Castleman's disease is a rare disorder characterized by benign tumors that may develop in the lymph node tissue throughout the body. Castleman's disease associated with myasthenia gravis is an especially rare disease. Only less than 10 cases have been reported in the world literature. The cause of Castleman's disease is associated with immune mediated reaction, and myasthenia gravis also develops due to an antibody-mediated process. The cause of myasthenia gravis is the immune activity of Castleman's disease, which may be the promoter of the antibody-mediated process. We report here a case of Castleman's disease, which was incidentally found in a patient diagnosed with myasthenia gravis.
Harlequin syndrome is a rare disorder of the sympathetic nervous system characterized by unilateral facial flushing and sweating. Although its etiology is unknown, this syndrome appears to be a dysfunction of the autonomic nervous system. To the best of our knowledge, thus far, very few reports on perioperative Harlequin syndrome after thoracic surgery have been published in the thoracic surgical literature. Here, we present the case of a 6-year-old patient who developed this unusual syndrome following the resection of a posterior mediastinal mass.
Aortic thrombi are important because it can cause the central and peripheral embolizations. Aortic thrombi can occur anywhere in the aorta but extremely rare in ascending aorta without atherosclerosis, aneurysm, cardiosurgical or traumatic state. Systemic sclerosis (SSc) is an autoimmune disorder of connective tissue and it can involve multisystem. Enhanced coagulation pathways, decreased fibrinolysis, and endothelial dysfunction probably contribute to vascular events in SSc. We report a case of a highly mobile thrombus in the ascending aorta, presented as an acute embolic stroke in the patient with systemic sclerosis. Surgical removal was performed to prevent recurrent embolic events.
점막하 식도 박리는 드문 질환으로 식도 천공 없이 점막하 출혈 및 혈종에 의해 점막하층과 근육층이 박리되는 상태를 의미하며, Mallory-Weiss, Boerhaave's syndrome과 함께 제3의 급성 식도 손상질환이다. 금식과 수액 요법 등의 보존적 치료만으로도 비교적 예후가 좋다. 하지만 출혈 소인이 있는 질환이나 정상적인 지혈에 영향을 미치는 약제(anticoagulants, anti-platelets, thrombolytic agents)를 복용중인 환자에서, 흉통 및 연하통 혹은 연하곤란 등을 호소하는 경우에 감별 진단으로 염두에 두어야 부적절한 치료를 피할 수 있다. 본 증례는 항혈소판 제재를 복용 중에 발생한 점막하 식도 박리를 보고하는 바이다.
기분장애에서 과체중과 비만의 문제가 일반인구보다 더 심각하고, 심혈관 치명률의 증가와 무관하지 않기 때문에 비만에 대한 치료가 필수적이다. 일반적인 상황에서 비만에 대한 치료와 동일하게 생활습관교정과 같은 비약물치료가 선행되어야 한다. 이 같은 시도가 실패하는 경우에는 약물치료가 필요하다. 기분장애의 비만치료로 공식 승인된 약은 없다. 따라서 일반인구의 비만에 승인된 약을 처방하거나 여러 연구를 통해서 효과가 입증된 약을 처방하게 된다. 여러 치료 지침과 연구 등은 orlistat, metformin, topiramate와 bupropion 등이 효과적임을 지지한다.
위식도역류질환은 서구에선 매우 유병률이 높으며 우리나라에서도 식생활의 서구화로 인해 유병률이 증가하고 있다. 최근 최소침습수술이 발달함에 따라 복강경을 이용하여 수술할 경우 미용효과, 단기재원기간 및 일상생활로의 조기복귀 등의 장점이 있어 장기적인 약물치료보다는 최소침습법에 의한 수술의 시행이 늘어날 것으로 예상된다. 저자들은 복강경을 이용하여 횡격막 탈장 및 탈장을 동반한 위식도역류성 질환에 대해 시행한 니센 위저추벽성형술 및 콜리스 위성형술 치험을 바탕으로 수술수기를 중심으로 문헌고찰과 함께 보고하는 바이다.
Advancement toward aging society has presented the importance of house planning in consideration of the elderly, increasing the necessity of such planning. Therefore the importance of indoor design guideline for elderly houses has been stressed and the understanding of the aging in regard to this trend will create more mature spaces. In line with this trend the purpose of this study is to study the medical factors related with bathroom interior design guidelines of the existing elderly houses from the viewpoint of deteriorating physical functions due to aging process. In order to research the deterioration of physical functions due to aging literatures of medical and exercise physiology were used and concerning the existing bathroom design guidelines for the aged people, the guideline items of behavior facilitation, physiological maintenance and perceptual maintenance aspects out of Murtha & Lee's user benefit criteria(1976) related with physical functions were selected to research in relation with the knowledge about the deterioration phenomena of physical functions. Physical aging and deterioration aspects were classified from the viewpoint of musculoskeletal disorder, cardiovascular disorder, respiratory disorder, gastronomy disorder, urology disorder, somatosensory disorder, endocrine disorder, immune disorder, nervous disorder and skin diseases and these were utilized in interpreting total 100 items of bathroom design guideline. Because bathroom is the space where many physical movements are done, it had the closest relationship with the deterioration of musculoskeletal health in general and as bathroom is the space where people use water and feel the difference in temperature and moisture more than other spaces, deterioration of skin and somatosensory health had the next closest relationship. The result of this study revealed that regarding the deterioration of physical functions of the elderly people in aspect of their perceptibility as designers will design creatively and sincerely based on its relationship with nervous system in future, the result of this study will be used to develop better spatial designs efficiently to meet for the aging society.

Attention-deficit hyperactivity disorder (ADHD) is characterized by inattention, hyperactivity, impulsiveness and problems in other higher cognitive processes such as executive function deficits. Currently, there are many treatment modalities, of which pharmacotherapy is the most strongly supported by scientific and clinical evidence. Stimulants, which are first choice in the pharmacological treatment of ADHD, block dopamine reuptake by binding the dopamine transporter and so increasing the concentration of dopamine in synaptic clefts. Stimulants are effective in improving core ADHD symptoms, as well as the nonspecific symptoms, such as aggressiveness and oppositional behavior. Frequently reported short-term adverse effects are decreased appetite, sleep disturbance, headache, dizziness and irritability. Although questions have been raised about the long-term side effects of stimulants, including growth suppression, cardiovascular events, and abuse potential, there is no clear evidence to support these concerns.
Sleep disorders such as insomnia, obstructive sleep apnea (OSA), and restless legs syndrome (RLS) are very common disorders and may cause significant burden in terms of individual as well as societal aspects. Sleep insufficiency from such sleep disorders may cause deleterious effects on daily work life and may be associated with other major medical or psychiatric disorders including cardiovascular disease, diabetes mellitus, depression, and anxiety disorder. Various motor or occupational accident may result from the sleep problems. In addition, recent researches provide the method to evaluate the lost productivity time in terms of absenteeism and presenteeism. Moreover, several studies on cost-effectiveness of treatment of sleep disorders show that it is cost-effective.
Autonomic dysfunction occurs frequently in multiple sclerosis (MS) and neuromyelitis optica spectrum disorder (NMOSD). Patients with either condition may present with autonomic symptoms such as bladder, sexual, cardiovascular, thermoregulatory, and gastrointestinal dysfunction, and fatigue, but autonomic symptoms that affect quality of life are underrecognized in clinical practice. The immunopathogenesis of MS has been considered to be associated with autonomic dysfunction. Applying appropriate treatment strategies for autonomic dysfunction is important to improve the quality of life of patients. Here we review autonomic dysfunction and how this is managed in patients with MS and NMOSD.
