188Re Labeled liver therapeutic drugs for hepatic carcinoma (HCC)

  • $^{188}Re$ is one of the most readily available generator derived and useful radionuclides for therapy emitting ${\beta}^-$ particles (2.12 MeV, 71.1% and 1.965 MeV, 25.6%) and imageable gammas (155 keV, 15.1%). The $^{188}W/^{188}Re$ generator is an ideal source for the long term (4-6 months) continuous availability of no carrier added (NCA) $^{188}Re$ suitable for the preparation of radiopharmaceuticals for radionuclide therapy. Rhenium-188 has been used for the preparation of therapeutic radiopharmaceuticals for the management of diseases such as bone metastasis, rheumatoid arthritis and primary cancers. Several early phase clinical studies using radiopharmaceuticals based on $^{188}Re$ -labeled phosphonates, antibodies, peptides, lipiodol and particulates have been reported. In this review, we addressed the current development status of $^{188}Re$ radiopharmaceuticals for liver cancer therapy and their applications.

Hypermethylation-mediated silencing of NDRG4 promotes pancreatic ductal adenocarcinoma by regulating mitochondrial function

  • The N-myc downstream regulated gene (NDRG) family members are dysregulated in several tumors. Functionally, NDRGs play an important role in the malignant progression of cancer cells. However, little is known about the potential implications of NDRG4 in pancreatic ductal adenocarcinoma (PDAC). The aim of the current study was to elucidate the expression pattern of NDRG4 in PDAC and evaluate its potential cellular biological effects. Here, we firstly report that epigenetic-mediated silencing of NDRG4 promotes PDAC by regulating mitochondrial function. Data mining demonstrated that NDRG4 was significantly down-regulated in PDAC tissues and cells. PDAC patients with low NDRG4 expression showed poor prognosis. Epigenetic regulation by DNA methylation was closely associated with NDRG4 down-regulation. NDRG4 overexpression dramatically suppressed PDAC cell growth and metastasis. Further functional analysis demonstrated that up-regulated NDRG4 in SW1990 and Canpan1 cells resulted in attenuated mitochondrial function, including reduced ATP production, decreased mitochondrial membrane potential, and increased fragmented mitochondria. However, opposite results were obtained for HPNE cells with NDRG4 knockdown. These results indicate that hypermethylation-driven silencing of NDRG4 can promote PDAC by regulating mitochondrial function and that NDRG4 could be as a potential biomarker for PDAC patients.

Anticancer effects of D-pinitol in human oral squamous carcinoma cells

  • D-pinitol is an analog of 3-methoxy-D-chiro-inositol found in beans and plants. D-pinitol has anti-inflammatory, antidiabetic, and anticancer effects. Additionally, D-pinitol induces apoptosis and inhibits metastasis in breast and prostate cancers. However, to date, no study has investigated the anticancer effects of D-pinitol in oral cancer. Therefore, in this study, whether the anticancer effects of D-pinitol induce apoptosis, inhibit the epithelial-to-mesenchymal transition (EMT), and arrest cell cycle was investigated in squamous epithelial cells. D-pinitol decreased the survival and cell proliferation rates of CAL-27 and Ca9-22 oral squamous carcinoma cells in a concentration- and time-dependent manner. Evidence of apoptosis, including nuclear condensation, poly (ADP-ribose) polymerase, and caspase-3 fragmentation, was also observed. D-pinitol inhibited the migration and invasion of both cell lines. In terms of EMT-related proteins, E-cadherin was increased, whereas N-cadherin, Snail, and Slug were decreased. D-pinitol also decreased the expression of cyclin D1, a protein involved in the cell cycle, but increased the expression of p21, a cyclin-dependent kinase inhibitor. Hence, D-pinitol induces apoptosis and cell cycle arrest in CAL-27 and Ca9-22 cells, demonstrating an anticancer effect by decreasing the EMT.

