Effects of Motivation-Enhancing Program on Health Behaviors, Cardiovascular Risk factors, and Functional status for Institutionalized Elderly Women (동기강화 프로그램이 노년기 여성의 건강행위, 심혈관질환 위험요인 및 기능적 건강상태에 미치는 영향)

  • Purpose: This study was to compare changes in health behaviors, motivational factors, cardiovascular risk factors, and functional status (SIP) after implementing the 6-month motivation-enhancing program to institutionalized elderly women. Methods: Sixty-four elderly women participated. Face to face interviews with blood sampling and anthropometric assessment were conducted at the pretest, 10 weeks and 6 months during the program. Results: 1. The program participants showed significantly better health behaviors over 6 months. The mean motivational level was also significantly improved, especially for perceived benefits, perceived barriers, and emotional salience. 2. The mean of cardiovascular risk factors for the participants was 21.8 at the level of low to moderate risk. After completing the program, total risk score was significantly decreased to 18.7 at 10 weeks, and further to 17.7 at 6 months. A significant reduction was also found in HDL and LDL-cholesterol levels, blood pressure, obesity, inactivity, and stress. 3. The functional status (SIP) was 11% at the baseline and significantly changed in positive direction at 10 weeks(M=9.3) and at 6 month(M=6.3). The significant improvement was also found in physical and psychosocial dimensions and sleep/rest dimension. Conclusion: The motivation enhancing program was effective to reduce cardiovascular risks and to improve the functional status of institutionalized elderly women by motivating them to perform better health behaviors.

Age-related Circulating Inflammatory Markers and Cardiovascular Disease Risk Factors in Korean Women (한국 성인 여성에서 연령에 따른 혈중 염증 표지자와 심혈관계 질환 위험 요인에 대한 연구)

  • The purpose of this study was to investigate the age-related changes of cardiovascular disease risk factors and inflammatory markers in non-obese Korean women. Subjects were 112 women over 20 years old with body mass index (BMI) less than $30 kg/m^2$ and were divided into 3 groups (< 40 years, $40{\sim}59$ years, ${\ge}60$ years). Mean weight and BMI in the oldest group were significantly higher than those in the other 2 younger groups (p < 0.05). Mean total cholesterol, triglyceride, LDL-cholesterol and apolipoprotein B/apolipoprotein A1 ratio (BAR) in the oldest group were significantly higher than those in the youngest group (p < 0.05), and mean HDL-cholesterol of the oldest group was significantly lower than that of the youngest group (p < 0.05). The older-aged group showed significantly higher mean values of atherogenic index (AI) and LDL/HDL ratio (p < 0.05) than the respective younger-aged group, and AI was significantly correlated with age, nitric oxide and thiobarbituric acid reactive substances (p < 0.01). In addition, mean vascular cell adhesion molecule-l (VCAM-1) tended to be higher in the older-aged group than the younger group. Tumor necrosis factor-${\alpha}$, a proinflammatory maker, was significantly positively correlated with serum homocysteine, a cardiovascular disease risk factor (p < 0.01). In addition, a significantly positive correlation was observed between C-reactive protein and BAR (p < 0.01). Overall results suggested that the aging might affect the increase of cardiovascular disease risk factors including the serum lipid profiles, weight and BMI, and age-related increases of weight and BMI might play a role in changes in certain biomarkers of inflammation.   • A lot of studies have investigated the role of psychosocial and behavioral risk factors in the etiology and pathogenesis of cardiovascular disease. On the basis of prospective population studies that found a relationship between Type A behavior and cardiac morbidity and mortality independent of traditional risk factors, Type A behavior was declared a risk factor for the development of coronary heart disease. Additionally, recently proposed, a new personality construct, Type D personality have been reported to be associated with the increased risk for cardiovascular morbidity and mortality. In Korea, however, there were few studies about these issues. This article attempts to provide an overview of accumulating evidences on the Type A behavioral pattern and Type D personality related with cardiovascular disease to offer new directions for research.

