Health Risk Assessment and Evaluation of Asbestos Release from Asbestos-cement Slate Roofing Buildings in Busan (부산시내 석면슬레이트지붕 건축물로부터의 석면 노출 및 건강위해성 평가)

  • This study was performed to evaluate the asbestos exposure levels and to calculate excess lifetime cancer risk (ELCR) for the risk assessment of the asbestos fibers released from asbestos-cement slate roofing (ASR) building. Total number of ASR buildings was into 21,267 in Busan, and 82.03 percent of the buildings was residential houses, and 43.61 percent of the buildings was constructed in 1970s. For this study, ten buildings were selected randomly among the ASR buildings. The range of airborne asbestos concentration in the selected ten ASR buildings was from 0.0016 to 0.0067 f/mL, and the concentration around no-admitted ASR buildings was higher than that around admitted buildings. The ELCR based on US EPA IRIS (integrated risk information system) model is within 3.5E-05 ~ 1.5E-04 levels, and the ELCR of no-admitted ASR buildings was higher than 1.0E-04 (one person per million) level that is considered a more aggressive approach to mitigate risk. These results indicate that the cancer risk from ASR buildings is higher than other buildings, and systematic public management is required for control of no-admitted ASR buildings within near future.

Evaluation of Biochemical Recurrence-free Survival after Radical Prostatectomy by Cancer of the Prostate Risk Assessment Post-Surgical (CAPRA-S) Score

  • Background: The cancer of the prostate risk assessment (CAPRA) score has been defined to predict prostate cancer recurrence based on the pre-clinical data, then pathological data have also been incorporated. Thus, CAPRA post-surgical (CAPRA-S) score has been developed based on six criteria (prostate specific antigen (PSA) at diagnosis, pathological Gleason score, and information on surgical margin, seminal vesicle invasion, extracapsular extension and lymph node involvement) for the prediction of post-surgical recurrences. In the present study, biochemical recurrence (BCR)-free probabilities after open retropubic radical prostatectomy (RP) were evaluated by the CAPRA-S scoring system and its three-risk level model. Materials and Methods: CAPRA-S scores (0-12) of our 240 radical prostatectomies performed between January 2000-May 2011 were calculated. Patients were distributed into CAPRA-S score groups and also into three-risk groups as low, intermediate and high. BCR-free probabilities were assessed and compared using Kaplan-Meier analysis and Cox proportional hazards regression. Ability of CAPRA-S in BCR detection was evaluated by concordance index (c-index). Results: BCR was present in 41 of total 240 patients (17.1%) and the mean follow-up time was $51.7{\pm}33.0$ months. Mean BCR-free survival time was 98.3 months (95% CI: 92.3-104.2). Of the patients in low, intermediate and high risk groups, 5.4%, 22.0% and 58.8% had BCR, respectively and the difference among the three groups was significant (P = 0.0001). C-indices of CAPRA-S score and three-risk groups for detecting BCR-free probabilities in 5-yr were 0.87 and 0.81, respectively. Conclusions: Both CAPRA-S score and its three-risk level model well predicted BCR after RP with high c-index levels in our center. Therefore, it is a clinically reliable post-operative risk stratifier and disease recurrence predictor for prostate cancer.

  • This study assessed the lifetime cancer and non-cancer risk of aldehydes and volatile organic compounds exposure of young children at child-care facilities and indoor playgrounds in Korea. The samples were collected at various children's facilities (40 day-care houses, 42 child-care centers, 44 kindergartens, and 42 indoor playgrounds) in summer (Jul~Sep, 2007), winter (Jan~Feb, 2008) and Spring (Mar~Apr, 2008) periods, and analyzed by GC-MS. We estimated the lifetime excess cancer risks (ECRs) of formaldehyde, acetaldehyde, benzene, chloroform, CCI4 and tetrachloroethylene, and the hazard quotients (HQs) of Non-carcinogens (toluene, xylene, benzaldehyde and formaldehyde). Formaldehyde evaluated both cancer and non-cancer risk. The average ECRs of formaldehyde for young children were $1\times10^{-4}{\sim}1\times10^{-5}$ level in all facilities. HQs of four non-carcinogens did not exceed 1.0 for all subjects in all facilities.

