A Preliminary Study on Mobile Medical Unit for Emergency Medical System (응급 의료체계용 이동 의료단위에 대한 기초연구)

  • Recently, the disaster situation has been diversified and compounded worldwide as well as in Korea. And the damage scale of each disasters have increased, and following economic loss is also increasing immensely. In Korea, especially the out break frequency of large-scale disasters has increased. Disasters caused by earthquakes are also very likely to occur. When a major accident such as Mauna resort collapse accident, Ferry sewol sinking accident occurred, systematic emergency medical care at the site at the time of disaster occurred in order to operate an emergency medical facility corresponding to the site, which is not systematic. The need for this service is urgent. If highly reliable emergency medical system function at appropriate times in the disaster site, secondary damage can be reduced and the whole situation would be back to normal shortly. Therefore, it is necessary to provide an emergency medical system capable of quickly and safely responding to disaster sites in relation to various damage scale. For the purpose, thorough analysis on mobile medical units of inside and outside the country was accomplished. As a result, Mobile Medical Unit of outside the country are trailer type, tent type, container type etc. At home, these types have been studied and will be proposed early next year as a temporary solution. Here we introduce suitable types for our country.

A Study on Influence Factors of Mobile Healthcare Service Using Structural Equation Modeling (구조방정식을 이용한 모바일 헬스케어 서비스에 대한 사용의도 영향요인 연구)

  • The purpose of this study is to investigate the factors influencing the intention to use mobile healthcare services based on smartphones. Data collection was conducted from March 10, 2014 to April 8, 2005. The collected data were analyzed by SPSS WIN 23.0 and AMOS 18.0 using Path analysis and Structural equation modeling analysis. The results showed that service quality and innovativeness, which are external variables, had a statistically significant effect on perceived usefulness, and these two factors had a positive effect on the intention to use mobile healthcare services. Usefulness also has a significant effect on perceived usefulness, and content characteristics and cost rationality have a significant effect on usability. The usefulness of the service also directly affects the intention to use mobile health care services, and various factors affect their effective use. In response to the recent rise in medical expenses, mobile healthcare using smartphones has emerged and there is a need to develop awareness of the various attempts by companies to develop such apps. The government should also make effort to improve accessibility to healthcare services by introducing suitable policies. It is expected that future studies will be continuously conducted to confirm the development of differentiated services for mobile healthcare subjects and their intention to use them.

  • Adolescent health is considered to contribute to health promotion in the home and community in the near future as well as individual health. However, adolescent health has been neglected from education field because of competitive school education focused on the university entrance examination That's why I suggest in this study that we should pay much more attention to adolescent health condition and try to make It better, in terms of man-power development and life-health promotion for nation development The purposes of this study are as follows First, to look into a variety of adolescent health problems Second, to find out the situation and problems of current adolescent health promotion and school health education Third, to make an effort to find, based on the current situation, various developments of adolescent health promotion and school health education in terms of practical, political and environmental change There are used study methods as adolescent-related, school heath-related literature review and anlysis of statistical data The results and suggestions are as follows Teenagers have a great variey of health problems including most Important physical, mental and social developments Recently, chronic diseases, emotional problems, health-risk behaviors linked With adolescents are on increase The complicated disorders of physical, mental, social health rather than paricular aspects of health or health-behavior problems influence adolescent health problems adolescence is regarded as the period when most health-related behaviors are formed. Therefore, adolescent health promotion would he assured by developing the ability of controlling multi-dimensional health determinants in the early stage. Health promotion is a positive concept that each individual, family and community makes real efforts to improve their health To achive this, we need health educational, organizational, political and environmental supports. Adolescent health promotion in Korea has been systematically treated in the category of school health Current school health services have had lots of systemic, constitutional, administrative and educational flaws Accordingly, I'm concerned that we can afford to accept a variety of adolescent health needs However, I would say that if were not to make those flaws better, it's certain that Korean national competitive power and the quality of the lives of most Koreans Will he threatened someday We have to develop Comprehensive School Health Crriculum(CSHC) and set up its standards to Improve adolescent health. CSHC is an organizational and costant process. CSHC means an Important part of overall curriculums. In addition, I could say that it's an Important school health education acivity including current school health services-health care service and school health environment. In conclusion, in order to develop CSHC, we require school nurse's role changes, establishment and management of intergrated subject of health education, striking revision of school health law(or legislation of school health promotion law), reorganization of administration system, big changes in curriculum for school health educators.