Impact of the Sasang Constitutional Type on the High-Sensitivity C-Reactive Protein, a Predictor of Cardiovascular Disease (사상체질(四象體質)과 심혈관질환 예측인자인 High-sensitivity C-reactive protein의 상관성에 관한 연구)

  • 1. Objectives The purpose of this study was to investigate the high-sensitivity C-reactive protein (hsCRP) related characteristics found in each Sasang Constitutional type. 2. Methods This cross-sectional epidemiological study was conducted using data from 2,842 men and women. After typing each participant into one of the four Sasang constitutional types, they were analyzed on various cardiovascular disease-related variables. 3. Results 1) The predictors of cardiovascular disease (metabolic syndrome, high risk FRS group, high risk hsCRP group) was found to show the highest prevalence in the Taeeum type. 2) In the group with metabolic syndrome, the mean hsCRP concentration in the Taeeum and Soyang types were higher than in the Soeum type. In the group without metabolic syndrome, the mean hsCRP concentration in the Taeeum type were higher than in the Soyang and Soeum types. 3) In the FRS low risk group, the mean hsCRP concentration were higher in order of Taeeum type > Soyang type > Soeum type. In the FRS high risk group, the mean hsCRP concentrations were not significantly different among the Sasang constitutional types. 4. Conclusions The results of this study suggest that the Taeeum constitutional type is a risk factor for high hsCRP and cardiovascular disease. These findings suggest that the prevention of cardiovascular disease is more important in the Taeeum type compared to other constitutional types. Moreover, preventive measures are warranted even in the Taeeum-type persons with low clinical cardiovascular risk.

The Relationship between Pattern Identification and Stroke Risk Factors of Acute Ischemic stroke Patients (급성기 허혈성 뇌경색 환자들의 변증 분형과 위험 요인들간의 관련성 연구)

  • Object : The purpose of this study was to evaluate the relationship between pattern identification (PI) and stroke risk factors, such as hypertension, diabetes mellitus, dyslipidemia, stroke history, obesity, abdominal obesity and metabolic syndrome. Methods : 46 patients with acute ischemic stroke were recruited from May 2012 to November, 2012. We analyzed the data of 32 patients, and pattern identification was identified by resident and specialist of Korean medicine. We analized patient's PI and risk factor by Fisher's exact test. Results : We found that Dampness-phlegm group was more related with patient's metabolic syndrome than non Dampness-Phlegm group. And Yin deficiency group had less relationship with patient's metabolic syndrome, obesity, abdominal obesity and dyslipidemia than non Yin deficiency group. Conclusions : According to the analysis, these results provide evidence for relationship between the Dampness-phlegm group, Yin deficiency and metabolic syndrome.

Do Blebs or Bullae on High-Resolution Computed Tomography Predict Ipsilateral Recurrence in Young Patients at the First Episode of Primary Spontaneous Pneumothorax?

  • Background: The relationship between the size of bullae and pneumothorax recurrence is controversial. The aim of this study was to retrospectively evaluate the role of blebs or bullae in predicting ipsilateral recurrence in young patients experiencing their first episode of primary spontaneous pneumothorax (PSP) who underwent conservative treatment. Methods: A total of 299 cases of first-episode PSP were analyzed. The status of blebs or bullae was reviewed on high-resolution computed tomography (HRCT). The dystrophic severity score (DSS; range, 0 to 6 points) was calculated based on HRCT. Results: The 5-year recurrence rate was 38.2%. In univariate analysis, age (<20 years), body mass index (<$20kg/m^2$), a unilateral lesion, and intermediate risk (DSS 4 and 5) were associated with recurrence. Sex; smoking history; and the presence, number, and maximal size of blebs or bullae were not related to recurrence. In Cox regression, age and intermediate risk were independent risk factors for recurrence. High risk (DDS 6) was not an independent risk factor. Conclusion: The presence, number, and size of blebs or bullae did not affect ipsilateral recurrence. DSS failed to show a positive correlation between severity and recurrence. The decision to perform surgery in patients experiencing their first episode of PSP should not be determined by the severity of blebs and bullae.

