A Study on the Risk of Occupational Infectious Disease of Police Scientific Investigator (The Bacterial Culture of the Putrefying Dead Body Specimen)

  • The unnatural dead body refers to a corpse which is suspicious of unusual death and the dead case has been occurred more than 20 thousands times every year during the recent five years and most of them are found decayed. Police Scientific Investigators investigate unnatural dead bodies and its surroundings in all-around way and determine whether the death is involved with a crime, and most of the Scientific Investigators are exposed to a danger to get infected with pathogenic bacteria which are generated during the decaying process of dead body and are mostly to cause serious injuries on human beings. In line with the fact, the present study conducted a bacterial analysis by collecting excretions from 60 dead bodies and culturing the bacteria to evaluate infection risk of the police agents. The study cultured bacteria from 60 bodies and classified pathogenic bacteria of 108 strains, and its main bacteria are found to be them (; Acinetobacter baumannii 20 strains (19%), Pseudomonas aeruginosa 19 strains (18%), E. coli 18 strains (17%), Klebsiella pneumoniae 11 strains (10%), Proteus mirabillis 10 strains (9%), Enterococcus faecium 10 strains (9%), Staphylococcus aureus 9 strains (8%), Bacillus spp. 5 strains (5%), Streptococcus pneumoniae 3 strains (3%), Candida albicans 2 strains (2%), Mycobacterium tuberculosis 1 strains (1%)). The study results are expected to be used as educational data for preventing the Police Scientific Investigator from infections with bacteria or as a minimum data for improving work environment of the agents.

Meroparamycin Production by Newly Isolated Streptomyces sp. Strain MAR01: Taxonomy, Fermentation, Purification and Structural Elucidation

  • Twelve actinomycete strains were isolated from Egyptian soil. The isolated actinomycete strains were then screened with regard to their potential to generate antibiotics. The most potent of the producer strains was selected and identified. The cultural and physiological characteristics of the strain identified. the strain as a member of the genus Streptomyces. The nucleotide sequence of the 16S rRNA gene (1.5kb) of the most potent strain evidenced a 99% similarity with Streptomyces spp. and S. aureofaciens 16S rRNA genes, and the isolated strain was ultimately identified as Streptomyces sp. MAR01. The extraction of the fermentation broth of this strain resulted in the isolation of one major compound, which was active in vitro against gram-positive, gram-negative representatives and Candida albicans. The chemical structure of this bioactive compound was elucidated based on the spectroscopic data obtained from the application of MS, IR, UV, $^1H$ NMR, $^{13}C$ NMR, and elemental analysis techniques. Via comparison to the reference data in the relevant literature and in the database search, this antibiotic, which had a molecular formula of $C_{19}H_{29}NO_2$ and a molecular weight of 303.44, was determined to differ from those produced by this genus as well as the available known antibiotics. Therefore, this antibiotic was designated Meroparamycin.

Probiotics in the Prevention and Treatment of Postmenopausal Vaginal Infections: Review Article

  • Bacterial vaginosis (BV) and complicated vulvovaginal candidiasis (VVC) are frequently occurring vaginal infections in postmenopausal women, caused by an imbalance in vaginal microflora. Postmenopausal women suffer from decreased ovarian hormones estrogen and progesterone. A normal, healthy vaginal microflora mainly comprises Lactobacillus species (spp.), which act beneficially as a bacterial barrier in the vagina, interfering with uropathogens. During premenopausal period, estrogen promotes vaginal colonization by lactobacilli that metabolizing glycogen and producing lactic acid, and maintains intravaginal health by lowering the intravaginal pH level. A lower vaginal pH inhibits uropathogen growth, preventing vaginal infections. Decreased estrogen secretion in postmenopausal women depletes lactobacilli and increases intravaginal pH, resulting in increased vaginal colonization by harmful microorganisms (e.g., Enterobacter, Escherichia coli, Candida, and Gardnerella). Probiotics positively effects on vaginal microflora composition by promoting the proliferation of beneficial microorganisms, alters the intravaginal microbiota composition, prevents vaginal infections in postmenopausal. Probiotics also reduce the symptoms of vaginal infections (e.g., vaginal discharge, odor, etc.), and are thus helpful for the treatment and prevention of BV and VVC. In this review article, we provide information on the intravaginal mechanism of postmenopausal vaginal infections, and describes the effectiveness of probiotics in the treatment and prevention of BV and VVC.

