Purpose: This study investigated the accuracy of free-hand implant surgery performed by an experienced operator compared to static guided implant surgery performed by an inexperienced operator on an anterior maxillary dental model arch. Methods: A maxillary dental model with missing teeth (No. 11, 22, and 23) was used for this in vitro study. An intraoral scan was performed on the model, with the resulting digital impression exported as a stereolithography file. Next, a cone-beam computed tomography (CBCT) scan was performed, with the resulting image exported as a Digital Imaging and Communications in Medicine file. Both files were imported into the RealGUIDE 5.0 dental implant planning software. Active Bio implants were selected to place into the model. A single stereolithographic 3-dimensional surgical guide was printed for all cases. Ten clinicians, divided into 2 groups, placed a total of 60 implants in 20 acrylic resin maxillary models. Due to the small sample size, the Mann-Whitney test was used to analyze mean values in the 2 groups. Statistical analyses were performed using SAS version 9.4. Results: The accuracy of implant placement using a surgical guide was significantly higher than that of free-hand implantation. The mean difference between the planned and actual implant positions at the apex was 0.68 mm for the experienced group using the free-hand technique and 0.14 mm for the non-experienced group using the surgical guide technique (P=0.019). At the top of the implant, the mean difference was 1.04 mm for the experienced group using the free-hand technique and 0.52 mm for the non-experienced group using the surgical guide technique (P=0.044). Conclusions: The data from this study will provide valuable insights for future studies, since in vitro studies should be conducted extensively in advance of retrospective or prospective studies to avoid burdening patients unnecessarily.
NobelGuide$^{TM}$ 임플란트 시스템은 골량과 골질을 수술 전에 판단하여 점막을 젖히지 않고 미리 계획된 스텐트와 보철물을 이용하여 즉시하중을 부여하는 CAD/CAM 기반의 임플란트 보철 치료방법이다. 환자의 불편감을 최소화하여 미리 예측 가능한 위치에 임플란트를 식립함으로써 술 후 합병증 및 보철과정을 간소화 할 수 있는 장점이 있다. 본 증례의 환자는 56세 남성으로 상,하악 부분 무치악 상태였으며, 상악전치부 보철물 탈락과 하악 전치가 흔들린다는 주소로 내원하였다. 이에 상악 잔존치 모두와 하악 4전치 발거 및 치주치료를 실시한 뒤 임시 의치를 6개월간 사용하여 발치와 치유 및 의치 적응여부를 평가하였다. 골량은 충분하다고 판단되었고 환자의 협조도와 경제적인 여건, 전신건강 등 여러 사항이 양호하여 CAD/CAM 기반의 수술템플렛을 이용한 무판막 수술을 통해, 미리 제작된 임시 고정성 보철물로 즉시하중을 부여하는 상악 NobelGuide$^{TM}$ 임플란트 시스템을 계획하였다. 사용중인 임시의치를 이용하여 제작한 방사선 스텐트로 CT를 촬영하고, computer based planning (Procera$^{(R)}$ software)을 통해 3차원적인 골형상과 각 부위별 단면상을 참고하여 상악에 8개의 임플란트 식립체를 생역학적 관점 및 골조건을 고려하여 분산 배치하였다. 미리 제작된 임시 고정성 보철물을 임플란트 수술 후 즉시 장착 및 하중을 부여하여 기능하도록 하였으며 6개월 후 골유착 정도를 평가한 뒤 금속구조물을 이용한 최종 보철물을 제작하였다. 하악의 경우 전치부 도재전장주조금속관과 우측 구치부 임플란트 보철물 제작을 통해 상, 하악 치료를 마무리하였다.