Determinant of Arterial Stiffness in Young Adults

  • Cardiovascular disease (CVD) risk factors may be acting several decades before CVD becomes manifest. Data from young subjects may be valuable in further elucidating at this issue. We evaluated the association between baPWV (brachial-ankle pulse wave velocity) and cardiovascular risk factors in apparently healthy young adults. A total of 46 male and 91 female adolescents aged $18{\sim}25 years$ were studied. baPWV increased in a dose-responsive manner as the number of metabolic syndrome components. In both gender groups, baPWV was positively correlated with age. In males, waist, circumference total cholesterol, and LDL-cholesterol were positively correlated with baPWV, and in females, blood pressure (BP) was positively correlated with baPWV. Age, gender, mean BP, and Homeostasis model assessment insulin resistance (HOMA-IR) were found to be independent factors associated with baPWV levels. In conclusion, mean BP, age, gender, and HOMA-IR were associated with baPWV in young adults. This result suggests that multiple cardiovascular risk factors may be associated with an increased risk of arterial stiffness in young adults.

Influencing Factors of the Metabolic Index and Cardiovascular Risk Factors on Depressive and Non-depressive Groups in the Vulnerable Diabetic Elderly Women (취약계층 당뇨병 여성노인의 대사증후군지표와 심혈관 위험정도가 우울증상 여부에 미치는 요인)

  • Purpose: The purpose of this study was to identify influencing factors of metabolic index and cardiovascular risk factors, on depressive and non-depressive groups, in vulnerable diabetic elderly women. Methods: Participants were 137 vulnerable diabetic elderly women, using health centers in D city. Data were collected through interviews September though December 2017. The metabolic index was measured using National Cholesterol Education Program-Adult Treatment Panel III (NCEP-ATP III), and cardiovascular risk factors were measured using Framingham Risk Score (FRS). Depressive and non-depressive groups were divided by the score of Geriartric Depress Scale Short Korea Version (GDSSF). Collected data were analyzed using a x2 test, independent t-test, and binary logistic regression, with the SPSS/WIN 25.0 program. Results: Vulnerable diabetic elderly women, did not exercise in the depressive groups, and had higher triglyceride (TG), total cholesterol (TC) and larger waists, than in the non-depressive group. Results show that lack of exercise (OR= 6.30), is the highest risk factor, influencing the depressive symptom in vulnerable diabetic elderly women. Conclusion: These results suggest that to reduce depressive symptom levels among vulnerable diabetic elderly women, nursing interventions are needed to increase exercise and decrease TG, TC, and waist size, particularly in improving exercise of vulnerable diabetic elderly women.

The Factors Affecting Recurrence of Symptoms after Infrainguinal Arterial Endovascular Angioplasty

  • Background: This study reports the result of endovascular treatment for arterial occlusive disease limited to femoropopliteal lesions, focusing on the recurrence of symptoms instead of patency. Methods: This was a retrospective, single-center study. From April 2007 to November 2011, 48 limbs in 38 patients underwent endovascular stenting or balloon angioplasty to treat femoropopliteal arterial occlusive disease. The factors affecting the recurrence of symptoms were analyzed. Results: The mean age of the patients was $69.60{\pm}7.62$ years. Among the baseline characteristics of the patients, initial hyperlipidemia was the most important factor affecting the recurrence of symptoms (relative risk=5.810, p=0.031). The presence of a dorsal arch was also a significant factor (relative risk=0.675, p=0.047). Conclusion: The major factors that affect the recurrence of symptoms after endovascular treatment for femoropopliteal arterial occlusive lesions are hyperlipidemia and the presence of a dorsal arch. Therefore, the usage of lipid-lowering agents after endovascular treatment and taking the presence of a dorsal arch into consideration are important elements of managing the recurrence of symptoms.