The Perceptions and Purchase Intentions of Health Food Consumers (건강기능식품에 대한 인식 및 구매의도 연구)

  • We investigated consumer perceptions of and purchase intentions for health foods. Data were collected from 454 adults over the age of 20 years living in Seoul and Gyeonggi province, from May 10 to June 5, 2010. Those that "hardly ever drink" (41.4%) were most prevalent in terms of drinking activity, whereas 80.8% of respondents did not smoke. Also, those who responded "hardly ever exercise" ranked highest; although 43.8% in fact exercised frequently. Of all respondents, 44.5% admitted to suffering slightly from stress. A total of 59.5% of respondents opined "I am healthy but I do worry about health", and "exercise" topped the list of approved (37.2% of respondents) health care methods. We also found that 83.5% of respondents claimed to have tried health foods, whereas in the case of having no taking experience, 60% had not purchased such foods because, in their view, this was unnecessary. The extent of concerns about health foods scored 3.09, and the level of purchase intentions for health food was high, with a score of 3.40. Therefore, all of government, producers, distributors, and academic researchers must provide consumers with accurate and complete information, and need to collaborate in the development of consumer education programs on health foods. This will enhance consumer interest in such foods, and empower logical choices.

Functional outcome predictors following mandibular reconstruction with osteocutaneous fibula free flaps: correlating early postoperative videofluoroscopic swallow studies with long-term clinical results

  • Background: Advancements in the field of microvascular surgery and the widespread adoption of microvascular surgical techniques have made the use of osteocutaneous fibula free flaps the standard of care in the surgical management of segmental mandibular defects. Although the literature possesses abundant evidence to support the effectiveness of fibula free flaps as a reconstructive method, there are relatively few studies reporting on outcomes as objectively measured by videofluoroscopic swallowing studies (VFSS). The purpose of this study is to explore the potential correlation between early postoperative VFSS and the long-term swallowing outcomes in patients who underwent mandibular reconstruction with fibula free flaps. Methods: We performed a retrospective chart review of 36 patients who underwent mandibular reconstruction with osteocutaneous fibular free flaps between 2009 and 2012. Demographics, clinical variables, VFSS data, and diet information were retrieved. Penetration and aspiration findings on VFSS, long-term oral feeding ability, and the need for gastrostomy tube were statistical endpoints correlated with postoperative clinical outcomes. Results: Thirty-six patients were reviewed (15 females and 21 males) with a mean age of 54 years (7-81). Seventeen cases were treated for malignancy. The size of the bony defect ranged from 3 to 15 cm (mean = 9 cm). The cutaneous paddle, a surrogate for soft tissue defect, ranged from 10 to 125 ㎠ (mean = 52 ㎠). A gastrostomy tube was present in patients preoperatively (n = 8), and postoperatively (n = 14). Seventeen patients had neoadjuvant exposure to radiation. Postoperative VFSS showed penetration in 13 cases (36%) and aspiration in seven (19%). Overall, 29 patients (80.6%) achieved unrestricted diet, and this was statistically correlated with age (p = 0.037), radiation therapy (p = 0.002), and preoperative gastrostomy tube (p = 0.03). The presence of penetration or aspiration on VFSS was a strong predictor for long-term unrestricted oral diet (p < 0.001). Conclusion: Early postoperative VFSS is an excellent predictor for long-term swallowing outcomes in patients undergoing mandibular reconstruction with osteocutaneous fibula free flaps.

