AdaBoost 알고리즘을 이용한 심전도 정보 판독 시스템의 설계 및 구현 (Design and Implementation of Electrocardiogram Data Interpretation system using AdaBoost Algorithm)

  • 통계청에 따르면 심혈관 등의 성인병 질환으로 연 600~800명이 사망하는 것으로 나타나고, 고혈압, 동맥경화증, 심장병, 뇌졸중 등은 혈액의 흐름에 장애가 생겨 발생하는 심혈관계질환으로 오늘날 성인병의 주종을 이루고 있는 사망률이 높은 질병으로 구분된다. 또한 사망한 심혈관질환자 중 올바른 응급처치를 했더라면 생존했을 환자가 약 40%를 차지하고 있어 응급상황 발생 시 신속한 대응이 요구된다. 따라서 본 논문에서는 AdaBoost알고리즘의 weak classifier를 결합하여 strong classifier를 생성하는 방법을 통하여 효과적인 분석으로 심전도를 측정할 수 있도록 하고, 심혈관 질환자에게 발생한 응급상황을 빠른 시간 내에 관리 데스크에 전달할 수 있는 시스템을 제안하였다. 이에 따라 심전도 센서를 기반으로 측정한 데이터를 ZigBee통신으로 단말기에 전송하고 응급 상황을 판정하여 관리데스크에 긴급경보와 모니터링을 제공함으로써 신속한 의료서비스 제공이 가능하도록 하였다.

A Pilot Study of APN-led Self-management Program to Improve Cardiovascular Health Status among Korean Women with Risk Factors

  • Purpose: The aim of this study was to examine the effects of an Advanced Practice Nurse (APN)-led self-management program on cardiovascular health status among Korean women at risk of developing or progressing cardiovascular disease. Methods: This pilot study used one-group pre- and post- test experimental design. At health fairs in a community, 30 women who had one or more risk factors for metabolic syndrome were recruited and agreed to participate in the study. A total of 25 women completed the study. The intervention consisted of weekly follow-up calls and self-monitoring diary after an hour of individual counseling regarding risk factors, fast walking, and healthy diet tailored to the participants' needs. Physical activity was assessed with the World Health Organization International Physical Activity Questionnaire and a pedometer. Results: Participants showed statistically significant improvements in blood pressure, body mass index, levels of triglyceride, total cholesterol and low density lipoprotein, numbers of metabolic syndrome factors, and the 10-year CV risk estimate after one month of concentrated intervention. In addition, their physical activity behavior significantly improved after the intervention. Conclusion: This APN-led self-management program targeting modifiable risk factors by offering tailored counseling and concentrated support during the transition might be effective in preventing progression to the cardiovascular disease.

소음 노출 근로자의 청력손실에 미치는 심혈관-대사성 질환의 영향 (Cardiovascular-metabolic Diseases Affecting Hearing loss in Workers Exposed to Noise )

  • Objectives: We examined the association of hearing with cardio-metabolic diseases, dyslipidemia, hypertension and diabetes mellitus according to the personal and occupational characteristics of workers exposed to noise. Methods: The subjects of the study were 237,028 workers who underwent 2, 3, and 4 kHz airway pure tone audiometry in 2015 and who underwent clinical tests to diagnose cardiovascular-metabolic diseases. Cardiovascular-metabolic diseases were defined using reference values for respective items including blood pressure (systolic/diastolic), fasting blood glucose, cholesterol, and triglycerides. The airway pure tone hearing threshold of 2, 3, and 4 kHz, the average threshold of 2-3-4 kHz, and the hearing loss by the average threshold of the primary examination were distinguished. Results: Workers with cardiovascular-metabolic disease had significantly higher average hearing thresholds and higher rates of hearing loss. Logistic regression analysis, which adjusted for demographic variables of gender and age and occupational variables such as workplace size, industry, and type of work, and cardiovascular-metabolic disease as independent variables, showed that the odds ratio of hypertension to hearing loss in the mid-frequency was 1.239 (95% confidence interval: 1.118-1.374). For hypertension was 1.159 (1.107-1.214) and for diabetes it was 1.166 (1.104-1.230) for hearing loss in the high-frequency. Hearing loss measured by mean hearing was 1.178 (1.105-1.256) for hypertension and 1.181 (1.097-1.271) for diabetes. Conclusions: Cardiovascular-metabolic diseases in noise-exposed workers are associated with an increased risk of hearing loss and should be accompanied by bio-monitoring of cardiovascular-metabolic diseases in addition to auditory surveillance.

