  • A method of reflection-transmission type ultrasonic inverse scattering image was presented using linearly arrayed transducers in inner surface of half-cylinder. In this method, to reduce the number of data, the mirror effect using a reflector behind object and pulse wave with finite frequency band, To verify the proposed method, a computer simulation was performed for organic phantom specimen, As the results. it was verified that the reconstructed image was satisfactory even when the limitation view angle was limited to around 30 deg.

  • Quantitative information of a three dimensional(3D) kinematics of joint is very useful in knee joint surgery, understanding how knee kinematics related to joint injury, impairment, surgical treatment, and rehabilitation. In this paper, an automated 2D/3D image matching technique was developed to estimate the 3D in vivo knee kinematics using dual X-ray images. First, a 3D geometric model of the knee was reconstructed from CT scan data. The 3D in vivo position and orientation of femoral and tibial components of the knee joint could be estimated by minimizing the pixel by pixel difference between the projection images from the developed 3D model and the given X-ray images. The accuracy of the developed technique was validated by an experiment with a cubic phantom. The present 2D/3D image matching technique for the estimation of in vivo joint kinematics could be useful for pre-operative planning as well as post-operative evaluation of knee surgery.

The Effects of Nonmagnetic Bolus on Contralateral Breast Skin Dose during Tangential Breast Irradiation Therapy

  • In this study the contralateral breast skin dose was decreased. It was to apply the results to the clinical study after analysis of different radiation dose amounts to contralateral breast with nonmagnetic bolus and without nonmagnetic bolus. A Rando phantom was computed tomography (CT) simulated, five treatment plans were generated: open tangents, open field in field, wedge 15, wedge 30, and intensity-modulated radiotherapy (IMRT) plan with 50.4 Gy to cover sufficient breast tissue. Contralateral breast skin dose was measured at 8 points using a glass dosimeter. The average contralateral breast dose using nonmagnetic bolus showed better excellence in decreasing the absorbed dose in the order of $168{\pm}11.1$ cGy, $131{\pm}10.2$ cGy (29%), $112{\pm}9.7$ cGy (49%), and $102{\pm}9.5$ cGy (64%) than changing the treatment plan. This study focused on decreasing the effect of scattered dose by use of a nonmagnetic bolus on the contralateral breast during radiotherapy in breast cancer patients and an intriguingly significant decrease was observed parallel to the opposed beam.

선형가속기를 이용한 뇌정위 방사선수술시 Isocentric sub system의 기하학적 오차

  • 목적 : 뇌정위 방사선수술은 AVM(ateriovenous malformation)이나 작은 크기의 종양에 1회에 고선량의 방사선을 조사하는 기술이다. 선형가속기를 이용한 방사선 수술을 하기 위하여 최근 본원에 설치한 Philips SL 75-5 선형가속기와 isocentric sub system(ISS)에 의한 뇌정위 방사선 수술에 있어서 표적의 위치선정과, gantry와 couch의 회전시 기하학적 오차가 중요시 되는데 isocentric sub system의 오차를 분석 하였다. 대상 및 방법 : 방사선원으로는 Philips SL 75-5 선형가속기의 5MV 광자선을 사용하였고, 원형의 작은 광자선속을 위하여 isocenter에서의 직경이 26mm인 secondary cone을 gimbal baaring에 삽입하여 사용하였다. 표적의 크기와 좌표를 정하기 위하여 CT나 angio localizer를 이용하고, 표적좌표 선정을 위하여 BRW phantom base와 target pointer를 이용하여 임의의 BRW-coordinator를 바꾸어 가면서 gantry angle와 ISS head 각도를 임의로 바꾸어 가면서 film에 방사선을 조사하였다. 흑화된 film을 view box 위에 놓고 광학판독기구로 film 가장자리의 오차를 scale 확대경으로 측정하여 오차를 분석하였다. 결과 : 표적좌표 선정의 정확도를 확인하기 위하여 임의의 표적좌표에 gantry의 10개각도 ISShead의 10개각도에서 각각 광자선을 조사시켜 film을 이용하여 오차를 측정한 결과 collimator cone의 직경이 26mm일때 전체 평균오차가 0.219+-0.03mm이었다. 결론 : Isocentric sub system은 gantry head와 ISS arm 사이에 gimbal bearing이 있어서 이 부위를 flexible하게 연결함으로 gantry의 회전에 무관하게 정확한 isocenter를 유지시켜 주고 ISS head는 couch와 독립되어 움직이므로 isocentric sub system isocenter의 오차를 최대한 줄일수 있음을 알았다.