대구(大邱).경북지방(慶北地方)에서 문제되는 젖소의 만성(慢性) 유방염(乳房炎)에 관한 연구(硏究) (Studies on Chronic Mastitis of Dairy Cattle in Taegu-Kyungpook Provinces)

  • 1984년(年) 10월(月)부터 1985년(年) 8월말(月末)사이에 대학부속가축병원(大學附屬家畜病院)에 검진의뢰(檢診依賴)된 대구(大邱) 경북지방(慶北地方)에서 발생(發生)한 83례(例)의 만성(慢性) 유방염(乳房炎)의 원인균(原因菌)을 분리(分離) 동정(同定)하고 이 분리균(分離菌)의 각종(各種) 항생제(抗生劑)에 대한 감수성(感受性)을 disc diffusion technique로 조사(調査)하였다. 대구(大邱) 경북지방(慶北地方)에서 문제되는 만성(慢性) 유방염(乳房炎)은 포도구균성 유방염(乳房炎)이 전체의 48.2%(83례중(例中) 40례(例))로 가장 많았으며 그 다음은 대장균군(大腸菌群) 유방염(乳房炎)으로 18.1%(83례중(例中) 15례(例))를 차지하였다. 효모양진균에 의한 유방염(乳房炎)도 10.8%(83례중(例中) 9례(例))나 되었다. Corynebacterium spp.에 의한 유방염(乳房炎)은 8.4%였으나 연쇄구균성 유방염(乳房炎)은 3.6%에 지나지 않았다. Bacillus spp.와 pseudomonad에 의한 것은 각각 3.6%와 8.4%이었으며 만성(慢性) 유방염(乳房炎)의 원인균(原因菌)을 밝힐 수 있었던 것은 4.8%(83 례중(例中) 4례(例))이였다. 만성(慢性) 유방염(乳房炎)에 관여한 포도구균과 대장균(大腸菌)은 공시(供試)한 항상제(抗生劑)에 대하여 내성(耐性)을 나타내는 경향이 뚜렷하였다. 포도구균은 penicillin, methicillin, linocomycin, novobiocin, ampicillin, tetracycline에는 각각 87.2%, 78.7%, 68.1%, 61.7%, 57.4%가 내성(耐性)인 반면에 gentamicin, cephalothin, chloramphenicol, erythromycin 등에는 각각 78.7%, 76.6%, 74.5%, 63.7%가 감수성(感受性)이었다. 대장균(大腸菌)은 streptomycin과 cephalothin에는 72.2%, tetracycline에 66.7%, ampicillin에 61.1%가 내성(耐性)인 반면에 colistin, gentamicin, chloramphenicol, neomycin에는 각각 83.3%, 77.8%, 66.7%, 55.6%가 감수성(感受性)이었다. Corynebacterium spp.는 ampicillin, chloramphenicol, erythromycin, gentamicin, oleandomycin, tetracycline에는 감수성(感受性)이었으나 novobiocin과 penicillin 등에는 내성(耐性)이었다. 한편 pseudomonas aeruginosa는 공시(供試)한 모든 항생제(抗生劑)에 내성(耐性)이었다.

활혈거어약(活血祛瘀藥)이 질내(膣內) 미생물(微生物)에 미치는 영향(影響) (Antibiotic Effects of blood-activating stasis-dispelling medicinals on Vaginal Microorganisms)

  • Purpose : The aim of this study is to investigate the antibiotic effects of 14 herbs among blood-activating stasis-dispelling medicinals on vaginal microorganisms. Methods : Staphylococcus aureus, Methicillin-resistant Staphylococcus aureus, Candida albicans, and Gardnerella vaginalis were used for vaginal pathogenic microorganisms. Lactobacillus gasseri, Streptococcus spp. and Escherichia coli HB101 were used for vaginal normal flora. The blood-activating stasis-dispelling medicinals, Mucunae Caulis, Salviae Radix, Persicae Semen, Myrrha, Zedoariae Rhizoma, Achuranthis Radix, Leonuri Herba, Melandrii Herba, Gleditsiae Spina, Lycopi Herba, Scirpi Rhizoma, Caesalpiniae Lignum, Corydlais Tuber and Polygoni Cuspidati Radix were used in this study. In vitro antibiotic activities were observed by optical density and colony test. Results : The optical density and colony test showed that Gleditsiae Spina, Scirpi Rhizoma, Corydlais Tuber, Polygoni Cuspidati Radix and Melandrii Herba of herbs among blood-activating stasis-dispelling medicinals had antimicrobial effect. Gleditsiae Spina had antimicrobial susceptibility and selective toxicity in Gardnerella vaginalis and MRSA. Scirpi Rhizoma had antimicrobial susceptibility and selective toxicity in Staphylococcus aureus and MRSA. Corydlais Tuber had antimicrobial susceptibility and selective toxicity in MRSA. Polygoni Cuspidati Radix had antimicrobial susceptibility and selective toxicity in Gardnerella vaginalis, Staphylococcus aureus and MRSA. Melandrii Herba had antimicrobial susceptibility and selective toxicity in Gardnerella vaginalis. Conclusion : According to the above results, we could suggest that Gleditsiae Spina, Scirpi Rhizoma, Corydlais Tuber, Polygoni Cuspidati Radix and Melandrii Herba of herbs among blood-activating stasis-dispelling medicinals be available to antimicrobial agent of vaginal pathogenic microbial species in vitro.