Association between p16 Promoter Methylation and Thyroid Cancer Risk: A Meta-analysis

  • Background: The aim of the meta-analysis was to derive a more precise assessment of the association between p16 promoter methylation and thyroid cancer risk. Materials and Methods: The PubMed, Web of Science databases and Chinese CNKI were searched for relevant articles. Ultimately, seventeen case-control studies were included with a total of 804 thyroid cancer cases and 487 controls analysis by R Software (R version 3.1.2) including meta. Crude odds ratios with 95% confidence intervals were calculated using the random-effects model which were used to assess the strength of relationship between p16 methylation and lung carcinogenesis. Funnel plots were carried out to evaluate publication bias. Results: The meta-analysis results showed that the frequency of p16 promoter methylation in cancer tissue/blood was significantly higher than that normal tissue/blood (OR=5.46, 95%CI 3.12-9.55, P<0.0001) by random effects model with small heterogeneity. Conclusions: Thus, p16 promoter methylation may be associated with thyroid cancer risk.

Dietary Exposure of Aflatoxin$B_1$ and Cancer Risk Assessment (아플라톡식 $B_1$ 노출에 의한 발암 위해성 평가)

  • Daily exposure of aflatoxin B1 (AFB1) was estimated in foods (rice, barley, soybean, peanut, soysauce, soybean paste) by ELISA (enzyme linked immunosorbent assay) using polyclonal antibody R101. Before ELISA, a simple extraction method was applied for the quantitation of AFB1 in foods using chloroform which showed high recovery (70$\pm$12%). AFB1 levels in foods were 0.32 ng/ml (rice), 0.24ng/ml (barley), 0.22 ng/ml (peanut), 0.30~0.78 ng/ml (soysauce), and 0.2 ng/ml (soybean paste). Based on food consumption, we estimated that Koreans were exposed to AFB1 at the level of 1.86$\pm$0.46 ng/kg/day and liver cancer incidence attributed to AFB1 exposure (assuming that AFB1 as a single hepatocarcinogenic agent) might be calculated to be 13.1 per 100, 000 population. Our data demonstrate that AFB1 levels in foods were below the regulation of 10 ppb in foods and might not be the major risk factor for the high incidence of lover cancer in Korea.

Quantitative Assessment of the Effects of MMP-2 Polymorphisms on Lung Carcinoma Risk

  • Background: Previous studies assessing associations between matrix metalloproteinase 2 (MMP-2) polymorphisms and lung cancer risk reported conflicting results. A meta-analysis was therefore performed to derive a more precise estimation. Method: Case-control studies assessing associations between MMP-2 C735T and C1306T polymorphisms and lung cancer risk were included. Pooled odds ratios (ORs) with 95% confidence intervals (CIs) were estimated. Results: 7 studies with a total of 3,189 lung cancer cases and 3,013 controls were finally included into this meta-analysis. Overall, the MMP-2 C735T polymorphism was associated with lung cancer risk under the homozygote model (CC versus TT: OR =1.44, 95% CI = 1.03-2.02, $I^2$ = 0%), while the MMP-2 C1306T polymorphism also associated demonstrated links with all four models (all P values less than 0.05). Subgroup analyses by race suggested obvious associations between MMP-2 C735T and C1306T polymorphisms and lung cancer risk in Asians but not in Caucasians. There was no evidence for publication bias. Conclusion: Currently available evidence supports teh conclusion that MMP-2 C735T and C1306T polymorphisms influence susceptibility to lung cancer in Asians.