Ex vivo Boosted Immune Cell Therapy for Canine Hepatic Disease

  • A 12-year-old male American Cocker Spaniel was diagnosed with a type of chronic hepatits (CH) called cholangioheaptits. Routine supportive medication was administered to the patient, and ex vivo boosted immune cell (EBI-C) therapy was used for the treatment. A histopathologic examination of the liver 19 months later revealed that the cholangiohepatitis had progressed to cholangiocarcinoma. The medication and immune cell therapy was maintained. Two months after the new diagnosis, the patient's state worsened, and the dog died 635 days after the first visit. EBI-C therapy is a type of immunotherapy, where immune cells are isolated from the patient's peripheral blood mononuclear cells, expanded ex vivo, and then infused into the patient intravenously every two weeks. EBI-Cs (mean: 2.78 × 108 cells) were obtained 38 times and infused every two weeks. Most EBI-C were T-lymphocytes (99.24% of total EBI cells). T-lymphocytes produce large interferon (IFN)-γ, and IFN-γ inhibits liver fibrosis in dogs with CH. Moreover, in bile duct cancer, an increase in T-lymphocytes correlates with decreasing tumor invasion and metastasis. Thus, we propose that EBI-C therapy is applicable as a new supportive therapy for canine liver disease if other treatments like drug medication, surgery, or radiation are unavailable.

담도 스텐트 시술 후 장천공 증례보고 및 장천공에 대한 고찰 (A case report of gastrointestinal perforation in patient after biliary stent insertion and the overview of gastrointestinal perforation)

  • Objectives: This study aimed to emphasize the importance of accurate and timely diagnosis of acute abdominal pain with simple radiography by reporting a case of gastrointestinal perforation. Methods: We closely observed the diagnosis and progress of acute abdominal pain after biliary stent and reviewed the outline of gastrointestinal perforation. Results: Patient diagnosed with urethral cancer metastasis to lung and peritoneum was treated with complex Korean medicinal treatments to deal with anorexia, abdominal pain, jaundice and oliguria. During hospitalization, the patient's acute abdominal pain after biliary stent was diagnosed with gastrointestinal perforation by using plain chest and abdominal radiography. Conclusion: Using simple radiography to find out the emergency diseases such as perforation in acute abdominal pain is important clinically.

  • 동결절편검사는 수술과 연계하여 암 전이 여부를 판단하기 위한 응급한 병리검사가 필요할 때 이용된다. 합성곱 신경망은 이미지 분류에 뛰어난 성능을 보이는 딥러닝 기법으로 본 논문에서는 이를 이용하여 유방암 전이 여부를 자동적으로 진단하는 방법을 제안한다. 실험과정은 전처리, 학습, 후처리의 과정으로 구성되어 있으며, 합성곱 신경망으로는 Resnet-18 모델을 사용하였다. 실험결과 예측 정확도 및 종양의 최대 길이 정합 여부를 점수로 환산하여 약 0.514 의 결과를 보였다.

Image texture feature를 이용하여 비소세포폐암 전이 예측 머신러닝 모델 연구 (Study of machine learning model for predicting non-small cell lung cancer metastasis using image texture feature)

  • 본 논문에서는 18F-FDG PET과 CT에서 추출한 영상인자를 이용하여 비소세포폐암의 전이를 예측하는 머신러닝 모델을 생성하였다. 18F-FDG는 종양의 포도당 대사 시 사용되며 이를 추적하여 환자의 암 세포를 진단하는데 사용되는 의료영상 기법 중 하나이다. PET과 CT 영상에서 추출한 이미지 특징은 종양의 생물학적 특성을 반영하며 해당 ROI로부터 계산되어 정량화된 값이다. 본 연구에서는 환자의 의료영상으로부터 image texture 프절 전이 예측에 있어 유의한 인자인지를 확인하기 위하여 AUC를 계산하고 단변량 분석을 진행하였다. PET과 CT에서 각각 4개(GLRLM_GLNU, SHAPE_Compacity only for 3D ROI, SHAPE_Volume_vx, SHAPE_Volume_mL)와 2개(NGLDM_Busyness, TLG_ml)의 image texture feature를 모델의 생성에 사용하였다. 생성된 각 모델의 성능을 평가하기 위해 accuracy와 AUC를 계산하였으며 그 결과 random forest(RF) 모델의 예측 정확도가 가장 높았다. 추출된 PET과 CT image texture feature를 함께 사용하여 모델을 훈련하였을 때가 각각 따로 사용하였을 때 보다 예측 성능이 개선됨을 확인하였다. 추출된 영상인자가 림프절 전이를 나타내는 바이오마커로서의 가능성을 확인할 수 있었으며 이러한 연구 결과를 바탕으로 개인별 의료 영상을 기반으로 한 비소세포폐암의 치료 전략을 수립할 수 있을 것이라 기대된다.