Psychometric Validation of the Bahasa Malaysia Version of the EORTC QLQ-CR29

  • Background: This study examined the psychometric properties of the Bahasa Malaysia (BM) version of the European Organization for Research and Treatment of Cancer (EORTC) Colorectal Cancer-specific Quality Of Life Questionnaire (QLQ-CR29). Materials and Methods: We studied 93 patients recruited from University Malaya and Universiti Kebangsaan Medical Centers, Kuala Lumpur, Malaysia using a self-administered method. Tools included QLQ-C30, QLQ-CR29 and Karnofsky Performance Scales (KPS). Statistical analyses included Cronbach's alpha, test-retest correlations, multi-traits scaling and known-groups comparisons. A p vaue ${\leq}0.05$ was considered significant. Results: The internal consistency coefficients for body image, urinary frequency, blood and mucus and stool frequency scales were acceptable (Cronbach's alpha ${\alpha}{\geq}0.65$). However, the coefficients were low for the blood and mucus and stool frequency scales in patients with a stoma bag (${\alpha}=0.46$). Test-retest correlation coefficients were moderate to high (range: r = 0.51 to 1.00) for most of the scales except anxiety, urinary frequency, buttock pain, hair loss, stoma care related problems, and dyspareunia (r ${\leq}0.49$). Convergent and discriminant validities were achieved in all scales. Patients with a stoma reported significantly higher symptoms of blood and mucus in the stool, flatulence, faecal incontinence, sore skin, and embarrassment due to the frequent need to change the stoma bag (p < 0.05) compared to patients without stoma. None of the scales distinguished between patients based on the KPS scores. There were no overlaps between scales in the QLQ-C30 and QLQ-CR29 (r < 0.40). Conclusions: the BM version of the QLQ-CR29 indicated acceptable psychometric properties in most of the scales similar to original validation study. This questionnaire could be used to complement the QLQ-C30 in assessing HRQOL among BM speaking population with colorectal cancer.

Cancer Screening Knowledge and Attitudes of Under- and Post-Graduate Students at Kasr Al Ainy School of Medicine, Cairo University, Egypt

  • Background: Increasing knowledge and awareness of cancer screening significantly influence health promotion behavior which could markedly reduce incidence rates. In many countries, health care providers are the principal source of information concerning cancer screening. This study was carried out to assess the level of knowledge concerning cancer screening among medical students, house officers and residents and to explore their attitude towards cancer screening practices. Materials and Methods: This cross-sectional study was conducted in Kasr Al Ainy Medical School at Cairo University in Egypt, with 300 undergraduate medical students and 150 postgraduates (interns and residents) enrolled. A pre-tested self-administered questionnaire was used to collect data from the study participants regarding personal and education-related information, knowledge about cancer screening and its sources, and attitude towards cancer screening. Results: More than 64% of participants had knowledge scores of ${\leq}10$ points (out of 24). The total knowledge score (out of 6 points) for breast cancer screening increased from $1.9{\pm}1.0$ to $2.3{\pm}1.2$ and $2.4{\pm}1.1$ for $4^{th}$, $5^{th}$ and $6^{th}$ year respectively, interns showed the highest score of $2.6{\pm}1.1$, P= 0.001. Year of enrollment at medical school was a significant positive predictor of acquiring knowledge about cancer screening (post graduate vs. undergraduate students) (OR= 1.30, C.I =1.01-1.63), lack of or none receiving of orientation/training about cancer screening was the sole negative significant predictor for proper knowledge about cancer screening (OR=0.50, C.I=0.31-0.82). Over 92% of students agreed that they had insufficient knowledge about cancer screening, 88.2% appraised the need to have enough knowledge in order to direct/advice patients, relatives and friends, and 93.7% required that the faculty should emphasize the importance of cancer screening in the delivered curricula at medical school. Conclusions: A relatively low to moderate level of knowledge about cancer screening was detected among the selected medical students regardless of their year of enrollment at medical school or their graduation status, which may implicate a negative impact on early cancer detection especially in a low resource country like Egypt.