A Study on the Process Analysis and the Risk Assessment for Removal Work of the Asbestos Cement Slate (석면 슬레이트 해체작업의 공정분석 및 위험성평가에 관한 연구)

  • Asbestos is given to a variety of six naturally occurring silicate minerals. These minerals possess high tensile strength, flexibility, resistance to chemical and thermal degradation, and electrical resistance. These minerals have been used for decades in thousands of commercial products, such as insulation and fireproofing materials, automotive brakes, textile products, cement and wallboard materials. When handled, asbestos can separate into microscopic-size particles that remain in the air and are easily inhaled. It is now known that prolonged inhalation of asbestos fibers can cause serious and fatal illnesses including malignant lung cancer, mesothelioma, and asbestosis. Therefore the use of asbestos and asbestos products has dramatically decreased in recent years. Also all constructions including asbestos should be removed under strictly controlled conditions and very tightly implemented health & safety management systems. In this study, the process of the removal work of the asbestos cement slate was analyzed by IDEF-0 modeling and evaluated by 4M risk assessment method. The results show that removal work of the asbestos cement slate was classified five process and eighteen detail process. The risk of safety side the higher than the risk of health side in 4M risk assessment.

CHEK2 1100delC Variant and Breast Cancer Risk in Caucasians: A Meta-analysis Based on 25 Studies with 29,154 Cases and 37,064 Controls

  • Links between the CHEK2 1100delC heterozygote and breast cancer risk have been extensively explored. However, both positive and negative associations with this variant have been reported in individual studies. For a detailed assessment of the CHEK2 1100delC heterozygote and breast cancer risk, relevant studies published as recently as May 2012 were identified using PUBMED and EMBASE and selected using a priori defined criteria. The strength of the relationship between the CHEK2 1100delC variant and breast cancer risks was assessed by odds ratios (ORs) under the fixed effects model. A total of 29,154 cases and 37,064 controls from 25 case-control studies were identified in this meta-analysis. The CHEK2 1100delC heterozygote was more frequently detected in cases than in controls (1.34% versus 0.44%). A significant association was found between CHEK2 1100delC heterozygote and breast cancer risk (OR=2.75, 95% CI: [2.25, 3.36]). The ORs and CIs were 2.33 (95% CI: [1.79, 3.05]), 3.72 (95% CI: [2.61, 5.31]) and 2.78 (95% CI: [2.28, 3.39]) respectively in unselected, family, early-onset breast cancer subgroups. The CHEK2 1100delC variant could be a potential factor for increased breast cancer risk in Caucasians. However, more consideration is needed in order to apply it to allele screening or other clinical work.

Assessment of Semi-Quantitative Health Risks of Exposure to Harmful Chemical Agents in the Context of Carcinogenesis in the Latex Glove Manufacturing Industry

  • Excessive exposure to chemicals in the workplace can cause poisoning and various diseases. Thus, for the protection of labor, it is necessary to examine the exposure of people to chemicals and risks from these materials. The purpose of this study is to evaluate semi-quantitative health risks of exposure to harmful chemical agents in the context of carcinogenesis in a latex glove manufacturing industry. In this cross-sectional study, semi-quantitative risk assessment methods provided by the Department of Occupational Health of Singapore were used and index of LD50, carcinogenesis (ACGIH and IARC) and corrosion capacity were applied to calculate the hazard rate and the biggest index was placed as the basis of risk. To calculate the exposure rate, two exposure index methods and the actual level of exposure were employed. After identifying risks, group H (high) and E (very high) classified as high-risk were considered. Of the total of 271 only 39 (15%) were at a high risk level and 3% were very high (E). These risks only was relevant to 7 materials with only sulfuric acid placed in group E and 6 other materials in group H, including nitric acid (48.3%), chromic acid (6.9%), hydrochloric acid (10.3%), ammonia (3.4%), potassium hydroxide (20.7%) and chlorine (10.3%). Overall, the average hazard rate level was estimated to be 4 and average exposure rate to be 3.5. Health risks identified in this study showed that the manufacturing industry for latex gloves has a high level of risk because of carcinogens, acids and strong alkalisand dangerous drugs. Also according to the average level of risk impact, it is better that the safety design strategy for latex gloves production industry be placed on the agenda.