췌장 선암의 절제 가능성 평가 (Radiologic Evaluation for Resectability of Pancreatic Adenocarcinoma)

  • 췌장 선암의 발견, 진단, 절제 가능성 평가, 병기 설정 및 치료 방침 결정에 영상 검사는 중요한 역할을 담당하고 있다. 최근에는 진단 당시 경계절제성 혹은 국소 진행성 췌장암을 진단 받은 경우 선행 보조 치료 또는 완화 치료 후 가능하면 근치적 절제술을 고려할 것이 권고된다. 이 종설에서는 췌장 선암의 절제 가능성 평가, 원격 전이의 진단 및 선행 보조 치료 또는 완화 치료 후 절제 가능성을 재평가할 때 영상 검사의 해석, 판독 기준 및 고려할 사항에 대하여 설명하고자 한다.

Mechanisms Underlying the Role of Myeloid-Derived Suppressor Cells in Clinical Diseases: Good or Bad

  • Myeloid-derived suppressor cells (MDSCs) have strong immunosuppressive activity and are morphologically similar to conventional monocytes and granulocytes. The development and classification of these cells have, however, been controversial. The activation network of MDSCs is relatively complex, and their mechanism of action is poorly understood, creating an avenue for further research. In recent years, MDSCs have been found to play an important role in immune regulation and in effectively inhibiting the activity of effector lymphocytes. Under certain conditions, particularly in the case of tissue damage or inflammation, MDSCs play a leading role in the immune response of the central nervous system. In cancer, however, this can lead to tumor immune evasion and the development of related diseases. Under cancerous conditions, tumors often alter bone marrow formation, thus affecting progenitor cell differentiation, and ultimately, MDSC accumulation. MDSCs are important contributors to tumor progression and play a key role in promoting tumor growth and metastasis, and even reduce the efficacy of immunotherapy. Currently, a number of studies have demonstrated that MDSCs play a key regulatory role in many clinical diseases. In light of these studies, this review discusses the origin of MDSCs, the mechanisms underlying their activation, their role in a variety of clinical diseases, and their function in immune response regulation.

소아 청소년 골육종 재발 후 치료성적 : 한국의 단일기관 성적 (Outcome after relapse in childhood and adolescent osteosarcoma : single institution experience in Korea)

  • 목 적 : 골육종은 소아 청소년기에 뼈에 발생하는 가장 흔한 악성종양으로 종양 제거술 및 집중적 항암 화학요법을 시행한 경우에도 약 30-40%에서 재발하는 매우 침습적인 질환이다. 하지만 아직까지 재발한 골육종의 예후 및 예후인자에 대한 연구가 부족한 상태이다. 저자들은 치료 후 재발한 골육종 환아의 치료성적 및 재발 후 생존율에 영향을 주는 요인들을 분석하고자 하였다. 방 법 : 1985년 2월부터 2004년 2월까지 원자력병원에서 골육종으로 치료한 461명의 환아 중 재발한 180명의 의무기록을 후향적으로 조사하였으며 재발 후 생존율과 재발 후 예후인자로 재발병소의 위치, 재발 후 치료 방법, 원발병소의 수술전 화학요법에 대한 병리학적 반응도, 첫 진단 시 전이여부 및 재발기간을 분석하였다. 결 과 : 전체 재발율은 39%였고 재발한 골육종 환아의 5년 및 10년 재발 후 생존율은 각각 13%와 4%였다. 평균 재발기간은 15개월이었다. 재발 후 5년 생존율에 영향을 주는 인자는 재발 부위(폐, 39%; 원발병소, 0%; 폐와 뼈의 재발, 25%; 기타 부위의 재발, 12%), 병소 부위 절제 여부(절제한 경우 38%; 절제하지 않은 경우 11%), 재발기간(1년 이내에 재발한 경우 13%, 1년 이후에 재발한 경우 44%)이었다. 결 론 : 재발된 골육종 환자의 생존율은 높지 않다. 하지만 본 연구에서는 재발된 병변을 적극적으로 치료할 때 생존율이 향상됨을 보여주고 있으며 특히 재발기간이 길고 폐에 재발된 경우에는 수술 및 항암 화학요법과 그 외의 적극적인 치료가 필요하다 하겠다.