Effects of Anger Expression on Self-Esteem in Dental Hygiene Students (치위생전공 학생의 분노표현이 자아존중감에 미치는 영향)

  • The purpose of this study was to examine the relationship between anger expression and self-esteem of female college students of dental hygiene. Participants were 598 female students who majored in dental hygiene from randomly selected colleges, located in Daejeon and Chungcheongbuk-do. Data were gathered from May 20 to June 5, 2014, using structured questionnaires. The major findings of the study were as follows: the correlation between anger expression and self-esteem was analyzed, and self-esteem was found to have a weak significant negative correlation with anger expression. On analyzing data to explore which variables affected self-esteem, it was found that self-esteem was influenced by grade, school record, harassment, language psychological violence, and bullying. The above-mentioned findings suggest that anger expression is related to self-esteem. After graduation from college, dental hygiene students, encounter a variety of interpersonal relationships in their work. Therefore self-esteem programs need to be developed and implemented at an individual, departmental, and collegiate level to help students learn to respect themselves and others, and to provide appropriate care.

A Study on Introduction and Application of Preceptor System in Clinical Practice Process of Dental Hygiene Students (치위생과 학생들의 임상 실습과정에서 프리셉터(가칭: 임상실무지도자)의 도입과 활용에 관한 연구)

  • The realistic problem about the dental hygiene education in our country is what the correlation between theoretical education and clinical education is short, and what the systematically clinical-practice education is failing to be realized. In order to solve this problem, the aim is to introduce preceptor and to present necessity in order to apply preceptor to the practice education for dental hygiene department. 1. What is the largest in the difference between the observation experience and the performance experience by element in clinical-performance activity is the patience-care service sphere(P < .001). 2. A hospital with the highest observation-practice experience level in children with the observation experience by hospital was in order of K hospital, Y hospital, H hospital, B hospital, and A hospital(P < .001, P < .01). 3. A hospital was the highest in the difference of the performance-practice experience level by hospital. And, the next was in order of H hospital, Y hospital, K hospital, and B hospital(P < .001, P < .01). 4. As for recognition on importance of clinical-practice activity in dental hygienists, the dental hygienists with more than the third year were indicated to be the highest than the dental hygienists with less than the 3rd year among 58 dental hygienists. Given seeing the results of this study, the more systematic and educational clinical-practice environment will need to be improved by suggesting a role of preceptor and to seek for a plan for application in the clinical-practice education.

A Study on the Introduction of Electronic Commerce for Purchasing Section in Hospitals - Focused on the Understanding of Purchasing Managers - (병원구매업무에 있어서의 전자상거래 도입에 관한 연구 - 구매부서 관리자의 인식도를 중심으로 -)

  • This study is focused that the electronic commerce(EC) on the purchasing section may improve the efficiency and transparency of the hospitals management. After reviewing the purchasing activity of hospitals, I study the introduction, expected effects, and problems of EC. So, I am going to provide basic information for activating EC. The samples are managers of 170 hospitals, which are located on Seoul. As a result of collection this survey, I analyze 79 hospitals. For data analysis, I use $X^2$-test and ANOVA for purchasing management and the relevance of EC according to the level of care. The results of this study are 1. The problems on the management of purchasing section are: firstly, they don't have sufficient time to study market. Secondly, it is difficult to find competitive suppliers. And, lastly, they cannot gather a lot of information about the price of products. 2. There are many answers of the needs on the introduction of B2B. However, some hospitals think they don't need it. But, the most answers are that the EC will be settled within 4 years. So, we can realize that these hospitals are getting interested on the EC. On the other hand, I find that they prefer outside EC companies for the introduction of EC. 3. On the expected effects on EC, first is the effectiveness of the market survey. The next is to collect information of adequate price of products owing to clear transaction, find easier new suppliers and gather useful data. 4. On the external problems of the introduction of EC, there is low credibility related to the security and the weakness of suppliers' information system. Especially, on the Real Transaction Price Payment system, the bigger bed size, the higher understanding on these problems. On the internal problems of the introduction of EC, first is the burden of the introduction of EC and operating cost. Especially, on the burden of the disclosure of revenue source, the smaller bed size, the higher understanding on this problem So, this is a point which deserves my attention statistically. However, this shows relatively little understanding about incomplete the standard of product category and the weak information system of hospital. Through this study, I am going to suggest 3 points for the activation of the introduction of EC on hospitals. 1. The reform of the Real Transaction Price Payment System on medical supplies and materials for medical treatment 2. The establishment of the standard of product category 3. The promotion of information system based on network.