PTT기반 운동 강도 모니터링 시스템 구현 (Implementation of PTT Change Monitoring System According to Exercise Intensity)

  • 심혈관질환은 전 세계 주요 사망 원인으로 다양한 원인에 의해 발생한다. 심혈관질환의 가장 높은 위험인자는 고혈압으로 뚜렷한 증상이 없지만 방치할 경우 여러 합병증을 유발한다. 고혈압을 치료하기 위하여 약물치료와 규칙적 운동을 진행해야한다. 고혈압 환자의 경우 과도한 신체 활동 시 심장에 큰 무리가 발생해 심혈관질환으로 이어질 수 있다. 따라서 적정 강도로 운동을 진행할 수 있는 PTT 계측을 통한 운동 강도 모니터링 시스템이 요구된다. 본 연구에서는 심전도와 맥파 계측을 통해 PTT를 산출하여 운동 강도에 따른 PTT 변화 모니터링 시스템을 구현하였다. 구현된 시스템은 심전도의 R-peak와 맥파의 P-peak를 미분하여 peak간의 시간차를 이용하여 PTT를 산출한다. 운동 강도에 따른 PTT 변화 모니터링을 위하여 달리기 실험을 진행하였다. 실험결과 저강도는 0.313s, 중강도는 0.220s, 고강도는 0.188s의 PTT가 측정되었으며, 운동 강도가 증가함에 따라 PTT는 감소하는 것을 확인하였다.

당뇨병환자에게 부적절하게 사용된 Metformin의 처방빈도 분석 (Frequency of Inappropriate Metformin Use in Patients with Diabetes Mellitus)

  • We evaluated the inappropriateness of metformin use in patients with type 2 diabetes and chronic medical conditions to identify the frequency of the prescription metformin in violation of the food and drug administration (FDA) black box warning. We reviewed medical records of 307 outpatients who received metformin at endocrinology department in a hospital setting between January 1, 2005 and August 30, 2009. Of the 307 outpatients, 25 discontinued treatment of metformin due to elevated serum creatinine level (Scr${\geq}$1.5 mg/dl in male, Scr${\geq}$1.4 mg/dl in female), cancers, and/or liver disease. 5 were lost to follow-up. 89 (29.0%) of the patients had cardiovascular disease, 54.1% for hypertension, 9.8% for liver disease, and 60 (20.8%) for chronic kidney disease. 12 patients (3.9%) with chronic kidney disease and/or elevated serum creatinine level, and 1 patient (0.3%) with lactic acidosis were contraindicated to metformin use. Metformin should be avoided in 7 outpatients (2.3%) with active hepatitis and 1 patient (2.6%) with liver cirrhosis. Of the 307 outpatients, 13 (4.2%) patients who received metformin at the first visit and 16 (8.7%) patients who received metformin at the last visit violated to black box warning. 8 (2.6%) of the patients were in precautionary conditions to metformin use. Adjusted mean difference of serum creatinine was -0.16 mg/dl [95% CI: -0.22 to -0.11 (p<0.05)] and adjusted mean difference of alanine aminotransferase was 4.46 IU/l [95% CI: 2.47 to 6.44 (p<0.05)] between the first visit and the last visit. Critical number of elderly patients who are at the high risks of drug-disease and drug-laboratory interaction is exposed to the inappropriate metformin use in violation of black box warning. The periodic evaluation of metformin use and monitoring prescription through drug utility review (DUR) system is needed to improve patients' safety and to reduce adverse drug events.

개심술후 조기 혈행동태심실중격결손증 교정과 승모판재건술에 대한 비교 (Hemodynamic studies early after open heart surgery: comparison of repair of ventricular septal defect and mitral valvular reconstruction)

  • After open heart surgery, the patient must be carefully observed and adequately managed for his survival. This report reviewed 10 cases of ventricular septal defect and 12 mitral valvular diseases as hemodynamics early after open heart surgery. For postoperative 24 hours, clinical status was evaluated for left atrial pressure, central venous pressure,DP[LAP-CVP], peak systolic pressure, heart rate, urine amount, and other clinical findings. Especially, on postoperative fourth hour, cardiac output was most decreased, when the changes of monitoring were compared with two groups with or without using cardiotonics. Finally, we concluded as followings; 1.Postoperatively, variation of CVP was noted in VSD, but mitral valvular disease was more variable change of LAP. 2.DP was 1.3\ulcorner.4 cmH2O in VSD, and 6.4\ulcorner.2 cmH2O in mitral valvular disease. 3.Parameter using cardiotonic was CVP in VSD, and LAP in mitral valvular disease.

