusion: Medical staff should provide PPC according to the priorities of parents' perceived needs.

만성질환자의 자가 간호수행과 가정간호 요구에 관한 연구 (Study of home Nursing Core Needs and Implementation of Self Care of Chronically Ill Patients)

  • This study was done to gather the basic information needed to identify how the home nursing care needs and implementation of self care is being carried out for chronically ill patients. The subjects of the study were 294 from chronically ill patients in a general hospital, a university hospital and seven primary health care center and the data was collected by a nurses, public health nurse practioners using questionairs from July 30 to September 30, 1993. The data were analyzed using percentage, mean, and T-test, ANOVA. Our objectives were to understand basic nursing information general characteristics, implementation of self care, home nursing care needs, implementation of self care depend on general characteristics. The results of the study were as follows 1) General characteristics of subjects. The majority of subjects are female (54.8%) 66.7% of residence are fishing and agrarian villages. 20.1% of disease are neurologic system (backache, neuralgia, HIVD, C.V.A). 2) Evnironmental offord reveals high point in implementation of self care.($2.76{\pm}1.37$) 3) B.P check reveals high peroentage in home nursing care needs.(84.7%) 4) Implementation of self care depend on general characteristics reveals significantly different by $sex^{*}$, educational $level^{**}$, monthly $income^{**}$, number of $family^*$,{\;}$disease^*$, and reason of $untreatment^{**}(^*<0.05,{\;}^{**}<0.01)$. In conclusion the study requires efforts of nurse practitioners, and the support of useful resouress by government.

개심수술후(Open-Heart Surgery) 환자의 교육 요구에 관한 간호원과 환자와의 비교연구 (A Comparison Study of Educational Needs Following Open-Heart Surgery Assessed by Nurses and Patients Themselves)

  • The purpose of this study was to compare educational needs following open heart surgery assessed by nurses & patients themselves, Data were collected from Nov. 1983 to Jan. 1984 through administration of Questionnaire developed for this study by the researcher. The subjects were 45 patients who experienced open heart surgery and 29 nurses who taking care of these patients in chest surgery ward at Seoul National University Hospital. The Data were analyzed by T-test and One Way ANOVA according to dependent variables. The result were as follows 1. There are significant difference between educational needs, assessed by nurses and patients themselves. (p<0.05) The nurses reported higher score of educational needs than patients. 2. In comparison of content area of educational needs, there was significant difference according to respiratory care, fain, chest tube care and operation including heart function between two groups. But, there was no significant difference in diet, activity, complication, emotional care and medication between two groups. 3. The priorities in content area of educational needs rated by nurses and patients were similar.

  • Purpose: The purpose of study was to identify the death orientation and cognition, attitude and nursing needs in hospice care according to training of volunteers. Method: Data were collected using a structured questionnaire which included characteristics, death orientation and cognition, attitude and nursing needs in hospice care. Data collection was done between June 1st and 20th, 2003. The participants in the study were the 86 trained hospice volunteers and 76 non-trained hospice volunteers. The data were analyzed using the SPSS/PC+ program. Result: The results are as follows: There was a significant difference in the average for the death orientation scores between non-trained hospice volunteers and trained hospice volunteers. There was a significant difference for the cognition of hospice and attitude, with higher responses being given by the hospice trained group. For the domains of nursing needs in hospice care, the highest response was to emotional needs. Conclusion: This study showed that training in hospice care was effective in changing the death orientation, cognition, attitude and nursing needs in hospice care.

근로자의 고용상태가 미충족 의료에 미치는 영향 (The Effect of Employment Status on the Unmet Needs of Medical Utilization in Workers)

  • Objectives: This study was designed to evaluate whether employment status is associated with the experience of unmet care needs. Methods: This study utilized the Korea Health Panel data from 2012 to 2016. A total of 4,083 workers were selected as baseline subjects in 2012 and were followed for four years. This study used the GLIMMIX procedure under the marginal model while adjusting for covariates. Results: A total of 12.4% of 4,083 people said they had failed at least once to have a treatment or checkup despite the needs. Those more likely to experience unmet care needs were women and people of lower income level, with worse health conditions and chronic disease. Precarious workers, the self-employed, and the unemployed were more likely to experience unmet care needs caused by economic burden than permanent workers (Odds Ratio: 2.14, 2.07, 2.74, respectively). Conclusions: This disparity means that precarious workers and the unemployed are more likely to face barriers in obtaining needed health services. Given their insecure employment status, meeting their needs for health care is an important consideration.

  • Purpose: This study aimed to investigate the self-care education experience, self-care performance, and home care service needs of patients with rare and incurable diseases who use a home ventilator. Methods: From2 ventilator rental companies and 5 hospitals, records of 162 subjects were collected to obtain data regarding self-care education experience (23 items) and home care service needs (25 items). Results: Of the subjects, 65.4% were male and 55.6% had a myotrophic lateral sclerosis. The items with the highest and lowest levels of self-care education experience were "using the ventilator"(94.0%) and "community-available resource information"(27.0%), respectively. Meanwhile, self-care was performed well in terms of "personal hygiene"(59.6%) but not performed well in "community-available resource information"(23.7%). "Health status assessment" had the highest need(88.2%) of home care services. Generally, the need of home care services was higher for the patients using invasive ventilators than that for those using non-invasive ventilators. Conclusion: Home care nurses need to not only provide physical care for the patients but also strengthen their roles of enhancing patient access to and utilization of various community resources to provide systematic and individualized training and care.

