Development and Evaluation of the Auditory Feedback Gait Training System Induced Symmetrical Weight-Bearing in Hemiplegic Patients (편마비 환자의 대칭적 체중부하 유도를 위한 청각적 피드백 보행훈련 시스템 개발 및 평가)

  • In this study, we developed a wireless rehabilitation auditory feedback gait training system for symmetrical weight-bearing in patient with CVA. The device consists of an instantaneous shoe equipped with two load-cell sensors. Auditory feedback can be applied according to the weight-bearing. For gait patterns analysis, cadence, walking velocity, stance/swing phase ratio and gait cycle were examined. The clinical test with six healthy volunteers and two hemiplegic patients was performed applying the auditory feedback system. Both normal subjects and hemiplegic patients were increased strength on weight-bearing in affected limb, walking velocity, and cadence after biofeedback device. Also, the stance time with weight-bearing was increased while the swing time was decreased in gait phase. It can be expected that by using the feedback system, the patient with lower limb disorder will be able to reach a better quality of weight-bearing during gait.

Analysis of the Effects and Nursing Intervention of Home Health Care in Public Health Centers (보건소 방문보건사업 효과와 간호중재 분석)

  • Purpose: The purpose of this study was to investigate the types and frequencies of nursing intervention of home health care in Public Health Centers and its effects. Method: The data collection period for this study lasted from March 1 to December 31, 2003. The clients were sampled by a stratified randomized method among those who had been cared for at least 3 months. The data was analyzed by SPSS for description. ANOVA, paired t test, etc. Result: The types and frequencies of nursing intervention in major chronic health problems were significantly different. The main types of nursing intervention in hypertension and DM patients included education on disease management, regular exercise, stress management, diets, etc. CVA patients were intervened in pain control (ice or hot pack, massage), position changes, and ROM exercise. Cancer patients received imaginary or relaxation therapy, pain control (ice or hot pack, massage), hospice, etc. After the intervention, the drug compliance of hypertension (8.2 days per month) and DM patients (6.2 days per month) improved. Blood sugar levels (FBS 7.6, post partum 2hrs $21.4(mg/d{\ell})$ and blood pressure(systolic 9.8, diastolic 4.3 mmHg) lowered significantly. All aspects of QOL also improved (total 3.68). Conclusion: The types and frequencies of nursing intervention were determined by characteristics of the health problems, and home health care nursing intervention in Public Health Centers was effective to the elderly of the community in many aspects.

Exploring Differences in Surgical Outcomes Depending on the Arterial Cannulation Strategy for Acute Type A Aortic Dissection: A Single-Center Study

  • Background: Type A aortic dissection (AD) and intramural hematoma (IMH) are critical medical conditions. Emergency surgery is typically performed under cardiopulmonary bypass immediately after diagnosis, which involves lowering the body temperature to induce total circulatory arrest. Selection of the arterial cannulation site is a critical consideration in cardiac surgery and becomes more challenging in patients with AD. This study explored the strengths and weaknesses of different cannulation methods by comparing each cannulation strategy and analyzing the reasons for patients' outcomes, especially mortality and cerebrovascular accidents (CVAs). Methods: This retrospective study reviewed the medical records of patients who underwent surgery for type A AD or IMH between 2008 and 2023, using the moderate hypothermic circulatory arrest approach at a single center. Results: Among the 146 patients reviewed, 32 underwent antegrade cannulation via axillary, innominate artery, aortic, or transapical cannulation, while 114 underwent retrograde cannulation via the femoral artery. The analysis of surgical outcomes revealed a significant difference in the total surgical time, with 356 minutes for antegrade and 443 minutes for retrograde cannulation (p<0.001). The mean length of stay in the intensive care unit was significantly longer in the retrograde group (5±16 days) than in the antegrade group (3±5 days, p=0.013). Nevertheless, no significant difference was found between the groups in the 30-day mortality or postoperative CVA rates (p=0.2 and p=0.7, respectively). Conclusion: Surgeons should consider an appropriate cannulation strategy for each patient instead of adhering strictly to a specific approach in AD surgery.