청열약(淸熱藥)의 질내(膣內) 미생물(微生物)에 대(對) 항균효과(抗菌效果) (Antibiotic Effect of Heat-Clearing Medicinals on Vaginal Microorganisms)

  • Purpose : This study was conducted to investigate the in vitro inhibitory effects of heat-clearing medicinal on common bacterias in gynecology. Methods : The heat-clearing medicinals ( Trichosanthis Radix, Sophorae Fructus, Phragmitis Rhizoma, Buddleiae Flos, Bambusae Folium, Anemarrhenae Rhizoma, Celosiae Semen, Gardeniae Fructus, Prunellae Spica, Sophorae Radix, Dictamni Radicis Cortex, Coptidis Rhizoma, Gentianae Scabrae Radix, Scutellariae Radix, Phellodendri Cortex) were used in this study. Staphylococcus aureus, MRSA, Candida albicans and Gardnerella vaginalis were used for vaginal pathogenic microorganisms. Streptococcus spp., Escherichia coli HB101, Lactobacillus gasseri were used for normal vaginal florae. We evaluated antibiotic effect by the optical density and the colony test. Results : The optical density and colony test showed that Celosiae Semen, Prunellae Spica, Scutellariae Radix and Phellodendri Cortex of herbs among heat-clearing medicinal had antimircobial effect. Celosiae Semen and Prunellae Spica had antimicrobial susceptibility and selective toxicity in MRSA. Scutellariae Radix and Phellodendri Cortex had antimicrobial susceptibility and selective toxicity in Gardnerella vaginalis. Conclusion : According to the above results, we could suggest that Celosiae Semen, Prunellae Spica, Scutellariae Radix and Phellodendri Cortex among heat-clearing medicinal be available to antimicrobial agent of vaginal pathogenic microbial species in vitro.

Benefits of Cervical Cancer Screening by Liquid-Based Cytology as Part of Routine Antenatal Assessment

  • Purpose: To determine the prevalence of abnormal cervical cytology, as diagnosed using a liquid-based cytology technique, in pregnant women attending the Antenatal Care (ANC) clinic at Siriraj Hospital. Materials and Methods: This cross-sectional study included 655 first-visit pregnant women who attended ANC clinic at Siriraj Hospital during June to November 2015 study period. After receiving routine antenatal care, cervical cytology screening was performed with the Siriraj liquid-based cytology technique. All specimens were reviewed by a certified cytopathologist using Bethesda System 2001 criteria. Patients with abnormal PAP results characterized as epithelial cell abnormalities were referred to a gynecologic oncologist for further management according to ASCCP Guidelines 2012. Results: Mean age of participants was $28.9{\pm}6.2$ years. Prevalence of abnormal cervical cytology was 3.4% (95% CI: 2.0-4.7). Among this group, there were ASC-US, ASC-H, LSIL, HSIL for 12(1.8%), 2(0.3%), 7(1.1%) and 1(0.2%), respectively. In 633 specimens of the normal group, infection was identified in 158 specimens (24.1%) which were caused by Candida spp. and Trichomonas vaginalis. Regarding patient perception about the importance of cervical cancer screening, although most women perceived screening to be important, 54% of participants had never been screened for cervical cancer. Rate of loss to follow-up in the postpartum period was as high as 41.8%. Conclusions: Prevalence of abnormal cervical cytology in pregnant women attending the ANC clinic at Siriraj Hospital was 3.4%. Inclusion of cervical cancer screening as part of antenatal assessment can help to identify precancerous lesions or cervical cancers in patients who might otherwise not be screened, thereby facilitating early treatment and improved patient outcomes.

