Shunt valves used to treat patients with hydrocephalus were tested to investigate influence of intracranial pressure pulsation on their flow control characteristics. Five commercial shunt valves were tested in the flow loop that simulates pulsed flow under pressure pulsation. As 20cc/hr of flow rate was adjusted at a constant pressure, application of $40mmH_2O$ of pressure pulse increased the flow rate by $67.9\%.$ As a 90cm length catheter was connected to the valve outlet, increase in the flow rate was substantially reduced to $17.5\%.$ As the flow rate was adjusted to 40cc/hr at a constant pressure, increase in the flow rate was $51.1\%$ with the same pressure pulsation of $40mmH_2O$. The results indicated that pressure-flow control characteristics of shunt valves implanted above human brain ventricle is quite different from those obtained by syringe pump test at constant pressures right after manufacture. The influence of pressure pulsation was observed to be more significant at low flow rate and the flexibility of the outlet silicone catheter was estimated to significantly reduce flow increase due to pressure pulsation.
수두증 환자를 치료하기 위해 머리에 시술 되는 션트밸브가 받는 뇌압 펄스가 밸브의 유량조절 특성에 미치는 영향을 수치적으로 해석하였다. 션트밸브를 통한 유량을 계산하기 위하여 밸브 다이아프램이 형성하는 유량 오리피스를 모델링 하여 맥동압력과 변동하는 압력에 의한 다이아프램의 움직임을 고려하였다. 연구 결과, 압력 펄스와 이에 따른 다이아프램의 움직임에 의하여 유량이 40% 이상 증가함을 보였다. 이 결과는, 뇌실에 시술 되는 션트밸브의 압력-유량 조절특성이 제조 직후 맥동이 없는 유량을 공급하는 시린지 펌프를 사용하여 구한 실험 결과와 상당히 다르다는 사실을 보여주었다.
Encephalitis is known as a rare complication of varicella zoster virus (VZV) reactivation. It is usually regarded as a complication of a cutaneous infection in patients with impaired cellular immunity. The reported incidence of herpetic motor involvement range between 0.5 and 31%, but is possibly more frequent as the weakness is readily obscured by pain. A 53-years-old woman, who presented with severe shoulder pain, fever, headache and seizure, which developed the day after skin eruptions, also developed motor paresis 7 days after the seizure. Her cerebrospinal fluid (CSF) was VZV-Polymerase chain reaction (PCR) negative, but VZV specific IgG antibody positive, and her brain MRI was found to be normal. With the early diagnosis and proper treatment, such as intravenous administration of acyclovir, stellate ganglion block and Yamamoto New Scalp Stimulation (YNSS), the patient completely recovered, without psychoneurological sequelae. Herein, we present this case, with a discussion of the relevant literature on the incidence, pathophysiology, diagnosis and management of central nervous system VZV involvement.
A 13-month-old intact female poodle dog presented with an acute history of circling and seizure episodes. On the basis of the results of neurologic examination combined with magnetic resonance imaging and cerebrospinal fluid analysis (CSF), meningoencephalitis of unknown etiology (MUE) was suspected. Therapy with mycophenolate mofetil plus prednisolone was initiated, following which the clinical signs showed improvement for only one month before gradually worsening again. Acute progression of the clinical disease was observed, and the patient was euthanized 91 days after initial presentation. This case was definitively diagnosed as necrotizing meningoencephalitis (NME) according to the results of post-mortem histopathological examination. This report describes the clinical findings, serial magnetic resonance imaging (MRI) characteristics, and histopathological changes in a case of acute NME.
We report a case of narcolepsy. A 25-year-old man has had excessive daytime sleepiness of about 10 years durations. He awakens daily feeling exhausted and continually falls asleep during the day while engaged in such situation like reading and watching television. He has exhibited cataplexy, a sudden loss of muscular tone, brought on by emotion, usually laughter. Polysomnogram revealed increased sleep stage 1, 2 and decreased deep sleep. Multiple sleep latency test (MSLT) showed that sleep latency was 1.33 minutes and there were 3 noted sleep onset rapid eye movement (SOREM) on 5 trials. The epworth sleepiness scale (ESS) was 17/24. Typing of HLA haplotype that was positive for the $DQB1^{\ast}0602$ allele, and hypocretin-1 (orexin A) could not be detected in cerebrospinal fluid (CSF). Brain MRI showed normal image. We diagnosed his case as narcolepsy based on history of cataplexy, and three occurances of SOREM, and positive of HLA haplotype.
Spinal meningeal cyst of the sacrum is uncommon congenital lesion. We experienced two cases of sacral meningeal cyst, so called Tarlov's cyst, who presented with radiating pain. Magnetic resonance imaging is a highly effective way of locating and approximating the size of these entities, which generally appear as intraspinal masses of low intensity on T1-weighted and high intensity on T2-weighted images, similar to cerebrospinal fluid(CSF). We evaluated 2 patients who had Tarlov's cyst diagnosed with conventional MRI. The clinical features, radiological findings, gross appearances of the lesion at surgery, surgical technique, histopathological features of the cyst wall, and surgical outcome are described. We conclude that excellent result can be expected in the case of symptomatic Tarlov's cyst by surgical decompression.