연구 목적: 컴퓨터-가이드 임플란트 수술은 전통적인 방법에 비해 여러 가지 장점을 가진다. 본 연구의 목적은 가이드 수술용 템플릿 제작을 위한 좌표동기화 5축 정밀가 공공정의 정확도를 범용 CAD 소프트웨어를 통해 역설계공학의 방법으로 평가하는 것이다. 연구 재료 및 방법: 악궁 형태의 모형에 거타퍼쳐 스타핑을 매식한 10 개의 모형을 만들고 상부에 실리콘 인상재를 이용하여 인공치은을 덮어 스타핑의 위치를 보이지 않게 가렸다. 모형의 하면에 동기화를 위한 좌표동기화 형상을 만든 뒤 Cone beam CT에서 3차원 영상을 얻었다. 임플란트 계획 소프트웨어의 CT 이미지 상에서 매식된 스타핑과 동일한 방향으로 스타핑의 1/2 깊이까지 가상의 시술계획을 하고, 스타핑의 방향벡터와 저지점 (1/2지점) 데이터를 석고모형의 영상으로 좌표동기화 하였다. 이후 모형하면의 좌표동기화 형상을 이용하여 가공기기상의 좌표로 좌표변환을 통해 가공좌표동기화를 하였다. 5축 밀링머신의 좌표동기화판에 모형을 고정한 후, 동기화된 가공데이터에 의거하여 스타핑과 동일한 직경의 드릴로 계획된 벡터와 깊이로 정확히 가공 하였다. 모델에 정확히 안착되는 인상트레이를 CT 장비에 미리 고정한 상태에서, 인상트레이에 모델을 적합하여 이미지를 획득한 뒤 3차원 재구성하는 방법으로 영상을 중첩하여 비교 분석하였다. SolidWorks (Dassault Systems, Concord, USA) 범용 CAD 상에 영상을 불러들여 역설계공학의 방법으로 실린더 상부, 하부의 중점에서의 위치편차와 각도편차를 조사하였다. 통계는 SPSS (release 14.0, SPSS Inc., Chicago, USA)를 이용하여 각 편차 사이의 상관관계를 분석하였다 ($\alpha$ = 0.05). 결과: 위치 편차로 인하여 모든 드릴 보어 (bore)에서 상부 1/2에 잔존하는 거타퍼쳐의 일부를 관찰할 수 있었다. 실험 모형상에서 계획된 이미지와 드릴링 후CT에서 역설계를 거친 이미지 사이의 위치편차는 상부에서 0.31 (0.15 - 0.42) mm, 하부에서 0.36 (0.24 - 0.51) mm, 각도편차는 1.62 (0.54 - 2.27)$^{\circ}$이었다. 실린더 상부와 하부 위치 편차는 양의 상관관계를 가졌다 (Pearson Correlation Coeffocient = 0.904, P= .013). 결론: 좌표동기화 5축 정밀가공 공정은 가이드 수술용 템플릿을 제작하는 데에 적합한 정확도를 가진다.
다수 치아의 상실 상태로 장시간 지속시 구치부 지지의 부족 및 치아들의 과정출이 발생되어 교합 부조화, 수직고경 상실 및 기능 장애 등의 심각한 문제가 초래된다. 본 증례는 장기간 다수치아 결손으로 인하여 대합치가 정출하면서 교합평면의 붕괴가 일어난 환자의 완전 구강 회복에 관한 증례이다. 상기 환자는 서울대학교 치과병원 치과보철과로 내원한 68세 남환으로 오래된 상악 의치를 빼다가 치아가 함께 발거되었으며, 하악 전치부가 상악 전치부와 강하게 닿아서 불편함을 호소하며 내원하였다. 여러가지 평가를 통해 수직고경을 4 mm 거상하였으며, 무치악부는 진단 및 치료계획시 설계한 최종 보철 수복물 제작을 고려하여 정확한 임플란트 식립을 위해 CAD-CAM (Computer-aided design-computer-aided manufacturing) 기술을 이용한 Computer guided implant surgery를 시행하고, 임플란트 고정성 보철 수복으로 진행하였고, 환자는 저작, 기능 및 심미 모두 큰 개선에 만족하였다.
디지털 방식을 이용한 치과 임플란트 진료에서 가장 주목받는 분야 중 하나가 디지털 소프트웨어와 컴퓨터 단층촬영을 이용하여 수술용 스텐트를 제작, 임플란트를 식립하는 분야이다. 컴퓨터를 이용한 디지털 임플란트 치료계획 시스템과 이를 이용한 수술용 가이드는 예지성과 심미성, 기능성을 갖춘 임플란트의 식립과 수복을 가능하게 한다. 특히 이러한 가이드 수술은 임플란트 수복물의 형태와 기능을 고려한 임플란트의 식립 시 더욱 유리하다. 최근 여러 제조사에서 보철물 제작을 위해 치과용 스캔부터 제작방법까지 다양한 디지털 방식의 제작과정을 제시하고 있다. 이러한 방법들은 정확하고 예지성 있는 예후를 위해 여러 연구자들에 의해 연구되어지고 있으며, 점차 발전하고 있다. 이번 증례는 완전무치악의 여성 환자에서 임플란트 고정성 보철물을 이용한 수복증례이다. 하악은 all-on-four 방식을 적용하여 4개의 임플란트를 식립하였으며, 상악은 이에 기초하여 6개의 임플란트를 식립하였다. 최종보철물에 기초한 임플란트의 식립을 위해 임플란트 수술용 스텐트를 이용하였으며, 환자는 치료 결과에 만족하였으며, 5년 추적관찰 결과, 임플란트와 보철물 모두 안정적으로 유지되고 있다.