Distribution of Allergen Reactivity in Serum of Allergy Patients

  • This study was conducted to evaluate the distribution, frequency, sensitization rate, and types of specific allergen in allergic patients. We analyzed allergens of 12,882 allergic patient's serum referred for Medical Laboratory using MAST Allergy Screen Test (Inhalation/food panel) from April, 2010 to March, 2011. Allergen reactivity were not detected in 833/12,882 (6.5%) allergic patients and the other allergic patients had two or more types of allergens of food and inhalant, respectively. In the distribution of the allergen food 73.4% (9,450/12,882 patients) was much higher than the pollen 26.6% (3,432/12,882 patients). The sensitization rate to each allergen showed garlic 26.7%, egg white 21.5% of food allergens, mugwort 24.9%, ragweed short 19.6% of pollen allergens, Candida albicans 10.5%, Alternaria spp. 7.9% of fungus allergens, and showed high sensitized rate to Dermatophagoides farinae 90.2%, Dermatophagoides pteronyssinus 77.8%, house dust 57% of indoor allergens, respectively. The specific allergens of food allergen, fungus allergen, and indoor allergen were more frequent in young child than adult, but pollen allergens were frequent in adult than young child. In seasonal distribution, the food allergen were frequent in summer and autumn, the pollen allergen in autumn, fungus allergen in spring and in winter, and indoor allergen in autumn and in winter. In conclusion, the mite of Dermatophagoides farinae and Dermatophagoides pteronyssinus, and house dust showed the highest sensitization rate in specific allergen of allergic patients. The allergens showed difference feature according to the age, region, and types of allergen.

Methicillin-resistant Staphylococcus aureus(MRSA), Candida albicans and Gardnerella vaginalis were used for vaginitis-induced microbes. Lactobacillus gasseri, Streptococcus spp. and Escherichia coil HB101 were used for normal vaginal florae. And herbs for removing dampness and promoting urination(Dianthi herbs. Tokoro Rhizoma, Saururi Herbs, Pyrrosiae Folium, Artemisiae Iwayomogii Herba, Plantaginis Semen, Tetrapanacis Medulla, Polygoni Avicularis Herba, Malvae Semen, Akebiae Caulis, Kochiae Fructus, Lygodii Spora) were used. Antimicrobial activities were estimated by the change of optical densities and colony test in vitro. Results : Plantaginis Semen, Artemisiae Iwayomogii Herba and Lygodii Spora had the antimicrobial susceptibility and selective toxicity against MRSA and Gardnerella vaginalis. Polygoni Avicularis Herba had the antimicrobial susceptibility and selective toxicity against Staphylococcus aureus. MRSA and Gardnerella vaginalis. Malvae Semen and Kochiae Fructus had the antimicrobial susceptibility and selective toxicity against MRSA. Dianthi Herba had the antimicrobial susceptibility and selective toxicity against Gardnerella vaginalis. Conclusion : According to these results, we can suggest that Plantaginis Semen, Artemisiae Iwayomogii Herba, Lygodii Spora, Polygoni Avicularis Herba, Malvae Semen, Kochiae Fructus and Dianthi Herba would be available to the antimicrobial agent for vaginitis-induced microbe in vitro.

Inhibition of Pathogenic Bacteria and Fungi by Natural Phenoxazinone from Octopus Ommochrome Pigments

  • Cephalopods, in particular octopus (Octopus vulgaris), have the ability to alter their appearance or body pattern by showing a wide range of camouflage by virtue of their chromatophores, which contain nanostructured granules of ommochrome pigments. Recently, the antioxidant and antimicrobial activities of ommochromes have become of great interest; therefore, in this study, the pH-dependent redox effect of the extraction solvent on the antioxidant potential and the structural characterization of the pigments were evaluated. Cell viability was determined by the microdilution method in broth by turbidity, MTT, resazurin, as well as fluorescence microscopy kit assays. A Live/Dead Double Staining Kit and an ROS Kit were used to elucidate the possible inhibitory mechanisms of ommochromes against bacterial and fungal strains. The results obtained revealed that the redox state alters the color changes of the ommochromes and is dependent on the pH in the extraction solvent. Natural phenoxazinone (ommochromes) is moderately toxic to the pathogens Staphylococcus aureus, Bacillus subtilis, Salmonella Typhimurium and Candida albicans, while the species Pseudomonas aeruginosa and Pseudomonas fluorescens, and the filamentous fungi Aspergillus parasiticus, Alternaria spp. and Fusarium verticillioides, were tolerant to these pigments. UV/visible spectral scanning and Fourier- transform infrared spectroscopy (FTIR) suggest the presence of reduced ommatin in methanol/ HCl extract with high intrinsic fluorescence.