암환아 부모의 호스피스 간호에 대한 인식과 요구 (Cognition'and Needs for Hospice Core among Parents of with Cancer)

  • Purpose: This study was done to investigate the cognition and needs for hospice care among.parentsofchildrenwith cancer. Methods: The participants were 73 parents of children with cancer. Data were collected through self-report questionnaires and analyzed using the SPSS/WIN Program. Results: Less than half of parents (49.3%) told the child about the disease. If the child could not be treated medically, 39.5% of the parents answered that they would have the child treated in a hospital until his/her last days, while 62.8% of the parents replied that it would be. appropriate for the child to get hospice care when all medical treatments for the child failed, or when the end of the child's life was near. Needs for hospice care for the parents were high, and the physical care of the child ranked as the most important. Conclusion: The above findings indicate that the parents were not cognitive enough about hospice care, but needed hospice care,especially as it is related to the physical care of the children. Therefore hospice care, based on cognition and needs of parents, should be provided for children and their families.

가정간호 교육요구도 조사 연구 (Education Needs for Home Care Nurse)

  • In 1990 Home Care Education Programs started when legislation established certification for Home Care Nurses. The Ministry of Health and Welfare proposed a home care education curriculum which has 352 class hours and 248 hours of 'family nursing and practice'. Though Home Care Education Programs have been offered in 11 home care educational institutes, there has been no formal revision for the home care education programs. Also a first and second home care demonstration projects have been carried out, but there has been no research on outcomes for home care education as applied in home care practice. The purposes of this study were to identify the important content areas for home care nursing as perceived by home care nurses, and to identify their clinical competence in each of these areas, and from these to identify the education needs. The sample was 107 home care nurses who were working in home care demonstration hospitals and community-based institutions which have been offering home care services. Responses were received from 88 nurses, comprising a 82.2% return rate, and 86 were included in the final analysis. The instrument used was a modification of the instrument developed by Caie-Lawrence et(1995) and Moon's(1991) instrument on home care knowledge. The instrument's Cronbach's coefficient was 0.982. Among the respondents, 64% were working at home care demonstration hospitals and 36% were working at community-based institutions. Their home care experiences were from one month to six years, with a mean of 20.6 months. The importance rating for home care education content was 3.42 0.325, which means importance was rated relatively high. Technical aspects of home care were identified the most important. Five items 'education skill', 'counseling skill', 'interview skill', 'wound care skill', 'bed sore care skill' received 100% importance ratings. The competency rating was 2.87 0.367 and 'technical aspects of home care' was the highest, and 'application to home care skill' was the lowest. Home care nurses' education needs were identified and compared to the importance ratings and competency ratings. Eleven items were identified as the highest in the importance areas and eleven items were in the lowest competency areas. High importance ratings matched with low competency ratings determined training needs, but there was no matching items in this study. In the lowest competency areas four items were excluded, because of not being applicable in current home care practice. Therefore total eighteen items were identified as home care education needs. These items are 'bed sore care skill', 'malpractice', 'wound care skill', 'general infection control', 'change and management of tracheostomy tubes', 'CVA patient care', 'Hospice care', 'pain management', 'urinary catheterization and management', 'L-tube insertion and managements', 'Respirator use and management skill', 'infant care', 'prevention to burnout', 'child assessment', 'CAPD', 'infant assessment', 'computer literacy', and 'psychiatry patient care'.

정상 신생아 어머니와 고위험 신생아 어머니의 신생아에 대한 지각 및 교육요구도 비교 (The Educational Needs and Perception of the Mothers of High Risk Infant and Normal Neonate)

  • The purpose of this study is to compare the educational needs and perception of mothers of normal neonates and high risk infants. This research was designed as a descriptive study. Data were collected for two months from April 2002 to March 2002. Subjects were 41 mothers of high risk infants and 60 mothers of normal neonates in one general hospital in Seoul participated in the study. Measurement tools used in this study were the educational needs scale developed by Cho Kyoul Ja et al and the neonatal perceptive inventories scale developed by Broussard. They ask mothers to rate each item on a four point Likert type scale. The collected data were analyzed using SPSS 8.0 program. The variables were listed as frequency, mean, standard deviation, X2 test, t-test, ANOVA. The results were as follows: The educational needs of the mothers of primi pregnancy was higher than the mothers of multi pregnancy. The educational needs on management of diseases such as prevention of infection, symptoms of disease, mental development, attachment promotion, congenital metabolism test, management of convulsion, care of vomiting and fever were higher than general care of infants such as immunization, measurement of temperature, hiccough care, follow up care. The informations must be included in nursing intervention program to reduce the mothers' stress level. In conclusion, in order to promote positive mother infant relationship, nurse need to give information and educate the mothers of high risk infant and normal neonate.