치과와 기공실을 오가며 복잡하게 진행되는 기존 CAD-CAM 컴퓨터 가이드 제작 방식을 단 순화하기 위하여, 진료실에 설치한 바로가이드 시스템(VARO Guide, CAD, Pre-Guide, VARO-mill, NeoBiotech, Seoul, South Korea)을 통하여 Top-Down 개념과 수복 주도개념을 적용하여 1-day 수술과 임시보철을 완료하는 증례를 보고하고자 한다. Pre-Guide를 통해 환자의 중심위에서 교합을 채득하고 CT를 촬영하며, 진료실 내 컴퓨터에서 CAD를 통해 바로가이드를 1시간 내에 제작하였다. 미리 제작된 광중합 레진이 담긴 Pre-Guide 트레이를 술부에 위치시킨 후 중심위에서 CT를 촬영하였다. CT 데이터가 진료실 내 컴퓨터로 전송되면 수복 위치를 정한 후 이어서 3차원적으로 턱뼈에서 임플란트를 식립할 위치를 디자인하여 STL 파일로 추출하였다. 그리고 진료실에 설비된 밀링 기계(VARO-mill)에 Pre-Guide를 고정하여 수술용 가이드를 15분 내에 제작한다. 이 가이드로 환자는 한번 내원하여 임플란트를 계획한 위치에 안전하고 정확하게 수술해 줌과 더불어, 초기고정력이 양호한 임플란트에 대해서는 즉시임시치아까지 장착해 주어 환자에게 즉시 기능이 가능하도록 할 수 있었다.
임플란트 시술에서 CAD/CAM을 포함한 컴퓨터 기술의 발달은 단순히 임플란트를 보다 정확하고, 예측 가능하게 식립하는 것뿐 아니라, 무피판(flapless) 수술과 즉시하중을 가능하게 하였다. 그러므로 술후 출혈과 불편감의 감소 및 시술시간과 치유기간을 단축시키는 장점이 있으며, 시술 후 즉시 보철물을 장착해 줌으로써 환자의 구강기능을 단기간에 회복시키고 경조직과 연조직의 형태를 보존하여 최종보철 시 유리한 환경을 만들어 줄 수 있게 되었다. 본 증례는 상 하악 무치악 상태로 내원한 40세 남성 환자로 하악 총의치의 불안정성 및 불편감 등을 호소하였으며, 하악에 임플란트 지지 고정성 보철물을 통한 수복을 원하였다. 충분한 골량, 환자의 참여도, 경제적인 여건, 전신적인 건강 등을 고려하여 하악에는 CAD/CAM 기반의 수술용 스텐트를 이용한 수술 및 미리 제작된 고정성 임시보철물을 이용하여 즉시하중을 부여하는NobelGuide 보철을 계획하였으며, 상악에는 총의치를 계획하였다. 환자의 안모를 평가하여 임시의치를 제작하였으며, 복제한 임시의치를 바탕으로 스텐트를 제작하고, 컴퓨터단층촬영을 시행하였다. 3차원으로 변환된 영상을 기반으로 하악에 7개의 고정체를 생역학적 조건과 치조골의 상태에 맞게 분산 배치하였다. 제작된 수술용 스텐트에 맞게 주모형을 제작하고, 임시고정성 보철물을 제작하여 장착함으로써 즉시하중을 부여하였다. 식립 3개월 후 골유착 정도를 평가하고 최종보철물을 제작하였다. 추후 발생할 수 있는 상악 골흡수를 방지하기 위해 주기적인 내원 및 검사를 통해 성공적인 치료가 될 수 있도록 노력하였다.