In this study, we intended to describe a human case of lumbosacral canal sparganosis in People's Republic of China (China). A 56-year-old man was admitted to Xiangya Hospital Central South University in Changsha, Hunan province, China after having an experience of perianal pain for a week. An enhancing mass, a tumor clinically suggested, was showed at the S1-S2 level of the lumbosacral spine by the examination of magnetic resonance imaging (MRI) with gadolinium contrast. The patient was received the laminectomy from S1 to S2, and an ivory-white living worm was detected in inferior margin of L5. In ELISA-test with cerebrospinal fluid (CSF) and serum samples, anti-sparganum antibodies were detected. He had a ingesting history of undercooked frog meat in his youth. By the present study, a human case of spinal sparganosis invaded in lumbosacral canal at the S1-S2 level was diagnosed in China. Although the surgical removal of larvae is known to be the best way of treatment for sparganosis, we administered the high-dosage of praziquantel, albendazole and dexamethasone to prevent the occurrence of another remain worms in this study.
In three dogs showing cerebellar ataxia, the onset of clinical signs varied from a young age of five months to age 13 years. Qualitative magnetic resonance imaging (MRI) revealed various degrees of cerebellar atrophy, and a tentative diagnosis of cerebellar cortical degeneration was made. Quantitative analysis using the brainstem to the cerebellar cross-sectional area ratio (BS:CBM ratio) and T2-signal intensity histograms were obtained to perform an objective evaluation. These techniques have the advantage of being easy and fast to evaluate. These quantitative analyses revealed the severity of cerebellar cortical degeneration in the three dogs as mild, moderate, and severe. Dogs 2 and 3 were identified as abnormal on the relative cerebrospinal fluid (CSF) space using T2-signal intensity histograms but were normal on the BS:CBM ratio. This suggests that the T2-signal intensity histograms may have higher sensitivity than BS:CBM ratio.
Amyloid-β (Aβ) is one of the amyloidogenic intrinsically disordered proteins (IDPs) that self-assemble to protein aggregates, incurring cell malfunction and cytotoxicity. While Aβ has been known to regulate multiple physiological functions, such as enhancing synaptic functions, aiding in the recovery of the blood-brain barrier/brain injury, and exhibiting tumor suppression/antimicrobial activities, the hydrophobicity of the primary structure promotes pathological aggregations that are closely associated with the onset of Alzheimer's disease (AD). Aβ proteins consist of multiple isoforms with 37-43 amino acid residues that are produced by the cleavage of amyloid-β precursor protein (APP). The hydrolytic products of APP are secreted to the extracellular regions of neuronal cells. Aβ 1-42 (Aβ42) and Aβ 1-40 (Aβ40) are dominant isoforms whose significance in AD pathogenesis has been highlighted in numerous studies to understand the molecular mechanism and develop AD diagnosis and therapeutic strategies. In this review, we focus on the differences between Aβ42 and Aβ40 in the molecular mechanism of amyloid aggregations mediated by the two additional residues (Ile41 and Ala42) of Aβ42. The current comprehension of Aβ42 and Aβ40 in AD progression is outlined, together with the structural features of Aβ42/Aβ40 amyloid fibrils, and the aggregation mechanisms of Aβ42/Aβ40. Furthermore, the impact of the heterogeneous distribution of Aβ isoforms during amyloid aggregations is discussed in the system mimicking the coexistence of Aβ42 and Aβ40 in human cerebrospinal fluid (CSF) and plasma.
본 연구에서는 콜린성 기전에 반응하여 분비되는 내피 의존성 이완물질(endothelium-derived relaxing factor, EDRF)나 nitric oxide(NO)가 마취 흰쥐의 뇌혈류 자가조절기전에 관여할 가능성을 관찰하였다. Acetylcholine($10^{-9}-10^{-6}M$)을 포함한 mock 뇌척수액(CSF)을 관류시 뇌연막 동맥은 농도에 의존하여 이완반응 나타내었고(평균; $19.3{\pm}1.7{\mu}m$, n=36), 이러한 이완반응은 $N{\omega}$-nitro-L-arginine(L-NNA, $10^{-5}M$)에 의해서 억제되었을 뿐 아니라 methylene blue($10^{-6}M$)나 oxyhemoglobin($10^{-6}M$)에 의하여도 억제되었다. 한편 이러한 acethlcholine에 의한 뇌연막동맥의 이완반응을 매게하는 무스카린 수용체는 무스카린 수용체 길항제의 봉쇄효과를 관찰한 실험에서 $M_1$과 $M_3$ 아형으로 생각되었다. L-Arginine을 함유한 mock CSF로 관류시 일어나는 일시적인 혈관이완반응은 NY 83583 ($10^{-5}M$)에 강력히 억제되었으나 L-NNA ($10^{-5}M$)에 의해서는 억제되지 아니하였다. 한편 acetylcholine과 L-arginine에 의한 혈관이완반응은 ATP-sensitive $K^+$ 통로 봉쇄제인 glibenclamide에 의해 유의하게 봉쇄되었다. 나아가 뇌연막동맥의 직경 변화를 동맥압의 변화에 대하여 검정한 결과 혈관이완과 혈관수축의 희귀 직선의 경사도는 $10^{-5}M$ L-NNA의 전처치에 의하여 영향을 받지 아니하였으나, $3{\times}10^{-6}M$ glibenclamide에 의해 유의하게 감소되었다. 이러한 결과로 보아 혈압하강에 대해 쥐의 뇌연막동맥에 나타나는 혈관이완반응은 EDRF(NO)에 의해 매개되지 않는다고 사료된다.