Effects of Physical Factors on Computed Tomography Image Quality

  • The purpose of this study was to evaluate the effects of X-ray photon energy, tissue density, and the kernel essential for image reconstruction on the image quality by measuring HU and noise. Images were obtained by scanning the RMI density phantom within the CT device, and HU and noise were measured as follows: images were obtained by varying the tube voltages, the tube currents and eight different kernels. The greater the voltage-dependent change in the HU value but the noise was decreased. At all densities, changes in the tube current did not exert any significant influence on the HU value, whereas the noise value gradually decreased as the tube current increased. At all densities, changes in the kernel did not exert any significant influence on the HU value. The noise value gradually increased in the lower kernel range, but rapidly increased in the higher kernel range. HU is influenced by voltage and density, and noise is influenced by voltage, current, kernel, and density. This affects contrast resolution and spatial resolution.

Absorbed and effective dose from newly developed cone beam computed tomography in Korea (최근 개발된 cone beam computed tomography의 흡수선량 및 유효선량 평가)

  • Purpose: Cone beam computed tomography (CBCT) provides a lower dose and cost alternative to conventional CT, promising to revolutionize the practice of oral and maxillofacial radiology. The purpose of this study was to evaluate the absorbed and effective doses of Implagraphy and VCT (Vatech Co., Hwasung, Korea) and compare them with those of panoramic radiography. Materials and Methods: Thermoluminescent dosimeter (TLD) chips were placed at 27 sites throughout the layers of Female ART Head and Neck Phantom for dosimetry. Implagraphy, VCT units, and Planmeca Proline XC panoramic unit were used for radiation exposures. Radiation weighted doses and effective doses were measured and calculated using 1990 and 2005 ICRP tissue weighting factors. Results: Effective doses in Sv (ICRP 2005, ICRP 1990) were 90.19, 61.62 for Implagraphy at maxillay molar area, 123.20, 90.02 for Implagraphy at mandibular molar area, 183.55, 139.26 for VCT and 40.92, 27.16 for panoramic radiography. Conclusion: Effective doses for VCT and Implagraphy were only about 2.2 to 4.5 times greater than those for panoramic radiography. VCT and Implagraphy, CBCT machines recently developed in Korea, showed moderately low effective doses.

  • Magnetic Resonance Image represents three-dimensional diagnostic imaging technique using both nuclear magnetic resonance phenomenon and computer. Compared with computed tomography (CT), MRI have advantages harmless to patient's body, three-dimensional image with high resolution and disadvantages long data acquisition time because of long T1 relaxation time, relatively low signal to noise ratio, high cost of setting, also. As physiologic motion of tissue results in motion ghost in MRI, high 2.0Tesla make improve low signal to noise ratio. This study have aim to improve image quality with controling motion ghost of tissue. Supposing a moving pixel in constant frequency, one pixel make two ghosts which are same size and different anti-phase. So, this study will show adjust parameter on locational control of motion ghost. Author made moving phantom replaced by respiratory movement of human, researched change of motion frequency, FOV by location shift, and them decided optimal FOV (field of view). The results are as follows: 1. The frequency content of the motion determines how far the image always appear in phase-encoding direction, the morphology of the ghost image is characteristic of the direction of the motion and its amplitude. 2. Double FOV of fixed signal object for locational control of motion ghost is recommended. Decreasement of spatial resolution by increasing FOV can compensate on increasing of matrix in spite of scan time increasement.

Factors affecting modulation transfer function measurements in cone-beam computed tomographic images

  • Purpose: This study was designed to investigate the effects of voxel size, the oversampling technique, and the direction and area of measurement on modulation transfer function (MTF) values to identify the optimal method of MTF measurement. Materials and Methods: Images of the wire inserts of the SedentexCT IQ phantom were acquired, and MTF values were calculated under different conditions(voxel size of 0.1, 0.2, and 0.3 mm; 5 oversampling techniques; simulated pixel location errors; and different directions and areas of measurement). The differences in the MTF values across various conditions were evaluated. Results: The MTF 10 values showed smaller standard deviations than the MTF 50 values. Stable and accurate MTF values were obtained in the 0.1-mm voxel images. In the 0.3-mm voxel images, oversampling techniques of 11 lines or more did not show significant differences in MTF values depending on the presence of simulated location errors. MTF 10 values showed significant differences according to the direction and area of the measurement. Conclusion: To measure more accurate and stable MTF values, it is better to measure MTF 10 values in small-voxel images. In large-voxel images, the proper oversampling technique is required. MTF values from the radial and tangential directions may be different, and MTF values vary depending on the measured area.