목적: 인상재와 석고모형을 사용하지 않고 디지털 영상 만을 사용하여 제작된 수술가이드를 이용한 치과 임플란트 수술의 정확도를 평가하고자 하였다. 재료 및 방법: 상하악 부분 무치악 모델 6개 세트를 본 연구에 사용하였다. 모델을 환자 구강구조로 간주하고 부분 무치악 모델을 Trios 구강스캐너로 디지털 인상을 채득하였다. 수술가이드의 정확도 평가를 위해 제작된 Reference plate를 모델의 교합면에 고정한 상태로 CBCT를 촬영하였다. CBCT에서 얻은 영상과 디지털 인상에서 얻은 영상을 정합하고 정합한 영상에서 임플란트 식립위치를 진단하고 수술가이드를 디자인하였다. 제작된 수술가이드를 이용하여 부분 무치악 모델에서 임플란트를 식립하였다. 시술 후, 시술 전과 동일한 방법으로 CBCT를 촬영하여 시술 전 컴퓨터 소프트웨어에서 측정한 수치와 시술 후 CBCT촬영하여 소프트웨어상에서 측정한 시술 후 수치 사이 차이점을 비교하였다. 결과: 평균거리오차가 임플란트 상단 X축에서 0.27 mm, Y축에서 0.29 mm 였으며, 임플란트 하단(Apex)부위에서는 X축에서 0.11 mm, Y축에서 0.14 mm 평균각도오차가 X축에서 $0.26^{\circ}$, Y축에서 $0.26^{\circ}$ 였다. 평균수직오차는 +0.44 mm로 시술 전 계획한 높이보다 평균 0.44 mm 더 위로 임플란트가 식립되었다. 결론: 본 연구의 결과는 인상재와 석고모형을 사용하지 않고 디지털 영상 만을 사용하여 제작된 수술가이드를 사용하여 임플란트를 시술할 경우 계획한 임플란트 식립 위치와 방향으로 높은 정확도를 가지고 임플란트를 식립할 수 있음을 보여 주었다.
Purpose : The purpose of this study was to develop a stent-based image guided surgery system and to apply it to oral and maxillofacial surgeries for anatomically complex sites. Materials and Methods : We devised a patient-specific stent for patient-to-image registration and navigation. Three-dimensional positions of the reference probe and the tool probe were tracked by an optical camera system and the relative position of the handpiece drill tip to the reference probe was monitored continuously on the monitor of a PC. Using 8 landmarks for measuring accuracy, the spatial discrepancy between CT image coordinate and physical coordinate was calculated for testing the normality. Results : The accuracy over 8 anatomical landmarks showed an overall mean of $0.56{\pm}0.16\;mm$. The developed system was applied to a surgery for a vertical alveolar bone augmentation in right mandibular posterior area and possible interior alveolar nerve injury case of an impacted third molar. The developed system provided continuous monitoring of invisible anatomical structures during operation and 3D information for operation sites. The clinical challenge showed sufficient accuracy and availability of anatomically complex operation sites. Conclusion : The developed system showed sufficient accuracy and availability in oral and maxillofacial surgeries for anatomically complex sites.
Background: The concept of the ideal morphology for the alveolar bone form is an important element to reconstruct or restore the in maximizing esthetic profile and functional alveolar bone restoration. The purpose of this preliminary study is to evaluate the normal alveolar bone structure to provide the standard reference and guide template for use in diagnosing for implant placement, determining the correct amount of bone augmentation in actual clinical practice and producing prostheses based on three-dimensional imaging assessment of alveolar bone. Methods: This study was included 11 men and 11 women (average age, 22.6 and 24.5 years, respectively) selected from among 127 patients. The horizontal widths of alveolar bone of maxilla and mandible were measured at the crestal, mid-root, and root apex level on MDCT (multi-detector computed tomography) images reconstructed by medical imaging software. In addition, tooth dimensions of the central incisors, canines, second premolars, and first molars of maxilla and mandible, including the horizontal width of the interdental alveolar bone crest, were also measured and statistically analyzed. Results: The horizontal alveolar bone width of the palatal side of maxilla showed a distinct increment from the alveolar bone crest to the apical region in both anterior and posterior areas. The average widths of the maxillary alveolar ridge were as follows: central incisor, 7.43 mm; canine, 8.91 mm; second premolar, 9.57 mm; and first molar, 12.38 mm. The average widths of the mandibular alveolar ridge were as follows: central incisor, 6.21 mm; canine, 8.55 mm; second premolar, 8.45 mm; and first molar, 10.02 mm. In the buccal side, the alveolar bone width was not increased from the crest to the apical region. The horizontal alveolar bone width of an apical and mandibular border region was thinner than at the mid-root level. Conclusions: The results of the preliminary study are useful as a clinical guideline when determining dental implant diameter and position. And also, these measurements can also be useful during the production of prefabricated membranes and customized alveolar bone scaffolds.

회원이 이용계약을 해지하고자 하는 때에는 [가입해지] 메뉴를 이용해 직접 해지해야 합니다.
제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는
경우 서비스 이용을 제한할 수 있습니다.
- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