  • Objectives : C V A(Cerebral Vascular Attack) is the severest and most important disease that makes doctors difficult in treating, also makes patients difficult to recover. I intended to research for imformations and standards of C V A patients of Sasang Constitutional Medicine through this clinical study. Methods : 106 C V A patients(61 men and 45 women) who had been inpatients from October 19, 1996 to August 19, 1999 in Conmaul Oriental Hospital were investigated and analysed the characteristics, past histories, symtoms, progress, relationship with other diseases for example DM and HTN, efficacy of oriental medical treatments. Results and Conclusions : The results of constitutional analysis showed 51 Taeumin(48.1%) 13 Soyangin(12.3%) 12 Soeumin(11.3%) 7 Taeyangin(6.6%). In the frequency of C V A, age of $60{\sim}69(43.4%)$ is major, both $50{\sim}59\;and\;70{\sim}79$ (17.9%) are next. and in pre-disease on the past, C V A is major, digestive disease and musuloskeletal disease are next. In chief complaint, kinetic disorder is major dyslogia is next. In efficacy of treatments, Slightly Improved(52.9%) is the most occupied part of that.

Correlation Between Body Composition, Lipid Metabolic Indicator and Bone Mineral Density in Old Patients (노인의 체성분 및 지방대사와 골밀도의 상관관계)

  • Objectives To observe relationship between BMD(BMD; bone mineral density) and lipid metabolic indicator which consists of cholesterol, HDL(HDL; high density lipoprotein), Trigliceride which influenced by BMI(BMI; body mass index), BFR(body fat rate) indirectly and directly in both ways. Methods Among 120 old patients aging above 65 who admitted to Dep. of Oriental Rehabilitation, Jeon-Ju Oriental Medical Hospital, Won Kwang University in order to prevent demetia and CVA from Mar. 2004 to May 2005 correlationship between BMI, BFR, BMD etc. and lipid metabolic indicator was statistically analysed. Results BMD of male patient was higher than that of female patients and body weight, height, BFR, BMI was significantly related to BMD as well. Relationship between Total Cholesterol, HDL, Triglyceride and BMD was not significant. Conclusions From the above results, Relationship between Total Cholesterol, HDL, Triglyceride and BMD was not significant.

Systematic Review of Occupational Therapy Programs for Upper Extremity Functions in Subacute Stroke Patients (아급성기 뇌졸중 환자의 팔 기능을 위한 작업치료프로그램에 대한 체계적 고찰)

  • Purpose : This study systematically reviewed occupational therapy programs for upper extremity functions in subacute patients implemented over the last decade at home and abroad to utilize the findings as the basis for intervention protocols. Methods : This study was conducted with domestic and foreign randomized controlled experimental studies published from January 2013 to May 2023. Acute or subacute, CVA or stroke, upper extremity function, and program or therapy were used as search keywords in the search databases Scopus, PubMed, Riss, and DBpia. A total of 3,723 documents were retrieved, and 1,007 duplicate papers were excluded. In addition, 2,640 papers that did not meet the inclusion and exclusion criteria were excluded by the researcher examining the titles and abstracts. Thereafter, the full texts were checked. Consequently, 67 documents were excluded, and nine documents were finally used for analysis. Results : Due to the evaluation of the quality of the documents of nine studies, five items were rated "low," and two items were rated "unclear" in many studies. Males predominated the genders, and infarction predominated the types. The average age was over 50 but under 60 years, and the number of days after onset was predominantly fewer than one month. Occupational therapy programs were classified into mental programs, mirror therapy, music programs, and virtual reality programs, and virtual reality programs were frequently used. The intervention periods were predominantly 15-30 sessions for less than 30 minutes, and the Fugl-Meyer assessment-upper extremity was predominantly applied as an evaluation tool. Conclusion : This study is significant because it attempted to present the direction of intervention protocols for upper extremity functions in subacute stroke patients in clinical settings. Studies should be conducted with an expanded scope for a literature review.