Accuracy Evaluation of Tumor Therapy during Respiratory Gated Radiation Therapy (호흡동조방사선 치료 시 종양 치료의 정확도 평가)

  • Purpose: To evaluate the accuracy of a target position at static and dynamic state by using Dynamic phantom for the difference between tumor's actual movement during respiratory gated radiation therapy and skin movement measured by RPM (Real-time Position Management). Materials and Methods: It self-produced Dynamic phantom that moves two-dimensionally to measure a tumor moved by breath. After putting marker block on dynamic phantom, it analyzed the amplitude and status change depending on respiratory time setup in advance by using RPM. It places marker block on dynamic phantom based on this result, inserts Gafchromic EBT film into the target, and investigates 5 Gy respectively at static and dynamic state. And it scanned investigated Gafchromic EBT film and analyzed dose distribution by using automatic calculation. Results: As a result of an analysis of Gafchromic EBT film's radiation amount at static and dynamic state, it could be known that dose distribution involving 90% is distributed within margin of error of 3 mm. Conclusion: As a result of an analysis of dose distribution's change depending on patient's respiratory cycle during respiratory gated radiation therapy, it is expected that the treatment would be possible within recommended margin of error at ICRP 60.

Radiation Therapy Using M3 Wax Bolus in Patients with Malignant Scalp Tumors (악성 두피 종양(Scalp) 환자의 M3 Wax Bolus를 이용한 방사선치료)

  • Purpose: Helmet type bolus for 3D printer is being manufactured because of the disadvantages of Bolus materials when photon beam is used for the treatment of scalp malignancy. However, PLA, which is a used material, has a higher density than a tissue equivalent material and inconveniences occur when the patient wears PLA. In this study, we try to treat malignant scalp tumors by using M3 wax helmet with 3D printer. Methods and materials: For the modeling of the helmet type M3 wax, the head phantom was photographed by CT, which was acquired with a DICOM file. The part for helmet on the scalp was made with Helmet contour. The M3 Wax helmet was made by dissolving paraffin wax, mixing magnesium oxide and calcium carbonate, solidifying it in a PLA 3D helmet, and then eliminated PLA 3D Helmet of the surface. The treatment plan was based on Intensity-Modulated Radiation Therapy (IMRT) of 10 Portals, and the therapeutic dose was 200 cGy, using Analytical Anisotropic Algorithm (AAA) of Eclipse. Then, the dose was verified by using EBT3 film and Mosfet (Metal Oxide Semiconductor Field Effect Transistor: USA), and the IMRT plan was measured 3 times in 3 parts by reproducing the phantom of the head human model under the same condition with the CT simulation room. Results: The Hounsfield unit (HU) of the bolus measured by CT was $52{\pm}37.1$. The dose of TPS was 186.6 cGy, 193.2 cGy and 190.6 cGy at the M3 Wax bolus measurement points of A, B and C, and the dose measured three times at Mostet was $179.66{\pm}2.62cGy$, $184.33{\pm}1.24cGy$ and $195.33{\pm}1.69cGy$. And the error rates were -3.71 %, -4.59 %, and 2.48 %. The dose measured with EBT3 film was $182.00{\pm}1.63cGy$, $193.66{\pm}2.05cGy$ and $196{\pm}2.16cGy$. The error rates were -2.46 %, 0.23 % and 2.83 %. Conclusions: The thickness of the M3 wax bolus was 2 cm, which could help the treatment plan to be established by easily lowering the dose of the brain part. The maximum error rate of the scalp surface dose was measured within 5 % and generally within 3 %, even in the A, B, C measurements of dosimeters of EBT3 film and Mosfet in the treatment dose verification. The making period of M3 wax bolus is shorter, cheaper than that of 3D printer, can be reused and is very useful for the treatment of scalp malignancies as human tissue equivalent material. Therefore, we think that the use of casting type M3 wax bolus, which will complement the making period and cost of high capacity Bolus and Compensator in 3D printer, will increase later.