A Review of Anesthesia for Lung Transplantation

  • Lung transplantation is the only treatment option for patients with end-stage lung disease. Although more than 4,000 lung transplants are performed every year worldwide, the standardized protocols contain no guidelines for monitoring during lung transplantation. Specific anesthetic concerns are associated with lung transplantation, especially during critical periods, including anesthesia induction, the initiation of positive pressure ventilation, the establishment and maintenance of one-lung ventilation, pulmonary artery clamping, pulmonary artery unclamping, and reperfusion of the transplanted lung. Anesthetic management according to the special risks associated with a patient's existing lung disease and surgical stage is the most important factor. Successful anesthesia in lung transplantation can improve hemodynamic stability, oxygenation, ventilation, and outcomes. Therefore, anesthesiologists must have expertise in transesophageal echocardiography, extracorporeal life support, and cardiopulmonary anesthesia and understand the pathophysiology of end-stage lung disease and the drugs administered. In addition, communication among anesthesiologists, surgeons, and perfusionists during surgery is important to achieve optimal patient results.

소화기 암환자 호흡곤란의 정확한 평가와 치료 (Accurate Evaluation and Treatment of Dyspnea in Patients with Gastrointestinal Cancer)

  • Dyspnea is a common symptom among patients with gastrointestinal cancer, and a comprehensive evaluation of their respiratory function is essential. Self-reporting aids in the assessment of the degree of dyspnea, while objective examination methods are performed to identify the potential underlying causes when subjective symptoms are present. Standard treatment protocols should be followed for potentially reversible and common causes of dyspnea, such as pleural effusion, pneumonia, airway obstruction, anemia, asthma, exacerbation of chronic obstructive pulmonary disease, pulmonary thromboembolism, or drug-induced interstitial lung disease. Careful and close monitoring is required due to the high frequency of pulmonary thromboembolism and the risk of cardiovascular accidents, drug-induced interstitial lung disease, or other complications from some anticancer drugs. In case of hypoxemia with an oxygen saturation of 90% or less, palliative treatment should comprise standard oxygen therapy such as nasal cannula, mask, or high-flow nasal cannula. If non-pharmacological oxygen therapy is not effective, pain control through systemic narcotic analgesics and anti-anxiety therapy with benzodiazepines may be helpful.

Unpredictable Postoperative Global Cerebral Infarction in the Patient of Williams Syndrome Accompanying Moyamoya Disease

  • We report a rare case of Williams syndrome accompanying moyamoya disease in whom postoperative global cerebral infarction occurred unpredictably. Williams syndrome is an uncommon hereditary disorder associated with the connective tissue abnormalities and cardiovascular disease. To our knowledge, our case report is the second case of Williams syndrome accompanying moyamoya disease. A 9-year-old boy was presented with right hemiparesis after second operation for coarctation of aorta. He was diagnosed as having Williams syndrome at the age of 1 year. Brain MRI showed left cerebral cortical infarction, and angiography showed severe stenosis of bilateral internal carotid arteries and moyamoya vessels. To reduce the risk of furthermore cerebral infarction, we performed indirect anastomosis successfully. Postoperatively, the patient recovered well, but at postoperative third day, without any unusual predictive abnormal findings the patient's pupils were suddenly dilated. Brain CT showed the global cerebral infarction. Despite of vigorous treatment, the patient was not recovered and fell in brain death one week later. We suggest that in this kind of labile patient with Williams syndrome accompanying moyamoya disease, postoperative sedation should be done with more thorough strict patient monitoring than usual moyamoya patients. Also, we should decide the revascularization surgery more cautiously than usual moyamoya disease. The possibility of unpredictable postoperative ischemic complication should be kept in mind.

Predictive Factors of Sustained Sinus Rhythm and Recurrent Atrial Fibrillation after the Maze Procedure

  • Background: We examined perioperative predictors of sustained sinus rhythm (SR) in patients undergoing the Cox maze operation and concomitant cardiac surgery for structural heart disease. Materials and Methods: From October 1999 to December 2008, 90 patients with atrial fibrillation (AF) underwent the Cox maze operation and other concomitant cardiac surgery. Eighty-nine patients, all except for one postoperative death, were followed-up with serial electrocardiographic studies, 24-hour Holter monitoring tests, and regular echocardiographic studies. Results: Eighty-nine patients undergoing the maze operation were divided into two groups according to the presence of SR. At the time of last follow-up (mean follow-up period, $51.0{\pm}30.8$ months), 79 patients (88.8%) showed SR (SR group) and 10 patients (11.2%) had recurrent AF (AF group). Factors predictive of sustained SR were the immediate postoperative conversion to SR (odds ratio, 97.2; p=0.001) and the presence of SR at the 6th month postoperatively (odds ratio, 155.7; p=0.002). Duration of AF, mitral valve surgery, number of valves undergoing surgery, left atrial dimension, and perioperative left ventricular dimensions and ejection fractions were not predictors of postoperative maintenance of SR. Conclusion: Immediate postoperative SR conversion and the presence of SR at the 6th postoperative month were independent predictors of sustained SR after the maze operation.