  • Background: This study was to investigate effects of Correlation Analysis between Cervical-Vertebra Angle and Neck Range of Motion, Muscle Strength, Sternocleidomastoid Thickness of Patients with Forward Head Posture Design: Correlation Analysis. Methods: The subjects of this study were a total of 54 people in the forward head position and their ages were between 30 and 50 years old. The subjects cranio-vertebral angles, neck extension, neck flexion, neck rotation angles, neck flexor strength, neck extensor strength, sternocleidomastoid thickness were evaluated through measuring instruments. The thickness of the sternocleidomastoid muscle was measured using an imaging ultrasound diagnostic device (ultra sound, Versana Premier, GE Medical systems, China). CVA was measured by measuring the side photo of the subject was taken with a camera and evaluated.. neck joint range of motion was measured through digital inclinometer for extension, flexion, and neck rotation. neck muscle strength was measured by measuring the using a digital sthenometer. Data analysis in this study was statistically processed using SPSS version 26.0 (IBM SPSS Inc., USA). Correlation analysis was used and the statistical significance level was set at 0.05. Results: The results neck extension(r= 0.70**), neck flexion(r= 0.67**), neck rotation(r= 0.56**), neck extensor muscle strengt(r= 0.85**), neck flexor muscle strength(r= 0.66**), sternocleidomastoid thicknes(r= -0.81**) It indicates that there is a correlation. Conclusion:These results improve the Cervical-vertebra angle of patients with forward head posture should include a program to improve the thickness of the SCM. In the future, study can be used as an evidentiary material for treatment interventions to improve the Cervical-vertebra angle of patients with forward head posture.

Characteristics of Hyperlipidemia in 1180 Subjects Examined with an Early CVA-detection Program in an Oriental Hospital

  • Objective: Hyperlipidemia is a main risk factor of stroke and cardiovascular disease. This study aimed to investigate characteristics of hyperlipidemia in adult subjects. Methods: Hyperlipidemia-related makers were analyzed using biochemical data from 1,180 (496 male and 684 female) subjects who took medical examination in an Oriental Hospital for the purpose of detecting cerebrovascular accident. Results: 19.7 % of subjects (male 14.7 %, female 23.2 %) had hypercholesterolemia while 21.9 % (male 25.8 %, female 19.0 %) had hypertriglyceridemia. 17.2 % of subjects (male 22.1 %, female 13.6 %) showed low HDL-cholesterol while 13.0% (male 10.3 %, female 14.9 %) showed high LDL-cholesterol. Conclusions: This study will provide helpful information for patients with hyperlipidemia and to develop therapeutics using traditional Korean medicine.

  • Purpose Because Taeyangin constitutions are very rare, There are few references about Taeyangin diseases. I suppose to gather basic materials on the case of a diagnosis and treatment on Taeyangin constitution patient. So I want to accomplish the theory of Taeyangin diseases.   • Purpose: This study was done to report nursing case for ADL improvement of elders who have CVA(Cerebrovascular Accident) sequelae. Methods: The client had registered in the C visiting nursing center after being decided a long-term care Grade 2. Data were collected through consultation logs for recipients, Activities of Daily Living (ADL) records, fall risk assessment (Huhn) sheets, decubitus ulcer risk assessment (Braden Scale) sheets, cognition assessment (K-MMSE) sheets, long-term care benefit provision records, and interviews with visiting nurse. Data were collected and analyzed according to the Omaha System problem classification. The intervention scheme and the problem rating scale for performance were applied to present the case for home-visit nursing. Results: The client registered in August, 2018, was provided home-visit nursing care once a week as of September 2020. ADL, cognitive levels and decubitus ulcer risks were found to have improved. Conclusion: This case report presents the value of classifying nursing problems and checking nursing intervention provided to patients with problems of ADL. The presentation of home-visit nursing cases applying a standardized nursing problem classification scheme for clients with various problems showed that a high quality level of care is guaranteed and evidence-based nursing can be provided by visiting nurses.
